        간호사의 문화적 역량 측정도구에 대한 고찰 : IAPCC와 CCA를 중심으로

        Purpose: As Korean society has rapidly become multicultural in the last few decades, it is essential for nurses to become culturally competent to provide effective care for ethnically and culturally diverse populations. Considering the advantages of standardized instrument, there is a need to evaluate current cultural competence instruments to assess adaptability to Korean nurses. Methods: Using Macdowell’s instrument evaluation guideline, a review and evaluation was done of the Inventory for Assessing the Process of Cultural Competence among Healthcare Professionals (IAPCC) and Cultural Competence Assessment (CCA), which were both developed based on cultural competence theoretical models and have been commonly used in nursing research. Two other Korean instruments were also evaluated. Results: The instruments reviewed have limitations in reliability and validity, as well as cultural background and development process, for measurement of cultural competence in Korean nurses. Conclusion: The results of this study indicate that it is necessary to discuss and agree on a definition of what cultural competence is and to develop instruments to measure cultural competence in Korean nurses.

        남성과 여성 사무직 근로자의 신체활동에 미치는 영향요인 비교

        Purpose: The purpose of this study was to determine gender differences in effects of self-efficacy, exercise benefits and barriers, and demographic factors on the physical activity. Methods: Seventy sedentary office workers, 35 male and 35 female, from a major airline company, completed a questionnaire from March 28 to April 5, 2012. Steps and body mass indices were measured using a CW-700/701(Yamax) pedometer and Inbody 720(Biospace), respectively. Data were analyzed using t-test, x2-test, multiple linear regression, and simultaneous quantile regression. Results: For male workers, exercise self-efficacy had a significant effect on physical activity, but only when respondents were at 10%(3,431 steps/day, p=.018) and 25%(4,652 steps/day, p=.044) of the physical activity distribution. For female workers, marital status was significantly related to physical activity, but only when respondents were at 10%(3,537 steps/day, p=.013) and 25%(3,862 steps/day, p=.014) of the physical activity distribution. Conclusion: Quantile regression highlights the heterogeneous effect of physical activity determinants among office workers. Therefore intervention strategies for increasing physical activity should be tailed to genders as well as physical activity levels.

        Development and Psychometric Evaluation of the Korean Version of the Cultural Competence Scale for Clinical Nurses

        Purpose: To develop and psychometrically test the Korean version of the Cultural Competence Scale for Nurses (K-CCSN). Methods: A multi-phase questionnaire development method was used to develop the scale from November 2012 to April 2013. The item pool was generated based on literature review, existing scales and in-depth interviews. The content validity was evaluated twice by an expert panel. The scale validation was conducted with a convenience sample of 456 general hospital nurses recruited from five general hospitals and a nursing college in the Seoul Metropolitan Area of South Korea. The constructrelated and criterion-related validity and internal consistency reliability of the scale were tested. Results: The 33-item K-CCSN comprised four subscalesdcultural awareness, cultural knowledge, cultural sensitivity and cultural skillsdexplaining 53.96% of the total variance. The criterion-related validity was supported by a known-group comparison. The reliability analysis showed an acceptable-to-high Cronbach's alpha in total and for subscales ranging from .879 to .932. Conclusion: This preliminary evaluation of psychometric scale properties demonstrated acceptable validity and reliability. The K-CCSN is able to provide scientific and empirical data regarding the cultural competence of clinical nurses. However, further studies are needed to test the applicability of the scale in different settings and contexts.

        Public Health Center Service Experiences and Needs among Immigrant Women in South Korea

        Purpose: To support implementation of comprehensive, person-centered healthcare, this study aimed to explore immigrant women's public health center (PHC) service experiences and needs while considering Photovoice's feasibility for this purpose. Methods: This qualitative study included 15 marriage-based immigrant women. Participants were recruited from churches and multicultural family support centers using purposive and snowball sampling. Data were collected through four focus group interviews and were subjected to inductive content analysis. Results: Five categories of experiences were identified: language barriers, hectic environment, affordable and practical primary healthcare, feeling ignored and discriminated against, and feeling frustrated. In addition, five categories of needs were identified: language assistance services, ease of access, healthcare across the lifespan, expansion of affordable healthcare, and being accepted as they are. This study provides preliminary evidence that the Photovoice approach can facilitate the interview process in a qualitative inquiry involving participants with limited ability to express their perspectives in the researchers' language. Conclusion: Study findings highlight the need to implement institutional policy and procedural changes within PHCs and to provide culturally competent, personcentered care for South Korea's marriage-based immigrant women and other ethnic minority populations. The findings also provide evidence-based direction for PHC service planning.

        이주민 취업자의 건강검진 참여와 인구사회학적 및 건강관련 특성과의 관계

        건강검진은 질병의 조기 진단 및 치료를 가능하게 하여 개인의 건강수준 향상과 국가의 질병부담 완화에 기여한다. 본 연구의 목적은 국내 이주민 취업자를 대상으로 건강검진 참여 현황과건강검진 참여에 영향을 미치는 요인을 인구사회학적 특성과 건강관련 특성을 중심으로 분석하는 것이다. 본 연구는 ‘2022년 광주광역시 이주민 건강실태조사’ 자료를 이용한 이차자료 분석연구이다. 본 연구에서는 20세 이상이며 취업 중인 이주민 243명을 연구대상자로 선정하였다. 연구변수는 최근 2년간 건강검진 참여, 인구사회학적 특성(10개 변수), 건강관련 특성(7개 변수)이었다. 건강검진 참여의 영향요인을 파악하기 위해 이분형 로지스틱 회귀분석(logistic regression analysis)을 실시하였다. 연구결과 이주민 취업자의 건강검진 참여율은 66.3%(161명)이었다. 건강검진 참여에 영향을 미치는 요인은 업종(OR=3.99, 95% CI 1.50-10.63), 월평균 소득(OR=4.09, 95% CI 1.48-11.32), 건강정보에 대한 관심(OR=3.27, 95% CI 1.26-8.50)이었다. 건설업·농림어업에 종사하고, 월평균 소득이 150만 원 미만이며, 건강정보에 관심이 낮은이주민이 건강검진에 참여하지 않을 가능성이 높은 취약 집단이었다. 이주민 취업자의 건강보호를 위해서는 이주민이 집중 고용되어 있는 농림어업·건설업 소규모 사업장에 대한 산업안전규제와 감독을 보완하는 제도적인 개선이 필요하다. 더불어 예방적 차원의 건강관리에 대한 이주민 취업자의 인식제고를 위해 이주민의 언어와 문화에 적절한 건강정보를 개발하고 활용해야할 것이다. Health examinations facilitate early diagnosis and treatment of diseases, contributing to the improvement of individuals’ health and the reduction of the burden on the nation's healthcare services. The purpose of this study was to identify the sociodemographic and health-related factors associated with participation in health examinations among migrant workers in South Korea. This study was a secondary data analysis study using data from the 2022 Gwangju city migrant health status survey. In this study, 243 employed migrants aged 20 years or older were selected as the study subjects. We used 18 variables and conducted logistic regression analyses to identify predictors for participation in health examinations. A total of 66.3% of the subjects participated in health examinations. The factors that influenced health examination participation were industry type (OR=3.99, 95% CI 1.50-10.63), monthly income (OR=4.09, 95% CI 1.48-11.32), and interest in health information (OR=3.27, 95% CI 1.26-8.50). Migrants who worked in agricultural, fishery, and construction industries, had a monthly income of less than 1.5 million won, and had low interest in health information were more likely to not participate in health examinations. To protect the health of migrant workers, institutional improvement is needed to supplement industrial safety regulations and supervision for small businesses in the agricultural, fishery, and construction industries, where migrants are concentrated. In addition, to raise the awareness of migrant workers about the importance of preventive health care, it is necessary to develop and utilize health information that is appropriate for their language and culture.

        Home-based walking intervention for middle-aged migrant women using 360-degree virtual videos and a wearable activity tracker: A mixed-methods pilot study

        Purpose: To sustain behavior change, an intervention strategy that considers the contribution of affect to daily physical activity behavior regulation is needed. Although virtual reality-based physical activity interventions have the potential to improve emotional status, interventions using virtual reality videos in a free-living environment are lacking. This pilot study assessed the feasibility and preliminary efficacy of a home-based intervention using 360-degree virtual videos and wearable activity trackers to improve mood and physical activity. Methods: A one-group pilot study of 12 middle-aged migrant women asked participants to watch virtual reality videos and perform moderate-intensity walking 5 days per week for 4 weeks, then complete surveys and focus group interviews. The intervention’s feasibility and preliminary efficacy were assessed by examining recruitment, retention, adherence, acceptability, mood, physical activity, and exercise self-efficacy. Results: A word-of-mouth approach was effective for recruiting participants. Although the weekly intervention adherence rate ranged from 53.5% to 83.5%, retention (92.3%) and acceptability (91.7%) were high. Participants were satisfied with the “visual status indicators,” “sense of accomplishment and confidence,” “emotional engagement and sense of presence,” “joy from exercise,” “external motivation through supervision,” “easy to control virtual reality device,” and “extra benefits” of the intervention. Participants had significant decreases in negative affect (p=.016). Positive affect, physical activity, and exercise self-efficacy showed trends toward improvement. Conclusion: This home-based intervention employing virtual reality videos and Fitbit activity trackers is feasible and shows preliminary efficacy in improving mood. Further research is warranted to evaluate its effectiveness in a more rigorous randomized controlled trial.

        국내 이주근로자의 주관적 건강에 미치는 영향요인

        Objectives: The purpose of this study was to identify factors associated with the self-rated health of migrant workers and to compare the difference between factors according to the length of stay. Methods: Study subjects were 8,696 migrant workers in Korea who worked at least 40 hours a week. Data were analyzed using logistic regression. Results: Of the subjects, 24.1% rated their health as poor. In the group with more than 10 years of residence in Korea, two times more people rated their health poor than the group of less than 3 years. Factors associated with health were relationship satisfaction, life satisfaction, leisure satisfaction, roommates, discrimination, residential environment satisfaction, unmet health needs, job satisfaction, and income satisfaction. Regardless of the length of stay, relationship satisfaction, unmet health needs, and job satisfaction were common influences. As the length of stay increased, while the significance of employment insurance and life satisfaction decreased, leisure satisfaction, income satisfaction, and Korean language ability increased. Conclusions: The findings suggest that measures to protect the health of migrant workers should begin early in migration. Furthermore, for influencing factors differ depending on the length of residence, health policies for migrant workers should be developed according to the different lengths.
