        단태임신에서 융합된 단일 탯줄을 가진 두 개의 태반 1예

        단태임신에서 두 개의 독립된 태반과 각각의 태반으로부터 형성된 탯줄이 하나의 탯줄로 융합되어 태아에게 연결되는 것은 매우 드문 현상이다. 37세 초산인 여성이 임신 8주 5일에 vanishing twin syndrome이 의심되는 절박유산으로 입원했으며, 이후 산전 초음파 진찰 결과 자궁벽 전면과 후면에 위치한 두 개의 독립된 태반이 진단되었다. 분만 후 시행한 검사결과 각각의 독립된 태반의 가장자리에서 형성된 탯줄 중주태반의 탯줄은 2개의 동맥과 1개의 정맥, 부태반의 탯줄은 1개의 동맥과 1개의 정맥으로 이루어져 있었으며 주태반의 탯줄 시작부위에서 하나로 융합되는 형태를 보였다. 단태임신에서 관찰된 두 개의 태반과 탯줄 융합은 여러 가지 기전-체외수정시술, 부태반의 형성, fetus in fetus 등-에 대해 고려할 수 있으나, 향후 좀 더 체계화된 연구가 필요한 실정이다. Two placentas in singleton pregnancy with fused umbilical cord which has its own placental insertion site forming 3-vessel cord at fetal end is an extremely rare case. This present case describes two placentas with fused umbilical cord with an episode of vanishing twin syndrome and there seems to be a strong relationship between these two events. A 37-year-old woman, gravid 0, para 0, visited emergency room with an episode of vaginal bleeding without pelvic cramps at 8 weeks and 5 days of gestation and repeated ultrasonic exams revealed reabsorption of vanishing twin and two separate placentas on anterior and posterior body of uterus. At 40 weeks and 4 days, the patient delivered a viable female infant weighing 3,900 g via Cesarean section and postpartum examination of the placentas and membranes confirmed two placentas with fused umbilical cord. Two placentas were almost equal in size and there were 2 cord insertions, 1 into each placenta. The cord at each of the placental disc had marginal insertion site and main placental disc cord had 2 arteries with one vein (3 vessel-cord) whereas side placental disc cord had one artery with one vein (2 vessel-cord), Several hypothesis for this two placentas with fused umbilical cord in singleton pregnancy, were proposed including placenta abnormalities after in vitro fertilization-embryo transfer procedure, succenturiate lobes and fetus in fetus, however, further evaluation is need.

        This study was conducted to investigate employee food-hygiene and nutrition awareness and performance and to study the current education programs for children at child care centers and kindergartens located in Seoul, Republic of Korea. A self-evaluated type of questionnaire was developed for foodservice employees and teachers using a five-point Likert scale. Employees with an employment history of <1 year and who were <30 years old had tile lowest scores for food-hygiene and nutrition awareness and performance. Employees with a cooking license had a higher awareness score regarding cross-contamination than those without a license. Higher nutritional awareness and performance scores were also observed for employees who had more experience as cooks and who had received a nutrition education than those who did not. Approximately 94% and 76.4% of the employees had experience in sanitation and nutrition education, respectively. Most teachers responded mal food sanitation-nutrition education for children seemed necessary in a child-care center; however, only 63.9% of the teachers actually taught sanitation-nutrition education to children. The biggest limitation to conduct food sanitation-nutrition education was the "lack of professionalism" of teachers, because only 48.1% of teachers had received a sanitation-nutrition education. Thus, a food sanitation-nutrition education program for foodservice employees and teachers must be developed and implemented actively to improve the quality of food service and nutrition education in child care centers and kindergartens.

        식중독 위험성 인식이 높은 잠재적 위해식품 섭취실태조사

        식중독의 원인이 될 수 있는 가능성이 있는 식품을 잠재적 위해식품이라 하며 이 식품들의 섭취행태가 정확이 파악되어야 정확한 미생물 위해평가를 할 수 있다. 본 연구는 50가지 잠재적 위해식품을 선정하고 대한민국의 성인을 대상으로 잠재적 위해식품의 섭취 실태를 조사하기 위해 전국의 성인 남녀 1,000명을 5월과 8월에 나누어 설문조사를 하였다. 월 1회 이상 섭취하는 빈도를 보았을 때 쌈야채는 소비자의 91.3%가 월 1회 이상 자주 섭취하는 것으로 가장 높게 나타났으며 데친 나물류는 72.3%, 닭튀김은 68.1%였으며 그 뒤로 파채(67.9%), 맛살/어묵(67.3%), 부추(65.3%), 삼각김밥(62.5%), 계란지단/찜(62.4%), 샐러드(59.8%), 오징어/문어(58.7%) 순으로 나타났다. 소비자의 위험인식도가 가장 높은 식품은 조개로 4.36점으로 조사되었으며 홍합(4.35점), 생굴(4.30점), 육회(4.29점), 생선회(4.27점), 해삼/멍게(4.19점), 꼬막(4.16점), 참치회(4.10점), 초밥(4.08점), 날치알(3.95점)의 순서로 나타나 수산물 및 수산가공품의 비율이 높게 나타났으며 이들의 위험인식도는 5월보다 8월이 더 높게 나타나 기온이 올라감에 따라 소비자들의 위험 인식도가 증가함을 볼 수 있다. 반면에 소비자는 위험하다고 생각하지 않는 식품으로는 부추가 1.93점으로 가장 낮게 나타났으며 쌈야채(2.00점), 파채(2.02점), 새싹채소/무순(2.12점), 생식/선식(2.40점), 샐러드(2.40점), 데친나물류(2.47점), 오징어채/포류(2.54점), 조미쥐포류(2.59점), 육포(2.63점) 순으로 나타났으며 미생물의 오염도가 높음에도 불구하고 위험 인식도가 낮아 이들 식품의 위험성에 대한 교육이 필요한 것으로 나타났다. 잠재적 위해식품의 섭취실태조사는 식중독의 원인이 될 수 있는 식품의 위해도 분석 시 기초자료가 될 수 있으며 소비자들의 위험 인식 수준에 대해 지각하고 식중독 예방교육의 방향을 제시해 줄 수 있다. This study investigated the frequency, amount and consumption patterns of 50 potentially hazardous foods(PHF) along with consumers’ risk perceptions towards PHF in Korea. A quantitative survey was performed from May through August by trained interviewers, surveying 1,000 adults aged over 18 who were randomly selected from six major provinces in Korea. Consumers perceived seafood, including shellfish, mussel, sashimi and sushi, as the top foods with the highest risk, followed by raw sliced beef. The food with the highest frequency intake per month was leafy vegetables, which is used to wrap other foods, followed by blanched vegetables, fried chicken, etc. The group of middle aged individuals with economic stability had the highest frequency intake of sashimi and sushi. Respondents living in small regions consume greater PHF portions at once. Food safety education with regard to the risk of PHF is essential for consumers, with high frequent intake of PHF.

        전처리 근채류의 저장과정중의 품질평가

        During the peeling, cutting, and shredding of wild root vegetables, the surface is exposed to air. This results in a rapid deterioration in quality and an increase in the potential of contamination by microorganisms, both of which reduce the shelf-life of wild root vegetables in retail markets. Thus, in this study, the effects of various washing treatments on the quality of wild root vegetables, including lotus root, burdock root, and bellflower root, were investigated at 10 and 24oC. Lotus root, burdock root and bellflower root were washed with 0.2% acetic acid (AA), 0.2% citric acid (CA), 500 ppm acidified sodium chlorite (ASC), and tap water (TW), which was used as a control, and stored at 10oC and 24oC. The changes in total plate counts, coliform groups, polyphenol oxidase (PPO) activity, color, pH, and exterior appearance of the samples were then evaluated. The pH and initial microbial contamination levels were reduced when the root vegetables were washed with AA, CA, and ASC. In particular, initial population levels of total plate counts and coliform groups were not detected in lotus root and burdock root that had been washed with ASC and their growth was significantly (p<0.05) inhibited during storage at 10 and 24oC when compared to the control (TW). In addition, the polyphenol oxidase (PPO) activities of the root vegetables washed with AA, CA and ASC were lower than that of root vegetables washed with TW. ASC was determined to be the most effective treatment for preventing microbial growth, tissue softening, and the development of browning and an unpleasant smell. At 10oC, the overall qualities of the wild root vegetables were maintained longer when compared to 24℃.

        월경전불쾌장애의 약물치료 3상 임상 연구 메타분석

        Premenstrual dysphoric disorder (PMDD) is a common condition that temporarily, but repetitively affects patient`s global function. Patients and physicians alike are often uncertain whether prescription medication for PMDD is sufficiently effective. The primary objective of this analysis is signal detection in efficacy of pharmacological treatments in PMDD. Secondary objective is to review which symptoms are likely to respond to which medications. The review included otherwise healthy women with clinician confirmed diagnosis of PMDD who participated in phase 3 clinical trials for the treatment of PMDD. Twelve pair-wise comparisons of drug and placebo for 2,420 patients with PMDD were performed. Oral contraceptives and selective serotonin receptor inhibitor were effective in alleviating symptoms of PMDD compared to placebo. Both Intermittent and continuous administration were more effective than placebo. This meta-analysis provides a signal that pharmacological treatment of PMDD is effective.
