        방문 물리치료의 안정화 운동이 뇌졸중 환자의 균형능력과 일상생활동작 및 폐기능에 미치는 영향

        Purpose: To analyze the effect of home-visiting physical therapy, after applying a stabilization exerciseprogram that can easily be performed at home by professionals to chronic stroke patients and analyzingbalance ability, daily life movements, and pulmonary function. Methods: A sample of 30 patients diagnosed with hemiparesis due to stroke was assigned to two groups:a group that received on-site physical therapy (group I) and a group that received on-site physical therapycombined with trunk stabilization exercise (group II). After the intervention twice a week for eight weeks, thebalance ability, daily life movements, and pulmonary function of the subjects were measured. A paired-samplet-test was performed to determine differences before and after intervention within the groups, and analysisof covariance was performed to determine differences between the groups. Results: After the intervention, the Berg balance test score, MBI score, and lung function increased in bothgroups, with a statistically significant pre-post difference (p<.05). In the comparison between groups, theBerg balance test score, MBI score, and lung function increased more significantly in group Ⅱ than in groupⅠ, showing a statistically significant difference (p<.05). Conclusion: It was found that an intervention combining home-visiting physical therapy and trunk stabilizationexercises increases more efficiently the balance ability, ability to perform activities of daily living, andpulmonary function in stroke patients at home.

        Effects of Various Types of Bridge Exercise on the Walking Ability of Stroke Patients

        Purpose: The purpose of this study is to examine the effect of various bridge exercises on walking ability. Method: The subjects were 30 stroke patients. They were divided into a bridge exercise group on a stable support surface (Group I), a bridge exercise group on an unstable support surface (Group II), and a bridge exercise group combined with whole body vibrations (Group III). 10 subjects were randomly assigned into each group. The subjects of this study had 30 minutes of nervous system physical therapy including gait training and strength training. In addition, each group underwent a 30 minutes session five times a week for eight weeks. Before intervention, LUKOtronic was used to measure step width and step length, time was measured with a 10 m walking test, and time and number of steps were measured with the figure 8 walking test. After the intervention, remeasured and analysis was performed for each group. Results: As a result of comparing and analyzing the change of walking ability between groups, there was a statistically significant difference. As a result of the post hoc analysis according to the change of walking ability among groups, the change of walking ability was larger in Group III than in Group I and Group II. Conclusion: Based on these results, it is confirmed that the bridge exercise combined with whole body vibration was more effective for walking ability. Based on these findings, this study proposes an effective program for elite athletes as well as stroke patients.

        게임기반 체중지지 균형훈련이 뇌졸중 환자의 국면별 일어서고 앉기 동작과 기능적 수행력에 미치는 영향

        PURPOSE :This research intends to identify the effects of game-based weight bearing exercises on balance, muscular activation, sit to stand to sit motions of stroke patients. METHOD : 30 patients who were diagnosed as hemiplegia by stroke less in than a year were sampled and they were classified into two group, one of which was game-based weight bearing balance exercise group, and the other was functional weight bearing exercise group. 15 people were randomly selected for each group. Each exercise was coordinated by this research for 8 weeks, 5 days a week, 1 time a day and 30 minutes a day. 3D motion analyzer was used to measure the sit to stand to sit motions and a stopwatch was used to measure the time for stand-up motions for 5 times. RESULT : In terms of analyzing sit to stand to sit motions by phases, game-based weight bearing balance exercise group showed significant reduction compared to functional weight-bearing exercise group in phase Ⅰ, Ⅱ, Ⅲ, Ⅳ and total time. In terms of functional stand-up performance analysis, game-based weight bearing balance exercise group showed significant reduction compared to functional weight-bearing exercise group in 5 times stand-up examination. CONCLUSION : It was verified that game-based weight bearing balance exercise had positive impact on function recovery of stroke patients by enhancing sit to stand to sit capabilities. It is considered that game-based exercise was an effective intermediary for functional improvement of stroke patients, while also inducing consistent and voluntary participation by causing interest and motivation. 목적 본 연구는 뇌졸중 환자에게 게임기반 체중지지훈련을 중재하여 균형과 다리 근활성도 및 일어서고 앉기 동작에 미치는 영향에 대해 알아보고자 하였다. 방법 연구 대상자는 뇌졸중으로 인한 반신마비 진단을 받은 1년 미만의 환자 30명을 표본 추출하여, 게임기반 체중지지 균형훈련 그룹과 기능적 체중지지훈련 그룹으로 나누었으며 대상자들은 15명씩 무작위 배치하였다. 본 연구의 각각의 훈련을 8주 동안, 주 5회, 1일 1회, 1일 30분씩 중재하였다. 3차원 동작분석기로 일어서고 앉기 동작을 측정하였고, 초시계를 이용하여 5회 일어서기 수행 시간을 측정하였다. 결과 단계별 일어서고 앉기 동작 수행 시간 분석에서는 phase Ⅰ, phase Ⅱ, phase Ⅲ, phase Ⅳ 그리고 total time에서 기능적 체중지지 훈련 그룹보다 게임기반 체중지지 균형훈련 그룹에서 유의한 감소를 보였다. 기능적 일어서기 수행력 분석에서는 5회 일어서기 검사에서 기능적 체중지지훈련 그룹보다 게임기반 체중지지훈련 그룹에서 유의한 감소를 보였다. 결론 뇌졸중 환자에게 게임기반 체중지지 균형훈련은 일어서고 앉기 능력을 증가시켜 기능적 회복에 영향을 미침을 알 수 있었다. 게임을 기반으로 한 훈련이 뇌졸중 환자의 기능향상에 효과적인 중재로 생각되며, 흥미유발과 동기부여를 이끌어 지속적이며 자발적인 참여를 유도할 수 있다고 생각된다. 향후 본 연구를 바탕으로 뇌졸중 환자들의 성별, 나이, 관심사에 맞는 다양한 게임기반 훈련들의 도입으로 중추신경계 손상 환자들의 특성에 맞는 효율적인 중재 프로그램이 필요할 것으로 생각된다.

        편마비 환자의 반 무릎서기 자세가 일어서기 동작 수행에 미치는 영향

        Purpose: The purpose of this study was to investigate the kinematic characteristics and muscle activities during the following two conditions: transition from half-kneel to standing on the affected leg and non-affected leg. Methods: Twenty-one hemiplegic patients participated in the study. A motion analysis system was used to record the range of motion and angle velocity of the hip, knee and ankle from the half-kneel to the standing position. Electromyography was used to record the activity of 4 muscles. Results: The statistical analysis showed that the minimum ROM of the hip joint was less on the affected leg during transition from half-kneel to standing. However, the minimum ROM of the knee and ankle joints was less on the non-affected leg during transition from half-kneel to standing. The angle velocity of the knee and ankle joints was less during transition from half kneeling to standing on the non-affected leg. Muscle activity of the rectus femoris and tibialis anterior was less while moving from half-kneel to the standing position on the affected leg. Conclusion: These results show that greater active ROM of the knee and ankle was required on the affected leg for transition from half-kneel to the standing position than for normal gait. Muscle activity of the rectus femoris and tibialis anterior is normally required for movement from the half-kneel to the standing position during normal gait. Further studies are needed to investigate the antigravity movement in healthy subjects and hemiplegic patients in order to completely understand the normal and abnormal movement from the half-kneel to the standing position.

        경두개 직류 전기자극을 결합한 호흡 저항 운동이 뇌졸중 환자의 호흡 기능에 미치는 영향

        Purpose: This study aimed to investigate the effect on the pulmonary function of stroke patient by mediatingrespiratory resistance training combines transcranial direct current stimulation. Methods: 30 stroke patients were selected as the subjects of study, and divided them randomly into arespiratory resistance training group combining with transcranial direct current stimulation (Experiment group)and general respiratory resistance training group (Control group). All subjects of this study were providedgeneral neurologic physical therapy for 30 minutes, and then additionally provided each respiratory resistancetraining with transcranial direct current stimulation and general respiratory resistance training according togroup, 5 times a week for 30 minutes for totally 8 weeks. Pulmonary function was analyzed before theintervention. The strength index, peak inspiratory flow and 6-minute walking test were measured in order toanalyze the breathing function. The analysis between the groups was performed by re-measuring above itemsequally after 8 weeks of intervention. Results: Respiratory resistance training group with transcranial direct current stimulation (Experiment group)has statistically meaningful difference in pulmonary function compared to the general respiratory resistancetraining group (Control group) based on the comparative analysis of strength index, peak inspiratory flow and6-minute walking test between two groups. Conclusion: As a result of above, it was found that respiratory resistance training with transcranial directcurrent stimulation was more effective for pulmonary function. This training shall be proposed as an effectiveprogram not only for the stroke patients but also for normal people or pro-athletes based on this study. Futurestudies on muscle changes related to pulmonary function are necessary in the future based on this study.

        The Biofeedback Scapular Stabilization Exercise in Stroke Patients Effect of Muscle Activity and Function of the Upper Extremity

        Purpose: This study attempts to understand the effect of stabilization exercise of biofeedback scapular on muscle activity and functional evaluation of the upper extremity in stroke patients. Methods: Patients were divided into two groups; a biofeedback scapular stabilization exercise group comprised of 8 patients and a task-oriented training group including another 8 patients, and 30-minute exercise was performed 5 times a week for 8 weeks. Electromyogram was used to measure muscular activity of lower trapezius, deltoid middle, and serratus anterior. Fugl-Meyer Assessment and Manual Function Test were used to evaluate functions of the muscles mentioned. Results: Significant difference was observed in the comparison group before and after exercise in muscular activity of lower trapezius, deltoid middle, and serratus anterior, Fugl-Meyer Assessment, and Manual Function Test. Conclusion: Therefore, we could see that biofeedback scapular stabilization exercise is more effective than task-oriented training in facilitating muscle activation and functional capacity of upper limb.

        테이핑을 결합한 생체되먹임 균형 훈련이 발목 불안정성을 갖고 있는 배드민턴 동호인의 다리 근활성도에 미치는 영향

        Purpose: To examine the effect of biofeedback balance training combined with taping on the lower extremitymuscle activity of badminton club members with chronic ankle instability. Methods: Thirty badminton club members were selected as the study participants and divided into abiofeedback balance training group combining taping (experimental group), and a general biofeedbackbalance training group (control group). Fifteen participants were randomly assigned. Additionally, the studyparticipants implemented biofeedback balance training combining taping and general biofeedback balancetraining for 30 min five times a week over an 8-week period, after receiving general badminton training. Lowerextremity muscle activity was analyzed before the intervention. The peroneus longus, peroneus brevis, andtibialis anterior were measured by maximum voluntary isometric contraction in the bare-handed musclestrength test to analyze the lower extremity muscle activity. The above outcomes were equally remeasured,and intergroup analysis was conducted after 8 weeks of intervention. Results: The biofeedback balance training group combining taping (experimental group) showed a significantdifference in lower extremity muscle activity compared to the general biofeedback balance training group(control group) (p<0.05). Conclusion: Biofeedback training combining taping is more effective in improving lower extremity muscleactivity. Basic data on the possibility of using taping and biofeedback in the field of sports rehabilitation ofbadminton players could be provided based on this study.
