Syphilis and human immunodeficiency virus(HIV) disease may coexist, and syphilis is often the presenting infection of patients with concurrent HIV infection, because syphilis may accelerate development of immunodeficiency in HIV-infected persons, and conversely HIV infection causes reactivation of dormant syphilis. Cutaneous secondary syphilis in HIV-infected patients is often associated with unusual and s clinical features. We report a 44- year old man in that syphilis reactivated by HIV infection, and a 23-year-old m in that syphilis was a presenting sign of HIV infection.

The case of an unusual desmoplastic variant of trichilemmoma arising in the face of a 72-year-old man is reported. Desmoplastic trichilemmoma is characterized by the presence of a densely sclerotic stroma, surrounded by lobules of epithelial cells with features of outer root sheath differentiation, including glycogen-rich, clear cytoplasms and peripheral palisading. In the central part of the tumor, irregular cords and nests of epithelial cells entrapped in the desmoplastic stroma are found. The pathological appearance can mimic trichilemmal carcinoma, morpheic basal cell carcinoma, and squamous cell carcinoma. Recognition of this benign neoplasm as such will prevent misdiagnosis and unnecessary aggressive treatment. (Korean J Dermatol 2001;39(12):1407~1410)
한국 성인여성의 인유두종 바이러스(HPV)백신 접종 및 접종의도에 영향을 미치는 요인 : 체계적 문헌고찰

Purpose: This study was conducted to identify the factors influencing of HPV vaccination and vaccination intention among Korean adult women through a systematic review. Methods: A systematic review was conducted across international and domestic electronic databases and nine of 193 studies were identified for this review. Results: Meaningful variables influencing HPV vaccination and its intention included socioeconomic status (age, economic level, and education level), knowledge level, health beliefs, attitudes toward HPV vaccination, and previous experiences. Economic level and knowledge influenced HPV vaccination and vaccination intention, which was consistent with the results of previous studies. Eight studies measured knowledge level regarding cervical cancer or HPV vaccination; however some instruments lacked reliability and validity report. Conclusion: There is a need for governmental support to reduce the burden of HPV vaccination cost, to publicize the benefits and to provide general information regarding HPV vaccination. Finally, it is necessary to develop an instrument with confirmed reliability and validity to measure HPV knowledge.

Premature rupture of membrane is the most frequent cause of low birth weight infant delivery which increase the maternal and fetal morbidity and perinatal mortality. A retrospective case-control study was performed on 315 mothers who delivered low birth weight infants($\leq$2.5kg) with premature rupture of membrane and as control group 546 mothers who delivered normal birth weight infants(2.9-3.7kg) without premature rupture of membrane were chosen. The results obtained from this study were as follows: 1. The proportion of low birth weight infants due to premature rupture of membrane among all low birth weight infant deliveries was 14.5%, and this is equivalent to 1.1% among all deliveries. 2. The most significant maternal risk factor of low birth weight infant deliveries with premature rupture of membrane was infections on vagina, cervix and uterus during pregnancy. Compared with control, adjusted odds ratio was 7.61(95% confidence interval(CI) 1.88-30.88, p=0.004). Other significant maternal risk factors were the history of induced abortion, spontaneous abortion, and the experience of premature delivery. The risk ratios were 1.82, 2.07, 4.42, respectively. 3. Breech presentation did increase the risk of low birth weight infant delivery with premature rupture of membrane compared with control(Adjusted Odds ratio=2.66, 95% CI 1.35-5.26, p=0.005). 4. Mothers who had not taken antenatal care were having higher risk of low birth weight infant delivery with premature rupture of membrane against control(Adjusted odds ratio=1.73, 95% CI 1.19-2.53, p=0.004). These study results show that maternal factors such as the infection of genital organs during pregnancy, the history of induced abortion and breech presentation are significantly associated with the premature rupture of membrane in the low birth weight deliveries, and that most of these risk factors are controllable ones through proper antenatal cares.

This study explores Almanac of the Dead, the Native American writer Silko``s apocalyptic novel. This novel is a harsh indictment of the United States, and demands justice for the dead and the oppressed. In order to resist cultural and spiritual genocide, Silko, deconstructing the form of the novel, experiments with fragmented narratives and weaves dozens of interconnected tales to rewrite five hundred years of American history and envision a future where the tribal people of the Americas retake the land from governments that are corrupt at every level. So this essay is designed to elaborate one of the ways she replaces with Indian cultures those destructive western cultures which have oppressed the Americas throughout the past 500 years. Above all, she wants to alter the reader``s sense of time, because Indian time is circular while Western time, justifying the European progress model, is linear. Further, each day has its own identity, and the day returns to the same place where the story had happened earlier but with its details changed. This process symbolically strengthens the potential for the ancestor``s prophecy. Especially in a cycling time, a person is thought of as “a moving event shaped by and shaping human and non-human surroundings” (Allen 149). So in Almanac of the Dead, there is no character development. Consequently it means that to restore Indian time is to turn back to the pre-colonized time over the Americas, leading to a resurrection of Indian cultural identities.
