      • Impact of Interferon-Based Treatment on Quality of Life and Work Related Productivity from the Korean Cohort in the MOSAIC Study

        Aims: Chronic Hepatitis C Virus (HCV) infection increases the risk for progressive liver disease, hepatocellular carcinoma and negatively impacts the patient’s quality of life. HCV treatment is evolving with direct acting antivirals but IFN based therapy has been the standard of care for many years and remains available in some countries. The MOSAIC study aims to characterize patients with chronic HCV infection and assess the impact of IFN-containing treatment on health-related quality of life, work related productivity and health care utilization. Methods: MOSAIC is an international prospective multicenter observational study that has been conducted in 20 countries. Consecutive patients with chronic HCV infection were enrolled and those who initiated an IFN based regimen were prospectively followed for 48 weeks. We report results from the Korean cohort Results: 100 patients were enrolled: 86 were treatment naïve and 14 were treatment experienced. 33 patients initiated an IFN based regimen: 6 patients started IFN + RBV, 26 patients started Peg-IFN + RBV, none started Peg-IFN + RBV + DAA and 1 patient received other treatment. Among the treated cohort, demographic and disease characteristics were the following: the mean age was 54.5 years; 14 patients were male. 14 had minimal or no fibrosis, 2 portal fibrosis, 3 bridging fibrosis and 6 patients suffered from cirrhosis. HCV Genotype distribution was as follows: genotype 1: 11; genotype 2: 19 and genotype 3: 3. Table 1 describes the results at baseline and changes over 4, 12 and 48 weeks and end-of-treatment (EOT) for the quality of life and work productivity outcome measures (EQ-5D-5L, HCV-PRO and WPAI). Conclusions: Results from the Korean cohort of the MOSAIC study show a moderate trend for deterioration of health-related quality of life and work productivity associated with IFN based treatment for patients with chronic HCV infection during treatment period.       • Rocuronium의 애별투여방법이 모지내전근의 이완에 미치는 영향

        Objective : There are controversies in pnmmg technique of rocuronium whether it can or can't shorten the onset time. We want to compare the effects of two different priming doses and intervals of rocuronium. Materials and Methods : After giving propofol, 50 patients were randomly assigned to 5 groups. Group I received a placebo, followed 3 min later by rocuronium 0.6 mg/kg. Priming doses and intervals of Group II, Ill, N, V were 0.06 rng/kg and 2 min, 0.12 mg/kg and 2 min, 0.06 rug/kg and 3 min, 0.12 mg/kg and 3 min, respectively. Total dose of rocuronium was 0.6mg/kg. Train of four (TOF) count or ratio and 1st twitch height of TOF (T1) were monitored with 15 secondsintervals. TOF ratio of each groups just before bolus injection, duration for TOF count to reach zero and duration for Tl to reach 95% depression after bolus injection were compared. Results : Duration for TOF count to reach zero and duration for Tl to reach 95% depression after bolus injection were decreased in priming groups compared to Group 1 (P < 0.01) In group V, significant depression of TOF ratio just before bolus injection was seen (P < 0.05). Conclusion : Priming technique with rocuronium is effective method to shorten the onset of neuromuscular block. But priming dose of rocuronium itself could cause hypoxia and aspiration in awake patients and should be used carefully. Priming dose of rocuronium 0.06 mg/kg and 2 minutes interval maybe relatively safe method.

        비골골절 환자에 대한 정복 후 만족도에 대한 임상적 연구

        In the evaluation of the result of reduction of fractured nasal bone, 'the degree of satisfaction of the patient' is the most important factor. There were several reports regarding 'patient's satisfaction'. However they are not related to individual factors such as age, sex, cause of injury, method of reduction, etc but are the 'overall' rate of satisfied patients. We tried to know what should we do in the individual steps in treating fractured nose to reduce the rate of dissatisfaction. We analysed 120 consecutive patients who underwent reduction in Inje University Sanggye Paik Hospital due to fractured nose from August of 1989 to March of 1993. The results were as followings ; 1) There were many dissatisfied cases in second and third decades(p=0.054). 2) In case of severe combined injury requiring general anesthesia, the rate of dissatisfaction was high(p=0.061). 3) In cases with septal fracture, the rate of dissatisfaction was high(p=0.042). 4) The operation should be performed before 24 hours after trauma or on the fifth, sixth, and seventh posttraumatic day because the rate of dissatisfaction was low(p=0.023) 5) To evaluate the effect of splinting, further study is necessary. We should follow-up for longer time and it is difficult to define criteria of satisfaction of individual persons. We think that further research can clerify the protocol in the treatment of nasal bone fractures.

      • 경막 천자 후 두통 치료 목적의 자가 혈액 봉합술 시행 중 발생한 기뇌증

        A 57-year-old woman who was suffered from chronic back pain and radiating pain to left leg visited the local pain clinic. An accidental dural puncture happened at the L4-5 intervertebral space during the procedure with the loss of resistance technique using air. The epidural needle was removed, and a new insertion was done at the L3-4 intervertebral space with administration of the 0.5% lidocaine 8 ml mixed triamcinolone 40 mg. On the next day, she visited again the local pain clinic with complaining of headache. During an epidural autologuous blood patch procedure, she complained of a severe headache, nausea, vomiting, and developed generalized tremor of abrupt onset. There was no evidence of a dural puncture during the procedure. She was referred to our hospital at once. A brain CT scan showed air-filled cavities in basal cistern. She was given conservative treatment during 14 days. After 10 days, follow up brain CT scan showed no air-filled cavity in basal cistern. She was discharged from our hospital without complications.

      • 췌장암 환자에서 알코올 내장신경 신경파괴블럭 후 발생한 간염

        Splanchnic neurolytic block(SNB) with alcohol improves the management of pancreatic cancer pain. Adverse effects of alcohol SNB which include regional pain, hypotension, diarrhea, and acute alcohol intoxication are common, but acute hepatitis caused by alcohol neurolytic block is rare. A 63-year-old patient with pancreatic head cancer and liver metastasis had complete pain relief after both retrocrural tansdiscal SNB with 100% alcohol 10 ml. But, 1 day later, liver function test showed a high elevation in the aspartate aminotransferase (AST, 2182 IU/L) and alanine aminotransferase (ALT, 1807 IU/L). The patient had slight jaundice, general weakness, chilling, nausea and vomiting after SNB. Both AST and ALT decreased to 46 and 119 IU/L within 10 days with only supportive therapy. But, 19 days later, the patient died due to pneumonia. Our case illustrates the importance of clinical surveillance especially in patient with hepatic disease after alcohol SNB.

        Objective: Electroconvulsive therapy (ECT) is often associated with transient hypertension and tachycardia. Acute hemodynamic change may produce serious complications. Remifentanil is a potent ultra short acting opioid and has been found to reduce blood pressure and heart rate. The goal of this study was to evaluate effects of remifentanil on seizure duration, recovery parameters and hemodynamic changes during ECT. Methods: Ten ASA Ⅰ, Ⅱ patients enrolled in this prospective, randomized trial, each receiving more than four electroconvulsive therapy. Patients were randomized to receive placebo, group P (propofol 1mg/kg+0.9% normal saline 1ml, n=10), and propofol with remifentanil, group R (propofol 1mg/kg+remifentanil 1ug/Kg n=10). Supplemental dose of propofol was given as required to achieve loss of consciousness. We recorded hemodynamic parameters, seizure duration, and recovery parameters. Results: Seizure duration and recovery parameters were similar in group R and group P. Blood pressure and Hear rate after ECT were significantly lower in group R. Conclusions: Remifentanil(1ug/kg) attenuated the acute hemodynamic response to ECT under propofol anesthesia without adversely affecting the duration of seizure activity and recovery profile. Adjunctive use of remifentanil may be good alternative regimens for ECT.

      • Estradiol enhances alternative activation of macrophages in white adipose tissue

        목적: Purpose - Our aim is to investigate how estradiol affects adipose tissue immune milieu and determine whether it improves glucose homeostasis in the long-term absence of estrogen. 방법: Materials and Methods – Female C57BL/6 mice were fed either normal chow (NCD) or 60% high-fat diets (HFD) ad libitum for 8 weeks after sham-operation (sham) or ovariectomy (OVX). Male C57BL/6 mice were used as controls. Female mice underwent long-term estrogen deficiency were administered 25 or 100/ug/kg/day of 17beta-estradiol (E2) subcutaneously via implanted ALZET mini-osmotic pump and sacrificed 2 weeks later. Snice E2 replacement reduced food intake, subjects treated with vehicle were subdivided with two groups as food intake: ad libitum and pair feeding. For detecting immune milieu of visceral adipose tissue and glucose homeostasis, flow cytometry measurements, oral glucose tolerance test (OGTT) and insulin tolerance test (ITT) were performed, respectively. 결과: Results – E2 replacement reduced body weight and calorie intake compared with group received HFDs ad libitum. We classified ATMs into 2 groups: M1 (CD11c+CD206-) and M2 (CD11c-CD206+) macrophages. E2 replacement increased M2 ATMs compared with control group regardless of food control. To elucidate underlying mechanisms for increased ATMs in E2 group, we performed Ki67 and adoptive transfer of GFP labeled monocytes. It is identified that GFP positive M2 ATMs is detected in visceral adipose tissue after E2 administration in OVX mice. Next, to determine whether estrogen improves glucose metabolism, GTT and ITT were performed. Compared to pair-fed group, 25 or 100 ug/kg/day of E2-treated groups did not improved glucose metabolism regardless of E2 concentration. 결론: Conclusions - Our results suggest E2 increases M2 ATMs by inducing peripheral monocytes recruitment to visceral adipose tissue and does not improve glucose metabolism under long-term estrogen deficiency.

        We propose a damascene gate FinFET with Si nanocrystals implemented on bulk silicon wafer for low voltage flash memory device. The use of optimized SRON (Silicon-Rich Oxynitride) process allows a high degree of control of the Si excess in the oxide. The FinFET with Si nanocrystals shows high program/erase (P/E) speed, large VTH shifts over 2.5V at 12V/10㎲ for program and -12V/1㎳ for erase, good retention time, and acceptable endurance characteristics. Si nanocrystal memory with damascene gate FinFET is a solution of gate stack and voltage scaling for future generations of flash memory device.
