증례 : 혈액종양 ; 전립선암에서 발생한 수막암종증 1예

수막암종증은 암세포가 뇌수막에 침윤한 상태로 정의하는데 자기공명영상이 진단방법으로 쓰이기 전까지 진단이 용이하지 않았다. 또한 전립선암 환자에서 수막암종증이 생긴 경우는 매우 희귀하여 증례보고 형식으로 소수에서 보고 되고 있는 실정이다. 내원 11년 전 전립선암으로 근치적 전립선 절제술을 받고 내원 4년 전 골전이로 항암치료를 받은 병력이 있는 환자로 경련을 주소로 내원하여 시행한 반복적인 뇌척수액 검사에서 세포진을 발견하지 못했지만 뇌척수 액 전립선 특이항원 양성소견과 뇌 자기공명영상 소견으로 수막암종증을 진단하고 조직검사를 통해 확진하여 적극적인 치료로 생명연장을 할 수 있었던 사례를 경험하였기에 문헌 고찰과 함께 보고한다. Meningeal carcinomatosis in patients with prostate cancer is very rare. Recently, we experienced a case of meningeal carcinomatosis in a patient with prostate cancer. He had undergone a radical prostatectomy 11 years before presentation and had undergone chemotherapy 4 years before presentation due to bone metastases. This time, he visited the emergency department because of a seizure. Although cerebrospinal fluid (CSF) examinations did not reveal malignant cells, we diagnosed meningeal carcinomatosis based on positive CSF prostate-specific antigen and magnetic resonance imaging (MRI) findings. The diagnosis was confirmed pathologically. We treated the patient with whole brain radiotherapy and intrathecal chemotherapy. We report a case of prostate cancer with meningeal carcinomatosis with a literature review. (Korean J Med 2011;81:262-265)

옛이야기 속 인물의 옷이 삶에 미치는 상징적 의미 연구

In this study, the origin and function of clothes, and the meaning of the symbolism of clothes in folk tales were investigated. Clothing has played a role in protecting people and expressing psychological stability and affiliation. In the early days, they wore animal fur as it was, then gradually processed it, spun yarn, and made cloth for In fairy tales, the unconscious, clothes. meeting clothes the and enter meant integrating with persona and anima animus, division and boundary, and protection. The characters in the folk tales went down to the unconscious through an effort to recover the clothes they lost, and sometimes they took off the overlaid persona or built the necessary persona. The clothes distinguish the outside from the self and the shadows, so that the characters in the folk tales meet the anima animus become and individuation. The results of this study shows that the symbolic meaning of the clothes in the folk tales accompanies the character's psychological journey and acts as a helper so that the character can live their life well. 본 연구는 옛이야기 속에 나타난 옷의 상징성이 가지는 의미를 알아보았다. 옷은 사 람들을 보호하고, 심리적 안정감이나 소속 지위 등을 표현하는 역할을 해 왔다. 초기에 는 동물의 털가죽을 그대로 사용해 입다가 점점 가공하고 실을 잣고 옷감을 만들어 옷 을 해 입었다. 옛이야기 속에서 옷은 무의식으로 내려감, 페르소나, 아니마⦁아니무스와 의 만남과 통합, 구분과 경계, 보호를 의미하였다. 옛이야기 속 인물은 잃어버렸던 옷을 되찾는 노력을 통해 무의식으로의 내려가기도 하고, 덧씌워진 페르소나를 벗겨 내거나 필요한 페르소나를 구축하기도 하였다. 옷은 외부와 자신을 구분 짓고 그림자를 구분하 여 이야기 속의 인물이 아니마⦁아니무스를 만나 개성화 과정을 가는 것을 표현하였다. 연구 결과 옛이야기 속의 옷의 상징적 의미는 인물의 심리적 여정에 동반하여 인물이 자신의 삶을 잘 살아갈 수 있도록 조력자 같은 역할을 하고 있다는 것을 알 수 있었다.
