Trans-Scaphoid Perilunate Dislocation with Fractured Carpal Bones in a Child- A Case Report -

Transcarpal fractures and dislocations in children are rarely reported in the orthopedics literature. This is a case report of a 10-year-old boy who sustained a trans-scaphoid perilunate dislocation with fractures across the carpal structure: these included injuries to the capitate and triquetrum bones. Treatment consists of a closed reduction for the dislocation and using the dorsal approach, an open reduction with internal fixation of the fractures. The injury healed well with a full return of good wrist function. This unusual pattern of injury is described so that it may be more readily appreciated in the future. 소아에서 수근골 경유 골절 및 탈구는 문헌상에서 드문 보고를 보이고 있다. 이 증례 보고는 주상골 경유 월상골 주위 탈구 및 두상골과 삼각골의 골절을 동반한 10세 남자 환자의 예이다. 탈구의 도수정복과 골절에 대한 후방도달 이후 관혈적 정복과 내고정을 하였다. 골절의 유합과 탈구의 정복이 잘 유지되었으며, 수근관절의 기능도 정상으로 회복되었다. 흔하지 않은 유형의 손상에 대한 증례 보고로 향후 더 나은 치료를 기대할 수 있을 것이다.

대퇴골 전자간부 개방형 내반 절골술을 이용한 9세 이상의 LCP 병의 치료

목적: 9세 이상의 LCP 병에 대한 치료로써 대퇴골 전자간부 개방형 내반 절골술의 결과를 보고자 하였다. 대상 및 방법: 1989년 8월부터 2002년 8월까지 처음 LCP 병으로 진단된 9세 이상이며, 대퇴골 전자간부 개방형 내반 절골술로 치료받은 33명, 35예를 대상으로 하였다. 이 중 Herring 그룹 A는 3예, B는 21예, C는 11예이었으며, Waldenstrom 분류상 무혈성 괴사기는 22예, 분절기는 13예였다. 방사선학적인 결과는 Stulberg의 분류를 따랐으며, 평균 추시기간은 7.7년(범위: 3.2-14.1년)이었다. 진단 당시 평균 연령은 10.2세(범위: 9-14세)였으며, 남자가 30명, 여자가 3명이었으며 이 중 두 명이 양측성이었다. 결과: 최종 추시 검사상 Stulberg Ⅰ/Ⅱ가 10예, Ⅲ/Ⅳ가 15예, Ⅴ가 10예였다. Herring과 Catterall의 분류상병의 침범범위가 넓을수록 Stulberg의 분류상 불량한 결과를 보였으나, 범위가 적거나 초기에 발견되어 치료받은 경우에는 좋은 경과를 보였다. 결론: 9세 이상이라도 LCP병의 진행 상태 및 범위를 고려하고, 이를 개별화하여 전자간 개방형 내반 절골술을 이용하여 치료할 경우 좋은 결과를 얻을 수 있다. Purpose : To evaluate the outcome of intertrochanteric varus open wedge osteotomy in LCP in patients over 9 years old. Materials and Methods : Thirty-three patients over 9 years old, who were diagnosed with LCP were treated by intertrochanteric varus open wedge osteotomies, from August 1989 to August 2002. The mean age of the patients was 10.2 years old (range, 9 to 14 years old), with a mean duration of follow-up of 7.7 years (range, 3.2 to 14.1 years). According to the Harring classification system, there were 3 group A, 21 group B, and 11 group C patients. There were thirty male and three female patient. There were 2 cases of female patients with bilateral extremity involvement of LCP. Radiographic outcome was assessed utilizing Stulberg's classification to grade residual deformities. Results : 10 cases were Stulberg classes Ⅰ and Ⅱ (spherically congruent), 15 were classes Ⅲ and Ⅳ (aspherically congruent), and 10 were class Ⅴ (aspherically incongruent). Less involvement of the disease and treatment in the early stages produced better outcomes. Conclusion : Intertrochanteric varus open wedge osteotomy is a reliable treatment for LCP after the age of 9 years old, if the disease is evaluated individually according to the extent and stage of its involvement.
