상부요로 요로상피암종: 다중검출기 전산화단층촬영을 이용한 병기결정 및 조영증강에 대한 연구

Purpose: This study was designed to evaluate the diagnostic accuracy of multidetector-row spiral CT (MDCT) for the staging of urothelial carcinoma of the upper urinary tract and to assess the enhancement pattern. Materials and Methods: Sixty-seven patients with 76 histologically proven urothelial carcinomas of the upper urinary tract from 2003 to 2008 were included in this study. Imaging was performed with either a 64-channel or 16-channel MDCT scanner in the precontrast, cortical and excretory phases. Two radiologists who were blinded to the pathological results retrospectively reviewed the MDCT examinations in consensus. Tumor location, margin, peripelvic or periureteral spread, decreased renal parenchymal enhancement, dilatation of the upper urinary tract, the presence of lymphadenopathy and of a metastasis were described. Enhancement patterns of tumors were analyzed and were compared according to the WHO grade. Results: Thirty-eight tumors were identified in pelvocalyceal systems and 35 tumors were indentified in the ureter. The overall staging accuracy was 68.7%. The mean number of Hounsfield units of tumors in the cortical phase (91.4 25.2 HU) was statistically higher than for tumors in the excretory phase (80.5 15.1 HU). No significant differences for attenuation of tumors correlated with WHO grades. Conclusion: MDCT is an excellent modality for the detection and staging of upper urinary tract urothelial carcinoma. The enhancement pattern can be a helpful finding in the detection of urothelial carcinoma. 목적: 상부요로 요로상피암종의 병기평가에 대한 다중검출기 전산화단층촬영(multidetectorrow spiral computed tomography, 이하 MDCT)의 정확도와, 악성도에 따른 암종의 조영증강정도를 알아보고자 하였다. 대상과 방법: 2003년부터 2008년까지 병리조직학적으로 상부요로 요로상피암종으로 진단된 67명의 환자의 76예의 암종을 대상으로 하였다. 64 혹은 16 채널 MDCT로 조영제 주입 전, 피질기, 배설기 촬영을 시행하였다. 영상은 병리학적 병기를 모르는 두 명의 영상의학과 의사의합의로 후향적으로 분석하였다. 각각의 종양에 대해서 병변의 모양, 크기, 위치, 경계, 주변 신실질의 조영증강 변화, 주변조직으로의 침윤, 상부요로의 확장, 림프절비대와 원격전이를 기술하였고, 조직학적 악성도가 다른 그룹 간의 조영증강 감쇠치의 평균값을 비교하였다. 결과: CT는 68.7%의 병기결정 정확도를 보였다. 신배와 신우에 있는 병변은 38예였으며, 요관에 국한되어 있는 병변은 35예였다. 3예의 병변은 신배와 요관을 같이 침범하였다. 신우신배암의 경우 CT의 T 병기 정확도는 60.5%였으며, 요관암의 경우 T 병기 정확도는 82.9%였다. 신우신배암의 경우 조영증강정도는 피질기에서 배설기에 비해 높았으나, 요관암의 경우에는 양기간에 조영증강정도의 차이는 없었다. 신우신배암 및 요관암 내에서 조직학적 악성도에 따른 조영증강의 차이는 없었다. 결론: MDCT는 상부요로 요로상피암종의 진단 및 병기결정에 있어서 우수한 검사이다. 조영증강정도를 측정함으로써, 신종괴의 감별 및 타 충만결손과의 감별에 도움을 줄 수 있다.

역대기와 성전건축가 솔로몬

역대기에 나타난 왕들의 연설에 대한 연구에서부터 나타난 증거들이 솔로몬의 이상화를 입증하고 있는지 혹은 그렇지 않은지 결정하는 것은 쉽지 않다. 모든 경우에 있어서 본문에 의해 나타나는 강조점들은 솔로몬을 이상화라는 것이라기보다는 하나님을 영화롭게 하는 것이며, 하나님의 선택이며, 혹은 하나님의 왕국이다 솔로몬에 대한 연구에 관하여 야뱃은 솔로몬에 대한 역대기 저자의 관점에는 명백히 대조되는 두 가지의 경향들이 나타나고 있다고 말한다. 성전 건축가로서의 솔로몬의 역한에 놓인 강조는 많은 다른 요소들을 생략하게 하였다. 그러나 성전 건축가로서의 그의 활동의 많은 부분들이 다윗의 업적으로 돌려진다. 본문의 이와 같은 조정은 두 왕들의 공헌들을 이해하는데 균형을 제공하고 있으며 따라서 그플의 통치들이 본질적으로는 하나의 시대로 이해되게 한다. 아들의 엽적은 부친의 업적이 된다. 그러나 이와 같은 균형성은 이상화로 여겨질 수 없다.

길가메시 서사시와 성경

길가메시는 기원전 2600년경 우룩(창 10:10 에렉, 참고 스4: 9)을 다스리던 통치자이다. 당시는 도시 국가들 사이에 생존 경쟁이 치열하던 시대였다. 그의 실제적인 치적에 관해 알려진 바가 거의 없지만, 길가메시 서사시에 따르면 그는 영웅적인 전사이며, 우룩의 성벽을 건축한 사람이었다. 수메르 왕 목록에 따르면 그는 쿨랍의 대제사장의 아들이며, 우룩을 126년 동안 통치하였다 . (ANET. 266) 그가 죽은지 얼마 되지 않아서 그는 신들의 명부에 포함되었다. 한편 서사시에 따르면 그의 아버지는 우룩의 왕이자 신으로 숭배되었던 루갈반다 (Lugalbanda) 이며, 그의 어머니는 여신(女神) 닌순(Ninsun) 이었다.

Peroneal neuropathy scarcely can develop after massive weight reduction. A 21-year-old man complained left foot drop after 28% weight reduction (from 94 kgs to 67.5 kgs). During previous seven months, he played PC games with sitting cross-legged more than seven hours a day. In addition, he started a heavily restricted diet three months ago. Except for those, he had neither any medical history nor trauma to his knee. Electrophysiologic study showed the partial conduction block of left peroneal neuropathy at the fibular head. Four-week well balanced diet and physical therapy improved his foot drop. For the prevention of peroneal neuropathy related to weight reduction, well balanced diet and lifestyle modification are needed.

The Correlations of Parameters Using Contrast Enhanced Ultrasonography in the Evaluation of Prostate Cancer Angiogenesis

Purpose: The purpose of this study is to investigate the correlations of various kinetic parameters derived from the time intensity curve in a xenograft mouse model injected with a prostate cancer model (PC-3 and LNCaP) using an ultrasound contrast agent with histopathologic parameters. Materials and Methods: Twenty nude mice were injected with human prostate cancer cells (15 PC-3 and five LNCaP) on their hind limbs. A bolus of 500 μL (1 × 108microbubbles) of second-generation US contrast agent (SonoVue) was injected into the retroorbital vein. The region of interest was drawn over the entire tumor. The time intensity curve was acquired and then fitted to a gamma variate function. The maximal intensity (A), time to peak (Tp), maximal wash-in rate (washin), washout rate (washout), area under the curve up to 50 sec (AUC50), area under the ascending slope (AUCin), and area under the descending slope (AUCout) were derived from the parameters of the gamma variate fit. Immunohistochemical staining for VEGF and CD31 was performed. Tumor volume, the area percentage of VEGF stained in a field,and the count of CD31 (microvessel density, MVD) positive vessels showed correlation with the parameters from the time intensity curve. Results: No significant differences were observed between the kinetic and histopathological parameters from each group. MVD showed positive correlation with A (r=0.625, p=0.003), washin (r=0.462, p=0.040), AUC50 (r=0.604, p=0.005), and AUCout (r=0.587, p=0.007). Positive correlations were also observed between tumor volume and AUC50 (r=0.481, p=0.032), washin (r=0.662, p=0.001), and AUCout (r=0.547, p=0.012). Washout showed negative correlations with MVD (r=-0.454, p=0.044) and tumor volume (r=-0.464, p=0.039). The area percentage of VEGF did not show any correlation with calculated data from the curve. Conclusion: MVD showed correlations with several of the kinetic parameters. CEUS has the potential for prediction of tumor vascularity in a prostate cancer animal model.

Characteristics and Direction of Fruit and Vegetable Joint Sales Business in Korea - Focused on the Cases of Cooperative Joint Business Corporations in Jinju, Namwon, and Muju -

This study aims to examine the development process and characteristics of the joint agricultural product sales business carried out by cooperative joint business corporations founded through a joint investment of regional agricultural cooperatives, which entrusted them with the authority to execute sales. To this end, this study selected the cases of the Jinju Cooperative Joint Business Corporation, which had been converted from a joint business group; the Namwon Cooperative Joint Business Corporation, which had led joint sales in producing areas of multiple crops that cover several administrative districts; and the Muju Banditbul Cooperative Business Corporation, which had performed joint sales business in a major apple producing area. The analysis of these three cases was based on the internal data of each corporation with regard to their sales. The findings are as follows. Co-selected products have merits in terms of price compared to individually selected products. Agricultural products processing centers in producing areas are operated beyond the boundaries among the regional agricultural cooperatives for the efficient operation of the business. Co-selection and shipment associations are incorporated into several federations that established themselves on the same footing as operating partners. In addition to farms participating in co-selection associations, farms performing individual selection are also involved in the business, enabling the corporation to play the main role in creating a wide-area production center. Cooperative joint business corporations have shown their potential to lead local agriculture, continuously evolving to maintain stable supply of agricultural products and the strengthening of the bargaining power of participating farms in the market by scaling up the production of strawberries and adjusting the cropping season, as well as by promoting the introduction of new varieties of products and fostering them as new income crops for local communities.
