        한국의 건강도시 현황 및 평가: 제 4기 유럽 건강도시 평과결과를 중심으로

        Objectives: The Healthy Cities Project was proliferated in the European region as the WHO-EURO adopted the concept in 1984, while it was accelerated in Korea with the pilot project in 2004. In line with needs for evaluating Korean Healthy Cities in terms of environmental improvement, this study compares progress level between Korea and European cities and explores development strategy which fits into Korean Healthy Cities. Methods: To examine progress of the Healthy Cities in Korea, a survey was conducted with 56 respondents, who are project managers at local level, for a month(3-4) in 2010. The questionnaire includes 6 key factors; partnership, city health profiles, city health development plans, active participation by community, healthy equity and health impact assessment, which are derived from the evaluation of phase Ⅳ of the European Healthy Cities Network. Results: City health profiles and city health development plans are considered as an essential factor in the European Healthy City Network, while some of the Korean Healthy Cities do not meet those requirements. In terms of healthy equity and health impact assessment, European Healthy Cities move on the practice stage, while Korean Healthy Cities are still in the awareness stage. Conclusion: There are still many challenges for the Healthy Cities in Korea compared to the Europe, especially central government should support local governments` commitment in many ways to achieve qualitative progress. In conclusion, developing growth strategy and activating participation by community are priorities for Korean Healthy Cities to step forward.

        혼성 체육수업의 성차별 요인이 학생들의 잠재적 태도성향에 미치는 영향

        The purpose of this study was to the effect of gender discrimination felt by students in cosex physical education classes on student subconscious attitudes. To this end, Cluster Random Sampling was used to set up the population with middle and high school students in Kyunggi-do. The survey tool was questionnaire and Collected data hereof were analyzed by one way ANOVA analysis and Multiple Regression Analysis with SPSS WIN Ver 11.0. The aforementioned method and procedure led this research to the conclusion as follows. Firstly, First, there were significant differences in sex discrimination according to individual student characteristics. Male students felt sex discrimination more than their female counterparts and the level of ability, students with lower ability felt more discrimination than those with higher ability. Second, there were significant differences in student subconscious attitudes according to individual student characteristics. Male students' subconscious attitudes were more affected, compared with that of female students and the level of ability, the subconscious attitudes of students with academic performances above average were more affected. Third, even gender discrimination factors were a significant influence upon students' subconscious attitude, partially uneffective student' subconscious attitude factors were interact others.

        코로나19 유행에 따른 보건소 진료량 변화분석

        연구목적 : 한국은 코로나19 유행기간 동안 보건소의 건강증진사업과 진료서비스를 긴급히 축소하고 의료진을 방역 업무에 배치하였다. 이 연구는 코로나19 대응이 보건소의 진료제공에 미치는 영향을 평가하고자 하였다. 연구방법 : 2018년 12월부터 2020년 8월(85주)까지 국민건강보험과 의료급여 청구 자료를 사용하였다. 코로나19 유행 전, 확산기, 회복기의 세 단계를 기준으로 단적절 시계열 분석을 시행하였으며, 지역별 차이를 확인하기 위한 시도별 하위 그룹 분석 또한 수행되었다. 주요결과 : 코로나19의 발생(β: -64,448, P value <0.0001)과 확산(β: -7,346, P value: 0.0003)이 보건소 이용량을 감소시키는 것으로 나타났다. 진료량은 사회적 거리두기를 완화한 시점에(β: 27,650, P value: 0.0176) 증가했으며, 이후에도 점차 증가했다(β: 8,507, P value <0.0001). 결론 : 코로나19 유행에 따라 보건소에서 의료 서비스 제공량이 감소했으며, 유행이 약화 되었음에도 불구하고 완전히 회복되지 않았다. 이는 보건소의 의료제공에 공백이 존재했음을 의미하며, 보건소의 주요 이용자인 취약 계층을 위한 지원의 필요성을 시사한다. Background : In the Republic of Korea, medical personnel at the Public health centers (PHCs) were urgently deployed to the quarantine while the healthcare projects and medical treatments are scaled back during the COVID-19 pandemic. This study aimed to evaluate the impact of COVID-19 response on the number of visits at PHCs. Materials and methods : The Republic of Korean National Health Insurance and Medical Aid claims data between December 2018 and August 2020 (85 weeks) were used. Interrupted time series and segmented regression model was conducted for statistical analysis by three stages (pre-wave period, the spreading period during the wave and recovery period). Subgroup analysis was performed based on the regions. Results : The results showed that the outbreak (β: -64,448, P value < 0.0001) and the spreading (β: -7,346, P value: 0.0003) of COVID-19 decreased the number of visits. The volume increased when the social distancing was eased (β: 27,650, P value: 0.0176), and gradually increased (β 8,507, P value < 0.0001). Conclusion : In conclusion, the COVID-19 pandemic decreased the volume of healthcare services at PHCs. However, it was not fully recovered again despite the weakening of the prevalence, implying that the vacuum of healthcare has existed at PHCs. The findings suggest the importance of alternative supports for healthcare for the vulnerable, who are the main user of health centers.

        간호사 야간근무 현황과 야간근무 가이드라인에 대한 인식

        간호사 야간근무 정책 개선방향 마련에 기초자료를 제공하고자, 3교대 근무 간호사 2,841명을 대상으로 야간근무 현황과 근무하는 병원의 야간근무 가이드라인 운영 현황, 가이드라인에 대한 인식, 야간근무의 어려움 등에 대해 조사하였다. 한 달간 평균 근무일수는 20.0일이었으며, 야간근무 일수는 6.1일, 최대 연속 야간근무 일수는 평균 2.6일이었다. 야간근무는 평균 21시 40분에 시작하여 다음날 7시 34분에 종료하였으며 근무시간은 평균 9.9시간으로 8시간 근무원칙이 ‘지켜지지 않는다’가 62.0%를 차지하였고, 2일 이상 연속 야간근무시 48시간 이상 휴식 보장 여부에 대하여 67.7%가 ‘보장되지 않는다’고 응답하였다. 반면 특수건강검진 경험은 94.5%가 ‘있다’고 응답하였고, 근무외 행사참여는 81.1%가 ‘없다’고 응답하였다. 야간근무시 추가 수당을 받은 적이 있는지 여부에 ‘모른다’가 45.3%로 가장 많았고, 가이드라인에 대해 조사 시점 이전에 알고 있었는지 여부를 조사한 결과 ‘모른다’는 응답이 전체의 82.0%를 차지하고 있었다. 야간근무시 가장 어려운 점에 대한 질문에 건강악화가 60.9%로 가장 많았고, 다음이 업무가중 13.3%, 일·가정 양립 어려움이 12.6%, 환자응대 4.5% 순이었다. 가이드라인은 적용 대상을 확대 중에 있으며, 이에 따라 병원의 가이드라인 운영 변화와 간호사의 상세 근무환경 등에 대한 추가 연구와 모니터링이 필요하다. This study aimed to explore the night shift working conditions of nurses in Korea, and to provide a reference for the develop government guidelines. A survey was conducted with 2,841 three-shift work nurses using a web-based structural questionnaire. The questionnaire included questions about night shift working conditions and perceptions of the government guidelines. The average night shifts work was 6.1 days per month, and maximum consecutive night shifts work was 2.6 days. Night shifts work, on average, started 21:40 and ended 7:34 of the next day, the average overtime hour was 1.9 hours. 62.0% of the respondents were not guaranteed the rule of 8 hour working time, and 67.7% were not guaranteed the between-shift break over 48 hours after a consecutive night shift works. On the other hand, 94.0% had experienced special health examination. 45.3% of the respondents didn’t know whether they had experience with additional allowances or not, and 82.0% didn’t know about government guidelines. The most difficulty about night shift work was ill health, over-work, work-life balance, and so on. The government guideline is expanding, so additional study and monitoring is necessary.
