Ocular or orbital pain has been associated with neurological disorders such as migraine, trigeminal neuralgia, trigeminal autonomic cephalalgia or trigeminal neuralgia. Systematic history taking is most important for an accurate and differential diagnosis to rule out secondary headache. Generally, brain images are not required in primary headache with or without ocular pain. In this review, we focus on primary headache and trigeminal neuralgia with normal ocular and neurologic examinations and discuss methods for differentiating them based on history taking.
