목적: 원위 요골 골절 후 발생한 장무지 신전건 파열의 원인과 수술 결과에 대하여 알아보고자 하였다. 대상 및 방법: 원위 요골 골절 후 발생한 장무지 신전건 파열에 대한 수술적 치료 후 1년 이상 추시가 가능한 19예를 대상으로 하였다. 19예 중 14예에서 보존적 치료, 4예에서 비관혈적 정복술 및 K-강선 고정술, 그리고 1예에서 관혈적 정복술 및 금속판 고정술 후에 장무지 신전건 파열이 발생하였다. 전 예에서 시지 고유신전건 이전술을 시행하였다. 결과: 장무지 신전건 파열의 원인으로는 K-강선에 의한 자극에 의한 경우 2예, 돌출된 나사못에 의한 경우 1예, 그리고 가골에 의한 경우 1예였다. 파열의 진단 시기는 보존적 치료를 시행한 14예에서 평균 3.1개월 (0.7∼17), 수술적 치료를 시행한 5예에서 평균 12.8개월 (1∼48)이었다. 최종 추시상 전 예에서 만족스런 무지의 신전이 관찰되었다. 결론: 원위 요골 골절 후 발생한 장무지 신전건 파열은 허혈성 손상이 주원인이었으므로 요골 골절 후 장무지 신전건 파열에 대한 주의깊은 관찰이 필요하며, 수술적 치료 시 나사못이나 K-강선의 방향과 길이에 대한 세심한 주의가 필요할 것으로 생각된다. 또한 시지 고유 신전건 이전술은 무지 신전 기능 회복에 효과적인 수술 방법이라 생각한다. Purpose: To evaluate the cause and surgical outcome of extensor pollicis longus rupture after distal radius fractures. Materials and Methods: Nineteen cases in which the patients underwent surgical treatment for rupture of the extensor pollicis longus after distal radius fractures were followed for more than one year. Among the nineteen cases, fourteen extensor pollicis longus ruptures occurred after conservative treatment, four occurred after closed reduction with K-wire fixation, and one occurred after open reduction and internal fixation with a plate. All cases were treated by extensor indicis proprius transfer. Results: Extensor pollicis longus ruptures were caused by K-wire irritation in two, by a protruding screw tip in one, and by a callus in one. In the conservative treatment group, tendon ruptures were diagnosed at an average of 3.1 months (0.7∼17). Tendon ruptures were detected in the surgical treatment group at an average of 12.8 months (1∼48). All the patients showed favorable recovery of the extension capability of the thumb at the final follow-up. Conclusion: The main cause of extensor pollicis longus rupture after distal radius fracture was ischemic damage. Therefore, during the surgery, the length and direction of screws and K-wires should be fixed carefully to avoid such damage. Distal radius fracture also requires careful observation of the extensor pollicis longus during follow-up. Furthermore, extensor indicis proprius transfer is considered to be an effective method for extensor pollicis longus rupture.
Performance Characteristics of Water-to-Air Heat Pump under Partial Load Heating Operation

Performance of water-to-air heat pump using raw water has been analyzed under part load heating operation in March, 2010. The water source heat pump of 30 RT was installed for 24 hours cooling and heating ventilation, and the gravity inflow water from Daechung dam is used as the heat source. The daily averaged water and air temperatures are 5.7?C and 9.9?C respectively, and the heat pump is operated under part load condition for 7.5 hours in 24 hours. The daily averaged heat pump COP calculated with heat transferred from the brine water is 2.49 and the monthly averaged COP is 2.25 in March. Based on the database of the California Energy Commission, the monthly averaged COPs of air source heat pumps installed in U.S.A. are 1.97 in March and 2.03 in April. Therefore it is confirmed again that the performance of the heat pump using raw water is better than that of air source heat pumps.

Riverbank filtration wells have been developed to supply a heat source/sink of water in the alluvium aquifer to ground water heat pumps for cooling and heating of a green house. In order to look for an appropriate site to carry out the research, two sites of Jinju and Gumi areas were investigated. In the results of the electrical resistivity surveys, Jinju and Gumi areas have the alluvium aquifer in the depth of 6~17 m and 10~20 m under the ground respectively. Two boreholes have been drilled in each site of both areas. The averaged water level at Jinju site is about 3 m under the ground, and 3.5 m and 6.5 m of sandy gravel aquifer layers are existed in each site. While Gumi site has 10 m water level and 2.5 m and 4.6 m of sandy gravel aquifer. Therefore, it is expected that 1,000m³/day of water could be withdrawn at Jinju site rather than Gumi site.

Objectives This study was performed to evaluate the clinical effect of acupuncture on children's myopia. Methods Among the outpatients with myopia who visited to Department of Acupuncture & Moxibustion, National Medical Center from June 2000 to May 2001, we selected 50 cases that did not involve ,any other pathological condition of eyes and patient age was below 17, whose eyes including with glasses or naked examined by monoocular and biocular visual acuity test per each visit and followed up for 6 weeks. Results We investigated 29 female and 21 male patients. The most distribution of age was 12.26. First mean biocular visual acutity was $0.2{\pm}0.074$. Last mean biocular visual acuity after 6 weeks was $0.8{\pm}0.046(p<0.001). Improved cases on biocular test were 38 cases($76.00\%$). Conclusions These results suggest that the acupuncture therapy is effective treatment modality on children's myopia.

To investigate the effect of protein klnase on melanin production via CAMP-dependent pathway, we measured the melanin amount and tyrosinase activity in Bl6 melanoma cells stimulated by alpha-melanocyte stimulating hormone (MSH), forskolin and 8-Br-Camp MSH, forskolin and 8-Br-Camp significantly increased both melanin production and tyrosinase activity in Bl6 cells. Melanin production and tyrosinase activity by MSH are significantly inhibited by cyclic AMP-dependent protein kinase inhibitor (KT5720) and protein kinase C down-regulation treated with PMA. Bisindolmaleimide (1μM), protein kinase C inhibitor, significantly inhibited melanin production and tyrosinase activity stimulated by MSH, for-skolin and 8-Br-Camp with the following order of potency: MSH>forskolin>8-Br-Camp. Tyrosine kinase inhibitor, genistein and DHC, significantly inhibited both, but the inhibitory effect was more potent in 8-Br-Camp-stimulated Bl6 cells than MSH-stimulated cells. NFkB inhibitor (parthenolide) significantly inhibited melanin production and tyrosinase activity. Neither melanin production nor tyrosinase activity induced by MSH, forskolin and 8-Br-Camp were affected by KN-62 (calm-odulin-dependent protein kinase II inhibitor), PD098059 (mitogen-activated protein kinase inhibitor, MAPKK) and worthmannin (phosphatidylinositol 3-kinase inhibitor). These results suggest that both protein klnase C and tyrosine kinase are involved in melanin production by cyclic AMP-dependent pathway and NFrB pathway may play an important role in cyclic AMP-dependent melanin production in Bl6 melanoma cells.

This study is performed to investigate the effects of citrus essential oils on melanin production in Bl6 melanoma cells. Five kinds of citrus essential oils (Bergamot, Grapefruit, Lemon, Mandarin, Petigrain) did not have any influence on DPPH radical scavenger activity cell growth and cytotoxicity in Bl6 melanoma cells. Both mandarin and petigrain essential oils dose-dependently inhibited purified tyrosinase activity, but bergamot did not. In 1μM MSH-stimulated Bl6 melanoma cells, all of 5 citrus essential oils inhibited melanin production in a dose dependent manner. From the above results, it is possible that citrus essential oils may be development to be an anti-melanogenesis agent on the basis of their inhibitory effect on MSH-induced melanin production.
