최근 업무 전산화로 인한 분산 컴퓨팅 환경이 확대되고 네트워크 기반의 응용 서비스들이 다양하게 개발됨에 따라 네트워크 트래픽은 증가 일로에 있으며 어느 호스트에서 어떤 형태의 트래픽이 얼마만큼 유발되는지를 알아내는 일은 한정된 네트워크 자원을 효율적으로 활용하는데 매우 중요한 일이 되었다. 하지만, 현재의 네트워크관리시스템들은 트래픽 모니터링에 따른 통계값 제공 등의 단순 평면적인 정보만을 제공할 뿐, 네트워크를 정밀 분석하고 그에 따른 대책을 마련하기에는 미약하다. 따라서 효율적인 네트워크 자원 활용을 위하여 네트워크 트래픽을 자세히 분석하며, 네트워크 계층별, 지역별, 호스트별 및 응용 서비스별로 트래픽을 측정 및 분석할 수 있는 시스템의 개발이 필요하다. 본 논문은 한국전력 데이터통신망을 대상으로 네트워크 트래픽의 발원지로부터 목적지까지의 백본 및 LAN 구간에서의 트래픽을 플로우별로 수집, 분석할 수 있는 트래픽 분석 시스템을 설계 및 구현하였다. 본 시스템은 트래픽 측정을 위해 백본에서는 Netflow 기능을 이용하며, LAN 구간에서는 패킷 캡처 방식의 Probe를 이용한다. 이 때 기존의 패킷 캡처 방식의 기능을 개선하여 이를 플로우화 함으로써 LAN 구간에서도 동일한 인터페이스를 통하여 네트워크 트래픽을 분석할 수 있게 하였다.
Augmentin 투여후 Thromboelastography 를 이용한 혈액응고 변화 측정

Background: Antibiotics are used prophylactically in surgery to prevent postoperative infection. However, antibiotics administered in large doses can cause a bleeding diathesis as a result of platelet dysfunction. We wondered whether these antibiotics might impair platelet function by interfering with the initial step of platelet activation: the binding of agonists to their specific receptors on the platelet surface. Methods: In 30 patients(male 18, women 12) undergoing primary elective knee arthroscopic surgery, the whole blood coagulation system was prospectively evaluated before, and 10 and 40 minutes after administration of 1 g of augmentin. All patients who had abnormal preoperative coagulation profiles or who received anticoagulant or antiplatelet, antibiotics therapy within 7 days prior to surgery were precluded. Results: At 10 minutes after augmentin administration 25 of 30 patients had a significant impairment in all phases of whole blood coagulation as monitored by thromboelastography. In contrast, three of 30 patients had a significantly decreased coagulation time. Two of 30 patients had no significant changes of TEG variables. TEG variables were restored toward baseline in fourty minutes after augmentin administration. Conclusion: Augmentin can cause a significant but transient change in the viscoelastic properties of blood. Coagulation parameters of the TEG should be measured prior to augmentin administration to prevent and prospect a bleeding diathesis as a result of platelet dysfuntion. Aortic aneurysm has been associated with degeneration of the media of the aorta. Hypertension is the most important predisposing factor for aortic dissection, being present in 70% to 90% of afflicted patients. Morbidity in patient with aortic aneurysm is most commonly due to cardiopulmonary, neurologic and renal complication. Perioperative mortality for elective abdominal aortic aneurysm repair is 2% to 5%. During performance of Dacron graft interposition, left atriofemoral pump bypass was undertaken with centrifugal blood pump, having lesser hemolysis and lower microemboli, to provide adequate circulation distal to the level of aortic occlusion and to prevent serious left ventricular strain by maintenance of blood pressure at near normal levels. A 26-year-old male patient with extensive dissecting aneurysm involving descending thoracic and abdominal aorta was operated successfully with use of centrifugal blood pump, he was improving hemiplegia, even with brief duration of oliguria. 경막외마취시 진정목적으로 투여한 Ketamine 과 Midazolam 의 병용 효과

Background : Epidural anesthesia is becoming an increasingly important aspect of anaesthetic practice because it has many advantages. To achieve the appropriate sedation, various methods have been described. The authors applied the ketamine for this purpose in subanesthetic dose and compared with the midazolam that has been most commonly used for intravenous sedation. Methods : Fifty-seven adult patients undergoing lower abdominal and extremity surgery who were receiving epidural anesthesia were randomely enrolled into this clinical study. They were allocated to three groups to receive only normal saline(group C), midazolam 0.05 mg/kg(group M) and ketamine 0.5 mg/kg midazolam 0.05 mg/kg(group K) in normal saline 10ml, respectively. Hemodynamic and respiratory measurements were recorded at baseline, 1 minute, 3 minute, 5 minute, 10 minute, 20 minute and 30 minute after sedation. The degree of sedation was assessed by a blinded observer using sedation score. Results : There were no significant differences for hemodynamic and respiratory parameters, but decrements of arterial pressure at 1 minute after sedation were most remarkable in the group M. Sedation was satisfactorily achieved by combination of ketamine and midazolam without significant emergence reaction. Conclusions : The combination of ketamine and midazolam was judged to be suitable alternative for sedation during epidural anesthesia. (Korean J Anesthesiol 1997; 33: 721∼727)
