Turner`s syndrome (TS) is characterized by short stature and gonadal dysgenesis. It is often associated with systemic manifestations, such as cardiovascular, gastrointestinal, and musculoskeletal disorders. Although very rare, it is possible for TS to accompany autoimmune disease, including thyroid disease, inflammatory bowel diseases, diabetes mellitus, psoriatic arthritis, and juvenile rheumatoid arthritis. A 39-year-old woman was referred for symmetric polyarthritis of her hands and feet. She had been diagnosed with Turner`s syndrome with 46,XO,-X,+fragment before the age of 22 years and had developed autoimmune hypothyroidism treated with thyroid hormone replacement. At the time of first visit, she had polyarthralgia with morning stiffness for more than 3 months. The musculoskeletal examination revealed symmetrical polyarthritis affecting the metacarpophalangeal, proximal interphalangeal, and metatarsophalangeal joints, fulfilling the ACR 1987 revised criteria for rheumatoid arthritis (RA). Here, we present an unusual case of RA associated with TS. It is important to pay meticulous attention to patients with TS so that inflammatory arthritis is not neglected and the diagnosis is not delayed.

기능성과 보온성 향상을 위한 공군 정비파카 개발 및 평가

This study was designed to develop air force mechanic parka, evaluate it, and ultimately provide functionally superior parka to the air force. The development process was 1) conducting a survey, 2) identifying problems and shortcomings of currently-supplied parkas, and 3) improving the design, pattern and materials. The newly-developed parkas were evaluated in terms of their ease of fit, clothing mobility, and insulation. Ease of fit was evaluated by subjects` sensory tests, and clothing mobility was by fitness-for-motion tests and range-of-motion tests using a Goniometer. Evaluation on insulation was conducted by thermal manikins. Findings of this study were as follows: 1. In the subjective evaluation on clothing mobility, new parkas were considered to have sufficient ease of fit while previous ones scored much lower, confirming the improvement of the new version. 2. Both subjective tests and ROM measurements on fitness for motion verified the superiority of the new parkas. 3. Insulation tests found that although insulation capability of newly-developed parkas was at a similar level to those of the previous ones, their insulation capability per unit weight was superior, demonstrating that new parkas were better at blocking heat conduction, When making changes in parka patterns and designs to enhance the mobility, it was necessary to maintain the insulation function, The new parkas developed by this study was verified to be superior to the previous ones in their insulation and clothing mobility.

감염 ; 파상풍 환자의 합병증 빈도와 원내 폐렴발생의 위험인자 분석

목적: 파상풍 환자에서 발생하는 합병증의 빈도와 그 중 원내 폐렴을 일으키는 위험인자에 대해 조사하고자 하였다. 방법: 1999년 1월부터 2008년 6월까지 파상풍으로 전남대학교 병원에 입원 치료한 환자들을 대상으로 하였다. 환자들의 임상양상과 치료방법, 합병증을 조사하였으며 APACHE II score와 Ablett score를 이용한 중등도 평가를 시행하였다. 또한 환자들을 원내 폐렴 발생여부에 따라 두 군으로 나누어 폐렴발생의 위험인자에 대해 평가하였다. 결과: 42명의 환자 중 29명(69%)이 여자, 13명(31%)이 남자였으며 평균 나이는 64.3세였다. 이 중 3명의 환자만이 파상풍 기본예방접종을 시행한 상태였으며 나머지 환자들은 기본예방접종 혹은 추가접종을 전혀 시행하지 않은 상태였다. 21명의 환자는 후두 및 호흡기 근육 경직에 의한 호흡부전으로 인공 기계환기를 시행하였다. 입원치료를 받은 38명의 자율신경계 합병증으로는 심한 혈압변동(31.6%), 부정맥(26.3%), 심정지(7.9%) 등이 빈번하게 발생하였다. 가장 흔한 감염성 합병증은 폐렴(47.4%)이었으며, 다음으로 혈류감염(15.8%), 요로감염(13.2%)이었다. 다변량 분석에서 자율신경계 이상(OR=32.0, 95% CI 1.9~524.2.1; p=0.005)과 중환자실 입원기간(OR=1.145, 95% CI 1.02~1.29; p=0.024)이 원내 폐렴 발생의 독립적인 위험인자로 분석되었다. 결론: 파상풍의 발생위험이 높은 지역에서는 질환에 대한 교육과 예방접종이 발병률을 감소시키는데 매우 중요한 역할을 하며, 파상풍을 치료하는데 있어서 질환의 특성을 고려한 치료 및 합병증에 대한 주의 깊은 감시와 치료가 필요하다. Background/Aims: Tetanus is a fatal disease and various complications affect its prognosis. In Korea, tetanus cases have been reported, but no studies, thus far, have examined its complications. We investigated the complications of tetanus and analyzed the risk factors for nosocomial pneumonia, one of most common complications of tetanus. Methods: We retrospectively reviewed the clinical features, treatment, and complications of 42 patients diagnosed with tetanus at Chonnam National University Hospital from January 1999 through June 2008. The severity of tetanus was assessed using the APACHE II and Ablett scores. We compared two groups, one with nosocomial pneumonia and the other without it, and analyzed the risk factors for nosocomial pneumonia. Results: Forty-two patients (13 men, 29 women) were enrolled. Only three of them had received a primary tetanus vaccination; the others had not received primary or booster vaccinations. Twenty-one patients needed mechanical ventilation. The most common infectious complication was pneumonia (47.4%), followed by blood stream (15.8%) and urinary tract (13.2%) infections. Regarding noninfectious complications, dysautonomia was also common, including fluctuating blood pressure (31.6%), arrhythmias (26.3%), and cardiac arrest (7.9%). Nosocomial pneumonia was associated with autonomic disturbance (OR=32.0, 95% CI 1.9-524.2; p=0.005) and length of stay in the intensive care unit (OR=1.145, 95% CI 1.02-1.29; p=0.024) in the multivariate analysis. Conclusions: Careful monitoring and treatment of complications are essential in managing tetanus, along with specific therapy for tetanus itself. Education and tetanus immunization are important for preventing the disease, especially in high-risk areas. (Korean J Med 77:84-90, 2009)

This study is designed to understand current wearing conditions of air force mechanic parkas and evaluate their functionality by examining the wearing conditions and wearers` subjective assessment. By doing so, it also intends to identify issues that require improvements; and to provide basic data for future development of air force mechanic parkas. A survey was conducted as a study methodology, and the collected 1,628 questionnaires were analyzed. Findings of this study are as follows. 1. In the usability assessment, it was found that visibility of body sides was needed to be enhanced since reflective tapes were attached only to the front and back of the body; pockets were too big, but not convenient to keep mechanic tools; the design of hiding hood was not suitable for water-proof clothing; and a new design of size-controllable hood was called for since the hood blocked eye sight. With respect to the fabric, dissatisfaction was identified with durability, cold-proof and fire-proof features. 2. In the mobility assessment, respondents showed low satisfaction with collar heights and neck girths. Mobility score was over moderate level on average, except the parka length that obstructs wearers` movement, and neck and collar size. 3. In the wearability assessment, respondents presented moderate satisfaction with pleasantness. weight, pressure and dampness. However, insulation was found unsatisfactory. Based on the result, this study proposed improvement plans on design, pattern and fabric; and is expected to serve as basic data for developing more effective and functional force mechanic parkas.

Bruton-type agammaglobulinemia is primary hypogammaglobulinemia followed by severe recurrent infection, including bacterial otitis media, bronchitis, pneumonia, and meningitis. Septic arthritis is a main musculoskeletal disorder that can occur in association with Bruton-type agammaglobulinemia. But the development of rheumatoid arthritis (RA) is rarely reported in a patient with hypogammaglobulinemia. Here, we describe a case of 34-year-old male with Bruton-type agammaglobulinemia, who presented with multiple symmetric polyarthritis. He was diagnosed as having a RA according to ACR criteria. His symptoms of polyarthritis had been improved after the introduction of medications including DMARDs (disease modifying anti- rheumatic drugs). Our case suggests that RA can be developed in the setting of agammaglobulinemia, and even in this situation, anti-rheumatic agents were effective to control arthritis without complication such as severe infection.

Sjogren`s syndrome is a chronic autoimmune disease characterized by decreased function of the lacrimal and salivary glands. In addition, many other organs can be involved in patients with Sjogren`s syndrome. Overt or latent renal tubular acidosis is an uncommon extraglandular manifestation in Sjogren`s syndrome, and osteomalacia is a rare complication of renal tubular acidosis. It has been rarely reported that osteomalacia is associated with distal renal tubular acidosis in patients with Sjogren`s syndrome. We report a case of a 34-year-old female patient who was initially presented with muscle weakness in both lower extremities. The patient was then diagnosed with Sjogren`s syndrome complicated by osteomalacia and renal tubular acidosis.

피부근염은 피부와 근육에 침착된 자가 항체와 면역 복합체에 의해 비화농성 염증이 생기는 자가 면역 질환으로 피부 병변은 잘 알려져 있으나 피하지방 병변의 임상양상과 조직학적 소견은 잘 알려져 있지 않다. 1924년 Weber에 의해 피부근염에 동반된 지방층염이 보고된 이래 지금까지 23예의 증례가 보고되어 있다. 저자들은 내원 7년전 피부근염으로 진단되어 외래에서 경과추적을 하고 있던 27세 여자 환자에서 양측 상지와 안면부를 포함한 전흉부에 통증을 동반한 경화성 결절을 발견한 후 피부 조직 생검을 통해 지방층 염으로 진단하였고, 고용량의 프레드니솔론과 메토트렉세이트를 투여함에도 피부 병변의 호전이 없어 아자치오프린을 추가하였고, 이후 호전을 경험하였기에 문헌고찰과 함께 보고하는 바이다. Dermatomyositis is an autoimmune disease wherein autoantibodies and immune complexes cause non-suppurative inflammation in skin and muscle. Although the skin lesions of this disease are well known, the clinical and pathological features of subcutaneous fat lesions remain uncharacterized. Since Weber (1924) reported the development of panniculitis in a patient with dermatomyositis, 23 subsequent cases have been reported. Here, we present the case of a woman who had been diagnosed with dermatomyositis 7 years previously, and presented with diffuse, painful, indurated nodules on her face and upper limbs. A skin biopsy allowed us to diagnose these nodules as panniculitis. The patient was initially treated with high doses of prednisolone and methotrexate. She responded inadequately to these medications, so azathioprine was added to the regimen. The skin lesions subsequently improved and the patient has remained disease-free for 1 year. Here, we present the clinical profile of this patient and review the relevant literature. (Korean J Med 2011;80:122-127)

IT 패션에 대한 국내 연구 동향

The purpose of this study was to analyze research trends and make suggestions regarding the future of information technology (IT) in the fashion industry. In this study, 437 papers written regarding IT fashion from five major journals published between 2000 and 2011 were examined. The research areas were then organized by subject and keyword, and divided into 16 high-context categories. Two IT fashion maps were constructed, one from a fashion consumer`s perspective, and the other based on the fashion industry`s supply chain. This study identified important trends in IT fashion such as: 3D scanners, 3D digital renderings of the human form, 3D digital garments, smart garments, mass customization, production automation, online shopping, home shopping, online communities, e-commerce, digital media, virtual reality, e-tail, the digital generation, E-CRM, and education. Data from body scans was collected and applied to production, and research on smart textiles was also carried out. As for IT fashion`s service areas, the majority of the research focused on online shopping or online communication. Additionally, research done on avatars and cyber space, and studies on social networking services are shown. The results of this study indicated that a new field of research has opened and that current research has been developing. Also, this study showed what is needed to expand and strengthen IT fashion.

류마티스관절염 양상으로 발현된 췌장암의 부종양성 증후군

Paraneoplastic cancer polyarthritis is a very rare manifestation of musculoskeletal disorders associated with several solid and hematologic malignancies. We describe a 71-yearold woman who presented initially with an abrupt onset of rheumatoid arthritis-like symmetrical polyarthritis. The patient was diagnosed with pancreatic cancer with multiple hepatic metastases approximately six months later. Her symptoms of polyarthritis improved after the introduction of medication including non-steroidal anti-inflammatory drugs and disease modifying anti-rheumatic drugs. This case suggests that a hidden malignancy should always be considered in elderly patients, who present with an abrupt onset of symmetric polyarthritis for the first time in their life.
