Eosinophilic enteritis is characterized by marked submucosal edema and massive infiltration of eosinophiles throughout the entire intestinal wall. The etiology of this disease is not clear until now, although some authors insisted that some gastrointestinal allepens, parasites or collagen diseases. Authors report the case of eosinophilic enteritis of ileum with embedded larva of Anisakis, which is thought as etiology. The patient is 47-year-old male, who was healthy, received operation due to acute abdominal pain with small bowel obstruction. He lived in Eastern sea side of Korea and liked to eat the raw cuttle fish. The specimen is segmental resected product of ileum and shows central small ulcer lesion and marked submucosal edema. On histiology examination, marked eosinophilice infiltration throughout the mucosa to serosal layer, accompanied with marked submucosal edema. The three segmented body of Anisakis larva is present in mucosa to submucosal area, which site is just correlated with small ulcer lesion on gross examination. We concluded that the possibility of Anisakiasis as etiology of eosinophilic enteritis may be high especially in the cases of sea-side inhabitants. So we think that careful seria1 sections for pathologic examination is needed in such cases.

N/A We studied 12 cases of gastric anisakiasis who contacted the Dept. of Internal Medicine of St. Mary's Hospital and Sun Rin Hospital, Pohang, from March 1986 to Feb. 1989 and were confirmed by gastrofiberscopic findings and removal of larva of Anisakis. Our analysis gave the following results: 1) Of 12 cases, the sex ratio was 1 to 1,4 (male to female) and the 40s age group was most common, followed by the 30s, the 20s and 60s, in order of frequency. 2) In terms of seasonal incidence, May to July showed 6 cases (50%). Among causative fish, Astroconcer myriaster caused 6 cases (50%) and showed highest freguency. 3) The most common symptoms when the patients contacted the hospital were epigastric pain in 9 cases (75%) followed by nausea, epigastric fullness, and vomiting in order of frequency. 4) The onset of symptoms in 8 cases (66.7%) was within 6 hours since ingestion of those fish. 5) Six cases (50%) underwent gastrofiberscopic exam within 24 hours of onset of symptoms. 6) The most commonly involved site of the stomach was the anterior wall of the mid body (4 cases). The gastrofiberscopic findings were, in frequency of incidence, normal mucosa with larva (5 cases), erosion, hyperemia, mucosal thickening, petechia and ulceration. 7) We performed a biopsy in 4 among 12 cases, and the microscopic findings of the gastric mucosa were congestion and edema, broadening of the foveolar ridge and inflammatory cell infiltration, especially eosinophils and lymphocytes. The antral mucosa revealed findings of chronic superficial gastritis and small fragmental necrotic debris was seen along the surface of the mucosa. Early gastroscopic examination may be very useful in the diagnosis and treatment of anisakiasis if anisakiasis is clinically suspected, In addition, the general public should be enlightened that ocean fish are not necessarily safe when they are eaten a raw and a more detailed epidemiologic study should be conducted for anisakiasis.
대장내시경 용종절제술 후 지연성 출혈의 위험인자: 환자-대조군 연구

Background/Aims: Colonoscopic polypectomy is a valuable procedure for preventing colorectal cancer, but is not without complications. Delayed bleeding after colonoscopic polypectomy is a rare, but serious complication. The aim of this study was to identify risk factors of delayed bleeding after colonoscopic polypectomy. Methods: A retrospective case-control study was conducted in a single university hospital. Forty cases and 120 controls were included. Data collected included comorbidity, use of antiplatelet agents, size and number of resected polyps, histology and gross morphology of resected polyps, endoscopist`s experience, resection method, use of sedation, and use of prophylactic hemostasis. Results: In univariate analysis, size, histology and number of resected polyps, endoscopist`s experience, resection method and use of prophylactic hemostasis were significant risk factors for delayed bleeding after colonoscopic polypectomy. In multivariate analysis, risk of delayed bleeding increased by 11.6% for every 1 mm increase in resected polyp diameter (OR, 1.116; 95% CI 1.041-1.198; p=0.002). Number of resected polyps (OR, 1.364; 95% CI, 1.113-1.671; p=0.003) and endoscopist`s experience (OR, 6.301; 95% CI, 2.022-19.637; p=0.002) were significant risk factors for delayed bleeding after colonoscopic polypectomy. Conclusions: Size and numbers of resected polyps, and endoscopist`s experience were independent risk factors for delayed bleeding after colonoscopic polypectomy. More caution would be necessary when removing polyps with these factors. (Korean J Gastroenterol 2012;59:423-427)
