        강변저류지 횡월류부의 흐름 형태 변화를 고려한 HEC-RAS의 하도 내 부정류 모의 정확도 분석

        강변저류지를 홍수방어대책에 포함시키기 위해서는 정확한 홍수조절효과 산정이 필요하며, 이를 위해 현재 실무에서는 1차원 부정류 수치모형인 HEC-RAS를 사용하고 있다. 그러나 강변저류지의 저류용량이 부족한 경우에 발생하는 잠긴 횡월류 흐름에 대해서는 HEC-RAS 부정류 수치모의의 정확도 분석이 수행되지 않았다. 따라서 본 연구에서는 직선수로에 강변저류지를 설치한 경우에 대하여 횡월류부의 다양한 흐름 형태를 재현할 수 있는 부정류 수리실험을 수행하였다. 또한 부정류 수리실험 결과를 이용하여 HEC-RAS 모형의 부정류 수치모의 결과의 정확도를 분석하여 하도 내 수위 오차 및 강변저류지의 홍수조절효과 산정 오차를 제시하였다. 분석 결과 횡월류부에서 잠긴 횡월류 흐름이 발생하는 경우에 대한 HEC-RAS의 수위 계산 결과는 최대 -5% 오차를 보였으며, 홍수조절효과 오차는 최대 2.4%로 나타나 HEC-RAS의 부정류 모의 결과가 비교적 정확한 것을 확인하였다. Accurate quantitative assessment of flood control effect of side-weir detention basin as a flood countermeasure was highly required, in which one-dimensional HEC-RAS model has been widely utilized in practice. When the submerged overflow occurred particularly driven by limited storage capacity of a given detention basin, HEC-RAS model could not be sufficiently applicable by guaranteeing acceptable accuracy without reliable benchmark dataset. From this perspective, a dedicated unsteady experiment was planned and carried out to physically realize such submerged overflow for accommodating better accuracy. Subsequently, the experimental results were applied to validate and calibrate HEC-RAS unsteady modeling to provide flood control effect of the detention basin for various inflow scenarios. After following this procedure, the modelled results indicated that there appeared within -5% of difference in stage height and maximum 2.4% accuracy to assess the flood control effect, thereby ensuring the calibrated HEC-RAS unsteady model to be accurate with practically acceptable error range.

      • 홍대용 혼상의(渾象儀)의 복원 연구: 구조와 작동 메커니즘을 중심으로

        홍대용(洪大容, 1731~1783)이 저술한 "담헌서(湛軒書)" 외집 권6의 <주해수용(籌解需用)>에는 수격식 천문시계인 혼상의(渾象儀)에 대한 기록이 나온다. 우리는 홍대용 혼상의에 대한 구조와 작동 메커니즘에 대한 연구를 통해 복원 모델의 개념설계를 진행하였다. 혼상의는 세 개의 환(環)과 2개의 층(層)으로 되어 있다. 외층에 해당하는 환 구조는 자신이 제작한 혼천의인 통천의(統天儀) 구조와 유사하다. 내층에 해당되는 혼상구에는 별자리와 은하수를 그려넣고, 태양과 달의 모습을 구슬 형태로 매달아 천상의 운행 모습을 구현하였다. 통천의는 추동력으로 운행되며, 혼상의는 물에 의한 수격방식으로 운행된다. 홍대용의 수격방식은 조선에서 전통적으로 사용한 동력체계로 중국의 대표적 천문시계인 수운의상대의 동력체계를 간소화시켰다. 혼상의에 대한 기록은 조선시대 수격식 동력체계를 다룬 거의 유일한 문헌기록으로 수격식 작동 메커니즘 연구에 중요한 단서를 제공해 주고 있다.

        체질진단설문지 정확률 연구의 연구방법론 고찰

        Objectives For the methodological review on the accuracy study of questionnaire for Sasang constitution diagnosis, we searched the various diagnostic accuracy study of the questionnaires for Sasang constitution. Methods We searched MEDLINE, the Cochrane Library, KISS, and DBPIA. Additionally, We hand-searched the main oriental medical journals. All articles were independently reviewed and selected by two evaluators. And selected articles were assessed by "Quality Assessment of Diagnostic Accuracy Studies Tool"(QUADAS Tool) for the methodological review. Results The twenty eight studies initially identified studies were included in the methodological review. The part of "Acceptable reference standard", "Uninterpretable results reported" and "Withdrawals explained" was very weak in the risk of bias. The part of "Representative spectrum", "Acceptable delay between tests", "Incorporation avoided", "Reference standard results blinded", "Index test results blinded" was unclear in the description. Conclusions For the further study on the accuracy study of Sasang constitution diagnosis, we have to improve the aforementioned errors. Additionally, the checklist for the description of study might be needed.

        태음인 표병군과 리병군의 증후 차이에 대한 분석 : 다기관 약진 대상자를 중심으로

        1. Objectives : The aim of this study was to survey the ordinary symptoms of TE type(Tae-eumin) and to compare the tendency of their symptoms according to the Exterior- or Interior- disease. 2. Methods : We recruited 659 TE type(Tae-eumin)-subjects in multi-oriental hospitals and researched their ordinary symptoms. Subjects were diagnosed by the Sasang Constitutional specialists in oriental hospitals, and had remarkable improvement in the chief complains after Sasang Constitutional Medication. Subjects were divided two groups. One group had the Exterior-cold disease and they took the prescription for the Exerior-cold disease(such as Taeumjowi-tang, Jowiseungcheong-tang, Nocyongdaebo-tang etc.). The other group had the Interior-febrile disease and they took the prescription for the Interior-febrile disease(such as Galgunhaegui-tang, Yeoldahanso-tang, Cheongsimyeonja-tang, Gongjinhugwon-dan etc.). All subjects answered the questionnaire about their symptoms. We found the differences of answers using the Chi-square test between-group comparisons. We analyzed using SPSS 12.0 for Korean. 3. Results and Conclusions : In symptoms, Quantity of meal(p=.009), Perspiration of head and face(p=.001), Aspect of Excrement(p=.027), Presence of pain after excrement(p=.001), Frequency of urination at night(p=.003), Clarity of urine(p=.041), Sleep during the day(p=.049), Warm or cold feeling on abdomen(p=.007), etc. are different according to the Exterior- or Interior- disease. We found the differences of symptoms between not only the constitutional types but also the types of disease.

        혼탁한 친수성 아크릴 인공수정체안의 초음파 생체계측계를 이용한 안축장 측정

        Purpose: To investigate the accuracy of measuring ultrasonic axial lengths using A-scan (Ocuscan®, Alcon, USA) on opaque intraocular lenses after hydrophilic lens (ACRL-160®, Ophthalmed, USA) implantation. Methods: We measured axial length through ultrasonic biometry prior to intraocular lens exchange. Twelve eyes of 14 patients who had intraocular lens opacity following hydrophilic acryl lens implantation were examined in a clinical study. We compared pre-cataract operative axial lengths to pre-IOL exchange axial lengths. The pre-IOL exchange axial lengths were measured in aphakic and pseudophakic modes. In the pseudophakic mode, the ultrasound velocity through an IOL was set at a rate ranging between 1,500 m/sec to 2,200 m/sec. Results: The pre-IOL exchange axial lengths in the pseudophakic mode at the rate of 1,600 m/sec in lens velocity were the closest to pre-cataract operative values (p=0.88). Conclusions: When pre-cataract operative axial length is known with a hydrophilic acrylic intraocular lens, previous values may be used for IOL exchange of an opacity patient. If not, however, the closest values to pre-cataract operative axial lengths may be obtained by setting the velocity in the pseudophakic mode to a lens velocity rate of 1,600 m/sec.         단축형 사상체질 진단설문지(KS-15)의 베트남인 적용 예비연구

        Objectives The purpose of this study was to evaluate the possibility of Korea Sasang Constitutional Diagnostic Questionnaire(KS-15) when was applied to the Vietnamese. Methods The subjects of this study were Vietnamese participants who visited National Hospital of Traditional Medicine in Hanoi in 2012-2013 among the data established at the Korean Medicine Data Center. Among them, 188 people were used to assess the reliability, and 116 people were used to assess the validity. The reliability test was conducted through test-retest analysis, and the validity test was conducted through cross analysis comparing to diagnosis by Sasang Constitutional experts. Results & Conclusions Three items of total 14 items, except for BMI, showed 0.4 or below in Kappa, 3 items were ranged in 0.4-0.6 and 8 items showed 0.6 or over, using 188 people. The concordance rate between the results of KS-15 and diagnosis of experts was 66.4% in 116 people (85.4% in Taeeumin, 47.4% in Soeumin, and 64.9% in Soyangin). The KS-15 could be usable for supporting to diagnose the Sasang constitution of the Vietnamese.

        열다한소탕과 태음조위탕·조위승청탕의 소증 분석을 위한 의사결정나무 구성

        Objectives The purpose of this study is to analyze the decision making process of prescribing Yeoldahanso-tang and Taeeumjowi-tang Choweseuncheng-tang using decision tree. Methods We used collected the prospective clinical data of TE type from September 2012 to July 2015. In this study, we used gender, BMI, blood pressure, pulse and clinical symptoms (digestion, sweat, defecation, urination, sleep, physical status, emotion, heat-coldness, water consumption, facial color) as variables. Decision trees were analyzed using open source R version 3.3.2. Results & Conclusions We found that the decision trees differed among institutions. However, in all institutions, it was found that stool type (ordinary symptom), urine frequency (ordinary and present symptom) and anxiety (ordinary symptom) were important in the decision of prescription. Besides, clinical informations such as sex, Body Mass Index and blood pressure affected the prescription decision.

        혼천시계의 시보시스템 구조 분석

        Song I-Yeong (1619 ~ 1692), who was an astronomy professor of Gwansanggam (觀象監, Bureau of Astronomy), created the Honcheonsigye (渾天時計, Armillary Clock) in 1669 (10th year of King Hyeonjong Era). Honcheonsigye was a unique astronomical clock which combined an armillary sphere, the traditional astronomical instrument of the Far East, with the power mechanism of western alarm clock. The clock part of this armillary clock is composed of two major parts which are the going-train, power unit used the weight, and the time signal system in a wooden case. The time signal system is composed of four parts which are the time-annunciator, the striking train, the 12 different time-announcing medallions and the sound bell. This clock has been neglected for many years and its several components have been lost. This study is to understand the structure of time signal system and suggests the restoration process.

        원발성 항인지질증후군에서 발생한 양안의 망막중심정맥폐쇄

        Purpose: To Report a case of bilateral central retinal vein occlusion (CRVO) caused by primary antiphospholipid syndrome. Methods: A 38-year-old male is referred to the department of ophthalmology for the bilateral visual loss. Results: On initial visit, both visual acuity was 0.3. Upon consider changing to fundoscopic examination, the patient was diagnosed with bilateral CRVO. We performed hematologic tests including thrombophilia examination. There were no abnormal findings on routine hematologic tests. Antinuclear antibody and rheumatoid factor were negative but anticardiolipin antibodies presented high titer, on two occasions six weeks apart. We prescribed oral aspirin and performed intravitreal bevacizumab injection under the diagnosis of bilateral CRVO in primary antiphospholipid syndrome. Conclusions: It may be necessary to check antiphospolipid antibody in cases of bilateral CRVO in young patients without medical problem. J Korean Ophthalmol Soc 48(10):1433-1437, 2007
