        The purpose of this study was analyse the necessity for the Scam work at home care service. The subjects of the study were 14 home m nurses who work in community and in physical ana 16 physical therapist. The data were collected from Sep. 1 to Dec. 31, 1999. The result were analysed statistically by $X^2$, t-test. The main results of this study were as follows : 1. There were statistical significances of the general characteristics between the physical therapists and the home care nurses, 2. There were statistical significances of currencies of the home us service which the subjects experienced. 3. There were no statistical significances of the necessity of team work and the participation in home care service. 4. Regarding the effects of expectation which consisted 9 items, there were statistical significances in 8 items.

        수중운동과 타이치운동 후 여성 골관절염 환자의 근력변화

        Arthritis is one of the most common chronic degenerative joint disease in elderly. Osteoarthritis is a widespread, slowly developing disease, with a high prevalence increasing with age in women. The large joints mostly involved by the disease are the knees. But there are no treatments available that cure the underlying process of osteoarthritis diseases. Physical exercise helps in increasing cartilage nutrition and remodeling, increases the synovial blood flow, decrease swelling, and improves muscle strength. Thus, exercise has been suggested as an important nursing strategy in osteoarthritis. Purpose: The purpose of this study were to compare muscle strength between Tai-Chi exercise and aquatic exercise for women with knee osteoarthritis. Methods: A quasi-experimental study with pretest and posttest measures was used. The study subjects were those who had been enrolled in a community health center, and agreed to participate in the study for eight weeks, signed the consent form, and obtained the physicians approval. The study dropout rates were 13.2% with the final study subjects of 17 on Tai-Chi exercise, 16 on aquatic exercise program. The collected data were analyzed using SPSS for Window (version 12.0). Independent sample t-test and paired t-test was performed to compare of muscle strength for women with osteoarthritis after 8-week Tai-Chi exercise and aquatic Exercise. Results: The homogeneity tests of demographic characteristics and study variables at the pretest data revealed no significant differences between two groups. After 8-week Tai-Chi and aquatic exercise, there was significant result in pre-post test comparison on muscle strength on Tai-Chi group, but no significant in aquatic group. There were no significant differences of knee extensor (p=.078), and hand grip(p=.118) in group comparisons on muscle strengths. But there were significant differences of knee flexor(p=.024). Conclusion: Tai-chi exercise was effective in improving knee flexor. So, it seems that Tai-chi exercise may be more suitable for aquatic exercise in osteoarthritis exercise programs. Further studies with other comparisons in physical and psycho social outcomes are necessary to confirm the more effects of exercise.

        류마티스 관절염 환자의 운동프로그램 적용효과와 망분석(network analysis)

        Purpose: This study aims to analyze the outcome of exercise programs for Rheumatoid Arthritis patients and its tendency with network analysis. Method: 30 articles from Medline search of foreign nursing journals(1966-2002) and 5 from three Korean nursing journals(1970-2002) were surveyed. The contents analyses were focused on outcome measures and network analysis of exercise program. Results: Exercise programs results in improving muscle strength(91.7%) aerobic capacity(72.9%) and 50 feet walk time (57.3%) Among several exercise water exercise and Tai-chi are in the middle of weight-bearing exercise. In the experimental design non-randomized control trials were 40% and the functional status of subjects were not classified in 40% of the articles surveyed. Conclusion: Considering the results muscle strength aerobic capacity and 50 feet walk time were tested as valid outcome measures. Tai-chi exercise could be recommended as proper exercise for Rheumatoid artiritis patients because of its low intensity of weight-bearing exercise. In terms of research methods the randomized trials and functional classifications of rheumatoid arthritis should be done more strictly.<br/> <br/>

        노인에게 적용한 타이치운동의 논문분석

        Purpose: This study aims to conduct a systematic review of the physical, psychosocial and physiological effects of Tai Chi exercise in elderly. Method: 37 articles from Medline search of foreign journals (1966-2006) were surveyed by the key words 'Tai Chi', Tat-chi program' and 'Tai-chi and elderly' limiting the range to age 65 or older, choosing clinical trial and randomized controlled trial research in English articles. Four articles were excluded due to methodological study, pilot study and review. The research was analyzed according to health status of the subjects, styles and forms of Tai Chi exercises, factors for physical, psychosocial and physiological measures. Result: A short forming Yang style was commonly used in chronic diseases and health for elderly. Variable outcome measures were used in Tai Chi studies relating to balance, muscle strength, walking and mobility, flexibility and cardiorespiratory function in physical measure, quality of life, depression, self-efficacy, health status, cognition and impact questionnaire in psychosocial measure, lipids, insulin resistance and hormone in physiological measure. Conclusion: Tai Chi exercise appears to have physical and psychosocial benefits and also appears to be safe for elderly and chronic diseases. It is suggested that future studies analyze statistical part of systematic reviews through meta analysis.

        재가 중ㆍ노년 여성의 골밀도와 낙상두려움 및 낙상효능감에 관한 연구

        Purpose: The purpose of this study was to investigate bone mineral density(BMD) and fear of falling and falls efficacy in the middle and old aged women over 50 years. Methods: The subjects consisted of 409 women. One-way ANOVA, Pearson's correlations and multiple regression were used to test the BMD, fear of falling and falls efficacy scale by using SPSSWIN 12.0. The BMD of the calcaneus were measured with peripheral dual energy x-ray absorptiometry(DEXA). Results: The average age was 63 years old and the average T-score was -3.21 in patient with osteoporosis, -1.72 with osteopenia, and .13 with normal. There were significant differences in the status of the BMD according to age(p=.000), height(p=.000), weight(p=.000), married status(p=.000), age of menarche(p=.002), and menopause(p=.002). The fear of falling was related with falls efficacy (r=-.247, p=.01), BMD(r=-.337, p=.01). Falls efficacy(β=-.21, p=.000)and BMD(β=-.26, p=.000) were predicting variables of fear of falling. The model explained 13% of the variance in fear of falling(F=27.38, p=.000). Conclusion: Fear of falling and falls efficacy were related with the bone mineral density. Falls efficacy and BMD may be useful for the predicting fear of falling for women in middle and old age. Further studies with assessment of fall-related risk-factors and a longitudinal study are necessary to assess with falls efficacy, and BMD with age.

        류마티스 관절염환자를 위한 타이치운동과 이압요법의 효과

        Purpose: The purpose of this study was to evaluate the effects of a program of Tai Chi and auricular acupressure in patients with rheumatoid arthritis. Methods: The study design was a one-group pretest-posttest design. The subjects were 14 outpatients with a diagnosis of rheumatoid arthritis based on the American College of Rheumatology Standards. The setting was a Academic Section of Musculoskeletal Disease in the Leeds Institute of Molecular Medicine in United Kingdom. The program was Tai Chi exercise with auricular acupressure per two times a week, for twelve weeks. Results: A program of Tai Chi with auricular acupressure showed significant improvements in amount of pain of RADAI, 28 Joint count swollen, fatigue, physical, affect and symptom of AIMS2, function and symptom of ASES (p<0.05). However, no differences were found in the score of self-efficacy relate to the pain, and quality of healthy related life to social interaction. Conclusion: In order to improve the quality of healthy related life and self-efficacy relate to the pain, it requires long-term intervention. Also different research designs are needed to confirm the effect of this study.
