        간경변증 환자에서 t - PA , Euglobulin 섬유소용해능 및 Protein C , S 의 변화와 식도정맥류출혈과의 관계

        N/A Background: Bleeding is common complication and a leading cause of death in cirrhotic patients, Accelerated fibrinolysis and coagulation inhibitor were contributing factors to bleeding. Method: In a study of 20 normal control and 20 liver cirrhosis (10 liver cirrhosis without esophageal variceal bleeding and 10 liver cirrhosis with esophageal variceal bleeding), we tried to evaluate fibrinolytic activity 8r inhibitory factors of the coagulation in liver cirrhosis and to find correlation between variceal bleeding and parameters of fibrinolysis % coagulation inhibitors. Results: 1) t-PA antigen was significantly increased(p <0.05) in patients with liver cirrhosis(22.2±12.5ng/ml) than in normal contro1(3.9±1.9ng/ml), but there was no statistically significant increase in cirrhotic patients with bleeding(26.2±13.8ng/ml) as compared to cirrhotic patients without bleeding(18.2±10.1ng/ml). 2) Euglobulin fibrinolytic activity was significantly higher(p<0,05) in patients with liver cirrhosis(145±37.6 BAU) than in normal control(91±7.8 BAU), but there was no statistically significant increase in cirrhotic patients with bleeding(150±64 BAU)as compared to cirrhotic patients without bleeding(131±22 BAU). 3) The activities of protein C was significantly lower(p<0.05) in patient with bleeding(41±9%) than in patient siwthout bleeding(54±18%). 4) The activities of protein S was lower in patient with bleeding(62±18%) than in patients without bleeding group(77±17%), but there was no statistical significance. 5) There was a significantly linear correlatian between t-PA antigen and euglobulin fibrinolytic activity in normal control and in patients with liver cirrhosis (r=0.807, p<0.01). Conclusion: Fibrinolytic activity(t-PA k euglobulin fibrinolytic activity) was increased in cirrhotic patients, but there was no statistically significant increase of fibrinolytic activity with relation to esophageal variceal bleeding. The activities of protein C was significantly decreased in liver cirrhosis with relation to esophageal variceal bleeding.

        B 형 급성간염에서 IgM Anti-HBc 의 진단적 의의

        To investigate the diagnostic significance of IgM anti-HBc in acute hepatitis B, IgM anti-HBc was measured by radioimmunoassay in 41 pateitns with acute hepatitis B, 18 patients with chronic hepatitis, 29 patients with liver cirrhosis, 1S patients with primary hepatoma, and 23 healthy carriers. The results obtained were as follows; 1) Distribution of Hepatitis B virus markers in acute hepatitis B Of the 41 patients with acute hepatitis B, 29 patients (70.8%) were seropositive for HBsAg and IgM anti-HBc, 6 patients (14.6%) were seropositive for HBsAg and seronegative for IgM anti-HBc, 5 patients (12.2%) were seronegative for HBsAg and serapositive for IgM anti-HBc, and 1 patient (2.4%) was seronegative for HBsAg and IgM anti-HBc. 2) IgM anti-HBc in acute hepatitis B Of the 41 patients with acute hepatitis B, 34 patients (82.9%) were IgM anti-HBc positive. IgM anti-HBc was tested in 33 patients with positive IgM anti-HBc within 2 weeks after onset of jaundice, and tested in 7 patients with negative IgM anti-HBc more than 4 weeks after onset of jaundice. IgM anti-HBc titer decreased gradually with time in 12 patients with positive IgM anti-HBc in whom the follow up evaluation was available. In 3 patients, IgM anti-HBc became negative seroconversion at 4 weeks after onset of jaundice, In 3 patients, IgM anti-HBc became negative seroconversion at 9 weeks, 11 weeks, and 20 weeks after onset of jaundice, respectively. IgM anti-HBc was more frequently detected in acute hepatitis B compared with chronic liver diseases (p$lt;0.05) and 23 of 41 patients (56.1%) with acute hepatitis B had P/N ratio over 4. 3) IgM anti-HBc in chronic liver disease IgM anti-HBc was postitive in 2 of 1S patients (11.1%) with chronic hepatitis B, 5 of 29 patients (17.2%) with liver cirrhosis, 3 of 18 patients (~26.7%) with hepatoma, and 1 of 23 (4.3%) healthy carriers. Among these, only 2 patients (7%) with liver cirrhosis had P/N ratio over 4. 4) The relationship between HBeAg and IgM anti-HBc in acute hepatitis B There was no significant difference in positivity and P/N ratio of IgM anti-HBc between HBeAg positive and negative patients with acute hepatitis B(p$lt;0.1), In conclusion, IgM anti-HBc testing can aid in the diagnosis of acute hepatitis 8. But if IgM anti-HBc testing is performed more than 4 weeks after onset of jaundice, it could be negative in acute hepatitis B. And it should be careful to diagnose acute hepatitis B because there is false negative results on conventional radioimmunoassay in acute hepatitis B.

      • 기니픽 대뇌 피질에서 [^3H]norepinephrine 유리에 대한 오피오이드 수용체의 상호작용

        포유류의 뇌에 존재하는 opioid수용체의 종류에는 μ, δ 그리고 k의 3가지 주된 형이 있으며 그 기능을 발현함에 있어서 상호작용을 하고 있음을 시사하는 많은 증거가 제시되고 있다. 본 연구에서는 [³H]NE 유리 조절에 있어서 서로 다른 opioid 수용체가 관여하는 기니픽과 백서의 대뇌피질 절편에서 prototype μ-agonist인 morphine 고선택적 μ-agonist DAMGO 그리고 k₁-agonist인 U69,593의 [³H]NE 유리억제효과에 대한 δ ₁ 수용체, agonist인 DPDPE 그리고 δ₂수용체 agonist인 deltorphin Ⅱ의 영향을 검토하여 opioid 수용체 상호작용을 시험관내 실험으로써 증명하고, 이를 매개하는 δ 수용체의 subtype을 규명코자 하였다. 본 실험에서 morphine, DAMGO 및 U69,593은 기니픽 대뇌피질절편에서 고칼륨에 의한 [³H]NE 의 유리를 용량의존적으로 억제하였으며, 자체로써 효과를 보이지 않는 농도의 DPDPE또는 DLTP에 의하여 유의하게 강화되었다. 그 강화효과의 정도는 U69,593, morphine 그리고 DAMGO의 순이었다. Morphine 또는 U69,593의 [³H]NE 유리억제효과에 대한 강화작용은 DPDPE보다 DLTP이 더욱 강력하였다. 그러나 백서에서는 기니픽의 경우와 달리 DPDPE 또는DLTP의 첨가는 morphine는 DAMGO의 효과에 전혀 영향을 주지 못하였다. 본 연구의 결과는 opioid δ 수용체와 μ 또는k 수용체간에 기능적인 상호관계를 시험관내 실험을 통하여 증명하는 것이며, 이 상호작용에 의한 효과는 두 수용체의 자극효과가 동일한 경우에 출현하는 것을 시사한다.

        Mo-Cu 계 구상흑연주철의 파괴특성에 미치는 오스템퍼링 온도의 영향

        The mechanical properties of ductile east iron can be significantly improved by austempering, isothermal transformation in the temperature range from Ar" to Ar' point flollowing austenitization. Fracture experiments were performed on 0.24 % Mo-0.5 % Cu alloyed ductile cast irons which were austenized for 1.5 hr at 900℃ and austempered for 2 hr in the temperature range from 280℃ to 415℃. Fracture toughness values higher than K_(IC) 80 MPa m^(½) were obtained with the bainite ductile irons which were austempered in the range of 365 to 385℃, and then values of yield strength, elongation and hardness were in the ranges of 810 to 880 MPa, 8.8 to 10.4 % and 270 to 310 H_B, respectively.

        ADI 의 파괴인성과 잔류 오스테나이트량에 미치는 오스테나이트화 온도의 영향

        The fracture properties obtained in austempered ductile iron castings are dependent on metallurgical process variables, including chemical composition and heat treatment conditions. In the present work alloyed ductile irons of 0.27%Mo-1.08%Ni and of 0.24%Mo-0.57%Cu were studied for their response to austempering. The alloys were austenitized for 1.5 hrs at 850-1000℃ and austempered for 2 hrs in salt at 385℃. The optimum mechanical properties and fracture toughness were obtained when the alloys were austenitized at 900-950℃.

        Mo-Ni 계 구상흑연주철의 오스템퍼링 온도에 따른 파괴특성

        The fracture properties of ductile cast iron (DCI) can be significantly improved by austempering, isothermal transformation in the temperature range from Ar$quot; to Ar' point following austenitization. According to the results of experiment, when a 0.27%Mo-1.08%Ni alloyed DCI was austenitized for 1.5hr at 900 ℃ and austempered for 2hr at temperature range from 280 to 415℃, as the austempering temperature was increased, strength and hardness decreased, while ductility increased. Also, the fracture toughness values K_IC obtained were over about 80 MPa m^(½) when yield strength, elongation and hardness were the ranges of 750 to 850 MPa, 5 to 8% and 270 to 330 H_B respectively. The homogeneous bainitic microstructure of the austempered ductile iron (ADI) was obtained by the austempering temperature range of 340 to 385 ℃, and its SEM fractography showed a fibrous structure of dimple pattern, and it had the high fracture toughness.
