A 34-year-old womar was admitted to the department of neurology, Seoul City Boramae Hospital for altered rnentality, food refusal and general weakness. She was a vagabond and had received anti-convulsant and anti-psychotic therapy over the past several years. She was referred to the department of dermatology for evaluation of skin lesions. There were light brownish, verrucous, hyperkeratotic plaques on the dorsum of both hands, wrists and soles, and pdoriasifom plaques on both elbows. Other skin lesions such as tina pedis, tinea crurir, and plaques on both elbows. Trichophyton ruhrum was isolat.ed from the dorsum of the hand, elbow, ingui nal fold, sole, and fingernail. On histopathologic examination, papillomatosis and hyperkeratosis were observed from the hand lesion and psoriasiforrn acanthosis from the elhow lesion. She was anergir to a Multitest CMI examination. When richophytin w;is injectecl intradermally, wheal de veloped after 20 minutes but induration did not developed untill 72 hours. We report this case because of its rare clinical manifestation and the interesting restilts of the immunologic test. (Korean J Derltlatol 1997;35(6): 1234-1240)

Diaper dermatitis is an infant dermatosis associated with the wearing of diapers, Despite of its high frequency in clinical pediatrics the clinical characteristics are not well described in the korean literatvre. We investigated the clinical and mycological characteritics of diaper dermatitis in 50 serial patients. Clinically diaper dermatitis was classified. into five categories,irritant contact derniatitis(ID), candidiasis(CD). combined rash of ID and CD(CR.), intertrigo, and miliari. Of 50 patients with diaper dermatitis, 18 had ID; 16, CI3; 10, CR; 5, intertrigo, and 1, miliaria, The mean age of patients with ID (11.8 months) was higher than that of patients with CD (4.0 months). Candida albicans(C. albicans) w is isolated from 25 patients with diaper dermatitis. The rate of positive cultures by clinical diagnosis was 130% in CD, 11.1% in ID, 60, 0% in CR, and 20.0% in intertrigo. C. albicons was isolated from 23 patients(82.1%) of patients with clinically suspected Ci) and 2 of 22 patients without the characteristic erythematous papulesThe presence of eryhematous papules was the most characteristic clinical feature of infection, Other candida species isolated from cut,aneous lesions were C. tropicalsis, C.'. parapsiolosis, and C. stellatoidea (Kor J Dermatol 29(2):159 166)

This study was undertaken to observe the effect of the preirradiation of UVA on the eryt,hematous reaction by UVB. Twenty-five healthy Korean adult male volunteers were taken as subjects. The results were as follows: 1)Preirradiation of 10J/cm and 20j/cm of UVA had no influence on the erythematous reaction induced by UVB. Preirradiation of 30 J/cm of UVA decreased the minimal erythemal dose(MED) of UVB significantly (p<0,05), and 4pJ/cm of UVA dicl very significantly(p<0. 001). 2) Preirradiation dose of UVA(x) and MED of UVB(y) had statistically significant relationship (y=33. 23 - 33 x, r= -0.39, p<0. 05). So preirradiation of UVA decreased MED of UVR in relation with the preirradiation dose. 3) For the objective criteria of skin erythematous reaction induced by the UV irradiation, MED was considered more accurate and efficient than minimal perceptible dose (MPI).

N/A Background : Standardization of antifungal susceptibility testing for dermatophytes is important and several victimless such as inoculum size, length and temperature of incubation, media, and end point determination has recently been established. However, a more improved test model which can evaluate bioavailability and has clinical relevance is still needed. Objective : We performed antifungal susceptibility testing by using three dimensional keratinoctyte culture model, living skin equivalent (LSE), as an in vitro model. Methods : LSE was prepared and various concentrations of antifungals were added to media. Microconidia of Trichophyton mentagrophvtes was inoculated onto LSE and incubated for 6 days at 35℃. Results : Inhibition of fungal proliferation and in .won were observed at 0.1 ㎍/ml of terbinatine, 0.01㎍/ml of itraconazole solution, 0.l㎍/ml of itraconazole powder, and 10㎍/ml of fluconazole, respectively. Conclusion : Our culture model is similar to in vi o situation and the results were relatively well accordant to those of other previous studies. Our LSE model is considered t vi promising in vitro system for evaluating the activity and safety of antifungal agents. However, further study u ore various species of dermatophytes and n ore strains is still needed.

Tinea pedis is frequently found in people with poor personal hygiene and in hot, humid environments. The authors investigated the clinical, epidemiological, and mycological characteristics of tinea pedis in 170 garbage workers(166 males and 4 females) employed in a garbage plant in Seoul, Korea. 40 were clerical workers and 130 were field workers. Tinea pedis was found in 134 with a prevalence rate of 78.8%. Of those 134, 61(45.5%) also had onychomycosis. The prevalence of tinea pedis increased with age and the period working in garbage plant. However, there was no difference in the prevalence of tinea pedis between clerical workers and field workers, mitigating against the conclusion that their tinea pedis may be of occupational origin. Clinically tinea pedis was classified into 4 types ; interdigital (41.8%), vesicular(23.1%), dry squamous(22.4%), and mixed(12.7%). KOH smear positivity and culture positivity was related to clinical types of the lesion the lowest in the interdigital type. In the interdigital type, the etiology of nonmycotic lesions could not be identified. Negative fungal cultures could have been due to secondary bacterial infections. 63 strains of dermatophytes were isolated; 54 strains of Trichophyton rubrem(85.7%), 8 strains of Trichophyton mentagrophytes(12.7%) and 1 mixed infection of T. rubrum and T. mentagrophytes. T. mentagrophytes was isolated only from the vesicular lesions. 8 strains of yeast forms, positive in KOH smears and yielding pure colonies, were also isolated, and repeated mycologic examination yielded the same results. Among them, at least 4 cases of Trichosporon beigelii, and 1 case of Candida parapsilosis were considered to be of pathogenic significance. (Kor J Dermatol 1992; 30(3): 340-346)

Background: Nail involvement in psoriasis is common. But their manifestions may often be difficult to differntiate clinically from onychomycosis, and results on fungal infections in psoriatic nails are still conflicting. Objectiye : The purpose of this study was to observe the clinical signs and mycolgical profiles of psoriatic nails. Methods : The frequency and types of clinical signs were observed prospectively according to sex, age, site, and accompaning tinea pedis. Repeated direct micrscopic and culture examinations, and KONCPA(KOH+Nail Clipping+PAS) study were done in the nails showing subungual kera tosis, and/or onycholysis. Results : Fifty nine(59%) out of 100 patients with psoriasis had psoriatic nail changes. Nail changes were more common in fingers than in toes(55% vs 34%), in males than in females(68.9 % vs 43.6%). The types of frequent nail changes were different in fingers and in toes. Dermatophytes were isolated in 2 cases(8%) and yeasts and non-dermatophytic moulds were isolated in 20-40% of psoriatic nails Conclusion : Psoriatic nail changes of fingers and toes showed different clinical signs. Thus we recommended psoriatic nails to be diagnosed according to the involved site. The isolation of dermatophytes in the psoriatic patients were not so rare. Repeated culture was thought to be the most valuable test because yeasts and moulds were frequently isolated as saprophytes or contaminants. (Kor J Dermatol 1996;34(4): 629-636)

Tinea pedis is frequently found in those people with poor hygine and in hot and humid environments. The authors investigated the clinical, epidemiologicol a id mycological characteristics of tinea pedis in 138 sewerage workers attending a sewerage plant in Seoul. Tinea pedis was found in 82, with a prevalence of 59.4%. The prevalence of tinea pedis increased with age and the period working at sewerage plant, however, there was no statistical significance. Also there was no difference in the prevalence of tinea pedis between the clerical workers and the field workers. Positive rate for KOH smear was 73.2%, and culture positive rates were 42.7%, producing 35 strains of dermatophytes. Twertyeight strains of Trichophyton Rubrum and 7 strains of Trichophyton mentagrophytes were isolated. Twentyseven yeast-form colonies were isolated, and Trichosporon beigelii was foungl in 19 samples. Most of the yeast forms were found mixed with dermatophytes and moulcis, However, 6 were isolated from direct smear positive cases and yieIded pure colonies of yeast. These included 4 cases of T. beigelii, 1 case of Candida parapsilosis, and 1 case of Candida hormicola. In view of the recent report of these fungi as pathogenic organism, these isolates, rspecially T. beigelii, were considered as a causative agent of tinea pedis in certain groups like sewerage workers. We report a case of 64-year-old female patient who has had factitious skin lesions for 20 years. She complained formification sense and severe itching on her face. These symptoms were improved with squeezing and bleeding. There were scuare shaped ulcerated nodule on the right cheek and white depressed scar on the left cheek. She was treated with wet dressing and occlussive dressing, and skin lesions were nearly cleared within 3 weeks, but new lesion appeared. Pimozide 2mg #1 p.o were given under the diagnosis of delusion of parasitosis from June, 1984. Her symptoms were improved without recurrence till now. Treatment with pimozide in monosymptomatic hypochondriacal syndrome is discussed.
