This study was aimed at developing the Structured Intake Interview Form. This intake form was for diagnosing the severity of addiction, predicting prognosis, and reflecting the evaluation into future treatment plans. This intake form is based on constructs of causes, internal mechanism, and pathological symptoms, which were introduced in Davis"(2001) Cognitive-Behavioral Model of Pathological Internet Use and Cheong-Taek Kim and his colleagues"(2002) Internet-Addiction Structure Model. The items of intake form are composed of demographic variables and symptoms. We have revised this form through the interviewee and interviewers" feedbacks, two pilot tests and several experts" meetings. In addition, we asked 80 interviewers to complete this intake form, self-reported KS scale, Observer rating scale on one internet overusing adolescent. Totally 53 cases were gathered. The total correlation coefficient between this intake form and KS scale was .56(p<.001), and that between this form and Observer rating scale was also .56(p<.001). In addition, the results showed statistically significant correlations between this form"s and KS scale"s subscales(disturbance of adaptive functions: r=.52, p<.001, withdrawal: r=.46, p<.001). This results was identical in correlations between this form"s and Observer rating scale"s subscales(disturbance of adaptive functions: r=.49, p<.001, withdrawal: r=.34, p<.05). This intake interview form was for counselors and teachers. It would be the basic information to examine the severity of addiction and causes, and establish the treatment plan. 본 인터넷 과다사용 청소년 접수면접진단지는 인터넷 중독 자가진단 척도상에서 과다사용자로 판별되거나 일상생활 관찰을 통해 인터넷 과다사용이 의심되는 청소년의 인터넷 중독수준, 원인, 치료적 예후를 평가하고 평가 내용을 향후 치료와 지도 계획에 반영하기 위한 목적으로 개발되었다. 본 접수면접진단지는 Davis(2001)의 병리적 인터넷 사용의 인지-행동적 모형, 김청택, 김동일, 박중규, 이수진(2002)의 인터넷중독구조모형에서 인터넷 중독의 원인, 내적 기제, 병리적 증상으로 제시된 구인들을, 인구학적 변인과 증상을 묻는 문항들로 구현한 것이다. 본 접수면접진단지는 피면접자와 면접자로부터의 피드백, 예비조사, 지속적인 전문가 회의를 통해 수차례 수정, 보완하였다. 그 결과, 피면접자의 거부 반응을 최소화하면서 인터넷 중독수준, 원인, 치료적 예후의 정보 획득이 가능한 접수면접진단지를 구성할 수 있었다. 또한 본 접수면접진단지의 중독수준 평가의 타당성을 확보하기 위해 53개의 사례를 대상으로 다른 청소년 인터넷 중독 진단척도들(청소년 인터넷 중독 자가진단 간략형 척도, 청소년 인터넷 중독 관찰자용 척도)과의 상호관련성을 알아본 결과, 유의한 정적 상관(각각 r=.56, p<.001)이 존재한다는 것을 확인할 수 있었다. 본 접수면접진단지는 상담기관이나 학교 현장에서 인터넷 과다사용 청소년을 만나야 하는 상담자나 교사가 청소년의 인터넷 중독수준, 원인, 치료적 예후를 평가하고 향후 치료나 지도계획을 수립하는 데 매우 유용한 역할을 할 것으로 기대된다.
