        During the immediate postoperative period following cataract surgery, the intraocular pressure (IOP) often rises significantly to impede the ocular circulation and threaten the visual function of the optic disc in some patients with vulnerable ocular disease such as advanced glaucomas. Some kind of ocular hypotensives, acetazolamide and beta-adrenergic blockers were reported to be effective for preventing or aborting the immediate postoperative IOP rise. Other ocular hypotensives, pilocarpine and carbachol are used for the purpose of constricting the pupil at the end of cataract surgery. However, they were not evaluated to prevent the postoperative IOP rise after uneventful extracapsular cataract extraction and posterior chamber lens implantation. Fifty-three eyes were randomly assigned to the three groups: group 1 as a control group consisting of 24 eyes in which no ocular hypotensives were administered; group 2 consisting of 16 eyes in which pilocarpine 4% was instillated at the end of surgery; group 3 of 14 eyes in which 0.1ml of carbachol 0.01% was infused into the anterior chamber at the end of the surgery. A slit-lamp examination and applanation tonometry were done at 6 hours, day 1, 2, 3, week 1, 2, the first and second month after operation. Preoperative baseline IOP means were not different between three groups(P>0.05). An average postoperative 6-hour mean IOP were lower than those baseline IOP in group 2 and 3 by 1.8 ±3.36mmHg and 0.6±4m3mmHg respectively although these were not statistically significant(P<0.05). Mean IOPs measured at other postoperative periods were not different between three groups and did not increased comparing with the baseline mean IOP(P>0.05). Therefore a single administration of pilocarpine 4% or carbachol 0.01% was found to be effective to prevent the immediate postoperative IOP rise.

        국가직무능력표준(NCS)에 의한 교과과정 개발에 관한 연구

        새로운 패러다임의 문화적 전환기를 통해 사회에서는 지식과 기술의 융,복합을 통한 새로운 4차 혁명의 시대를 맞고 있으며, 학문 분야에서는 지식의 통합과 융합이 강조되는 시대를 맞이하고 있다. 최근 교육정책의 핵심적인 기조는 산업 현장에서 필요로 하는 맞춤형 인력을 양성하는 데 있다. 2016년 1월에 발표한 교육부의 업무보고에 따르면, ‘사회수요 맞춤형 인력 양성 및 일학습 병행’을 상위목표로 첫째는 사회수요 맞춤형 대학교육 강화, 둘째로 일학습 병행제 및 선취업, 후진학 확산을 하위 목표로 설정하였다. 우리나라의 주요 인적자원개발 시스템에는 자격제도와 교육훈련제도가 있지만, 고등학교와 대학의 교육을 받고 취업한 신규 인력은 기업에 입사한 후 많은 비용과 시간 그리고 노력을 투자하여 재교육을 받고 있는 것이 현실이다. 정부의 국가직무능력표준(NCS) 추진배경에는 첫째, 산업사회의 수용에 부응하는 지식과 기술변화에 능동적으로 대응하는 인력개발의 필요성, 둘째, 직업훈련과정과 자격제도가 직업에 필요한 지식, 기술, 태도를 충분히 반영하지 못한다는 지적으로 효율적인 운영을 위한 국가적인 직무 수행 틀이 필요하며, 셋째, 교육시장과 노동시장의 미스매치(mismatch)를 해결하는 국가적 차원의 인력자원개발 시스템이 필요하기 때문이다. 따라서 교육과학기술부와 고용노동부는 현장과 밀접한 교육을 유도하기 위해 국가직무능력표준(NCS) 개발 사업을 통해 교육훈련기관들이 현장적합성 높은 교육훈련과정을 운영하도록 권장하고 있다. 본 연구가 갖는 의미는 대부분 전문대학교에서 이루어지는 NCS 교과 과정을 K대학교 글로벌 MICE 연계전공을 통해 개발 하면서 생길 수 있는 문제점을 찾고자 하는데 있다. 또한 다른 대학교와의 차별화를 위해 인문학을 기초로 한 산학 연계전공 개설을 통해 경쟁력을 높이며, 교과과정 개발 시 산업 현장 전문가와 교육훈련 전문가가 함께 참여하여 다양한 SME(Subject Matter Expert)의 특강을 통해 융합교과목 운영을 보완하였다. 전략적 과제 도출을 통해 인력양성 및 현장 실무형 성과 도출에도 많은 도움이 될 것으로 사료된다. 향후 본 연구를 바탕으로 보다 다양한 융합교과목에 대한 후속 연구와 기존 강의와 NCS 강의와의 차이점을 비교하는 후속 연구가 진행되어야 할 것이다. Through the cultural turning point of a new paradigm, the society is facing the era of the new 4th Revolution through the fusion of knowledge and technology. In the field of academia, the integration and convergence of knowledge is emphasized. The key focus of the recent education policy is to cultivate customized manpower needed in the industrial field. According to the report of the Ministry of Education, Industry and Energy in January 2016, the superordinate goal is "training manpower customized to social demands and combining work and study.’ First subgoal is to reinforce college training customized to social demands, and second is to spread job training, and ‘employment first, college later.’ Although there are qualifications systems and education and training systems in Korea"s major human resource development systems, the reality is that new employees who have received high school and university education are being reeducated after joining a company, investing a lot of money, time and effort. The background of National Competency Standard(NCS) of Korea consists three: first, the necessity of manpower development to actively cope with changes in knowledge and technology in response to the acceptance of the industrial society; second, the need for a national working framework for efficient and intellectual operation that vocational training courses and qualifications systems do not fully reflect the knowledge, skills and attitudes required for the job; third, the need for a nation-wide human resource development system that solves the mismatch between the education market and the labor market. Therefore, the Ministry of Education, Science and Technology and the Ministry of Employment and Labor encourage education and training institutions to carry out highly site compatible courses through the NCS development projects to induce training sessions close to the field. The significance of this research is to find out the problems that may arise when developing the NCS curriculum, on most colleges, through K-University’s Global MICE interdisciplinary program. In order to differentiate itself from other universities, K-University’s Global MICE enhanced competitiveness through the establishment of an industry-university-related specialty based on humanities. In addition, industry field experts and education and training experts participated in the development of curriculum, and complemented the operation of convergence curriculums through special lectures of SME(Subject Matter Expert). It is expected to be of use to the training of manpower and delivering results on field work through strategic assignments. Future studies should be conducted to implement diverse interdisciplinary subjects and compare the differences between existing lectures and NCS lectures.

      • e-PTFE막과 실리콘 관을 이용한 녹내장 수술

        A new glaucoma drainage implant, silicone tube attached to an expanded polytetrafluoroethylene (e-PTFE) membrane, was used in patients with recalcitrant glaucomas. Aqueous was shunted via the silicone tube to reservoir which was formed under the posterior Tenon's capsule by an oval piece of e-PTFE memberane, (Japan Gore-tex Inc., Tokyo) was 0.1mm thick, malleable and foldable. Therefore, the e-PTFE membrane of the implant could be introduced through a small incision of a quadrant conjunctiva in folded form and then spreaded over the sclera. The implant had been placed in 18 eyes of 17 patients with one or more unsuccessful glaucoma filtration surgeries including three neovascular glaucoma (NVG) patients. After follow-up of peroids ranging between seven and 18 months (mean: 10 months), 15 of the 18 eyes (83%) had controlled intraocular pressure (IOP) using 20 mmHg as normal. Three patients were not on any glaucoma medication. Seven patients required one β-blocker. Five patients were on one β-blocker and pilocarpine or a small dose of acetazolamide on their final visits. Three eyes with an unsuccessful surgery had an IOP over 25 mmHg in spite of maximum tolerated medical therapy. A postequatorial filtration bleb was found in each operated eye following removal of 5-0 cat-gut threaded through the silicone tube for the prevention of aqueous overdrainage. Postoperative complications included hyphema and fibrinous aqueous in the eyes with NVG(17%), prolonged shallow chamber(22%), obstruction of the silicone tube in the anterior chamber(6%), and choroidal datachment(6%) silicone tube in the anterior chamber (6%), and choroidal detachment(6%). The required conjuctival incision for the installation of the e-PTFE drainage implant was less than 90° and the overall surgical procedures seemed to be simple.

        Endophthalrnitis is a rare but potentially devastating complication of intraocular surgeries and penetrating injury. We experienced three consecutive cases of pseudomonas endophthalmitis and performed vitrectomy combined with intravitreal antibiotics. Repetitive intraocular antibiotic injections or vitrectomies were performed in these cases, but all had loss of vision. Vitrectomy with intraocular antibiotic injections for endophthalmitis has been known to eliminate the intraocular infection effectively. But pseudomonas aeruginosa are extremely difficult to eradicate and resistant to antibiotics. An earlier total vitrectomy rather than a partial vitretomy and repetitive intravitreal antibiotics would be recommended particularly for pseudomonas endophthalmitis
