The 'sinus pericranii" is a rare vascular anomaly involving an abnormal communication between extracranial and intracranial venous system, which usually involves the superior sagittal sinus. sometimes the transverse sinus. It is found beneath or in the periosteum of the cranial vault and is connected with an intracranial sinus through anomalous diploic veins of congenital or acquired origin. It is a benign scalp mass, and should be differentiated from vascular anomalies of the scalp and cystic masses of the scalp communicating with the cerebrospinal space. The authors present 2 cases of sinus pericranii which was spontaneously developed without any history of head injury. The lesion was found to be changed in it's volume according to patient's head position. Clinical characteristics of sinus pericranii is discussed.

Recent advances in neurosurgical field have markedly reduced the mortality and morbidity after skull base surgery. Despite of new skills of skull base surgery, the complication rates remain high. Postoperative cerebral infarction is a main cause of complication. Although arterial compromise is responsible for a majority of these events, the venous side of the circulation can also play a role in producing cerebral infarction. One of the key areas of cerebral venous drainage is at the junction of the transverse sinus and vein of Labbe. We have analyzed 50 cases of cerebral angiograms performed to rule out several vascular diseases. The results are as follows : 1) The selected carotid artery influences opacification patterns of the transverse sinus. 2) The incidence of transverse sinus atresia is about 14%. 3) Anterior and posterior circulation may have different opacification patterns of the transverse sinus. We concluded that neurosurgeons must perform complete 4-vessel angiography and analyze various opacification patterns of the transverse sinus and the vein of Labbe to prevent venous infarction after skull base surgery.

Through the use of stereotactic neurosurgical operation, some authors have employed chronic electrical stimulation of the dentate nucleus reduce spasticity and each trial on spastic patients has show marked clinical improvement. We investigated the eletrophysiological changes of cats after stereotactic electrical stimulation of the dentate nucleus to elucidate the possibility of clinical application and to evaluate the clinical results The M-wave is an initial response and the F-wave is a late response in the electromyography of a muscle, which is stimulated by its original controlling nerve itself. There is evidence that the sue of the F-wave is dependent on motor neuron excitability and its amplitude is increased significantly at spastic condition. If such a relationship exists, procedures which are cam& out to relieve spasticity might be exerted to change the F-wave sue and thus it might be possible to use the F-wave as an objective monitor during electrical stimulation of the dentate nucleus. We investigated this possibility in experiments on 10 cats weighing betHieen 27kg and 4.4kg. We studied the change% of M and F waves, recorded in EMG, after stereotactic stimulation to cerebellar dentate nucleus in cats. The results were as follows 1) The change of mean value of M-wave amplitude was not significant(contro1 group, 3591 ± 1029μV stimulation group, 3424± 927μV, 00.05). 2) The mean value of F-wave amplitude was significantly reduced about 56.7% after the dentate stimulation(contro1 p u p , 443.2± 119μV stimulation p u p , 251.3±99.4μV, P<0.05). 3) The F/M ratio also significantly reduced about 56.8% after the dentate stimulation(contro1 group, 12.5±1.9%, stimulation group, 7.1± 1.2%, P<0.05). Our experimental results demonstrated that the dentate stimulation markedly decreased the sue of F-wave amplitude and F/M ratio in the experimental cats and we concluded that theseelectrophysiological changes can be applied as a parameter of clinical evaluation of electrical dentate stimulation for the spasticity.
다지역산업연관모형을 이용한 식품산업의 지역간 생산 및 소비 연계성 분석

The purpose of this study is to analyze the structure of inter-regional trade and the inter-regional linkages in production and demand of food industry. We estimated inter-regional demand-demand multipliers as well as inter-regional demand-output multipliers in food industry, and investigated potential complementarities in production and demand of regional food industry. Our analysis shows that inter-regional trade linkages between Seoul metropolitan area and other regions in food industry is much more stronger than those of other regions. The analysis on production multiplier effects shows that Seoul metropolitan area has the largest intra-regional production multiplier effect and Kangwon demonstrates larger inter-regional production multiplier effect. We also found that Seoul metropolitan area and Jeolla region has relatively high inter-regional complementarities in production with many other regions. Seoul metropolitan area also shows high inter-regional complementarities in consumption with many other regions such as Chungcheong, Jeolla, Gyoungnam. Besides Seoul metropolitan area, inter-regional complementarities in consumption between Gyoungnam and Gyoungbuk are also found to be significant.

We report the unusual case of multiple intraorbital and intracranial germinoma primarily arising from optic nerve, pineal gland extending to the splenium of corpus callosum. The patient was 27-year-old male who presented with progressinve bilateral visual loss for 2yrs and suddenly developed right painful exothalmus. The neuroimaging studies disclosed tumors in the right optic nerve, optic chiasm, pineal gland and posterior corpus callosum. The tumor markers of serum and cerebrospinal fliud were all negative. The intraorbital and intracranial optic nerve tumor was removed via fronto-orbitotomy and pterional approach. The histological diagnosis was germinoma. Follow up brain MRI demonstrated complete disappearance of the multiple masses after completion of three courses of combinant chemotheraphy with Ifosfamide-Cisplatin-Etoposide. This case is believed to be the first case report as multiple intraorbital and intracranial primary germinoma.

Objectives : An in vitro biomechanical study of posterior lumbar interbody fusion(PLIF) with threaded cage using two different approaches was performed on eighteen functional spinal units of bovine lumbar spines. The purpose of this study was to compare the segmental stiffnesses among PLIF with one long posterolateral cage, PLIF with one long posterolateral cage and simultaneous facet joint fixation, and PLIF with two posterior cages. Methods : Eighteen bovine lumbar functional spinal units were divided into three groups. All specimens were tested intact and with cage insertion. Group 1(n=12) had a long threaded cage($15{\times}36mm$) inserted posterolaterally and oriented counter anterolaterally on the left side by posterior approach with left unilateral facetectomy. Group 2(n=6) had two regular length cages($15{\times}24mm$) inserted posteriorly with bilateral facetectomy. Six specimens from group 1 were then retested after unilateral facet joint screw fixation in neutral(group 3). Likewise, the other six specimens from group 1 were retested after fixation with a facet joint screw in an extended position(group 4). Nondestructive tests were performed in pure compression, flexion, extension, lateral bending, and torsion. Results : PLIF with a single cage, group 1, had a significantly higher stiffnesses than PLIF with two cages, group 2, in left and right torsion(p<0.05). Group 1 showed higher stiffness values than group 2 in pure compression, flexion, left and right bending but were not significantly different. Group 3 showed a significant increase in stiffness in comparison to group 1 for pure compression, extension, left bending and right torsion(p<0.05). For group 4, the stiffness significantly increased in comparison to group 1 for extension, flexion and right torsion(p<0.05). Although there was no significant difference between groups 3 and 4, group 4 had increased stiffness in extension, flexion, right bending and torsion. Conclusion : Posterior lumbar interbody fusion with a single long threaded cage inserted posterolaterally with unilateral facetectomy enables sufficient decompression while maintaining a majority of the posterior elements. In combination with a facet joint screw fixation, adequate postoperative stability can be achieved. We suggest that posterolateral insertion of a long threaded cage is biomechanically an ideal alternative to PLIF.
