      • 전산화 신경인지기능 프로그램(COMCOG, CNT)을 이용한 뇌졸중 환자의 기억력과 주의력 증진효과

        Purpose: The purpose of this study was to evaluate the effect of computerized neurocognitive function program on cognitive function about memory and attention with stroke. Methods: 24subjects with stroke were recruited. Twelve of subjects received conventional therapy including physical therapy, occupational therapy and language therapy. Another subjects received additional computer assisted cognitive training using Computer-aided Cognitive rehabilitation training system(COMCOG, MaxMedica Inc., 2004). All patients were assessed their cognitive function of memory and attention using Computerized Neurocognitive Function Test(CNT, MaxMedica Inc., 2004) before treatment and 6 weeks after treatment. Results: Before the treatment, two groups showed no difference in cognitive function(p>0.05). After 6 weeks, two groups showed significantly difference in digit span (forward, backward), verbal learning(A5, $A1{\sim}A5$), auditory CPT(n), visual CPT(n)(p<0.05). After treatment, the experimental group showed a significant improvement of digit span(forward, backward), verbal learning(A5, $A1{\sim}A5$), visual span (forward, backward), auditory CPT(n, sec), visual CPT(n, sec), and trail-making (A, B)(p<0.05). Conclusion: Computerized neurocognitive function program would be improved cognitive function of memory and attention in patients with stoke.

        알파파 음악과 미술 감상이 손 기능에 미치는 영향

        Purpose: The purpose of this study was to investigate the effect of $\alpha$-wave music and art appreciation on hand function. Methods: A total of 22 university students participated in this study (10 males and 12 females). Twelve subjects received $\alpha$-wave music and art appreciation. The other subjects received neither. All subjects were assessed for hand function (manual dexterity, power grip, pinch, lateral pinch, tactile sense) using a Purdue pegboard, dynamometer, pinch gauge, and Semmes-Weinstein monofilament wire. The data were analyzed using paired and independent t-tests. Results: The results were as follows: 1. In the experimental group, manual dexterity and tactile sense were significantly increased between pre- and post-intervention (p<0.05). Within the control group, manual dexterity and power grip were significantly increased between pre- and post-test (p<0.05). 2. With regard to dexterity and tactile sense, the experimental group experienced a significant post-intervention increase compared to the control group (p<0.05). There was no significant difference in power grip, pinch, or lateral pinch changes between the two groups (p>0.05). Conclusion: The results of this study show that $\alpha$-wave music and art appreciation affect hand function with regard to manual dexterity and tactile sense.

        잡기동작 수행 시 입모양에 따른 뇌활성화 분석

        Purpose:This study was performed to understand the relationship between hand and mouth shapes using functional magnetic resonance imaging(fMRI). Methods:Two healthy volunteers without any previous history of physical or neurological illness were recruited. fMRI was done that volunteers was 6 repeated of natural mouth, close mouth and open mouth while power grip and pinch grip movement. Results:Cerebral cortex activation was not well observed for the natural mouth during the power grip exercise. For the closed mouth, the temporal lobe, Broca`s area, the prefrontal area related to thinking and judgment, the supplementary motor area, the auditory area and Wernicke`s area were activated. For the open mouth, cortical activation was also observed in the temporal lobe, Wernicke`s area, the prefrontal area related to thinking and the orbital frontal area related to visual sense. During the pinch grip exercise, cortical activation was observed for the natural mouth in the primary sensory area, Wernicke`s area, the primary and supplementary motor area, and the prefrontal area. For the closed mouth, cortical activation was observed in the temporal lobe, Wernicke`s area, the prefrontal area related to thinking, the secondary visual area, the primary sensory area and the supplementary motor area. In the case of the open mouth, cortical activation was observed in a few parts in the temporal lobe as well as Wernicke`s area, the prefrontal area related to thinking, and other areas related to visual sense such as the primary visual area, the secondary visual area and the visual association area. Conclusion:Brain was more activation for close mouth and open mouth more than natural mouth movement.

        알파파 음악과 미술감상이 뇌졸중 환자의 손 기능에 미치는 영향

        Purpose:The purpose of this study was to investigate the effect of α-wave music and art appreciation on hand function in stroke with hemiplegia. Methods:A total of 32 stroke with hemiplegia participated in this study experimental group(16 subjects) received α-wave music and art appreciation with general neurologic therapy. Control group (16 subjects) received general neurologic therapy, All subjects were assessed for hand function (manual dexterity, power grip, pinch grip, two point discrimination (parm, finger), tactile sense (parm, finger) using a purdue pegboard, dynamometer, pinch gauge, two-point anethesiometer and semmes-weinstein monofilament wire, The data were analyzed using paired and independent t-test, Results:The results were as follows: 1, In the experimental group, manual dexterity were significantly increased between pre and post intervention (p<.05). 2. In the experimental group, tactile sesne in finger were sifnificantly increased between pre and post intervention (p<.05). Conclusion:The results of this study shows that α-wave music and art appreciation affect the hand function of hemiplegic side with regard to manual dexterity and tactile sense.

        감각통합치료효과에 대한 단일대상연구의 특성과 질적 수준에 대한 고찰

        목적 : 본 연구는 감각통합치료효과에 대한 단일대상연구의 특성을 파악하고 질적 수준을 알아보고자 하였다. 연구방법 : 대한작업치료학회, 국가과학기술정보센터(NDSL), 누리미디어(DBpia), 학술연구정보서비스(RISS), 한국학술정보(KISS), 국회도서관의 원문제공 서비스를 이용하여 2002년부터 2013년 까지 발간된 논문 중 ‘감각통합’, ‘단일대상연구’, ‘응용행동분석’ 을 주요검색용어를 사용하여 검색하였다. 17편의 단일대상연구 논문을 선별하여 연구방법과 연구 설계의 질적 수준을 분석하였다. 결과 : 연구 설계 방법으로는 반전설계가 가장 많았으며, 총 회기와 연구기간, 중재시간은 다양하였고 중재 장소는 작업치료실과 감각통합실에서 이루어졌다. 대상자는 지적장애가 가장 많았다. 질적 수준은 모든 연구들이 중간 수준이상으로 나타났다. 결론 : 단일대상연구의 질적 수준은 어느 정도 보장되고 있었으나 연구방법에 대한 논의와 보완이 필요한 것으로 보인다. Objective : The purpose of this study is learning the characteristics of literature applying single subject researches in the field of sensory integration therapy of Korean occupational therapy and evaluating the quality level of them. Methods : Analyzed the characteristics of 17 single subject research papers (independent variable, study design, study subject, total number of session, study period, intervention time, intervention place, dependent variable, measuring tool and result) published between 2002 and 2013 using the document delivery service of Korean Society of Occupational Therapy, National Discovery for Science Leaders (NDSL), Nuri Media (DBpia), Research Information Sharing Service (RISS), Korean Studies Information Service System (KISS) and National Assembly Library and evaluated the literatures using quality evaluation scale. Results : According to the analysis result on the literature characteristics, reversal design was the most used study method. Total number of session was 10 sessions to 34 sessions. Study period was 4 weeks to 16 weeks. Intervention time was 8 minutes to 70 minutes and most interventions were done in occupational therapy room or sensory integration therapy room. According to the result of quality level evaluation, 4 papers out of 17 papers were on high level and remaining papers were on intermediate level. However, there was no study which included intervention blind. Conclusion : Suggested standards that need to be supplemented and discussions on qualitative improvement including repetitive research on the mediation effect during the application of the single subject research methodology, the securement of adequate data sections, and processing of a mediation blind. It is expected to be used as basic data for conducting a better qualitative research in the future.

        우리나라 물리치료학과 교육과정 실태 분석

        Purpose: The purpose of this study was to investigate purpose of education, goal and curriculums of physical therapy department. Methods: It was carried out a questionnaire survey with total 74 colleges that have the department of physical therapy including the 3-year course (36 colleges) and 4-year course (38 college) from June 10 to August 7, 2010 and collected the data from total 64 colleges including 35 colleges with 4-year course and 29 colleges with 3-year course. Results: First, the purposed and objectives of the physical therapy education of the individual colleges were all excellent, but those of the colleges with 4-year course was a little higher than those of the colleges with 3-year course. Second, the evaluation of the curricula of the individual colleges showed that they have low degree of connection, continuation and integrity and the theory and practice were not harmonized, and scores were low especially in the practice. Third, we have classified the curricula of the individual colleges and found that the practice-based education was lacking among the curricula of Korea. Conclusion: In this study, the fundamental data for the establishment of the integrated and professional physical therapy education system was provided to improve the physical therapy education and the efficiency of the clinical work in the current, fast-moving status of physical therapy. Further study is necessary to present a more standardized curriculum.

        Effects of Modified Cervical Exercise on Respiratory Functions in Smartphone Users with Forward Head Posture

        Purpose: This study was conducted to investigate the effects of modified cervical exercise on respiratory functions in smartphone users with forward head posture. Methods: Thirty-three smartphone users with forward head posture participated in this study. Subjects were divided into three groups that performed modified cervical exercise one time a day (A group), two times a day (B group), and three times a day (C group). All subjects performed the exercise for four weeks, during which time respiratory functions were measured. Results: There were significant differences in respiratory functions such as forced vital capacity, forced expiratory volume at one second, and maximal voluntary ventilation in the C group after four weeks (p<0.05). Moreover, the forced vital capacity differed among groups, and the post hoc test revealed a significant difference between A group and C group (p<0.05). Conclusion: The results of this study confirmed that modified cervical exercise improved respiratory functions in smartphone users with forward head posture. These findings indicate that smartphone users with forward head posture should perform modified cervical exercise to build correct posture and respiratory functions.

        Purpose:This study investigated to find the therapeutical effects of functional weight bearing exercise on the balance and gait in stroke. Methods:The subjects of this study were 13 hemiplegia was exercised using functional weight support exercise for 5 weeks, all of whom agreed to participate in the study. All subjects were measured to see their balance and gait with a Pro-3 balance system and Gait analysis. In order to assure the statistical significance of the results, we used for SPSS 12.0 for windows. Results:The results of this study were as follows : 1) There were statistically significant difference in medial-lateral stability and overall stability index. 2) There were statistically significant in distance and gait velocity index. Conclusion:According the results of this study, functional weight bearing exercise is effect on the balance and gait for hemiplegia
