      • 비인강암 환자에서 발생한 경동맥동 과민증후군

        저자들은 극소진행형 비인강암(제 4기, N₄N₂(c)) 환자에서 종양으로 인한 경동맥동 과민증후군과 관련되어 실신이 발생하였으며 방사선치료후 호전된 환자 1예를 경험하였다. The syndrome of carotid sinus hypersensitivity is cardovascular symptom complex including lightheadedness, transient loss of consciousness, perspiration, pallor, weakness of lower extremities, transient hypotension, bradycardia and seizure, which occurs from firing of hyperactive carotid sinus reflex. Most cases are idiopathic, but maliganant tumors involving parapharyngeal space have been occasionally implicated. We present a case of carotid sinus hypersensitivity due to nasopharyngeal carcinoma invading the parapharyngeal space with bilateral cervical lymphnode metastases(stage IV, T₄N₂M(0)). The patient experienced several episodes of severe syncope proceeded by blurring of vision, dizziness and nausea, which were used to occur while he was working in erect position. The tumors were markedly regressed by external radiation therapy with successful resolution of syncope.

        신혈관성 고혈압 진단과 Captopril 검사

        N/A Background: The Captopril test (Muller, 1986) appears to represent a simple and useful screening tool in the diagnosis of renovascular hypertension. The limitations of the sennsitivity, specificity have been reported under certain circumstances. We performed this study to evaluate the sensitivity, specificity and predictive value of the Captopril test in the diagnosis of renovascular hypertension. Methods: We calculated the sensitivity, specicity and predictive value of the Captopril test in 26patients with clinical suspicion of renovasculr hypertension. We performed the Captopril test and renal angiography in these patients. Results; 1) The Captopril test was positive in 8 patients (30.7%), in whom 26renovascular hypertension were suspected. 2) The mean value of basal and stimulated plasma renin activity were 4.55±1.74ng/ml/hr and 19.89±7.19ng/ml/hr respectively, in 8patients with positive Captopril test. The mean value of percent increase of stimulated PRA was 384±227.9% in these patients. 3) The mean value of basal and stimulated plasma renin activity were 7.27±5.29ng/hr and 8. 78±5.89ng/ ml/hr respectively, in patients with negative Captopril test. The mean value of percent increase of stimulated PRA was 34.61±30.56% in these patients. 4) In patients with postivie Captopril test, there were 4true positive (50%) and 4false positive (50%) by renal angiography. In 18patients with negative Captopril test, there was 1false negative who had abnormal angiography. 5) The sensitivity, specificity and predictive value of the Captopril test were 80%, 81% and 50% respectively. Conclusion: The Captopril test seems to have limitations of sensitivity, specificity and predictive value in the diagnosis of renovascular hypertension. The careful clinical clinical application of Captopril test and renal angiography should be considered in the diagnosis of renovascular hypertension.

        폐포 단백증 1 예

        Pulmonary alveolar proteinosis is a rare non- inflammatory alveolar disease which results from the abnormal accumulation of surfactant like phospholipids and proteinaceous material in the alveolar space. The etiology and pathogenesis of pulmonary alveolar proteinosis is still obscure but several kinds of lung insults that derange normal surfactant homeostasis probably responsible for development of pulmonary alveolar proteinosis. Pulmonary alveolar proteinosis is found all industrialized countries so it will be found repeatedly in our country for the future. Because of possibility of increasing incidence, pulmonary alveolar proteinosis must be considered as a differential diagnosis of non-inflammatory alveolar disease with unknown etiology. So we report this case of pulmonary alveolar proteinosis in a 53 year old male that is confirmed by open lung biopsy and improved by repeated brochoalveolar lavage and conservative care.

      • Cefepime 의 호기성 그람음성 간균 및 그람양성 구균에 대한 시험관내 항균력

        Cafepime and the other three third generation cephalosporins were tested for their antimicrobial effects. Minimal Inibitory concentrations of total 407 clinical isolates were determined by the agar dilution method. Cefepime showed the lowest MIC?? against Citrobacter freundii and Enterobacter cloacae, 1 ug/ml and 8 ug/ml respectively. MIC?? of cefepime, 16 ug/ml, was the lowest against Pseudomonas aeruginosa and Acinetobacter calcoaceticus. Cefepime showed the lowest MIC?? against Serratia marcescens and oxacillinsensityive Staphylococcus aureus. Against Klebsiella pneumoniae, cefepime showed low MIC??, 0.5 ug/ml. MIC?? of cefepime, 16 ug/ml, was the lowest against Streptococcus species. However, cefepime showed high MIC?? against Xanthomonas maltophilia, oxacillin resistant Staphylococcus aureus and coagulase negative Staphylococcus, 128 ug/ml, > 128 ug/ml, and 64 ug/ml, respectively. Compared to the other 3 third generation cephalosporins, cefepime showed the lowest MIC?? against all isolates except Xanthomonas maltophilia, oxacillin resistant Gram-positive cocci. These high active in vitro test results are concluded to indicate the usefulness of cefepime for the treatment of Gram-negative bacilli and oxacillin sensitve Gram-Positive cocci including nosocomial ones.
