Newton's physics and the outlook on the world after Descartes had accelerated pragmatism which emphasizes on dualism philosophy dividing cause and effect and analytic reasonable thought, so that the only reality is considered as what can be a concrete existing by human ability. As the result, industrial society controlled by physical power of materialism and quantitative extension can be built up ignoring invisible emotion of human and nature to be conquered. Modern design, which has recognized as a good design in industrial society, has pursued rational and functional from depriving of its mental and emotional elements. But Modern design have to face many problems for frustrating continuity of human history and desire of decoration, as it sometimes has collided with environment to be used and set up. And there are also many criticism against it for its tendency by the philosophy of Descartes to consider individual person as parts of machine, so that it occurred not only human isolation but also destruction of ecosystem in earth increasing entropy. In the meantime, a new insist that Modern design which had focused on extension of physical and quantity should be re-illuminated has appeared, and this is considered as the same effort that in the past had tried to recognize new design concept in its time by pioneer. Now, we are in the Post-Industrial Society which is also called a high developed information society with advanced technology. In the post-industrial society, its subject becomes holon instead of mass as modicum production using tool and order made system had changed from mass production. It bases on intention to soft society from hard society which emphasized on intuition without scientific and reasonable thing. The best goal is to recover the lost humanity during seeking after-self-regulation for human and qualitative growth and to achieve harmony between human and nature. If the tool can be considered as the way to connect human and nature, its past concept was analytic and mechanical solution, while proper concept in the future can connect human and nature organically and lively in total system. So the designed tool is no longer a separated from human and nature, it is rather a organic body should live with them. In this context, we can find the root of this thought which understand two contrast thing as harmonious extreme thing in intuitive oriental The Great Absolute(太極)-the source of the dual principle of Yim und Yang(陰陽) Which had focused on balance between nature and human as a part of organic nature. "Man learns from the earth, the earth learns from the heaven, the heaven learns from the tao(道), the tao comes from nature." St. Lao-tzu(老子) As Lao-tzu said, this oriental thought shows us the harmony among human and nature and tool which organizes completely and lively.

후방불안정성을 동반한 요추 추간판 탈출증에서 Graf 고정술후 발생한 합병증 분석

본 포항성모병원 정형외과에서는 1991년 5월부터 1995년 12월까지 척추불안정성과 관련된 요추부질환 중 연성고정술을 시행한 145례 중 문제점이 발생된 22례를 분석하여 다음과 같은 결론을 얻었다. 1. 총 145례 중 22례(15.1%)에서 연성고정술로 유발되는 문제점을 발견하였다. 2. 문제점으로 주위분절의 불안정성 증가 10례, 추간판 간격의 감소 8례, 척추경 나사못의 해리 3례, Graf band의 파열이 1례였다. 3. 추간판 간격의 감소 8례 중 초기 질환의 원인으로 추간판이 파열된 경우가 6례로 의의있는 소견을 보였다. 이상과 같은 문제점이 관찰되었으나 주위분절의 불안정성 증가 및 척추경 나사못의 해리는 견고한 고정술에 비해 그 빈도가 낮은 것으로 보이며, 추간판 간격의 감소의 경우 역시 단순 수핵 제거술후 발생된 추간판 간격의 감소보다 빈도가 적었으나, 보다 장기간의 추시 및 계속적인 관찰과 연구가 필요할 것으로 사료된다. The Graf stabilization has been introduced in treating lumbar spinal disorder associated with posterior instability. This study reviewed some problems of the Graf instrumentation as a soft stabilizer. The purpose of this study is to analyse the problems of the soft stabilization in spinal instability. We reviewed 145 cases which were operative treatment using the Graf instrument for lumbar spinal disorder associated with posterior instability at our department from May, 1991 to Dec, 1995. The mean follow up periods was 29 months ranging from 24 months to 6 years 8 months. Of the 145 cases,22 cases were showed the problem. The diagnostic method were simple x-ray, flexion-extension lateral stress view and CT scan. Results were as follows: Adjacent segmental instability was 10 cases(6.9%), disc space narrowing was 8 cases(5.5.%), screw loosening was 3 cases(2.1%) and breakage of the Graf band was 1 case(0.6%). The problems of the soft stabilization were adjacent segmental instability, disc space narrowing, screw loosening, and breakage of the graf band. But the rate of adjacent segmental instability and disc space narrowing was lower than other lumbar spinal instrumentation.

Between Feb. 1992 and Apr, 1995, the authors have performed arthroscopic abrasion arthroplasty in 78 knees of 76 patients with degenerative osteoarthritis. The followup period was between 24 and 58 months, with on an average of 41 months. All patients had Zarins grade IV articular cartilage change. The results were as follows. 1. Of the total 78 knees, results were excellent in 25(32%), good in 33(42%), fair in 12(17%), poor in 8(10%) knees respectively. 2, The best results were obtained patellofemoral abrasion arthroplasty. 3. The poor results were obtained in patients with the both femoral condyle lesion. 4. The results were much better in young age group (below 40 years). Aroscopic abrasion arthroplasty is not a curative but palliative method. But it could be an appealing altemative to total knee arthroplasty or high tibial osteotomy or can be performed postoperated after these reconstructive procedures.
