        The arrangement of anterior artificial teeth is an important factor on the esthetics in the construction of complete dentures and prostheses. During construction of the complete denture the position of maxillary central incisor plays an important role in the arrangements of artificial teeth. The proper position of maxillary central incisor provides natural appearance of anterior teeth and the anterior reference point of occlusal plane. Many methods have been utilized to eatablish the position of the maxillary central incisor in complete denture prosthodontics. However, there has not been provided for the reference for Korean yet. This study was aimed to extablish a guide for the position of the maxillary central inciosr in Koreans. The horizontal and vertical distance between the maxillary central incisor and incisive papilla in Koreans were measured. 1. The mean value of horizontal distance from the maxillary central incisor to the incisive papilla had significant sexual difference, The average distance was 12.648mm in male and 11.385mm in female. 2. The horizontal distance of the incisive papilla had sexual difference, the average distance was 6.182mm in male and 5.622mm in female. 3. The vertical distance from the maxillary central incisor to the incisive papilla had not sexual difference, the average distance was 7.21mm. 4. The crown length of the maxillary central inciosrs from the distal interdental papilla had not sexual difference, the right central incisor was 6.40mm(mean) and the left central inciosr was 6.43mm(mean). 5. The clinical crown length of the maxillary central incisors had not sexual difference, the right central incisor 10.35mm(mean) and the left central incisor was 10.43mm(mean). 6. The shapes of the incisive papilla were pear(68.4%), oval(16%), triangular(6%), irregular(5%), rectangular(4%), and inverted pear(1%) shape.

        OVERDENTURES AND OSSEOINTEGRATED IMPANTS

        저작압을 골에 직접 분산시키고 의치를 직접 골에 부착시켜 의치의 안정성을 증진시키는 가능성을 실현하는 것이 오랫동안 보철과의사들의 목표였다. 수십년 동안 골과 티타늄 임프란트가 결합(적합)되는 골융합성 술식이 발전되어 왔으며 이 임프란트의 상부구조물로 고정식 보철물이 받아들여져 왔다. 현재까지 가장 흔한 상부구조는 고정식 보철물이다. 요즈음 골융합성 임프란트에 지지를 받는 오버 덴춰의 술식이 고정식 보철물과 다른 하나의 방법으로 간주되어질 수 있게 되었다. 오버덴춰 술식이 심미적 경제적 이유 그리고 다양하게 적용할 수 있기 때문에 최근에 확대되는 경향이 있다. 골융합성 임프란트에 의한 고정식 보철물에 대한 광범위한 장기 결과들이 있는데 반하여 여러 어태치먼트를 이용한 오버덴춰 술식은 임상결과나 적용에 과한 지식들이 거의 발표되지 않았으며, 현재까지 임프란트 오버덴춰를 가진 환자의 구강기능에 대한 단지 몇개의 연구들만이 발표되었다. 우리는 장기 임상평가에 의해서 임프란트 오버덴춰를 아주 조심스럽게 평가해야 한다.

        Attachment를 이용한 후방연장 국소의치에 의한 압력분산에 관한 3차원 유한요소분석

        Although attachments have been utilized for a retainer of removable prostheses during several decades, there is little information on the force distribution by the attachments. This study was undertaken to evaluate the stress patterns developed by partial dentures employing rigid attachments on the supporting structures. Four types of the mandibular removable partial dentures were designed depending upon the position of the rigid attachment and the existency of lingual bracing. Under 100N of vertical and 25N of lateral loads on the 1st and 2nd lower molar of partial denture, stress distribution patterns and displacement were analysed with three dimensional finite element method by ANSYS version 5.3. 1. The highest stresses were concentrated on the distal alveolar crest of posterior abutment and the second stresses on the apical region of posterior abutment in the models. 2. The greatest displacement were shown on the distal alveolar crest of posterior abutment and the second displacement on the distal alveolar crest of anterior abutment in the models. 3. There was little difference between the models with intraoral attachment and those with extracoronal attachment. 4. There was little difference between the models with and without the lingual bracing.

        한국인 무치악환자의 인공치아선택방법에 관한 연구

        An esthetically pleasing result in denture service is dependent upon several consideration. Esthetic considerations for the edentulous patient include tooth size, shape, arrangement, positioning, gingival contour, age, sex, personality differences, and ethnic type. Especially, the form of anterior artificial teeth is an important factor on the esthetics. The selection of artificaial teeth requires understanding and knowledge on physical and biologic factors and has to meet the indivisul esthetics and functional needs of each patient. However, the selection of artificial teeth is based on the large degree of subjective judgement of the dentist, Therefore, this is one of the most unscientific processes. Many attempts have been made to find a guideline for the selection of artificial teeth. Temperamental theory by White and Hall utillized with the physical characteristics such as body size, body form, color of eyes and hair, and disposition. SPA theory by Frush and Fisher utilized with the basis of sex, personalities, and age of the indivisual. There has not been provided for a guideline and study on the selection of artificial teeth for Koreans yet. This study was aimed to evaluate the William's typal matching theory in Koreans. 1. The facial forms of korean adult were ovoid(46.3%), square(36.7%) and tapered(17.0%) form. 2. The anatomic forms of natural maxillary incisors were ovoid(45.3%), tapered(31.3%), square(23.3%) form. 3. The forms of face and natural maxillary incisors were ovoid(21.9%), squared(11%), tapered(6.3%). 4. The natural maxillary incisors were similar in form to the facial form only in 39%. There was no correlation between the form of the face and the form of natural maxillary central inisors in Koreans. 5. Artificaial teeth which was selected according to the typal matching theory did not represent the form of the natural teeth in 61 per cents of the fatal, but it felt that they harmonized with the form of the patient's face and produced good results.

        임플란트 직경이 골유착에 미치는 영향에 관한 연구

        Statement of problem : The survival rate of wide diameter implants was lower than of 3.75-mm implants in some clinical researches. Purpose : The purpose of this study was to investigate the influence of implant diameter on the osseointegration of implants in the rabbit femoral condyle and tibial metaphyses by means of removal torque measurements and histomorphometric analysis. Material and Method : Ten adult New Zealand White rabbits were used in this study Two 3.75-mm diameter implants were inserted through one cortical layer in the tibial metaphyses and one 3.75-mm diameter implant was inserted in the femoral condyle. 5.0-mm diameter implants were inserted in the other leg in the same manner. A total of 60 implants (3.75-mm diameter implants:30 : 5.0-mm diameter implants:30) were installed. After a healing time of 4 and 12 weeks, the peak removal torque values required to shear off the implants were recorded. From the removal torque values (Ncm) obtained, the mean shear stress ($N/mm^2$) was calculated. And the percentage of direct bone-to-implant contact and the percentage of bone area inside the thread were measured by Kappa Image Base-metreo. The Student's t-test was undertaken for statistical analysis (p<0.05). Results : The removal torque value of 5.0-mm diameter implants was higher than of 3.75-mm diameter implants (p<0.05). The difference of shear stress value between 3.75-mm and 5.0-mm diameter implants was not statistically significant (p>0.05). The percentage direct bone-to-implant contact had no statistical difference between two groups (p>0.05). The percentage of bone area inside the thread had no statistical difference between two groups (p>0.05). Conclusion It is concluded that the quality f osseointegration is not influenced by increasing implant diameter.

        강화형 간접복합레진과 치과용 합금의 결합강도에 관한 연구

        Indirect composite resins are used as an popular effective esthetic material in prosthetic dentistry, often with metallic substructure that provides support for restorations. Recently, new indirect composite resins as a substitute of ceramic have been developed. These resins provide good esthetics, with a wide range of hue and chroma. And the flexural strength of those is in the range of 120-150MPa, Which is higher than that of feldspathic Ceramic, and similar th that of Dicor. Although it has many merits, one of the major clinical problems of composite resins is the bond failure between metal and resin due to insufficient interfacial bond strength. The purpose of this study was to evaluate shear bond strength of the reinforced indirect composite resin to dental alloys. Three different composite resin systems($Artglass^{(R)},\;Sculpture^{(R)},\;Targis^{(R)}$) as test groups and ceramic($VMK\;68^{(R)}$) as control group were bonded to Ni-Cr-Be alloy($Rexillium\;III^{(R)}$) and gold alloy(Deva 4). All specimens were stored at $^37{\circ}C$ distilled water for 24 hours and the half of specimens were thermocycled 2000 times at temperature from $5^{\circ}C\;to\;60^{\circ}C$. The shear bond strengths of reinforced indirect composite resins to dental alloys were measured by using the universal testing machine, and modes of debonding were observed by stereoscope and scanning electron microscope. The results were as follows: 1 The shear bond strengths of reinforced indirect composite resins to dental alloys were approximately half those of ceramic to dental alloys(P<0.01). 2. There was no significant difference between the shear bond strength of several reinforced indirect composite resins to metal. 3. Alloy type did not affect on the shear bond strengths of resin to metal, but the shear bond strengths of ceramic to gold alloys were higher than those of ceramic to Ni-Cr alloys(P<0.05). 4. The shear bond strengths of Artglass and Targil to gold alloys were significantly decreased after thermocycling treatment(P<0.01). 5. Sculpture showed cohesive, adhesive, and mixed failure modes, but Artglass and Targis showed adhesive or mixed failures. And ceramic showed cohesive and mixed failures.

        섬유강화형 복합레진전장관의 변연적합도 및 변연누출에 관한 연구

        As Fiber-reinforced composite restorations cannot be made without leaving a marginal gap, luting cements play a pivotal role in sealing the margins as a prevention against margnal leakage. A recently introduced adhesive resin cement system is claimed to adhere chemically, as well as mechanically, to tooth substances, dental alloys and porcelain. But when considering the clinical variation conventional cementation using Zinc Phosphate and Glass-Ionomer can be requested. A vitro study was undertaken to compare microleakage and marginal fitness of Fiber-reinforced composite crowns(Targis/Vectris) depending upon luting cements. Fifty non-carious human premolar teeth were randomly divided into five experimental groups of 10 teeth each and luted with five luting cements. ($Bistite\;II^(R),\;Super-bond^(R),\;Variolink\;II^(R)$), Zinc phosphate and Glass-Ionomer cement) After 24 hours of being luted, all specimens were thermocycled 300 times through water bath of $5^{\circ}C\;and\;55^{\circ}C$ in each bath, then the quality of the marginal fitness was measured by the Digital Microscope and marginal leakage was characterized using Dye Penetration technique and the Digital Microscope The results were as follows : 1. The mean values of marginal fit were Bistite II($46.78{\mu}m$), Variolink II($56.25{\mu}m$), Super-Bond($56.78{\mu}m$), Glass-Ionomer($99.21{\mu}m$), Zinc Phosphate($109.49{\mu}m$) indicated a statistically significant difference at p<0.001. 2. The mean microleakage values of tooth-cement interface, restoration-cement interface were increased in the order of Variolink II, Bistite II, Super-Bond, Glass-Ionomer, Zinc Phosphate 3. Crowns luted with resin cement (Bistite II, Super-Bond, Variolink II, etc) exhibited less marginal gap and marginal leakage than those luted with conventional Glass-Ionomer and Zinc Phosphate cement. 4. The results indicated that all five luting systems yielded comparable and acceptable marginal fit.

        교합평면 설정에 관한 연구

        It is a very important procedure to establish the occlusal plane in the construction of complete denture. So many methods have been utilized to establish the occlusal plane in complete denture prosthodontics. However, no single method seems to fully accepted. This study was aimed to review the literature on establishing the occlusal plane in complete denture prosthodontics, to measure the distance from the lower border of the upper lip to the upper incisal edge and to investigate the correlation between the ala-tragus line and the occlusal plane. The results ware as follows ; 1. The average distance between the lower border of the upper lip and the upper incisal edge was $1.45{\pm}1.28mm$. 2. The distance between the lower border of the upper lip and the upper incisal edge had a tendency to decrease with age. 3. A angle of the ala-tragus line to the occlusal plane measured with Fox plane was $-1.41{\pm}2.33^{\circ}$. 4. The ala-tragus line to occlusal plane was nearly parallel, cosidering curve of spee in the upper natural teeth.

        표면처리가 다른 5종 임플랜트의 안정성에 관한 연구

        Purpose. The purpose of this study was to compare the effects of various surface treatments by measuring resonance frequency and histomorphometric analyses. Material and methods. In 5 adult dogs, the mandibular premolar were extracted. Six months later, 30 screw titanium implants (Dentium Co., Seoul, Korea) 6mm in length and 3.4mm in diameter, were placed in the mandibles of 5 dogs. Implants were divided into five groups following to surface treatment methods ; Group 1 is machined controls, Group 2 is sandblasted with large grit and acid-etched (SLA), Group 3 is anodized (Autoelectric Co., Korea, 660Hz, Duty10), Group 4 is hydroxyapatite(HA) coated by ion beam assisted deposition(E-beam), Group 5 is hydroxyapatite(HA) coated with Sol-gel coating process. Resonance frequency was measured implant placement immediately, and 3, 6 weeks and 10 weeks of healing perods. With the animal subject's sacrifice 10 weeks after implantation, implants were removed on bloc and histologic and computer-based histomorphometric analyses were performed. Histomorphometric analysis involved quantification of the entire bone to metal contact around the implants. Statistical analyses were performed using the SPSS for Windows (ver. 9.0 SPSS Inc.) Statistical differences were considered significant at P<0.05. Results. The results were as follows : 1) In five groups, mean value of resonance frequency analysis(RFA) were highest in group 5 (Sol-gel implant) at implantation and those of group 4 (E-beam)was highest at 10 weeks . but there was no correlation between surface treatments and RFA. 2) In all surface treatment groups, the RFA values of implants decreased until 3 weeks and increased to 10 weeks. 3) The percentage of direct bone-to-implant contact (BIC) had statistical significance between five groups in cancellous bone, (P<0.05) the percentage of bone density inside the thread had no statistical significance between five groups. (P>0.05) 4) There was a significant difference between cortical bone and cancellous bone in BIC. (P<0.05) and bone density. (P<0.05) 5) There was a correlation between the RFA value of implants at 10 weeks and BIC in cancellous bone, and between the RFA value of implants at 10 weeks and bone density in cortical bone. (P<0.05). Conclusions. These results indicate that surface treatment does not affect the implant stability in case of good bone quality.
