        비전형 안진을 보인 급성전정증후군을 동반한 Ramsay Hunt 증후군

        Ramsay Hunt syndrome is an acquired paralysis of the face specifically caused by a varicella-zoster virus infection in the facial nerve. Other cranial nerves in-cluding vestibulo-cochlear disturbance can be affected. Herein we reported a case of Ramsay Hunt syndrome with atypical vestibular syndrome. Although central vestibular signs including direction changing post head-shaking nystagmus or normal head impulse test are generally meaningful, clinicians need to be careful to interpret them because some findings can be observed not only in cases of central disorders but also in peripheral disorders. Clinical findings such as distinct ear pain and close observation of vesicles are important to diagnose Ramsay Hunt syndrome.

      • Fulminant Guillain-Barr Syndrome Mimicking Cerebral Death Following Acute Viral Hepatitis A

        A 32-year-old man was transferred to an intensive care unit due to respiratory difficulties with a 4-day history of progressive areflexic quadriparesis following acute hepatitis A. A nerve-conduction study revealed inexcitability of most nerves. The cerebrospinal fluid showed albuminocytologic dissociation, suggesting Guillain-Barré syndrome (GBS). The patient appeared brain dead on day 4, showing absent brainstem reflexes, respiratory failure, and fully dilated and fixed pupils. This case is an example of how GBS can evolve and simulate a brain-dead state from fulminant deafferentation following acute hepatitis A.

        알쯔하이머병 환자에서 감소된 망막두께

        Background: The loss of ganglion cells observed in Alzheimer’s disease (AD) patients might be attributable to a neurodegenerative process involving the neuroretinal structure. The objective of this study was to determine the retinal thickness in patients with AD using optical coherence tomography (OCT). Methods: Neuro-ophthalmologic examinations were performed in eight AD patients and eight age-matched control subjects. The average thicknesses of the retinal nerve fiber layer (RNFL) and the ganglion cell–inner plexiform layer (GCIPL) were measured using spectral-domain OCT in both subject groups, and the results were compared. Results: The RNFL was thinner in AD patients than in the age-matched controls (p<0.05), especially at the nasal and inferior retina, and the GCIPL was also significantly thinner (p<0.05). Conclusions: The decreased retinal thickness observed in AD patients suggests that OCT can be used as a biological marker of AD. The findings of this preliminary study suggest that the extent of the reduction in GCIPL and RNFL thicknesses should be investigated further.

        안와 통증의 신경학적 질환 감별: 원발두통과 삼차신경통을 중심으로

        Ocular or orbital pain has been associated with neurological disorders such as migraine, trigeminal neuralgia, trigeminal autonomic cephalalgia or trigeminal neuralgia. Systematic history taking is most important for an accurate and differential diagnosis to rule out secondary headache. Generally, brain images are not required in primary headache with or without ocular pain. In this review, we focus on primary headache and trigeminal neuralgia with normal ocular and neurologic examinations and discuss methods for differentiating them based on history taking. 안구 또는 안와통증은 편두통, 삼차자율신경두통 같은 일차두통이나 삼차신경통에서 흔히 동반되는 증상이다. 이차두통을 배제하고, 정확한 진단을 위해서는 체계적인 병력청취가 가장 중요하다. 대부분 일차두통에서는 안구, 안와통증과 상관없이 뇌영상검사가 진단적으로 필요하지 않다. 이 종설에서는 안구, 안와 통증을 호소하지만 안과검진에서 정상을 보이는 환자에서 감별해야 할 대표적인 질환인 일차두통과 삼차신경통의 주요 특징에 대해 병력청취를 기반으로 한 진단기준 중심으로 살펴보았다.

        OPC의 분말도 및 SO3 함량이 시멘트 광물계 급결제를 사용한 숏크리트 결합재 물성에 미치는 영향

        Shotcrete concrete is generally used in the form of ready-mixed concrete products using type I ordinary portland cement(hereinafter referred to as OPC) and about 5% of accelerator mixed separately in the field. In this study, we tested the effect of OPC fineness and SO3 content on a penetration resistance, compressive strength of binder for shotcrete using calcium aluminate type accerlerator. And we analysed hydrates and pore structure effects on mortar performance. In the future, it is expected to be useful for manufacturing optimized OPC as a binder for shotcrete. 숏크리트용 콘크리트는 일반적으로 1종 보통 포틀랜드시멘트(이하 OPC)를 100% 사용한 레미콘 제품에 현장에서 별도로 숏크리트용 급결제를 약 5% 혼합하는 방식으로 사용된다. 본 연구에서는 국내에서 사용률이 높은 시멘트 광물계 급결제(calcium aluminate)를 사용한 숏크리트용 결합재로서 OPC의 분말도 및 SO3 함량이 모르타르 관입저항 및 압축강도, 페이스트 수화물 및 공극구조에 미치는 영향을 시험 및 분석하여 재령별 수화물의 생성량과 공극구조가 숏크리트의 모르타르 성능에 미치는 영향을 파악하였다. 향후 숏크리트용 결합재로서 최적화된 OPC를 제조하는데 활용할 수 있을 것으로 기대된다.

        산업부산물 활용을 위한 고Al2O3 함량 OPC 클링커의 소성성 및 반응성에 관한 연구

        This study evaluated the burnability and hydration reaction of clinker burned with high Al2O3 content OPC to apply large amounts of industrial by-products in the cement manufacturing process. Specifically, after preparing a clinker with a high C3A content by burning the OPC raw material with a high content of Al2O3 in a laboratory electric furnace, the burnability of the clinker was evaluated through XRD Rietveld analysis and polarization microscopy, and clinker hydration reactivity was reviewed through the Isothermal conduction calorimetry analysis and the cement compressive strength. As a result, the kiln burning temperature for the production of high Al2O3 content clinker lower, and the compressive strength was equal to or higher than OPC. Therefore it was confirmed the possibility to manufacturing energy-saving high Al2O3 content clinker using a large amount of industrial by-products.

        급성일측전정병증 환자에서 전정기능검사의 임상적 유용성

        Objectives: Aim of this study is to investigate the clinical efficacy of the vestibular function tests (VFTs) and the predictability of lesion side of vestibular asymmetry parameters in acute unilateral peripheral vestibulopathy.Methods: Medical records and results of VFTs (caloric, rotatory chair, and head impulse tests) of 57 patients with acute unilateral vestibulopathy were reviewed retrospectively. The VFTs were examined within 7 days after the clinical onset.Results: For the caloric test, 74% showed significant canal paresis and the predictability of lesion side was 88%. For the sinusoidal harmonic acceleration test, 91% had low gain in at least 1 Hz, phase lead showed 70%, 89% showed phase asymmetry and the predictability of lesion side was 90%. For velocity step test, 67% had abnormal Tc asymmetry and the predictability of lesion side was 95%. In bedside head impulse test (HIT), abnormal catch up saccades were observed in 89% and the predictability of lesion side was 100%. For the video HIT, cover or overt catch-up saccades were observed in 95% and the predictability of lesion side was 100%. One hundred percent (100%) had low gain on the video HIT, but the lesion sides were uncertain because of bilateral involvements or artifacts. Conclusions: The most important things in the diagnosis of acute unilateral vestibulopathy are typical clinical symptoms and spontaneous nystagmus. A combination of rotatory, caloric, and HITs will result in a more complete examination of the vestibular system. Among them, HIT is recommended as the best tool in acute unilateral vestibulopathy.
