CRP measurement has been used in order to estimate the degree of acute inflammation or tissue necrosis caused by v:따ious factors, A lot of quantitative methods adopting different principle have been shown up since the international standards for CRP were confmned by IFCC. Through the ROC analysis the authors want to presents the efficiency of the quantitative CRP measurement among the patients who came to the emergency room of a university hospital appearing chest pains.

In European countries or Japan they made an attempt for the standardization of assays in AST, AL T, ALP, CK, ill and GGT 10 years ago. For the actualization of the trial, they managed to succeeded in development of certified reference material and enzyme reference material. And they are now forecasting the unification of test results produced differently urder the respective situatons. Then, throogh the highly developed communication system, someone can access any test results whenever or whereever only if they want to. It means that we can expect much greater improvements in medical services through the information exchanges. In this study the authors tried to ascertain the interchangeability comparing the obtained results of the CK tests between two general hospitals. Fortunately, the data got in the internal QC programs came to fall within the normal tolerances. The reference intervals for the CK test were different according to the chosen reagents and the results from the pooled sear were also shown differently each other. However. from the two hospitals. we could get glucose K factors, upon which the measurements were obtained and compared. Using K factors we measured the CKs, then we mulply the CK values by correction coefficients. We could found that the two results were showing good coincidence in the comparison. The main purpose of the standardization of enzyme assays is the creation of a new unified value can be expressed differently otherwise. Using the standardizad values we can exploit the data everywhere, reduce the medical costs from repeated or duplicated tests, save the foreign exchanges preventing the diversification of import connections.

Manuscripts dealing with the development and evaluation of performance of methods and instruments should discuss chemical sensitivity and specificity, detection limits, precision, accuracy, recovery, interference, comparison with other analytical methods ,and normal range, and when appropriate should include clinical data. Linear regression analysis is a commonly used technique in analyzing method comparison data. If a linear relationship between the test and reference method can be defined, then the slope and intercept of this line can provide estimates of the proportional and constant error between the two methods. However if the basic assumptions underlying the least-squares model are not met, the estimated line may be incorrect. The Syncron CX5, a random access type analyser used for routine clinical chemistry determinations, was compared to the Gilford 203 S. The analysis of data obtained by running serum samples in parallel on two instruments indicates that the Beckman CX5 correlates with the Gilford 203 S although there were some bias.

Peroxidase 짝지음 반응에 의한 혈청 Creatinine측정에 관한 연구

Recently, clinical laboratory employing enzymatic method for creatinine has been increased. In this study the enzymatic reaction are coupled to a sensitive peroxidase indicator system. We applied this assay to Hitachi 747. Comparison with the Jaffe method(XI:Hitachi 747, X2:Astra) gave for serum(n=51):y=0.890X1-1.129, r=0.999, range 5.1-220.6 and y=0.771X2-0.315, r=0.983, range 34∼287.3. Measurements of creatinine in clinical serum specimens agreed with those obtained by one enzymatic method and two picric acid methods. ANOVA revealed a significant(P<0.05) dependence of creatinine on method in some group (the concentration of creatinine & bilirubin 442∼1502μ㏖/L, 17∼205/㏖/L, respectively). In icteric samples, there were some tendencies of discrepant results between enzymatic method and alkaline picrate methods for creatinine determination. The enzymatic assay of creatinine in serum was not affected to the extent of 547μ㏖/L by the addition of bilirubin.
