        그레이브스병 환자에서 항갑상선제가 지질과산화에 미치는 영향

        N/A Objectives: The development of a significant increase in metabolic rate and total oxygen consumption leads to an enhancement in oxygen stress, which results in an increased lipid peroxidation. The study was undertaken to evaluate: 1) the lipid peroxidative stress of subjects with hyperthyroidism related to changes in metabolic activity, 2) the influence of the antithyroid agent, propylthiouracil on the oxidative stress status of Graves disease patients. Methods: We studied thyroid function indices(T3, free T4 TSH) and lipid peroxidation in Graves disease patients, before and after treatment with propylthiouracil for three months. Results: It is revealed that red blood cell membrane lipid peroxidation is significantly correlated with thyroid function indices(T3 r=0.676, p<0.01, free T4 r=0.683, <0.05), before PTU treatment and lipid peroxidation is diminished in parallel with attainment of euthyroid state of thyroid activity after propylthiouracil administration, after PTU treatment. Conclusion: It is shown that lipid peroxidation that is attributed to oxygen consumption and oxidative stress are significantly correlated with the state of thyroid function in Graves' disease patients.

        뇌졸중 환자의 균형능력과 타이밍 기능평가의 신뢰도와 타당도

        본 연구의 목적은 뇌졸중 환자를 대상으로 청각적 피드백을 이용한 타이밍-기능적 움직임 능력과 균형 능력 사이의 상관관계를 알아보고 이를 통해 타이밍-기능적 움직임 평가도구 개발의 기초자료를 제공하고자 하였다. 뇌졸중 환자 30명을 대상으로 균형능력을 알아보기 위해 Function in sitting test, Berg balance scale, modified Postural assessment scale for stroke, 10 Meter walk test, Timed up & go를 실시하였고, 청지각 능력 평가와 균형능력 카테고리에 따른 네가지 타이밍-기능적 움직임 측정 방법을 실시하였다. 타이밍-기능적 움직임의 검사-재검사 신뢰도는 급간 내 상관계수(ICCs)를 이용하였다. 측정 변수들 간의 선형적인 연관성을 알아보기 위해 피어슨 상관계수를 나타내었다. 절대적 신뢰도는 측정오차의 정량화를 위해 표준 오차 측정(standard error of measurement; SEM)을 이용하였다. 연구결과 FIST점수와 벽면에 손 짚기는 유의한 음의 상관관계를 보였다(r=-.693, r=-.465, r=-.364). BBS점수와 발판에 발 올리기는 유의한 음의 상관관계를 보였다(r=-.511, r=-.492). mPASS점수와 발판에 발 올리기는 유의한 음의 상관관계를 보였다(r=-.463, r=-.500). 10MWT 값과 한 발짝 내딛기는 유의한 양의 상관관계를 보였다(r=.376, r=.369). TUG 값과 한 발짝 내딛기는 유의한 양의 상관관계를 보였다(r=.437, r=.471, r=.379). 네 가지 측정 방법 모두 ICC=.986 이상으로 높은 검사-재검사 신뢰도를 보였다. 버튼 누르기 방법과 벽 짚기 방법의 SEM은 측정한 평균값의 10%보다 작아 신뢰할 만한 수준이었다. 연구결과를 통해 세 가지 측정 방법 모두 균형능력과 높은 연관성이 있어 타이밍-기능 평가 도구로 활용이 가능 할 것이다. The previous studies have shown that the use of auditory feedback to improve balance ability and the improvement of upper limb ability of stroke patients through Rhythmic auditory stimulation using Interactivity metronome have been actively studied. However, there are insufficient studies to evaluate the patient's balance ability including control of lower extremity and trunk. Therefore, the purpose of this study is to provide basic data for the development of Timing-functional movement test tool to evaluate the balance ability of stroke patients. The design of this study was a cross sectional design, involving 30 patients with stroke. Timing-functional movement test-retest reliability was measured by intraclass correlation coefficients (ICCs) and Kappa values. The Pearson correlation coefficient was used to examine the linear relationship between measured variables. Absolute reliability was measured by standard error of measurement(SEM). Timing-functional movement test methods showed high test-retest reliability with ICC=.986 or better. The Function in sitting test and Wall touch was significantly negative correlated(r=-.693, r=-.465, r=-.364). The Berg balance scale and Step on the stool was significantly negative correlated(r=-.511, r=-.492). The modified Postural assessment scale for stroke and Step on the stool was significantly negative correlated(r=-.463, r=-.500). The 10 Meter walk test and One step in front was significantly positive correlated(r=.376, r=.369). The Timed up & go and One step in front was significantly positive correlated(r=.437, r=.471, r=.379). In this study, we investigated the clinical usefulness of Timing-functional movement using auditory feedback in evaluating the balance ability of stroke patients and found that it is useful for evaluating the balance ability of stroke patients with high reliability and validity.

      • 요통환자에 대한 새로운 표면 근전도 방법의 신뢰도와 타당도

        요통환자의 공포-회피 반응검사(Fear Avoidance Belief Questionnaire; FABQ)와 요통 기능 장애 수준 검사(Oswestry Disability Index; ODI)를 통한 평가는 신체 기능장애 정도와 예후를 판단하는데 중요한 요인이다. 표면 근전도는 근 긴장도를 평가할 수 있는 도구로 카이로프랙터나 물리치료사가 환자의 진단 도구로 사용하고 있다. 이에 본 연구에서는 새로운 조이스틱형 표면 근전도 방법을 통한 심리사회적 수준 평가의 유용성을 검정하기 위해 장비의 검사-재검사 신뢰도와 타당도를 확인하고자 하였다. 본 연구를 통해 새로운 표면 근전도 방법은 요통환자의 요부 근활성도를 측정하는데 높은 신뢰수준을 확인할 수 있었으며 평가 변수들과의 유의한 상관관계를 보여 요통 환자가 겪는 심리사회적 문제를 평가하는데 타당도 높은 평가도구라 할 수 있겠다.

        앉은 자세에서 방석센서를 이용한 요통환자 골반가동성 측정의 신뢰도와 타당도

        PURPOSE: Postural and structural asymmetry due to muscle imbalances around the lower back and pelvis are the causes of back pain. Muscle imbalances in patients with chronic low back pain affect the pelvic tilt and movement, and it is necessary to assess the pelvic movement ability using the appropriate tools to determine the mediating effects of lower back pain. This paper reports the reliability and validity of the Sensbalance Therapy Cushion (STC) for pelvic movement and proprioception. METHODS: In this study, the Wii balance board (WBB) was used as a golden standard for pelvic movement measurements. FABQ, KODI, Myovision, and Pelvic movement were measured in 50 patients with chronic low back pain. The correlation between the lower-back muscle activity and pelvic movement was checked. The pelvic movement parameter was measured twice to determine the intra-rater reliability. RESULTS: The STC showed high test-retest reliability in the pelvic tilt measurements (ICC = .672 - .809). The testretest reliability of proprioception measurements (ICC = .588 - .859) and reaction time measurements (ICC = .542 - .836) were also high. The relationship between the WBB and STC showed a significant positive correlation with the pelvic tilt test (p < .01). The posterior pelvic tilt and lower-back muscle activity showed a significant negative correlation (p < .01). The pelvic left tilt and lower-back muscle activity showed a significant negative correlation (p < .05). CONCLUSION: The results revealed the high reliability and validity of the STC. Therefore, the STC can be used as an objective measuring device for evaluating pelvic tilt, proprioception, and reaction time in low back pain patients.

        뒤넙다리근 능동과 수동신장 운동이 요통 환자의 골반 가동성에 미치는 영향

        PURPOSE: Hamstring muscle shortening is related to low back pain, and it is important to check the hamstring muscle flexibility and pelvic mobility to analyze the effects of the intervention. This study examined the effects of hamstring muscle shortening on flexibility and pelvic mobility according to the method of stretching the hamstring muscle in patients with low back pain. METHODS: Forty Low Back Pain Patients participated. The subjects performed the Visual Analogue Scale (VAS), Fear Avoidance Belief Questionnaire (FABQ), Korean version of Oswestry Disability Index (KODI), Myovision, and sensbalance therapy cushion (pelvic mobility, proprioception). The subjects were divided into two groups to perform the passive and active stretching protocol. The intervention was conducted three times a week for four weeks. RESULTS: Hamstring muscle flexibility was increased significantly in both groups (p < .05), and there was no difference between the groups. VAS, FABQ was decreased significantly in both groups (p < .01), and there was no difference between the groups. The left lower back muscle impedance was decreased significantly in both groups (p < .05). The passive stretching group showed a significant increase in the pelvic anterior, posterior, and left tilt ROM (p < .05). The active stretching group showed a significant increase in the pelvic anterior, posterior tilt ROM (p < .05). CONCLUSION: Both stretching methods may be useful intervention methods for pelvic mobility and pain recovery and can assess back pain recovery.

        만성 뇌졸중 환자들의 낙상 예측을 위한 STS, TUG, FRT, MSL, BBS의 변별력과 예측 타당도

        본 연구의 목적은 만성 뇌졸중 환자를 대상으로 균형 능력 검사인 버그 균형 척도(BBS), 앉고 일어서기 동작 검사(STS), 일어나 걸어가기 검사(TUG), 기능적 팔 뻗기 검사(FRT)와 보호균형 반응능력 검사인 최대보장검사(MSL)를 이용하여 낙상의 위험성을 예측하거나 낙상을 예측하는데 변별력이 있는지 알아보고자 하였다. 만성기 뇌졸중 환자 61명을 대상으로 뇌졸중 후 연구시점까지 낙상 횟수를 기록하였다. 수용자 작업특성곡선(ROC curve)을 이용하여 측정 변수들의 낙상 예측 선별 기준값(cut-off value)을 구하였고 진단의 정확도를 알아보기 위하여 양ㆍ음성 예측도(PPVㆍNPV)를 구하였다. 측정 변수들간 연관성을 알아보기 위해 피어슨 상관계수를 나타내었고 변수들의 낙상 예측 선별 기준값은 낙상에 대한 위험정도를 알아보기 위하여 로지스틱 회귀분석을 통한 교차비를 구하였다. 연구 결과 STS의 선별 기준값은 15.88초(AUC=.77, 민감도=87%, 특이도=59%), TUG의 선별 기준값은 13.92초(AUC=.74, 민감도=80%, 특이도=72%), MSL의 선별 기준값은 32.57cm(AUC=.77, 민감도=80%, 특이도 =65%), FRT의 선별 기준값은 17.97cm(AUC=.79, 민감도=93%, 특이도=67%), BBS의 선별 기준값은 24점(AUC=.70, 민감도=60%, 특이도=91%)으로 수용할 만 하였다. 로지스틱 회기분석에서 MSL, FRT는 낙상을 예측할 수 있는 변수인 것이 확인되었고 각각 32.57cm 이하, 17.97cm 이하에 해당하는 피검자들은 낙상을 경험할 확률이 각각 1.098배, 1.236배 증가하는 것으로 확인이 되어 만성 뇌졸중 환자들의 낙상을 예측하는데 효과적인 평가도구로 활용이 가능 할 것이다. 추후 연구에서는 보행 지구력과 보행 속도, 피검자의 신체조건(신장, 하지 길이)을 고려하고, 최대보장 길이의 다방향 측정과 마비측에 따른 최대보장 검사 시 내딛는 다리의 기준을 제시하는 등 다양한 변인이 포함된 연구가 이루어져야 할 것이다. The purpose of this study was to evaluate the risk of falling or prediction validity for predictor of falls in patients with chronic stroke using the berg balance scale (BBS), sit to stand test (STS), timed up & go (TUG), functional reach test (FRT), and maximal step length (MSL). This study were included 61 patients with chronic stroke. 15 patients had once or no experience of falling, and 46 patients had experienced at least two falls. The receiver operation characteristic (ROC) curve was conducted to detect cut-off value of variables. The predictive value of measurement variables for accurately prediction of falls. The pearson`s correlation coefficients were calculated to determine the strength of the relationships between variables and Odd ratio was calculated by logistic regression analysis in order to predict the degree of risk falls and non-falls. The selection standard value (AUC, sensitivity, specificity) for risk of falls of STS was 15.88s (.77, 87%, 59%), TUG was 13.92s (.74, 80%, 72%), MSL was 32.57cm (.77, 80%, 65%), FRT was 17.97cm (.79, 93%, 67%) and BBS was 24 score (.70, 60%, 91%) which were in the acceptable range. Adjusted bivariate logistic regression found MSL (p=.008) and FRT (p=.008) to be predictors for falls. The patients with chronic stroke who were MSL >32.57cm, FRT >17.97cm were 1.10 times, 1.29 times more likely to experience falls. We also recommended further study of considering walking endurance, walking speed, body condition(height, led length) of the subject and include a multidirectional measurement of the maximum step length and suggesting the leg criteria for the maximal step length on the consider paralyzed side.

        Effect The effect of sintering additives ($SiO_2$, $Al_2O_3$, Clay) on the mechanical characteristics of sintered zircon was investigated. 1 vol% of additives in zircon powder was was sintered at $120{\sim}1500^{\circ}C$, the mechanical characteristics were measured, and microstructure analysis were was conducted. $Al_2O_3$ and clay additions increase the formation of monoclinic and tetragonal-$ZrO_2$ formation. An addition of SiO2 addition suppressed the formation of tetragonal-$ZrO_2$ formation., The A specimen sintered at $1400^{\circ}C$ showed the a density of $4.05\;g/cm^3$ and the a microhardness of 1120 HV, respectively.

        구조방정식 모델을 이용한 농업 마이스터 대학 교육프로그램 교육효과성 연구

        This study was designed to identify factors affecting the educational effect of Agriculture Meister College education programs with the sample of Agriculture Meister College students. The study set up a general structural model which included accessibility, self-efficacy, and social norm as exogenous variables and perceived easiness, perceived usefulness, attitude, and educational effect as endogenous variables. The result proved the model to be a good enough to understand the educational effect of Agricultural Meister Program based on the goodness fit statistical results. According to the results of parameter estimates and significance, easiness and usefulness among the endogenous variables were identified having direct effects on the educational effect, key endogenous variable. Meanwhile, only accessibility was found having direct effect on the educational effect. In addition, accessibility and self-efficacy were determined having indirect effects on the educational effect through easiness as an intervening variable. Further, self-efficacy and social norm were identified having indirect effects on the educational effect via usefulness. To sum up these results, it is suggested that easiness and usefulness are important variables compared to attitude as endogenous variables in the model.
