소아 상완골 외과 골절의 수술적 치료에 대한 임상적 고찰

소아 주관절 골절 중 비교적 드문 손상인 상완골 외과 골절의 수술적 치료 후 6개월 이상 추시한 26례를 분석, 문헌 고찰과 함께 보고하는 바이다. Lateral condyle fractures of the humerus is common in children, and it may be one of the most difficult fracture to treat, and has many controversy in treatment method. To obtain good result, anatomical reduction and firm fixation is obligatory. We analysed the 26 cases of lateral condyle fractures of the humerus in children that could be followed up at Paik hospital, Inje University, Seoul from March 1983 to March 1988. 1.Male predominate in a ratio of above 5 : 1. 2.The age ranged from 2 to 12 years, and its mean was 6.3 years. 3.The causes of the injury were falling down in 14 cases, slipping down in 11 cases, and traffic accident in only one case. 4.16 cases (61.5%) were Milch type 1 and 10 cases (38.5%) were Milch type 2. 5.We could not find the change of carrying angle during the follow up period in all cases. 6.The range of motion were nearly full in all cases. 7.There were 2 cases of the superficial infection in the K- wire inserted site. 8.We thought that early anatomical reduction and internal fixation with K- wire will promise good functional results.

Purpose: We wanted to investigate the radiographic and clinical results of internal fixation with cannulated screws in elderly patients with femoral neck fracture. Materials and methods: We reviewed the results of 47 cases of elderly patients with femoral neck fractures that were treated by osteosynthesis from May 2000 and March 2009 and these patients could be followed up for more than one year. There were 11 males and 36 females with a median age of 72 years (65~85 years). They were classified by the Garden stage. The number of stage I and II fractures was 34 and 16, respectively. The postoperative follow up period was 24 months (12~84 months). The union time, horizontal shortening and complications were investigated. Walking ability was evaluated by Koval`s scoring system and the quality of life was measured by the Korean EQ-5D. Results: The average union time was 4.4 months. The complications were one case of nonunion, 6 cases of avascular necrosis and one case of subtrochanteric fracture. The average of the horizontal shortening was 6.815 mm (0~20 mm) in the fracture site. The walking ability was on average 1 step down and it was possible to walk independently using a walker for 69% of the patients. The Korean EQ-5D quality of life was reduced from 0.856 to 0.561 (P <0.01). Conclusion: In elderly patients with femur neck fracture, the decreased abductor moment arm may reduce the quality of life and walking ability after cannulated screw fixation.
디메틸포름아미드 취급 사업장의 작업환경 및 보건관리 실태와 노출요인 조사

This study was aimed to assess the status of working environment, health care management status and cause of exposure in manufactories using dimethylformamide (DMF). For the purpose, airborne concentration of DMF in the workplaces and N-methylformamide (NMF) in worker`s urine were measured with job type and process. In addition, management of local exhaust ventilation system (LEV) and personal protective equipment (PPE) was evaluated at 35 work places (107 workers) located in Busan and Gimhae area. Mean DMF concentrations in work places by job type and process were of high level measured in printing and record media reproduction (5.23ppm) and flaking process (2.48ppm). Workers in adhesive job were measured a large amount of urine NMF (21.59mg/ℓ). 98.1% of DMF handling workers were provided respirators, but 67.3% of those workers used them. The main reasons for not using respirators were inconvenience and difficulty of breathing. Airborne concentrations of DMF were higher in the workplaces in which LEVs were working abnormally, but there was not statistically significant. In addition, the urine NMF levels were correlated with management of LEV within the workers who did not use the respirators (p<0.048). These results implied that LEV should be installed and maintained properly to protect the workers from the exposure to DMF. Management of PPE should be also necessary to protect the workers from chemical hazards.
