        객관적인 마취심도 조절하에서 전신마취 및 수술 전·후 사시각의 변화

        목적: 사시 수술 전후와 전신마취하 수술 전후 사시각의 변화에 대해 알아보고자 한다. 대상과 방법: 2014년 1월부터 2017년 10월까지 동일 술자에 의해 시행된 간헐성 외사시 수술 환자 20명을 대상으로 하였다. 환자는 먼저 수술 전 외래(preoperative angle of deviation, PreAn)에서 사시각을 측정하였다. 술 중에는 수술 침대에 누운 상태에서 전신마취 후 Bispectral index를 참조하여 충분한 정도의 진정이 되었음을 확인 후(angle of deviation under general anesthesia, PostAn), 근육 수술이 끝난 직후(angle of deviation before waking from general anesthesia after strabismus surgery, OP end) 디지털 카메 라로 수직으로 40 cm 높이에서 촬영하였다. 마지막으로 술 후 6개월째 외래(angle of deviation at 6th month after strabismus operation, 6mon) 방문 시 사시각을 확인하였다. 술 중 사시각은 촬영된 사진을 분석하여 측정하였으며 눈벌어짐은 양의 값으로, 눈모 음은 음의 값으로 처리하였다. 결과: 대상자는 남자 10명, 여자 10명, 평균 나이는 7.31 ± 3.59세였다. 술 전 사시각(PreAn)은 원거리 기준 30.0 ± 13.87 PD였으며, 전신마취하 측정된 사시각(PostAn)은 26.46 ± 5.39 PD였다. 술 전 사시각과 전신마취 후 사시각(PostAn)의 변화량은 술 전 사시각과 유의한 양의 상관관계(PostAN-PreAn=-7.67×PreAn+19.57 [R 2 =0.872, p<0.00])를 보였다. 전신마취 전·후(PostAn-PreAn) 사시각의 변화량과 근육수술 직후와 6개월 후 사시각의 변화량(6mon - OP end) 사이에는 유의한 양의 상관관계(6mon-OP end=0.317× [PostAn-PreAn]+13.098 [R 2 =0.334, p=0.01])를 나타내었다. 결론: 전신마취 후 측정된 사시각은 마취 전 측정값과 유의한 양의 상관관계가 있었다. 마취 전후 사시각의 변화 정도에 따라 수술 직후 보인 사시각과 추후 안정된 각 사이의 변화의 정도를 추정해 볼 수 있을 것이다. <대한안과학회지 2018;59(9):848-853> Purpose: Changes in angular deviation before and after general anesthesia and strabismus surgery were examined. Methods: Twenty patients with intermittent exotropia who were operated on by the same surgeon from January 2014 to October 2017 were included. The basic angle of deviation (preoperative angle of deviation, PreAn) was measured at an outpatient clinic. While lying on a surgical bed under general anesthesia, the bispectral index was referenced, and it was confirmed that there was a sufficient degree of sedation. Photographs were then taken at a height of 40 cm (angle of deviation under general anesthesia, PostAn); while still under general anesthesia, the same procedure was followed immediately after the end of surgery. At 6 months after surgery, the angle of deviation was measured. Results: There were 10 males and 10 females, and the mean age was 7.31 ± 3.59 years. The preoperative angle of deviation was 30.0 ± 13.87 prism diopters (PD) at far fixation; under general anesthesia, 26.46 ± 5.39 PD. There was a significantly positive correlation between the PreAn and angle of deviation under general anesthesia (PostAn - PreAn = -7.67 × PreAn + 19.57; R 2 = 0.872; p < 0.00). The angle of deviation changes between pre- and post-anesthesia (PostAn-PreAn) and at the end point of surgery (OP end) and at 6 months after surgery (6mon) also showed a significantly positive correlation (6mon-OP end = 0.317 × [PostAn - PreAn] + 13.098; R 2 = 0.334; p = 0.01). Conclusions: There was a significant positive correlation between the measured angle of deviation pre- and post-anesthesia. We could estimate the degree of change between the angle of deviation immediately after surgery and the stable angle according to the degree of deviation before and after general anesthesia.         유의한 황반부종이 있는 비증식당뇨망막병증 환자의 범망막광응고술과 베바시주맙병합치료의 장기효과

        목적: 유의한 황반부종(clinically significant macular edema, CSME)이 있는 환자에서 범망막광응고술(panretinal photocoagulation, PRP) 전 시행한 유리체 내 베바시주맙 주입의 효과를 알아보고자 하였다. 대상과 방법: CSME가 있는 심한 비증식당뇨망막병증 환자(91안, 70명)의 의무기록을 후향적으로 분석, PRP 단독군(51안)과 PRP 전 유리체 내 베바시주맙 주입술(1.25 mg/0.05 mL)을 시행한 군(병합치료군, 40안)으로 분류하였다. 치료 시작 전, 치료 후 1, 3, 6, 12개월째 최대교정시력과 중심황반두께를 측정하였다. 결과: 평균 최대교정시력(logMAR, 괄호 내 Snellen)은 PRP군이 치료 전 0.24 (0.575), 1, 3, 6, 12개월째 각각 0.27 (0.537), 0.28 (0.525), 0.28 (0.525), 0.30 (0.501) (p=0.13, 0.15, 0.56, 0.79), 병합치료군은 0.32 (0.479), 0.25 (0.562), 0.26 (0.549), 0.27 (0.537), 0.36 (0.436) (p=0.02, 0.04, 0.02, 0.13)이었다. 평균 중심황반두께(μm)는 PRP군이 257.66, 285.16, 282.21, 289.65, 309.85 (p=0.00, 0.00, 0.00, 0.00), 병합치료군은 349.39, 312.17, 331.15. 353.30, 333.55 (p=0.04, 0.94, 0.79, 0.06)였다. 결론: CSME가 있는 환자에서 PRP를 시행할 때 병합치료는 단독치료에 비해 3개월 이내 단기간의 황반부종 억제효과 및 6개월까지의 시력호전을 보였다. Purpose: To determine the effect of intravitreal bevacizumab injection before panretinal photocoagulation (PRP) in clinically sig-nificant macular edema (CSME) patients. Methods: A total of 91 eyes (70 patients) having severe nonproliferative diabetic retinopathy with CSME requiring PRP were en-rolled in the present study; the medical records were retrospectively reviewed and analyzed. The eyes were divided into the reg-ular PRP group (51 eyes) and PRP with preinjection of bevacizumab (1.25 mg) group (combination group, 40 eyes) and compared. Best corrected visual acuity (BCVA) and central macular thickness (CMT) at pretreatment and 1, 3, 6, and 12 months after PRP was evaluated. Results: BCVA (logarithm of the minimum angle of resolution, Snellen visual acuity in parentheses) at pretreatment and 1, 3, 6, and 12 months after PRP was 0.24 (0.575), 0.27 (0.537), 0.28 (0.525), 0.28 (0.525), and 0.30 (0.501) (p = 0.13, 0.15, 0.56 and 0.79) in the regular PRP group and 0.32 (0.479), 0.25 (0.562), 0.26 (0.549), 0.27 (0.537), and 0.36 (0.436) (p = 0.02, 0.04, 0.02 and 0.13) in the combination group, respectively. CMT (µm) at pretreatment and 1, 3, 6, and 12 months after PRP was 257.66, 285.16, 282.21, 289.65, and 309.85 (p = 0.00, 0.00, 0.00 and 0.00) in the regular PRP group and 349.39, 312.17, 331.15. 353.30, and 333.55 (p = 0.04, 0.94, 0.79 and 0.06) in the combination group, respectively. Conclusions: Pretreatment of anti-vascular endothelial growth factor injection before PRP affected the decrease of macular thickness for 3 months after PRP and improved visual acuity for 6 months after PRP when compared with PRP alone in patients with CSME.

        중심망막동맥 폐쇄로 인한 갑작스런 시력저하가 첫 증상으로 나타난 결절다발동맥염

        목적: 결절다발동맥염은 눈 조직을 포함하여 몸 전체의 중간, 작은 크기의 동맥에 이환되는 괴사성 혈관염이다. 저자들은 결절다발동맥염을 진단 받기 전 선행한 환자의 일측성 중심망막동맥 폐쇄를 경험하였고 이는 눈 증상이 선행된 비전형적인 결절다발동맥염의 증례로 이를 보고하고자 한다. 증례요약: 58세 남자 환자가 갑작스런 우안의 시력저하를 주소로 안과에 내원하였다. 현성굴절 검사를 통한 최대교정시력 우안 0.05, 좌안 1.0으로 확인되었으며, 고혈압, 당뇨 등의 전신기저 질환은 없는 상태로 10일 전 갑자기 발생한 우안의 시력저하와 간헐적으로 나타난 우측 편두통을 호소하였다. 대광반사에서 우안에 상대적 구심성 동공운동장애가 관찰되었으며 안저검사 및 빛간섭단층촬영검사상 우안의 시신경유두 및 황반주변으로 안저의 전반적인 부종이 관찰되었다. 환자는 형광안저촬영 후 우안의 중심망막동맥 폐쇄로 진단 받았고 이후 임상소견과 검사실 결과, 영상검사를 근거로 결절다발동맥염 진단하에 면역억제치료를 시작하였다. 1개월 후 우안의 최대교정시력은 0.05로 확인되었다. 결론: 결절다발동맥염 환자에서 중심망막동맥폐쇄로 인한 시력저하가 첫 증상으로 나타날 수 있으며 안과에 내원하는 경우 관련 문진과 실험실 검사를 추가하여 빨리 진단 후 전신적인 치료를 시행하는 것이 추가적인 다른 합병증을 예방할 수 있을 것이다. Purpose: Polyarteritis nodosa (PAN) is the necrotic vasculitis affecting middle and small-sized arteries throughout the body including ocular tissue. We report an atypical PAN case of unilateral central retinal artery occlusion in which the ocular involvement occurred before systemic symptoms. Case summary: A 58-year-old male visited the ophthalmology department due to abruptly decreased visual acuity. Best corrected visual acuity (BCVA) was 0.05 (in the right eye) and 1.0 (in the left eye) basedon the Snellen chart. He complained of acute decreased vision occurring ten days prior and intermittent migraine on the right side with no underlying diseases, such as hypertension or diabetes mellitus. Relative afferent pupillary defect was observed in the right eye. Generalized edema was found around the optic disc and fovea on fundus examination and optical coherence tomography. The patient was diagnosed with central retinal artery occlusion (CRAO) based on a fluorescein angiography. Subsequently, PAN was diagnosed based on clinical features, laboratory test results and imaging studies. The treatment was started with an immunosuppressive agent. One month later, the BCVA was 0.05 based on the Snellen chart. Conclusions: In PAN patients, decreased vision can occur as a first symptom due to CRAO. If the patient visits the ophthalmology clinic, history taking and laboratory tests for PAN can aid in early diagnosis and treatment, as well as preventing additional complications of PAN.

        미숙아 망막병증 환자에게서 공막 압박 안저 검사 후 발생한 다발성 망막출혈 1예

        Purpose: We report a case of widespread retinal hemorrhages of retinopathy of prematurity (ROP) after the fundus examination with scleral indentation. Case summary: A premature infant, gestational age (GA) 28 + 3 weeks, birth weight 1,250 g, is consulted to ophthalmology department for screening of retinopathy of prematurity. At GA 28 + 3 weeks, both eyes are diagnosed ROP (zone II, stage3, preplus) and a blot hemorrhage was found around temporal area on right eye. After 3 days, panretinal dot and blot hemorrhages are seen on fundus examination, and rapidly increased after the scleral depression with an indentator. We had systemic evaluations to search any underlying cause which makes retinal hemorrhage, like endocarditis, however there are no abnormal findings. Hemorrhages were starting to resolve from 3 weeks after first fundus examination and 3 weeks later all hemorrhages were disappeared. Conclusions: It has reported that ROP infants have immature retinal vessels and decreased autoregulation of circulation; therefore they can easily be destructed. In our case, though fundus examination was done carefully by retina specialist, panretinal hemorrhage was increased rapidly after scleral depression. Thus if the fundus examination should be done with sclera depression, it must be taken carefully.

        방열 특성에 따른 집광형 태양전지의 광전변환효율 변화에 관한 실험적 연구

        Under concentrated illuminations, the solar cells show higher efficiencies mainly due to an increase of the open circuit voltage. In this study, InGaP/InGaAs/Ge triple-junction solar cells have been grown by a low pressure metalorganic chemical vapor deposition. Photovoltaic characteristics of the fabricated solar cells are investigated with a class A solar simulator under concentrated illuminations from 1 to 100 suns. Ideally, the open circuit voltage should increase with the current level when maintained at the same temperature. However, the fabricated solar cells show degraded open circuit voltages under high concentrations around 100 suns. This means that the heat sink design is not optimized to keep the cell temperature at 25°C. To demonstrate the thermal degradation, changes of the device performance are investigated with different bonding conditions and heat sink materials.

        젊은 남성에서 발생한 Relentless Placoid Chorioretinitis

        목적: 16개월 동안 반복된 다발판모양 병변과 망막색소상피의 위축성 변화를 보인 relentless placoid chorioretinitis 증례를 보고하고자 한다. 증례요약: 21세 남자가 시야 흐림을 주소로 내원하였다. 교정시력은 우안 20/50, 좌안 20/20였다. 안저에 다발판모양 백색 병변이 관찰되었으며, 전방염증 소견은 관찰되지 않았다. 실험실 검사상 감염의 증거는 없었다. 임상 추정 진단으로 급성후부다발판모양색소상피병증이나 비감염성 뒤포도막염으로 생각하고 경구 스테로이드 치료를 시행하였다. 7개월간 병변이 양안에서 반복적으로 발생하여 경구 스테로이드 및 면역억제제 치료를 시행하였고, 내원 16개월 후 재발 없이 양안 교정시력 20/20으로 유지하고 있다. 수십개의 망막색소상피의 위축 소견을 남겼으며 임상경과와 병변의 소견을 통해 relentless placoid chorioretinitis로 진단하였다. 결론: 만성적인 다발판모양 백색 병변과 망막색소상피의 위축성 변화가 망막 중심 및 주변부까지 발생할 경우 relentless placoid chorioretinitis를 고려해야 하며, 시력개선을 위해 적극적인 면역억제제 치료가 필요하다. Purpose: To report a patient with relentless placoid chorioretinitis who showed recurrent multiple placoid lesions and retinal pigment epithelial atrophic changes in both eyes for more than 16 months. Case summary: A 21-year-old man visited our clinic because of visual disturbance in both eyes. The best-corrected visual acuities were 20/50 and 20/20 in the right and left eyes, respectively. The fundus showed multiple whitish placoid lesions from the posterior pole to the mid-periphery. There were no anterior chamber inflammation signs. To exclude systemic and infectious etiologies, laboratory workup was performed. All tests were negative except for the serological types HLAB13 and B51. Based on clinical estimation, we made a diagnosis of acute posterior multifocal placoid pigment epitheliopathy or posterior uveitis and prescribed oral methylprednisolone for the patient. However, multiple white dot lesions occurred repeatedly in both eyes for 7 months. With the oral methylprednisolone and immunosuppressive treatment, the best-corrected visual acuity of both eyes was maintained at 20/20 without further recurrence 16 months after the first visit. The lesions left atrophic findings in the retinal pigment epithelium. We diagnosed the patient with relentless placoid chorioretinitis, based on the prolonged clinical course and widespread lesion distribution. Conclusions: Relentless placoid chorioretinitis should be considered in patients with chronically progressive multiple white dot lesions and retinal pigment epithelium atrophic changes between the central pole and the periphery. Aggressive immunosuppressive treatment should be performed to improve the patient s visual outcome.

        금속 후면 반사막이 GaAs 태양전지의 효율에 미치는 영향

        To investigate the effect of metal back-reflective layers (MBLs) on the performance of GaAs solar cells, we fabricated GaAs solar cells on Al and Ag metal layers using the transfer printing technique. We also investigated the effect of MBL texturing on the performance of transfer printed GaAs solar cells. Transfer printed solar cells with MBLs exhibited improved photovoltaic performance compared to solar cells without MBLs due to light trapping. We demonstrated GaAs solar cells with MBLs on a flexible substrate and performed systematic bending tests. All the measured characteristics of solar cells showed little change in performance.

        우안 하안검에 국한되어 발생한 Merkel 세포 종양

        Purpose: To report a case of Merkel cell carcinoma localized to the right lower eyelid treated by surgical resection alone. Case summary: A 81-year-old woman visited the clinic with a painless and red-violet with 0.8 x 0.5 cm size tumor at the right lower eyelid, which developed 1 year ago. The patient complain that tumor begun to grow from 2 months ago. Surgical resection and biopsy were performed for treatment and diagnosis. We evaluated histologic exam using immunohistochemistry and it was positive for CD56 and chromogranin. Finally the patient was diagnosed as Merkel cell carcinoma of eyelid. The resection margin was not invaded and no additional resesction was performed. Local recurrence and metastasis were not observed for eight months. Conclusions: Merkel cell carcinoma is highly aggressive neuroendocrine malignancy and occurrence is rare. If Merkel cell carcinoma is localized to the skin, surgical resection is the primary treatment option and adjuvant radiation therapy can be used. We report a case of Merkel cell carcinoma localized to lower eyelid that was surgically resected and showed good outcome without recurrence and metastasis.

        인구역학적 요인에 따른 한국인의 사시 및 눈떨림 유병률 조사: 제5기 국민건강영양조사 자료

        목적: 사시 질환은 어느 연령대에서도 발병 가능하며 다양한 기능적 또는 비기능적 안과적 문제를 발생시킬 수 있다. 인구 기반 연구를 통해 다양한 인구역학적 요인에 따른 한국의 사시, 눈떨림 등을 포함한 사시 질환의 유병률을 확인하고자 한다. 대상과 방법: 2008년 7월부터 2011년 12월까지 실시된 국민건강영양조사(National Health and Nutrition Examination Survey, n=30,538)의 자료를 사용하여 분석하였다. 사시와 눈떨림의 유병률을 파악하고, 관련된 인구역학적 요인에 대해 평가하였다. 결과: 3세 이상에서 사시 및 눈떨림 환자의 전체 유병률은 1.4 ± 0.1% (95% confidence interval [CI] 1.3-1.6%, 남자 1.5 ± 0.1%, 여자 1.3 ± 0.1%)였다. 외사시(15프리즘디옵터 이상), 내사시(10프리즘디옵터 이상), 수직 편위, 기타 복잡한 사시 및 눈떨림의 유병률은 각각 1.0%, 0.2%, 0.2%, 0.1% 및 0.1%였다. 유병률은 6–15세 연령층에서 가장 높았으며(1.9 ± 0.3%), 40–49세(0.8 ± 0.1%)에서 가장 낮았다(p=0.005). 성별, 지역, 주거 지역, 가구 소득, 교육 수준 및 직업과 관련하여 통계적으로 유의한 차이는 없었다. 결론: 국민건강영양조사 자료를 분석한 결과 한국의 인구역학적 요인에 따른 사시 질환의 유병률은 연령 집단을 제외하고는 통계적으로 유의하지 않았다. 향후 사시 질환의 유병률과 관련된 추가적인 다른 보건 및 사회적 지표들의 검토가 유병률 연구에 도움이 될것으로 판단된다. Purpose: Strabismus affects any age and represents various functional or non-functional eye problems. This population-based study was conducted to determine the prevalence of strabismus and nystagmus in South Korea according to various sociodemographic factors. Methods: We acquired data from the Korea National Health and Nutrition Examination Survey, a nation-wide cross-sectional survey and examinations of the non-institutionalized civilian population in South Korea (n = 30,538), conducted from July 2008 to December 2011. The prevalence of strabismus and nystagmus were verified, and associated sociodemographic factors was evaluated. Results: The overall prevalence of strabismic disorder in participants over 3 years of age was 1.4 ± 0.1% (95% confidence interval [CI] 1.3-1.6%; 1.5 ± 0.1% in males, 1.3 ± 0.1% in females). The prevalence of exodeviation (15 or more prism diopters), esodeviation (10 or more prism diopters), vertical deviation, and other complicated strabismus and nystagmus was 1.0%, 0.2%, 0.2%, 0.1% and 0.1% respectively. The prevalence was highest in the 6 to 15-year age group (1.9 ± 0.3%), and lowest in the 40 to 49 years age group (0.8 ± 0.1%) (p = 0.005). There were no statistically significant differences for gender, region, residential area, household income, educational level and occupation. Conclusions: This nation-wide epidemiologic study demonstrated that the prevalence of strabismus and nystagmus according to various sociodemographic factors in South Korea was not statistically significant except for age group. Further investigations are required based on more surveys to better recognize the etiologic or risk factors that may be related to strabismus and nystagmus.         목적: 망막대동맥류에서 유리체내 베바시주맙주입술과 레이저광응고술 병합요법의 치료 효과에 대하여 알아보고자 하였다. 대상과 방법: 망막대동맥류로 인해 시력저하를 호소하는 환자 21명의 21안에 대하여 후향적 연구를 시행하였다. 비슷한 양상의 황반부종 및 출혈을 동반할 수 있는 질환이 동반된 경우 연구 대상에서 제외하였다. 유리체내 베바시주맙주입술과 레이저광응고술을 병합하여 치료한 11안(병합치료군)과 유리체내 베바시주맙주입술 혹은 레이저광응고술을 단독 치료한 10안(단일치료군)을 비교 분석하였다. 시력, 중심망막두께의 변화, 대동맥류의 크기 및 위치, 혈압, 지질 수치에 대하여 분석하였다. 결과: 평균 나이는 74.0 ± 10.5세, 평균 경과 관찰 기간은 7.0 ± 5.3개월이었다. 망막대동맥류의 크기는 병합치료군, 단일치료군에서 480.00 ± 292.30 μm, 328.75 ± 87.09 μm로 병합치료군이 유의하게 컸다(p=0.002). 경과 관찰 4개월째 시력은 0.33 ± 0.26, 0.17 ± 0.29로 병합치료군에서만 유의한 호전을 보였다(p=0.042). 4개월째 경과 관찰에서 중심황반두께는 293.60 ± 46.10 μm, 269.00 ± 48.34 μm로 병합치료군에서만 유의한 감소를 보였다(p=0.043). 초진과 비교하여 최종 시력이 두 줄 이상 호전된 환자의 비율은 각각 73%, 40%로 유의한 차이가 있었다(p<0.001). 결론: 망막대동맥류에서 유리체내 베바시주맙주입술과 레이저광응고술 병합치료요법은 시력의 개선과 황반부두께의 감소를 보이는 효과적인 치료 방법이다. Purpose: To investigate the effects of intravitreal bevacizumab injection combined with laser photocoagulation in patients with retinal microaneurysms. Methods: We retrospectively reviewed the medical records of 21 eyes of 21 patients who underwent intravitreal bevacizumab injection and/or laser photocoagulation to treat retinal macroaneurysms. Eleven eyes received the combination therapy (CT) and 10 eyes monotherapy (MT) (either bevacizumab injection or laser photocoagulation). Changes in visual acuity, central macular thickness, macroaneurysm size and location, blood pressure, and the lipid level were compared between the two groups. Results: The mean patient age was 74.0 ± 10.5 years and the mean study period 7.0 ± 5.3 months. The mean macroaneurysm diameter of the CT group was 480.00 ± 292.30 μm and that of the MT group 328.75 ± 87.09 μm. The diameter was significantly larger in the CT group (p = 0.002). The initial visual acuities were 0.91 ± 0.66 and 0.88 ± 0.83 in the CT and MT groups, respectively. At the 4-month follow-up, the visual acuities were 0.33 ± 0.26 and 0.17 ± 0.29 in the CT and MT groups, respectively, and had significantly improved only in the CT group (p = 0.042). The initial central macular thicknesses were 441.82 ± 226.81 and 541.63 ± 401.97 μm in the CT and MT groups, respectively. At the 4-month follow-up, the figures were 293.60 ± 46.10 and 269.00 ± 48.34 μm in the CT and MT groups, respectively, and had significantly decreased only in the CT group (p = 0.043). Compared to the initial findings, the proportion of patients whose final visual acuities improved by more than two lines were 73% and 40%, respectively, thus significantly higher in the CT group (p < 0.001). Conclusions: Combined intravitreal bevacizumab injection and laser photocoagulation treatment of retinal macroaneurysms improve visual acuity and decrease macular thickness.
