Although rare in childhood, gastric volvulus and wandering spleen share a common etiology, congenital absense of interperitoneal visceral attachments. Wandering spleen and gastric volvulus were diagnosed in a 5-year-old girl who presented with acute upper abdominal pain, abdominal distension and non-bilious vomiting. Intraoperatively, normal ligamentous connections between the stomach, spleen, and posterior abdominal wall were not found. Partial gastrectomy and gastropexy were performed. We experienced a case of acute mesenteroaxial gastric volvulus with wandering spleen. Therefore, we report this case with a brief review of literature. (Korean J Gastroenterol 2001;38:366-370)
고분자를 이용한 Adefovir Dipivoxil과 Suberic Acid의 공결정 제어

고분자가 adefovir dipivoxil(AD)과 suberic acid(SUB)의 공결정 형성에 미치는 영향을 연구하였다. 고분자 첨가제는 poly(ethylene glycol)(PEG)과 poly(acrylic acid)(PAA)를 이용하였다. 용액에서 AD와 SUB를 혼합한 후에 고분자를 첨가하여 공결정을 제조했을 때는, 고분자의 영향은 결정모양과 결정성을 일부 변화시키는 것에 한정되었는데, 이러한 영향은 고분자 없이도 용액에 과량의 SUB를 이용하거나 용매첨가 분쇄법으로 유도할 수 있었다. 한편,용액에서 AD와 고분자를 혼합한 후에 SUB를 첨가하여 공결정을 제조했을 때는, PEG의 경우 공결정의 변화를 훨씬 효과적으로 유도하였는데 결정성을 기준으로 같은 농도에서 3배 이상의 효과를 보였다. 또한 PAA의 경우에는결정의 형성을 완전히 억제하는 효과를 보였다. 본 연구의 결과는 공결정 형성에 미치는 고분자의 영향을 단순히혼합순서를 바꾸어서 조율할 수 있는 예를 보여 주었다. The effects of polymers on the cocrystallization of adefovir dipivoxil (AD) and suberic acid (SUB) were investigated. The polymeric additives in the present study were poly(ethylene glycol) (PEG) and poly(acrylic acid) (PAA). When the polymers were added to the solution of AD and SUB, their effects were limited to the morphology and crystallinity of the AD/SUB cocrystal, which could be also achieved without polymeric additives by the excess amount of SUB in the solution or through the solvent-assisted grinding. When the polymers were mixed with AD before adding SUB in the solution, PEG was dramatically more effective at the same amount with possible alteration of the cocrystal structure. Also, PAA completely inhibited the formation of crystals. The present study demonstrated that the effects of polymers on the cocrystallization could be tuned by simply modifying the mixing strategy.
유병기간에 따른 요통환자의 Ferguson 각과 요추중력중심선의 통계적 관찰

Objectives : The lumbosacral joint is unstable area from an anatomical viewpoint, while it is also a very mobile area in ordinary life, so that clinically major causes of low back pain originate in this joint. The purpose of this study is to assess the difference of the Ferguson angles and Lumbar gravity lines between the acute and chronic among lumbago patients. Methods : Ferguson angle and Lumbar gravity line were measured and evaluated in 81 lumbago patients in the standing position. Results and Conclusions : 1. Ferguson angle is decreased and Lumbar gravity line is posterior shift in acute stage of lumbago. 2. Ferguson angle is increased and Lumbar gravity line is anterior shift in chronic stage of lumbago. 3. Aging is relative with anterior shifting of lumbar gravity line. 4. Increasing of Ferguson angle is relative with anterior shifting of lumbar gravity line. 5. The instability of lumbasacral joint is increased in cases of chronic stage of lumbago and the weight bearing force is increased in cases of acute stage of lumbago.
양심탕가미방(養心湯加味方)을 기본으로 한 한방치료로 호전된 심인성 어지럼증 치험 3례

Objectives: To propose Korean medicine treatments based on effects of Yanshimtang-Gamibang on three patients diagnosed with psychogenic dizziness. Methods: Korean medicine treatments based on Yanshimtang-Gamibang were applied to three patients diagnosed with psychogenic dizziness after experiencing a traffic accident. Effects of Korean medicine treatments based on Yanshimtang-Gamibang were evaluated using DHI (Dizziness Handicap Inventory), BAI (Beck Anxiety Inventory), NRS (Numerical rating scale), NDI (Neck Disability Index), ODI (Oswestry Disability Index), and EQ-5D (EuroQol-Five Dimensions). Quality of sleep was assessed with total sleep time and number of times waking up during sleep. Results: After the treatment, there were significant decreases in DHI and BAI scores as well as values of NRS, NDI, ODI, EQ-5D. The sleep quality of three patients also showed improvement. Conclusions: Korean medicine treatments based on Yanshimtang-Gamibang might be effective in people with psychogenic dizziness.
간장에서 분리한 Bacillus licheniformis가 생산하는 박테리오신의 특성 및 정제

수집한 간장시료에서 형태학적인 특성에 따라 상이한 30주의 미생물을 분리하고 MRS 액체배지에서 37℃, 24시간 배양한 후 회수한 배양 상등액 중 paper disc법으로 항균활성이 있는 것을 일차 선별하여 proteinase K 처리에 의해 항균활성이 사라지는 시료 하나를 최종적으로 선별하였다. 선별한 분리주를 생화학적 분류와 분자계통학적 분류를 통해 동정한 결과 B. licheniformis로 나타났다. 이 균주의 생장온도와 배지의 초기 pH에 따른 세포생장 및 박테리오신 활성을 조사한 결과 배양온도는 37℃, 배지의 초기 pH는 7.0에서 박테리오신의 활성이 가장 높게 나타났다. B. licheniformis가 생산하는 박테리오신은 Bacillus sphaericus, Lactobacillus bulgaricus, Lactobacillus plantarum, Micrococcus luteus, Paenibacillus polymyxa 및 Pediococcus dextrinicus 등에 대해서 항균활성을 보였으며, pH 3.0~11.0에 이르는 거의 전 pH 영역에서 20시간 이상 처리하여도 그 항균활성을 완전히 잃지 않아 비교적 넓은 pH 범위 내에서 안정함을 알 수 있었다. Acetone, acetonitrile, chloroform, ethanol 처리 및 20~100℃에서 60분간 가열시에도 높은 항균활성을 유지하였다. 여러 가수분해효소에 대한 내성을 조사한 결과 trypsin, α-chymotrypsin, pepsin, α-amylase 및 carboxypeptidase A 등은 항균활성에 영향을 주지 않았으나 lipase는 항균활성을 약간 감소시켰으며 proteinase K는 항균활성을 완전히 사라지게 하였다. 75% 황산암모늄 침전, 양이온교환크로마토그래피, 역상 HPLC 등의 과정을 통해 정제된 박테리오신의 상대비활성(specific activity)은 배양상등액에서 보다 약 75배 증가하였고 회수율은 13.5%였다. 역상 HPLC를 통해서 정제된 박테리오신의 분자량을 tricine SDS-PAGE을 통해서 확인한 결과 약 2.5 kDa으로 나타났으며 염색 시 단일 band로 나타나 순수하게 정제되었음을 확인하였다. A bacteriocin-producing bacterium identified as Bacillus licheniformis was isolated from soybean sauce. Antibacterial activity was confirmed by paper disc diffusion method, using Micrococcus luteus as a test organism. The bacteriocin also showed antibacterial activities against Bacillus sphaericus, Lactobacillus bulgaricus, Lactobacillus plantarum, Paenibacillus polymyxa, and Pediococcus dextrinicus. Optimal culture conditions for the production of bacteriocin was attained by growing the cells in an MRS medium at a pH of 6.5~7.0 and a temperature of 37℃ for 36~48 hr. Solvents such as chloroform, ethanol, acetone, and acetonitrile had little effect on bacteriocin activity. However, about 50% of bacteriocin activity diminished with treatment of methanol and isopropanol at the final concentration of 50% at 25℃ for 1 hr. It was stable against a pH variation range from 3.0 and 7.0, but the activity reduced to 50% at a pH range from 9.0 to 11.0. It's activity was not affected by heat treatment at 100oC for 30 min and 50% of activity was retained after heat treatment at 100℃ for 60 min, showing high thermostability. The bacteriocin was purified to a homogeneity through ammonium sulfate precipitation, SP-Sepharose ion-exchange chromatography, and reverse-phase high-performance liquid chromatography (HPLC). The entire purification protocol led to a 75-fold increase in specific activity and a 13.5% yield of bacteriocin activity. The molecular weight of purified bacteriocin was estimated to be about 2.5 kDa by tricine-SDS-PAGE.
