        A Case of Hypomelanosis of lto Geun Soo Lee, M.D., Hong Zoon Jang, M.D., Yeon Lim Suh*, M.D., Kyu Joong Ahn, M.Dl., Jong Min Kim, M.D., Chong Ju Lee, M.D. Departments of Dermatology and Pathology*, Clooege of Medicine, Hallym University Seoul, Korea We report a case of hypomelansis of Ito developed in a 6-year-old male patient. He had numberous bizarre, hypopigmented streaky macules on the left side of the chest, and upper arm. The histopathologi findings demonstrated that hypopigmented areas contained slightly decreased basal pigmentation with decreased content of intracellualr melanin in keratinocytes and melanocytes and revealed neither inflammatory changes nor dropping of melanin granule into the dermis. The electron microscopic finding showed decreased number of melanosomes, incomplete melaniztion, and cytoplasmic vacuoles in keratinocytes and melanocytes. Extra-cutaneous alterations were not found.

        Correlation between Upper Extremity Strength and the Oral Health of Stroke Patients

        Objective: The objectives of this study were to evaluate the correlation between upper extremity strength and oral health in stroke patients, as well as to determine the need for improvement in oral health measures, including rehabilitation to enhance upper extremity strength, and education and training for patients and their guardians related to oral hygiene. Methods: To evaluate the oral health of stroke patients, we performed an examination of oral health and survey of oral hygiene behavior. Using the Oral Health Assessment Tool, an examiner evaluated oral health using a dental mirror. The upper extremity muscle strength was also assessed using the Manual Muscle Testing (MMT) grading system. Results: Patients with higher-grade upper extremity muscle strength had better oral health. Patients with higher MMT grades were more likely to brush their teeth independently, and more often, compared to those with lower grades. Conclusion: The MMT grade of patients with chronic stroke was significantly related to the oral health. Therefore, to improve oral health, patients should receive oral hygiene education and rehabilitation to increase their MMT grade. Combined efforts by rehabilitation and dental departments are required to improve the oral health of chronic stroke patients.

        새로운 노화 방지 성분으로서 글루쿠로닉 애씨드의 기능과 화장품 응용

        피부 세포는 외부의 유해 요소 즉, 스트레스 환경에 노출되었을 때 세포 자신을 보호하기 위하여 다양한 방어 및 복구 체계를 가지고 있는데 그 중 하나가 보호단백질인 열충격단백질 70 kDa의 발현이다 Glucuronic acid를 피부세포에 다양한 농도로 전처리한 다음 유해자극(열, 활성산소)을 주었을 경우, western blottting을 통해 0.12% 농도에서 세포 내 열충격단백질이 발현됨을 알 수 있었다 그리고 confocal microscopy 및 세포생존율 실험을 이용하여 열 및 활성 산소에 대한 우수한 세포보호 효과를 확인하였다. 또한 마우스 피부를 이용하여 glucuronic acid 및 수중유(O/W)형 에멀젼에 적용하였을 때 경피 흡수 양상을 비교한 결과, glucuronic acid는 빠른 경피흡수거동을 보였다(투과속도 0.83114 mg/㎠/h, 지연시간 1.2 h, 분배계수 0.114). 에멀젼에서는 투과속도는 0.04153 ㎍/㎠}/h, 누적투과량은 1456.25 ㎍/㎠로 감소하였지만 지연시간은 2.48 h으로 증가하였고 지속적인 경피흡수(서방성)를 보였다. Exposure to elevated temperatures, chemical (active oxigen), or physical stress (UV light) induces immediate physiological response, the expression of heat shock proteins in cells. Thus, cells with elevated Heat Shock Protein levels become more tolerant to stress conditions that are otherwise lethal. First, we studied on the new function of glucuronic acid (GA) as preventive material of skin aging. The application of the GA shows significant induction of Heat Shock Protein 70 kDa (HSP 70 kDa) in contrast to cells without it. GA at the concentration which can induce HSP 70 kDa, protects the cell death induced by second stress (heat shock and hydrogen peroxide) in NIH3T3 cells. Second, we studied on in vitro transdermal permeation characteristic of GA through the excised mouse skin. In this study, we compared the skin permeability of GA in water with O/W emulsion. As a result, skin permeation parameters of GA shows lag time 1.2 h, partition coefficient 0.114, permeation flult rate 0.83114 mg/㎠/h. In case of lag time, O/W emulsion containing GA increase 2.48 h. Also, the total accumulation permeation content decreased in contrast to GA solution after 24 h. But it has long-term permeability of glucuronic acid. These results suggest that glucuronic acid could be a good cosmetic active ingredient.

        새로운 주름개선 소재: 3,9-diferuloyl-6-oxopterocarpen (Tensolin-F<sup>®</sup>)의 개발과 화장품 응용

        3,9-Dihydro-6-oxopterocarpen과 ferulic acid의 에스테르 반응을 통해 페룰산 유도체인 3,9-diferuloyl-6-oxopterocarpen (Tensolin-F<sup>®</sup>)를 합성하여 이를 함유한 주름개선 화장품을 개발하였다. Tensolin-F<sup>®</sup>는 농도 의존적으로 DPPH와 superoxide radical에 대한 소거효과를 나타냈으며, 각각 0.8 mM에서 78%, 0.053 mM에서 92.9%로 DPPH와 superoxide radical을 소거하여 우수한 항산화 효과를 나타내었다. MMP-1 효소 활성 저해 효과도 0.16 mM에서 74%를 저해하였다. HDF에서 UVA에 의해 발현이 증가되는 MMP-1의 발현 저해 효과는 Tensolin-F<sup>®</sup> 0.8 uM에서 85.5%로 단백질 수준에서 모두 농도 의존적으로 발현 저해효과가 나타났다. Tensolin-F<sup>®</sup>를 함유한 제품의 피부 주름개선 효과 평가 결과, Tensolin-F<sup>®</sup>를 함유한 화장품을 약 8주 간 도포한 경우 유의한 주름개선 효과가 있음을 확인 할 수 있었다. 본 연구를 통하여 Tensolin-F<sup>®</sup>는 항산화 효과와 MMP-1활성 저해 효과 및 UVA에 의한 MMP-1의 발현을 저해하는 효과가 나타났으며 새로운 주름개선 기능성 화장품으로 이용될 수 있을 것이다. As a novel anti-wrinkle agent, 3,9-diferuloyl-6-oxopterocarpen (Tensolin-F<sup>®</sup>) has been synthesized and its anti-aging effects have been investigated. In the present study, to investigate the relationship between aging and Tensolin-F<sup>®</sup>, we examined its effect on scavenging activities of radicals and reactive oxygen species (ROS), in vitro inhibition activity of matrix-metalloproteinase (MMP) and expression of UVA-induced MMPs in human dermal fibroblasts (HDF). Tensolin-F<sup>®</sup> was found to show activities of scavenging radicals and ROS with the IC<sub>50</sub> values of 0.2 mM and 0.95 mM against 1,1-diphenyl-2-picrylhydrazyl (DPPH) radical and superoxide radicals, respectively, in the xanthine/xanthine oxidase system. Fluorometric assays for the proteolytic activities of MMP-l (collagenase) were performed using fluorescent collagen substrates. Tensolin-F<sup>®</sup> inhibited the activities of MMP-l in a dose-dependent manner and the IC<sub>50</sub> values calculated from semi-log plots were 0.025 mM. Also, UVA induced MMP-1 expression was reduced 85% by treatment with Tensolin-F<sup>®</sup> at 0.8 uM, which was reduced dose-dependent manner. The results of clinical study showed that 4.8 mM Tensolin-F<sup>®</sup> treated group reduced wrinkle significantly compared with placebo treated group (P<0.05). Taken together, these result suggest that Tensolin-F<sup>®</sup> act as an anti-wrinkle agent by taking effects to antioxidation and reducing UVA-induced MMP-l production.-l production.

        7-디하이드로콜레스테롤을 함유한 키토산 코팅 처리 Solid Lipid Nano-particle의 개발에 관한 연구

        불안정한 생리활성물질들은 외부 환경에 의해 빠르게 분해된다. 그러므로 이러한 물질들을 안정화시키기 위한 캡슐화 기술은 매우 중요하다. 비타민 D3의 전구체인 7-디하이드로콜레스테롤(7-DHC)은 일반인의 표피 각화세포에서 열충격 단백질(Heat Shock Protein)의 발현을 단백질과 mRNA의 수준에서 증가시키는 것으로 알려졌다. 하지만 7-DHC의 국소용 피부 제제로의 이용은 낮은 용해도와 화학적 불안정성 때문에 이용이 제한되었다. 본 연구에서 7-DHC는 나노에멀젼(NE), 고형 지질 나노 입자 (SLN) 그리고 키토산이 코팅된 고형 지질 나노 입자(CASLN)에 봉입하였다. NE와 SLN은 지질의 용융점 이상의 온도에서 고압의 호모제나이져를 통과시켜 제조하였다. CASLN은 SLN 분산액에 키토산을 용액을 첨가하여 제조하였으며 양(+)의 제타전위를 나타내었다. NE, SLN, CASLN 속에서 7-DHC의 안정도를 각각의 온도조건에서 시간의 경과에 따라 확인하였다. 열분석과 X선 회절 분석은 지질의 결정화 정도를 확인하기 위해서 수행하였다. 그 결과, CASLN은 기존의 SLN보다 불안정한 7-DHC를 효과적으로 봉입함으로서 안정성을 개선시켰다. Unstable cosmetic active ingredients could rapidly break down in chemical and photochemical process. Therefore, it has become a very important issue to encapsulate active ingredient for the stabilization. 7-Dehydrocholesterol (7-DHC), a precursor of vitamin D<sub>3</sub>, has been shown to increase levels of protein and mRNA for heat shock protein in normal human epidermal keratinocytes. However, topical dermal application of 7-DHC is restricted due to its poor solubility and chemical unstability. In this study, 7-DHC was incorporated into nano-emulsion (NE), solid lipid nano-particle (SLN), and chitosan coated solid lipid nano-particle (CASLN), respectively. In order to prepare NE and SLN dispersion, high-pressure homogenization at temperature above the melting point of lipid was used Hydrogenated lecithin and polysorbate 60 were used as stabilizer for NE and SLN. CASLN was prepared by high speed homogenizing after adding chitosan solution to the SLN dispersion and showed positively charged particle properties. Decomposition rate of 7-DHC in NE, SLN and CASLN was studied as a function of time at different temperature. Differential scanning calorimetry (DSC) and X-ray diffraction (XRD) studies were performed to characterize state of lipid modification. It appeared that CASLN is the most effective to stabilize 7-DHC and may be used for a useful topical dermal delivery system.
