        구급일지를 통한 병원전 심정지 환자의 임상적 특성과 병원전 응급처치

        본 연구는 심정지 환자에 특성 및 심폐소생술과 제세동 등 응급처치 시행에 관련된 구급활동일지와 심폐정 지환자 응급처치 세부상황표를 분석하여 심정지 환자의 소생을 위한 병원전 응급처치의 질향상에 기여할 목적으로 수행하였다. 연구결과는 다음과 같다. 제세동할 수 있는 심실세동과 심실빈맥 리듬은 20.3%이고 이중 제세동기에 의 한 쇼크는 66.5%가 시행되었다. 현장일반인 심폐소생술 시행시 소생률은 미시행시에 비해 높았으나 유의한 차이 (p=0.10)는 없었다. 심전도 기록을 판독한 결과 심실빈맥이거나 심실세동인 경우가 무수축이거나 무맥성 전기활동인 경우보다 소생률이 높고 유의한 차이(p=0.000)가 있다. 제세동을 적절하게 한 군은 부적절하거나 판단이 불가능한 경 우보다 소생률이 높았고 유의한 차이(p=0.000)를 보였다. 이상의 결과를 토대로 병원전 응급구조사의 기록과 제세동 등 적절한 응급처치로 심정지 환자의 소생률을 높이는데 기여하였으면 한다. In order to report characteristics of out-of-hospital cardiac arrest patient in whom 119 rescuers used prehospital care by Paramedic's Reports. 1,016 out-of-hospital cardiac arrest patients were transferred to hospitals by 119 rescuers between January 1st and December 31st, 2008. Prehospital reports of 983 cardiac arrest patients by 119 were analyzed. Shockable rhythm with AED use was 20.3%(VF 18.4%, VT 1.9%), then 66.5% of shockable cardiac arrest patients was resuscitated by AED. Bystander basic life support was 14.8%. There were significant differences in the recurrent survival rates between shockable rhythm and non-shockable rhythm(13.0% vs 2.0%,7.4%, p=0.000). There was also significant differences in the recurrent survival rates between adequacy rate of AED(21.6% vs 2.4%, p=0.000). But there was no significant differences in the recurrent survival rates between done bystander CPR and none(9.0% vs 5.5%, p=0.10). The performance of bystander CPR and usage of AED, and appropriate CPR done by 119 rescuers were unsatisfactory by paramedic's reports. To improve the adequacy of Basic life Support and to increase the performance on Advanced Life Suppport, we must challenge to develop the emergency medical systems.

      • 소방대원의 외상후 스트레스 실태

        Purpose : This study is a descriptive research to provide basic factors of posttraumatic stress in Firefighters. This study was carried out to develop the effective program for the fire fighters to cope with the posttraumatic stress following the disasters. Methods : The questionnaires were collected among fire fighters who serviced in K and I community from April 1 to June 30 in 2008. Total 304 questionnaires were analyzed by SPSS WIN program for descriptive statistics, Pearson's correlation coefficient and t-test. Results : 1. 48.0% of 300 fire fighters were at the age of 31-40, and 42.3% were under 30. 2. Work burden had a significant difference of 2.30 in low-risk group, 2.60 in high-risk group(t-value=-3.85, p=0.00). However, life event had no significant difference 0.79 event in low-risk group, 1.41 event in high-risk group(t-value=-2.27, p=0.24). 3. Concerning posttraumatic stress factors, there was positive correlation between mobilization impact level r=0.38(P<0.01), work burden r=0.38(p<0.01), and life event r=0.27(p<0.01). 4. According to the Symptom Check List-Revised(SCL-90-R), somatization had a significant differences(t-value=5.46, p=0.00), obsessive-compulsive(t-value=7.16, p=0.00), interpersonal sensitivity(t-value=6.15, p=0.00), depression(t-value=6.62, p=0.00), anxiety (t-value=7.33, p=0.00), hostility(t-value=5.94, p=0.00), phobia anxiety(t-value=6.85, p=0.00), paranoid ideation(t-value=5.55, p=0.00), psychotism(t-value=6.52, p=0.00) in low-risk and high-risk group. Conclusion : As a consequence, mobilization impact, work burden, and life event were the influential factors on posttraumatic stress. Also, high-risk group revealed significantly higher score on all 9 scales. The information obtained from surveys made recommendation to develop the intervention of stress management to control mobilization impact and posttraumatic stress.

        기관내 튜브삽관 후 커프 압력에 대한 교육 효과

        Purpose: This study investigated the effectiveness of education regarding cuff pressure following endotracheal tube intubation using a quasi-experimental design with a pre- and posttest non-equivalent control group. Methods: A total of 78 students from two universities participated in an education intervention on the importance of cuff pressure after endotracheal tube intubation between October and December, 2016. The intervention lasted 40 minutes. Data were collected from each student before the intervention and one week following the intervention. Analyses were conducted using chi-square tests, Fisher's exact tests, and analysis of covariance. Results: A total of 38 students were assigned to the experimental group and 40 to the control group. The educational intervention of cuff pressure following endotracheal tube intubation was associated with prevention of possible complications from excessive cuff pressure (F=121.02, p<.001). Conclusion: Training with a pressure gauge and an intubation manikin is necessary to determine the appropriate cuff pressure in the intubation protocol of the practical examination in the emergency medical technology.

        4차 산업혁명 시대 응급구조(학)과 융복합 교과목 개발을 위한 연구

        Purpose: This study aimed to examine the development and improvement direction of university education according to job type and investigate the types of knowledge required for employment after graduation for departments of emergency medical technology in preparation for the era of the fourth industrial revolution in a converged society. Methods: From June 3 to June 10, 2019, data were collected through structured questionnaires from 90 paramedics working in firefighting, hospitals, different industries, and emergency patient transportation. Data were analyzed using the software SPSS version 18.0. Results: For employment after graduation in departments of emergency medical technology, character and sincerity education should be strengthened. To prepare for the fourth industrial revolution, educational innovations such as improving the field practice system and devising a convergence curriculum are needed. Preparation for the fourth industrial revolution should be accomplished through the development of convergence subjects such as forensic science, information and communications technology, and safety engineering. Ninety percent of the study respondents indicated their need for convergence subjects. Conclusion: The demands of future society should be responded to flexibly by holding and conducting convergence subjects. Furthermore, preparations should be made in anticipation of a shortage of 120,000 college freshmen for the 2024-2025 school year by increasing the competitiveness of departments of emergency medical technology.

        전문대학 응급구조과의 국가직무능력표준(NCS) 기반 교육과정 개발 및 운영방안 연구

        Purpose: Although appointed as a national competency standards (NCS) based reserves department, the department of emergency medical technology, an NCS-based emergency department, is mainly focused on subject deduction for a NCS-based curriculum. Methods: Job models were formed and verified by combining the competency unit of NCS and the duty of Developing a curriculum (DACUM) based on the development procedure indicated in the guidelines for a NCS-based curriculum. The mapping method of the subject was performed by deducting necessary competency units (duty) and competency unit elements (task) by connecting with the composition items of NCS and DACUM. Results: Job models combined with job analysis for the NCS and DACUM were reduced to 13 competency units (duty) and 79 competency unit elements (task). A modified method such as the 1:N method was mainly applied as a subject-matching method with consideration of the competency level and size of the competency unit. Conclusion: It would be a desirable direction to develop a NCS-based curriculum in the center of the practice subject in consideration of the size of the competency unit and competency level of the competency unit element. The existing curriculum should be promoted as a field-oriented curriculum at the complementary level.

        Policy suggestions to educate and produce outstanding paramedics in Korea

        연구 목적: 본 연구는 우수한 1급 응급구조사 인력 양성 및 배출을 위해 응급구조사의 역량을 도출하여 교육제도를 재정립하고 국가시험 및 자격제도를 개선하기 위한 정책 제안을 하고자 하였다. 연구 방법: 본 연구는 전문가 회의 및 설문지를 이용하였다. 설문 자료는 교육 전문가(응급구조학과 대학교수)와 현장 전문가(소방 119구급대원 및 병원에 근무하는 1급 응급구조사) 총 277명으로부터 수집되었으며, 자료수집 기간은 2016년 9월 7일부터 9월 20일까지였다. 수집된 자료는 SPSS 22.0을 이용하여 분석하였다. 연구 결과: 1급 응급구조사 교육과정 강화를 위해 표준교육과정안으로 총 27과목 94학점을 제안하였고 1급 응급구조사의 핵심역량 9가지를 도출하였다. 국가 자격시험은 필기시험 과목을 통합하는 방안과, 실기시험에 시나리오형의 문제를 추가하고, 실기를 절차대로 수행하는 단순실기시험에 낙제기준(critical criteria)를 적용하며, 실기시험은 통과/실패만 결정하는 것을 제안하였다. 또한 1급 응급구조사의 보건의료적 업무특성을 반영하여 자격은 면허로 바꾸는 것을 제안하였다. 결 론: 본 연구 결과를 토대로 표준교육과정 운영으로 핵심역량을 갖추고 배출된 인력들이 신뢰성 있는 국가시험제도를 통해 자격을 취득하고 1급 응급구조사로서 전문성을 강화할 수 있는 자격관리제도가 선순환을 이룰 때 우리나라 응급의료서비스의 질적 수준이 향상될 것이다. Purpose: With the purpose of educating and producing outstanding paramedics by enhancing their competencies, this study aimed to make policy suggestions to re-establish the education system and improve the national examination and the certification scheme. Methods: This study used focus group interviews and questionnaires to collect data. Totally, there were 277 subjects, including experts from the education and field. Data were collected from September 9 to 20, 2016, and analyzed using SPSS 22.0. Results: To strengthen the curriculum of paramedics, this study suggested 27 courses with 94 credits as the standardized curriculum and derived 9 core competencies of paramedics. For the national examination, this study suggested consolidating written test subjects, adding scenario questions to practical tests, and applying critical criteria to simple practical tests that performs a procedure, grading these tests on a pass/fail basis. In addition, this study suggested converting certification into license, reflecting paramedics' healthcare job characteristics. Conclusion: The quality of emergency medical services in Korea will improve when those with core competencies that originated from the standardized curriculum based on the results of this study acquire their certification through the national test scheme, and the certification management system creates a virtuous cycle to further enhance paramedics' professionalism.

        응급구조(학)과 교육과정의 표준화에 대한 연구

        Purpose: This study describes current curricula for paramedic students in South Korea and proposes a standardization of the curriculum. Methods: Data were collected from 38 colleges and universities from March 1 to 31, 2016. Descriptive statistics were calculated using SPSS 23.0. Results: The proposed standard curriculum was below. Requisite liberal arts consisted of 2 subjects and 6 credits including biomedical ethics, communications and human relationships. Common major subjects were composed of 6 areas, 22 subjects, and 78 credits. The areas of basic medicine consisted of 6 subjects and 16 credits including medical terminology. Introduction to paramedicine consisted of 3 subjects and 7 credits. Emergency patient management consisted of 2 subjects and 9 credits. Particulars to paramedic care consisted of 8 subjects and 31 credits. The law area consisted of 1 subject and 3 credits. Other major areas consisted of 2 subjects and 12 credits including integrated simulation and physician assistance. Common field practice area consisted of 3 to 4 subjects and 9 to 12 credits. Conclusion: It is important to establish and adapt a standardized curriculum for paramedic students in order to ensure competence and to provide high quality emergency medical services.

      • 응급구조과 학생의 현장실습 수행활동 조사 연구

        This study was conducted to acquire the basic materials for effective field training to identify performance activities of emergency medical technology students. The method used in the study was a descriptive survey using a skill checklist. The subjects for the study were 43 emergency me야cal technology students who were 2nd grade in D college. The collected data were analyzed using the SPSS computer yielding frequencies and percentage. The results of study were as follows; 1. In 'emergency treatment skill', 26.6% of subjects are revealed as 'performed' in 'ambulance practice' and 22.1% of subjects are revealed as 'performed' in 'clinical practice' 2. In 'airway management', 41.9% of subjects are revealed as 'performed' in 'ambulance practice' and 14.1% of subjects are revealed as 'performed' in 'clinical practice'. 3. In 'oxygen therapy', 52.8% of subjects are revealed as 'performed' in 'ambulance practice' and 35.6% of subjects are revealed as 'performed' in 'clinical practice'. 4. 'ventilation skill', 17.8% of subjects are revealed as 'performed' in 'ambulance practice' and 10.7% of subjects are revealed as 'performed' in 'clinical practice'. 5. In 'vital sign check', 61.1% of subjects are revealed as 'performed' in 'ambulance practice' and 56.3% of subjects are revealed as 'performed' in 'clinical practice'. 6. In 'patient assessment', 40.7% of subjects are revealed as 'performed' in 'ambulance practice' and 20.0% of subjects are revealed as 'performed' in 'clinical practice'. 7. In 'basic life support(CPR)', 1.7% of subjects are revealed as 'performed' in 'ambulance practice' and 11.9% of subjects are revealed as 'performed' in 'clinical practice'. 8. In 'airway obstruction', 6.4% of subjects are revealed as 'performed' in 'ambulance practice' and 1.1% of subjects are revealed as 'performed' in 'clinical practice'. 9. In 'electrical therapy', 0.7% of subjects are revealed as 'performed' in 'ambulance practice' and 20.Off of subjects are revealed as 'performed' in 'clinical practice'.
