Estimated incidence of round ligament varicosities in pregnancy is not known and often times it is confused with inguinal hernia due to their clinical similarities. When a patient is presented with inguinal mass especially in association with varicosity in the genital region or lower extremity, round ligament varicosity must be considered as a plausible diagnosis. Depiction of bag of worms on color Doppler ultrasonography is diagnostic of the round ligament varicosity and it is known to resolve spontaneously following delivery. We report a case of round ligament varicosities that was diagnosed at 29 weeks of gestation with a brief review of the literatures.

목적 기존의 복강경기구들을 이용하여 2-공법을 이용한 복강경하 자궁절제술을 기존의 4개의 포트를 이용한 고식적인 방법과 비교하여 임상적인 효용성을 확인하고자 하였다. 연구방법 2011년 3월부터 2012년 2월까지 복강경하 자궁절제술을 시행받은 환자를 수술방법에 따라 두 군으로 나누어 후향적으로 의무기록을 분석하여 비교하였다. 결과 총 68명의 환자 중 2-공법 복강경하 자궁절제술을 시행받은 환자는 51.5% (35명), 기존의 고식적 복강경하 자궁절제술을 시행받은 환자는 48.5%(33명)였다. 2-공법군(318.38 ±137.73 g)에서 제거된 자궁의 무게는 고식적 복강경수술군(338.06 ±148.44 g)에 비해서는 가벼웠으나 통계학적으로 유의하지 않았다( P = 0.572). 수술 중 출혈량은 2-공법군, 기존 복강경수술군 각각 314.0 ± 227.18 mL, 293.64±282.35 mL로 2-공법군은 기존 복강경수술군과 출혈량은 차이를 보이지 않았다( P = 0.744). 평균 수술시간은 2-공법군은 194.94±46.08분, 기존 복강경수술군은 195.36±54.28분이었다(P = 0.973). 2-공법군의 평균 재원일수는 5.69 ±1.1일로 기존 복강경수술군의 5.85±0.8일과는 유의한 차이가 없었다( P = 0.476). 수술 직후 통증 정도는 2-공법군, 기존 복강경수술군 각각 4.49 ±1.56 점, 4.55 ±2.03점이었다( P = 0.892). 결론 2-공법 복강경하 자궁절제술은 수술시간, 수술 시 출혈량, 제거된 자궁의 무게, 수술 후 통증 정도 및 재원일수 등 수술 후 경과에 있어 기존의 복강경술과 견줄 수 있어 특히 단일통로로 접근하기 어려운 경우에 최소침습수술 방법으로 유용할 것으로 생각된다. Objective To compare two-port laparoscopic hysterectomy to conventional hysterectomy. Methods We retrospectively analyzed medical records of patients undergoing hysterectomy between March 2011 and February 2012. Patients were stratified into two groups by surgical approach. Clinical features and surgical outcomes were compared. Results Of a total of 68 laparoscopic hysterectomy, 35 (51.5%) were two-port laparoscopy, 33 (48.5%) were conventional laparoscopy. The weight of uterus were heavier in the conventional laparoscopic hysterectomy group (338.06±148.44 g) than in the two-port laparoscopic (318.38 ±137.73 g) group but there were no significant differences ( P=0.572). Estimate blood loss was reported in the two-port and conventional laparoscopic group with blood loss of 314.0 ± 227.18 mL, 293.64±282.35 mL ( P=0.744). Actual surgical time was 194.94 ± 46.08 minutes in the two-port, 195.36 ± 54.28 minutes in the conventional (P=0.973). Patients in the two-port group had similar length of hospital stay of 5.69 ±1.1 days as compared to 5.85 ± 0.8 days in the conventional group ( P=0.476) but there were no significant differences. Immediate postoperative pain by using a visual analogue scale was 4.49 ±1.56 point in the two-port group, 4.55 ± 2.03 point in the conventional laparoscopic group ( P=0.892). Conclusion Two-port group is a feasible method with comparable operative outcomes including operative time, blood loss, weight of uterus, postoperative pain, and hospital stay when compared with the conventional laparoscopic hysterectomy. Therefore two-port laparoscopic hysterectomy may replace the role of conventional laparoscopic hysterectomy.
