        Accuracy in the Interface between Implant Components after a 5-year Functional Period; A Clinical Report

        The reduction in the assembly accuracy of the implant complex could result from fatigue and micromovement during the functional phases and from the manufacturing process. The long-term functional integrity of implant prostheses can be ensured by accurate fit of the interfaces between the assemblies from a biomechanical perspective. A singleimplant restoration on the mandibular left first molar was removed after a 5-year functional period due to mobility, which was diagnosed as abutment screw loosening. The superstructure could not be retrieved, despite exceeding the torque value recommended by the manufacturer; therefore, the restoration was unscrewed and removed. The retrieved restoration, particularly the configuration of the abutment-gold cylinder and abutment-retaining screw interfaces, was analyzed with scanning electron microscopy. Moreover, the assembly accuracy of the old and new assemblies was compared. In the new specimen, a relatively an even linear gap was found between the threads of the abutment and screw and no vertical marginal gaps were found at the abutment-cylinder interface, wherein the horizontal and vertical contacts were incomplete. In the old specimen, an irregular pattern of screw-thread engagement, an irregular linear space at the abutment-cylinder interface, a partially worn-out screw configuration and remnants of the screw in the interface were observed.

        Dental trauma occurs mainly in maxillary anterior in children and adolescents. Because the jaw and teeth grow in adolescent and maxillary anterior teeth are aesthetically, psychologically very important, careful consideration is required in the choice of how tooth fracture repair. Tooth fragment reattachment is a more conservative method than composite resin restoration or prosthetic treatment. It was temporarily repaired because of discoloration and falling out of restoration in past, but recently many successful case have been reported with a brilliant development of dentin bonding systems and composite resins. So we reported a case who successfully underwent reattachment of tooth fragments in adolescent patient who was diagnosed with wide complicated crown fracture.

        하악 잔존 임플란트를 이용한 가철성 국소의치 수복 증례

        In a partially edentulous patient with a few remaining implants due to failure on multiple number of implants, implantassisted removable partial dentures (IARPDs) using remaining implants can be considered as an alternative treatment when further implant surgery is not available. IARPDs can be categorized as an overdenture type using attachment and a conventional removable partial denture type using surveyed crown. When the position of remaining implants is not ideal, it is known that the IARPDs using surveyed crown provides the ability to correct discrepancies and makes it easy to manufacture and maintain dentures although the evidence is still lacking. In this case, an 81-year-old female patient, the maxilla was restored with complete denture, and the mandible with IARPD using surveyed bridge on #i41i31=i33 and surveyed crown on #i36. She restored her masticatory function and was esthetically satisfied. Periodic follow-up after 3 months has been performed since the IARPD delivery to the patient. No sign of screw loosening of implants was detected. According to the reported design of IARPDs, it is recommended to place implants as an abutment of RPD in a symmetrical and strategic position to improve support, stability and retention of RPD. However, in this case, since the unilateral residual implant (#i36) was used as an abutment, there may be concerns about the lateral force applied to the implant due to rotation of the denture during mastication and deterioration of the stability of the denture. In order to compensate for these limitations, we intend to investigate considerations when manufacturing IARPDs.

        Warfarin 복용 환자의 임플란트를 이용한 전악 회복: 증례 보고

        When a patient with systemic disease needs complex treatment, a good prognosis can be expected if sequential treatment is performed based on evidence. For patients taking anticoagulants such as warfarin, it is very important to establish an oral rehabilitation that takes into account drug control and treatment intervals. In addition, in patients with lost posterior support, oral rehabilitation should aim to restore the existing vertical dimension as much as possible, and for this, it is necessary to apply and confirm provisional prostheses of the patient. In this case report, the patient was taking warfarin for mitral valve replacement surgery and atrial fibrillation 20 years ago. Full mouth rehabilitation with fixed implant prostheses was done, and 20 months of follow-up showed successful results.

        단국대학교 치과병원에 내원한 약물유해반응 경험이 있는 환자에 대한 1년간의 동향 보고

        Among adult patients over the age of 18 who visited Dankook University Dental Hospital for one year from March, 21, to February, 22, the chart was searched for 8641 patients who visited for the first time. 61 patients answered that they had a history of adverse drug reactions . the proportion of adverse drug reactions patients by age, and allergens and symptoms were investigated. Among the patients who came to the hospital for the first time, 41 women and 20 men responded that they had a history of adverse drug reactions, with females at a rate more than twice that of males. The ratio by age group was highest in the 30s (1.21%). According to the statistical data by allergen, the proportion of patients who answered that they had a history of adverse drug reactions to penicillin-based antibiotics was the highest (23.1%), and they responded that they experienced local skin redness and swelling, urticaria, bronchial stenosis, and laryngeal edema as related symptoms. As a result of this study, most of the adverse drug reactions were found to have a high ratio to NSAIDs (Non-steroidal anti- inflammation drugs) and antibiotics, which are the most frequently prescribed drugs for dental treatment. It was confirmed that moderate or high-risk symptoms such as respiratory distress and anaphylaxis were more likely to occur more frequently when allergic to nonsteroidal anti-inflammatory drugs. Therefore, a thorough detailed questionnaire for patients with a history of adverse drug reactions should be preceded prior to drug prescription during dental treatment.

        원심경사를 동반한 제3 하악대구치 발치로 야기된 설신경 감각저하: 증례 보고와 문헌 고찰

        Extraction of the mandibular third molar is a frequently performed dental treatment, but since various complications can occur, it is necessary to evaluate the difficulty of extraction before treatment and proceed with careful treatment. The lingual nerve is responsible for the sensation of the anterior 1/3 of the tongue and the lingual soft tissue of the mandibular molars. The frequency of hypoesthesia of lingual nerve after extraction is lower than that of the inferior alveolar nerve, but recovery is slower due to the difference in anatomical structure and location from that of the lower alveolar nerve. The inclination of third molars is classified into mesioangulation, distoangulation, vertical, and horizontal. It is known that extraction of molars with distoangulation is difficult and may increase the probability of hypoesthesia. We report a case of hypoesthesia of lingual nerve after extraction disto-angular lower third molar, and review the literatures on the characteristic of third molar of distoangulation and potential risk factors causing hypoesthesia of lingual nerve.

        Guide Surgery Using the Dual Optical Scan Method in a Patient with Distal Extension Base: A Case Report

        Implant-guided surgery using a surgical template was introduced for prosthetic-driven implant placement. In the case of a partially edentulous patient, a guide is created to fit the patient's tissue by overlaying the cone-beam computed tomography (CBCT) scan data on the optical scan data obtained by scanning the model or actual oral cavity. In the case of edentulous patients, because there are no teeth, a radiographic denture is manufactured, and the 'dual CBCT scan method’ is used. In this case report, a guide was created using a 'dual optical scan method' with two optical scan models because the CBCT and model scan data only overlapped with one tooth and one healing abutment. CBCT was taken after surgery to compare and analyze the deviation from the planned position, and errors within the average were recorded, except for the rearmost implant.

        단일 내원 근관 치료의 예후

        Traditionally, root canal treatment in multiple visits has been accepted and predictable protocol. In present, the benefits of single visit endodontic treatment have been proposed and many clinicians have considered it. The purpose of this review article is to find out evidence of single visit root canal treatment by discussing previous articles. In this article, it would discuss the outcome between single visit and multiple visits endodontic treatment as post-operative pain, flare-up and healing of periapical tissue. Overall, there are not significant different post-operative complication between single- and multiple- visit treatment. But the case of apical periodontitis and endodontic retreatment is still controversial.

        치근단 방선균증: 증례보고와 문헌고찰

        Actinomycetes is a type of normal inhabitants of the oral cavity and is an anaerobic bacteria mainly distributed on the root surface in contact with the gingival sulcus. A. israelii, a gram-positive bacterium, is normally present in carious teeth and calculus with the highest frequency. In the environment such as traumatic injury, apical infection, and pericoronitis that occurs around a partially impacted tooth, the nature changes to infectivity and causes an acute inflammatory response. For the treatment of actinomycosis, large doses of penicillin are administered for at least 10 days after surgical drainage and debridement. The apical lesions on radiographs can sometimes appear as malignant lesions, so differentiation is necessary and biopsy is essential. Therefore, in this classroom, actinomycosis was diagnosed and reported in the biopsy of the apical lesion of a tooth that had been extracted due to severe dental caries, and the literature review was conducted.

        발치 중 상악동으로의 치근 전위: 증례보고

        The immediate relationship between the maxillary molar root tip and the maxillary sinus floor is the cause of displacement of the root tip into the maxillary sinus. Moreover, sometimes the molar roots are widely separated, resulting in a high rate of root fracture during extraction. Accidently root fragments could be displaced into the maxillary sinus and when happened, retrieval of the root should be done urgently because of several complications such as oroantral communications, sinusitis, antrolith, and mucocele. In this case report, we introduce the root tips that have displaced into the maxillary sinus. In addition, we would like to discuss some considerations about some signs such as root projection into the maxillary sinus or darkening sign on the root apex that the surgeon can prevent displacement of root tip into the maxillary sinus before extraction.
