        CT 영상을 이용한 대퇴체부 휨의 3차원적 곡률 분석

        류마티스성 관절염 등의 관절 질환이나 내반 변형 등을 동반한 환자에게 정형외과적으로 인공슬관절 치환술을 시행하는데 수술용 금속정을 삽입할 때 대퇴체부의 휨 정도로 인해 대퇴피질부에 손상을 주거나 2차 골절 등의 부작용을 유발하기도 한다. 이때 대퇴체부의 중심 좌표값을 알면 금속정이 해부학적 모양의 중심으로 삽입될 수 있어 이러한 부작용을 예방할 수 있을 것이다. 본 연구에서는 인공슬관절 치환술이 필요한 10명의 환자와 대퇴골 질환이 없는 정상군 10명의 대퇴부 단층촬영영상을 이용하여 3차원 좌표값을 찾은 후 Matlab을 이용하여 중심좌표값을 찾아 해부학적 모양을 반영한 그래프를 얻었고, 구간별 3차원 곡률반경 값을 구하였다. 그 결과 실험군의 대퇴골 평균 곡률반경과 표준 편차는 758.15±206.3 mm로 측정되었고, 정상군은 1672.97±395.6 mm로 측정되었다. 측정된 결과는 통계적 유의성 확보를 위해 f-검정을 실행하여 두 집단간 평균의 차이를 검증하였다. 이 결과로 미루어 실험군의 대퇴체부 휨 정도가 더 심함을 확일할 수 있었으며 이러한 영상 분석방법을 이용하여 해부학적 중심점을 찾는다면 인공슬관절 치환술 같은 정형외과적 수술에 도움이 될 것이라 사료된다. For some patients with joint illnesses such as rheumarthritis or varus deformity, the total knee arthroplasty (TKA) procedures are performed. However, when inserting metal cutting guide for the procedures, due to the femoral shaft bowing, complications such as the cortex of the femoral shaft damages or secondary fractures can be caused. If the central coordinate value of the femoral shaft is known, the metal cutting guide could be inserted into the anatomical center, so such complications can be prevented. In this study, CT images of femoral shafts of 10 individuals in the experiment group who are in need of receiving the total knee arthroplasty procedures and those of 10 individuals in the control group without illness in the femoral shaft have been utilized to locate the 3-dimensional coordinate values. Then, Matlab was utilized to identify the central coordinate value in order to obtain a graph reflecting the anatomical shapes as well as to acquire the 3-dimensional radial curvature values by section. As a result, the average curvature range and standard deviation of femoral shafts of the experiment group was determined to be 758.15±206.3 mm whereas the that of the control group was determined to be 1672.97±395.6 mm. The statistical significance of the measured results was verified through f-distribution analysis. Based on these results, it was verified that the level of curvature of the femoral shaft of the experiment group was higher. If the anatomical central points are located and analyzed using this methodology, it would be helpful in performing orthopedic operations such as the total knee arthroplasty.

        CT 영상을 이용한 3D 프린팅으로 환자 맞춤형 대퇴골 첨삭가공

        대퇴골은 사람의 뼈 중 가장 길며 체중을 지탱하는 골격체로 대퇴골간은 긴 파이프 모양이면서 안에 해면골이 거의 없어서 골절이 발생되면 재생이 어렵다. 사고로 인하여 발생되는 대퇴골의 골절은 골간부가 가장 높은 빈도로 발생한다. 골절의 수술방법은 골수강에 IM Nail을 삽입하여 골절부위를 고정하는 IM Nailing이다. 수술 시 대퇴골의 중심으로 진입하지 못하면 2차 골절 등의 피해가 발생하기도 한다. 본 연구에서는 수술 전 대퇴골의 CT IMAGE을 이용하여 3D 프린터로 환자 맞춤형 대퇴골을 제작하여 골수강으로 IM Nailing 시뮬레이션을 할 수 있도록 하였다. 수술 중 발생할 수 있는 2차적 손상을 방지 하고 시간 단축, 정밀한 수술을 할 수 있을 것이다. Femur is the largest bone in the human body which supports the weight of body. A long pipeline shape of femur has little cancellous bone, so that regeneration is difficult when fracture happens. The fracture caused by an accident most frequently occurs at diaphysis. IM Nailing is the surgical method that implants an IM Nail into a medullary cavity for the fixation of fracture parts. However, a secondary fracture may happen if an IM Nail does not penetrate at the center of femur. In this study, a patient-specific femur was manufactured by a 3D printer using the computed tomography images scanned before surgery, which was used for the simulation of IM Nailing. It is expected that this result may prevent the secondary damage, reduce surgical operation time, and increase the precision.

        The Effect of Cryoprotectants on the Survival and In Vitro-Growth of Cryopreserved Mouse Preantral Follicles

        The aim of this study was to examine more effective cryoprotectant for the cryopreservation of mouse preantral follicles. Enzymetically isolated preantral follicles from 12-day-old mice were cryopreserved by a slow freezing protocol with 1.5 M propanediol (PROH), dimethyl sulphoxide (DMSO) or glycerol (GLY) and then grown and matured in vitro for 11 days after thawing. The survival of preantral follicles immediately after freezing and thawing was not different among the PROH (68.2%), DMSO (72.4%) and GLY (72.1%). After grown and matured in vitro, the rates of survival and metaphase II oocytes were 54.9% and 36.6% for PROH which was significantly higher rates (p<0.05) compared with the rates obtained from DMSO (16.9% and 9.0%) and GLY (16.3% and 7.5%). The diameter of metaphase II oocytes from preantral follicles frozen in PROH (67.4±1.8 μm) was significantly (p<0.05) smaller than that of the fresh preantral follicles (69.1±2.3 μm). The results from the present study revealed that PROH is more suitable cryoprotectant for the cryopreservation of mouse preantral follicles.

        3.0T 자기공명영상장치를 이용한 사람의 간지럼자극과 감각중추 자극의 활성화 차이

        기능적 자기공명영상법(fMRI)을 이용하여 일반 감각 자극과 다른 간지럼 자극 과제를 수행 할 때 대뇌 감각중추 신경 연결망을 규명하고, 간지럼이 웃음의 기전과 어떤 차이가 있는지를 알아보고자 하였다. 건강한 성인 남녀 16명(평균 : 28.9세)을 대상으로 두 종류의 감각 자극 과제 수행동안 3.0T 자기공명영상장치를 사용하여 기능적 자기공명영상을 얻었다. 감각 자극은 피험자마다 역균형화하여 제시되었으며, 블록 설계로 자극 제시와 영상 획득이 이루어졌다. 획득된 영상 데이터는 SPM 99 분석하였으며, 개별 분석과 그룹 분석을 실시하였다. 개별 분석 결과 두 과제 모두 체감각 영역의 활성화가 관찰되었고, 간지럼 자극 조건은 감각자극 조건에 비해 베리니케 영역(BA40)에서 더 많은 활성화를 보였다. 또한, 그룹 분석결과 일반 감각 조건에서는 양쪽 체감각 피질 영역(BA 1,2,3)이 활성화되었으며, 간지럼 조건에서는 양쪽 체감각 피질 뿐만 아니라 시상, 대상회, 대뇌섬엽 영역에서 커다란 활성화를 보였다. 간지럼 자극에서 감각자극을 뺀 결과에서는 우측 대상회와 좌측 MFG 영역 및 좌측섬엽 에서 유의미한 활성화를 보였다. 촉각을 통한 간지럼자극을 인지하는 대뇌영역에 대해 검증하였고, 간지럼과 같은 가장 원초적인 자극이 다양한 사회적 활동에서 중요한 기능을 담당하는 웃음과 밀접한 관련이 있음을 알 수 있었다. This study was performed to identify the cerebral network associated with sensation through the tickling stimulation, which is distinctive from the rest of other networks processing normal stimulation and to investigate the difference of laughing mechanism which is closely related to tickling using functional MRI(fMRI). A 16 healthy volunteers (mean age: 28.9) on a 3.0T MR scanner during two sensation conditions. Counterbalanced stimulus were presented across the participants, and the stimulation was used block design. Acquired data was analyzed by the statistical parametric mapping (SPM 99). Subject and group analysis was performed. Individual analysis showed the activation of somatic sensation area in both tasks and the tickling sensation test showed more activated area in the Wernicke's area(BA40) compared to the normal sensation. The group analysis result shows that under normal stimulations, both sides of somatosensory cortices(BA 1,2 and 3) were activated and under tickling stimulation, not only the cortices but also those huge activation on thalamus, cingulate gyrus and insular lobe were detected. When the tickling was stopped, significant activations were shown in right cingulate gyrus, left MFG area and left insular lobe. A cerebral area responsible for recognizing tickling sensation was examined and the primitive stimulation such as tickling is much closely related to laugh, which is an important factor for various social activities.

        Identification and Promoter Analysis of PERV LTR Subtypes in NIH-Miniature Pig

        Porcine endogenous retroviruses (PERVs) are integrated into the genomes of all pigs. Since some PERVs can also infect human cells, they represent a potential risk for xeno-transplantation involving pig cells or organs. The long terminal repeat (LTR) elements of PERVs show pro-moter activity that can affect human functional genes; therefore, we examined these elements in this study. We detected several expressed LTRs in the NIH-miniature pig liver, among which we identified 9 different subtypes. When these LTRs were compared, distinct structures that contained several insertion and deletion (INDEL) events and tandem repeats were identified in the U3 region. The transcriptional activity of the 9 LTR subtypes was analyzed in the PK15 porcine cell line and in the HepG2 and Hep3B human liver cell lines, and transcriptional regulation was found to be different in the 3 cell lines. The D LTR subtype was found to have stronger promoter activity than all other types in 4 different human cell lines (HepG2, Hep3B, U251, and 293). Using computational approaches, the D type was shown to contain 4 transcription factor-binding sites distinct from those in the U3 regions of the other subtypes. Therefore, deletion mutants were constructed and examined by a transient transfection luciferase assay. The results of this analysis indicated that the binding site for the Hand1:E47 transcription factor might play a positive role in the transcriptional regulation of PERV LTR subtype D in human liver cell lines.

        Aberrant methylation of Meg3 in alpha1,3-galactosyltransferase knockout pig induced pluripotent stem cells

        Pigs have been used as a good research model for xenotransplantation. Several groups have generated porcine-induced pluripotent stem cells (piPSCs) from differentiated somatic cells. Transgenic pigs with the alpha1,3-galactosyltransferase gene-knockout (GalT-KO) could successfully govern hyper acute rejection of organ transplants into primates. Thus, GalT-KO piPSCs could be a powerful cell resource for agricultural and biomedical applications. This study was performed to generate iPSCs from GalT-KO pigs and characterize their properties. We successfully generated a GalT-KO iPSC from a genetically modified pig using double alpha1,3- galactosyltransferase knockout alterations. Similar to mouse embryonic stem cells, the GalT-KO piPSCs were positive for classical pluripotency markers: POU5F1, NANOG, SOX2 and SSEA1, and were negative for: SSEA3, TRA-1-60 and TRA-1-81. Furthermore, these cells could form an embryoid body that differentiated into three germ layers in vitro, and were highly proliferative under leukemia inhibitory factor culture conditions. However, the methylation status in DMR2 of the Meg3 gene was higher in GalT-KO piPSCs than in porcine ear fibroblast. In conclusion, GalTKO piPSCs could be successfully generated by six human factors without expression of Galepitopes. Although aberrant methylation observed in GalT-KO piPSCs, this cell line maintained pluripotency and had differentiation properties into all three germ layers. Therefore, GalT-KO piPSCs might be a good cell source for biomedical application and basic research.
