        Background and Objectives : Branchial cleft anomalies are encountered rarely in clinical practice. We reviewed medical records of branchial cleft anomalies according to age and sex distribution, site distribution, characteristics of mass on physical examination, location of mass, preoperative diagnosis, type of branchial cleft cyst and report our experience with 25 cases of branchial cleft anomalies. Materials and Methods : A review of medical records was performed on 25 cases of branchial cleft anomalies diagnosed in pathologic report from January, 1990 to March, 2000. Result : Sex distribution was 11 male and 14 female, age distribution was most common in third decade which was 8 cases(32%). Site distribution was predominant in left side(13 cases), and in physical examination, the characteristics of the mass was nontender(76%), mobile(60%), soft(80%). The lesion site was submandibular area in 2 cases, upper 1/3 portion of sternocleidomastoid muscle(SCM) in 2 cases, middle 1/3 portion of SCM in 20 cases and parotid area in 1 case. The preoperative diagnosis was branchial cleft cyst in 16 cases, cervical lymphadenitis in 3 cases, tuberculous lymphadenitis in 3 cases, branchial cleft fistula in 1 case, dermoid cyst in 1 case and parotid mass in 1 case. Among 25 cases, first branchial cleft cyst was seen in 4 cases, second branchial cleft cyst in 20 cases and second branchial cleft fistula in 1 case. Conclusion : The clinical history of recurrent neck inflammatory episodes in young patients and nontender, mobile, soft neck mass should raise the suspicion of this entity. Investigation using CT scanning in combination with aspiration is useful. Treatment of all branchial cleft anomalies is complete surgical excision because there is lack of spontaneous regression, a high rate of recurrent infection, the possibility of other diagnosis, and rare malignant degeneration. Complete removal without complications depends on a good understanding of regional embryogenesis, a knowledge of the surrounding structure, and awareness of the different anatomical presentation.

        총상에 의한 하악결손의 재건술

        Most mandibulr reconstructions are performed primarily or secondarily for bony defects caused by injuries or removal of tumors. An autogenous bone greft is still the method of choice for the restoration of mandibular defec. But a conventional autogenous free bone graft does not possess an intact blood supply and is dependent on local nutrition to survive. Severely scarred or radiated tissue are unfavorable beds for bone graft and even more the risk of resorption and infection is inceased in these kinds of a pooly vascularized bed. To overcome this difficulty, free vascularized bone graft has been tried with microsurgical gical technuque. A free vascularized bone graft has significantly higher rate of survival, with less resorption and higher resistance to infextion than a connentional free bone graft. A free vascularized iliac bone graft pedicled on deep circumflex iliac vessels is used widely, as it have many advantages such as, availability of large bloc of composite tissue, simplicity of donor dissection, large and long vascular pedicle and few donor site morbidity. In this paper, we represent 4 cases of reconstruction of wide mandibular defects using free vascularized iliac bone graft and free dsteocutaneous groin flap, and review the characteristics of gunshot wound. The result is satisfactory.

      • 광주시내 업종별 음식점의 위생상태 및 식중독원인 세균의 분포 특성

        업종별 음식점 주방의 위생상태는 일식>중식>한식의 순이었고 개인위생의 경우는 일식>한식>중식의 순이었는데 주방 및 종업원의 위생상태를 향상시키기 위해서는 음식점 규모의 대형화, 45℃ 이상의 온수공급, 주방시설의 표준화, 위생비닐장갑의 착용, 및 유니폼 착용이 신속하게 이루어져야 할 것이다. 업종별 대중음식점의 주방기구중의 세균분포는 전반적으로 일반세균수가 높게 나타났고 특히 대장균군수가 높게 나타났는데 음식물이 아니고 조리기구이긴 하지만 우리 음식점의 위생상 문제가 많은 것으로 생각된다. 미국 육군 Natick연구소가 제시한 음식물의 미생물 기준한계인 1 ×10??마리 이하, 대장균수가 100마리 이하와 비교할때 일반세균수의 경우는 아주 만족할만한 위생상태로 평가되었으나 대장균수는 개숫물의 경우 업종과 관계없이 높게 나타났고 행주와 칼의 경우 일식음식점에서만 높게 나타났으며 도마의 경우는 모두 100마리 이하로 양호한 위생상태로 평가되었다. 한식 전문음식점간의 세균수가 큰 차이를 보인 것은 규모와 운영방법이 서로 다르기 때문으로 앞으로 한식음식점에 대한 시설 및 환경개선에 대한 노력이 요구되었다. 또한 일식음식점간에는 일반세균수의 분포가 크게 다는 것으로 조사되었는데 특히 개숫물, 행주 및 도마중에 존재하는 미생물수가 크게 차이가 있었다. 등급별 한식음식점의 세균분포는 호텔급 음식점> 일반대중급 음식점> 여관급 음식점의 순으로 나타나 업소규모가 클수록 위생시설 및 위생습관의 수준이 높아 위생상태가 양호한 것으로 분석되었고 여관급음식점의 경우 위생상태가 낮은 것은 이용빈도가 낮은 것이 그 원인이 아닌가 생각되었다. Sanitary evaluation of kitchen area in different types of restaurants was the best at Japanese style restaurants and the worst aat Korean style restaurants. In personal hygiene Japanese style restaurant were evaluated as the best, but Chinese style restaurants were evaluated as the worst. Total and coliform bacterial counts were mostly showed high levels at all types of restaurants and especially this tendency was predominated in coliform counts. As compared to the standards suggested by U.S. Army Natick, however, total bacterial counts in all of supplies were evaluated as the satisfactory state. On the other hand, coliform counts in dish-washing water were much higher than the standard, which were not affected by the types of restaurant, and Japanese style restaurants were higher in wiping cloths and knives, but cutting boards were evaluated as satisfactory state with the counts less than 1.0 ×10².Bacterial counts in Korean style restaurants were greatly different depended on the size or type of restaurant. Total bacterial counts in Japanese style were also different among restaurants, especially in dish-washing water, wiping cloths, and cutting boards. According to the results of the evaluation of Korean style restaurant, bacterial distribution based on the class of restaurants, classified as hotel, motel, and public classes, hotel class restaurants showed lower levels of bacterial counts than public class. This result suggested that the larger and higher classes of restaurants maintained the satisfactory state without potential hazards.

      • 광주시내 업종별 음식점의 주방기구와 관련된 주요 세균 및 이들의 계절별 변화

        업종별 음식점에서 사용중인 주방기구 가운데 개숫물중에 존재하는 세균들을 분리·동정한 결과, 한색음식점에서 37균주, 일식음식점에서 23균주, 및 중식음식점에서 33균주가 분리되었고 이들 중 한식음식점에서 E. coli 등 16균주, 일식음식점에서 Listeria sp. 등 20균주, 중식음식점에서 Micrococcus sp. 등 15균주를 각각 동정하였다. 이들 세균들은 업종별 음식업소에서 주로 사용되는 색품재료에 존재하는 세균들과 밀접한 관계가 있는 것으로 밝혀졌고 E.coli와 같은 일부 세균들은 주방의 불결한 환경이나 종사자로부터 오염된 것으로 추정되었다. 업종별 음식점에서 사용중인 개숫물, 행주, 칼, 및 도마에 존재하는 세균의 분포는 일반세균수 및 대장균수가 모두 2월에서 8월까지 점점 증가하다가 11월에는 다시 감소하는 경향을 보였다. 특히 이같은 경향은 일반세균은 개숫물에서 대장균군은 칼에서 가장 뚜렷하게 나타났다. 따라서 식중독 발생 가능성이 가장 높은 여름철에는 칼, 도마 등 주방기구들은 정기적으로 열탕소독하는 철저한 위생처리가 필수적인 것으로 생각되었다. Thirty-seven, twenty-three, and thirty-three bacterial strains were isolated from dish-washing water which was selected as a kitchen supply commonly loaded the highest level of bacteria among various supplies using at Korean style, Japanese style, and Chinese style restaurants. Among these isolates, sixteen strains including Esherichia coli, twenty strains including Listeria sp., and fifteen strains including Micrococcus sp. were identified from Korean style, Japanese style, and Chinese style restaurants, respectively. This result suggested that most isolated bacteria were closely related to bacterial flora of raw food materials used in each style of restaurants and others could be contaminated from the surrounding environment and the employees. Total and coliform bacterial counts distributed in dish washing water, wiping cloths, knives, and cutting boards used at different types of restaurants were gradually increased from February to August and then decreased until November. This changing tendencies of total bacterial counts and coliform group counts were obvious in dish-washing water and in knives, respectively. Therefore, kitchen supplies such as wiping cloths, cutting boards, and knives should be treated carefully with hot water of above 45℃ at every certain period of time to kill some of these potential hazardous bacteria and to prevent outbreaking foodborne-illness by them, especially during Summer.

        혈관평활근세포에서 산화에너지대사 억제에 의한 아밀로이드전구단백질 대사의 변화

        Background: A reduction in the activity of cytochrome c oxidase(COX) has been recently identified in mitochondria from platelets and postmortem brain tissue of AD patients Sodium azide (NaN₃). a COX inhibitor, is an effective chemical agent producing energy shortage and oxidative stress both in vitro and in vivo system Furthermore it has been suggested that vascular compromise could be either directly involved AD pathogenesis or indirectly associated with triggering pathogenetic events leading to AD This study was performed to investigate amyloid precursor protein (APP) metabolism by inhibition of mitochondrial energy metabolism in cultured vascular smooth muslce cells (VSMCs) Materials and Methods: VSMCs isolated from the aorta of seven weeks old Spraque-Dawley rat were treated with NaN₃in a low concentration (100-500μM) or in a high concentration (1-100mM) Cellular proliferation and viability were determined by MTS (3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenly)-2H-tetrazolium) assay Cellular APP was detected with N-terminal specific antibody 22C11. Celldeath was determined by observation of morphology and terminal deoxynucleotidyl transferase-mediated dUTP-fluorescein nick end labeling stain (TUNEL) We used ginkgo biloba extract(EGb761) and melatonin as anti-oxidants to investigate the mechanism of latered APP metabolism Results: The viability of VSMCs was increased after treatment with 1 mM and 10mM NaN₃(p<0.05) unitl 3 hr and then dimnished Many TUNEL positive cells were found in 10mM and 100mM treatment group. but were not apoptotic in nature 22C11 immunoreactivity was not changed at 3 hr, 6hr, 12hr Anti-oxidants reduced cellular proliferation (p<0.05). but did not block TUNEL positivities and did not influence the 22C11 immunoreactivity In a low concentration NaN₃ treatment group the viability of VSMCs was increased concentration dependently(p<0.05) Immunoblot with 22C11 showed the concentration dependent decrease at 145 kDa, 125 kDa. and high molecular weight range (>160kDa) TUNEL staining showed DNA fragmentations and condensations of nuclear chromatin suggesting apoptosis After treatment with anti-oxidants, the cellular proliferation was more decreased (p<0.05), and TUNEL positive cell deaths were blocked Immunoreactivities of 125 kDa (immature APP). 145 kDa (mature APP). and higher molecular weight bands were recovered below 400μM of NaN₃ Immunoreactivity of 145 kDa was recovered in 100 μM NaN₃ treated group Conclusions: The presumed mechanism of low concentration COX inhibitor is the overproduction of reactive oxygen species resulting from a depression of the mitochondiral electron transport chain. whereas potential consequence of high concentration COX inhibitor might be related to decreassion of ATP synthesis and bioenergetic impairment Reactive oxygen radicals in response to low concentration COX inhibitor alter the processing of APP in VSMCs This investigation demonstrated analtered APP metabolism as a peripheral marker of AD Therefore VSMCs treated with low concentration COX inhibitor could be concsidered as a novel in vitro model of AD.

      • 프리미돈의 한국인에서의 생체이용율시험

        A reversed phase HPLC method was developed and validated for the determination of primidone in human plasma. Propylthiouracil was used as an internal standard. Calibration curves were linear in the concentration range of 10-120 ng/㎖. The coefficient of variation of the intra- and inter-day precision were below 15%. The coefficient of variation of the accuracy were below 15% in the concentration range investigated. A bioavailability study was performed using the validated HPLC method. Twenty four healthy human male volunteers were orally administered 20 mg of primidone. The pharmacokinetic parameters were calculated using WinNonlin. The mean values of AUC_(LAST) was 85.0±17.7 ㎍ hr/㎖, C_(max) was 4.22±1.04 ㎍/㎖, T_(max) was 1.85±1.13 hr, t_(1/2) was 17.7±2.4 hr. The pharmacokinetic parameters and the HPLC method can be used for the desigm of bioequivalence study of primidone.

        Simple sequence repeat marker development from Codonopsis lanceolata and genetic relation analysis

        In this study, we developed 15 novel polymorphic simple sequence repeat (SSR) markers by SSR-enriched genomic library construction from Codonopsis lanceolata. We obtained a total of 226 non-redundant contig sequences from the assembly process and designed primer sets. These markers were applied to 53 accessions representing the cultivated C. lanceolata in South Korea. Fifteen markers were sufficiently polymorphic, and were used to analyze the genetic relationships between the cultivated C. lanceolata. One hundred three alleles of the 15 SSR markers ranged from 3 to 19 alleles at each locus, with an average of 6.87. By cluster analysis, we detected clear genetic differences in most of the accessions, with genetic distance varying from 0.73 to 0.93. Phylogenic analysis indicated that the accessions that were collected from the same area were distributed evenly in the phylogenetic tree. These results indicate that there is no correlative genetic relationship between geographic areas. These markers will be useful in differentiating C. lanceolata genetic resources and in selecting suitable lines for a systemic breeding program.

      • Hydrogen for the Future Green Energy

        Many agree that the future energy will be hydrogen after the recent energy crisis which brought us to reconsider seriously about possible alternatives after fossil fuel for mankind. There are abundant sources for hydrogen-water, and it burns to form water. However, in order for us to adopt the new energy system, many hurdles have to be overcome. They are how to produce hydrogen economically, ways to store and transport safely, and ways to generate electricity efficiently, to name a few. Electrochemical generation of hydrogen needs a high overvoltage, thus, it is a very expensive way at the moment. Also, storage and transport require many fundamental researches. Efficient use of hydrogen is fuel cell. Fuel cells have been received a considerable interest as power sources because they exhibit high energy efficiency and are environmentally friendly. Despite these advantages, fuel cells in general need to overcome economic barriers for wide applications. Main barriers are the cost of materials such as platinum used in catalysts and proton conducting polymer. Catalysts are the core material for fuel cell and many attempts have been made to find out the most active, efficient and durable formulae in the MEA preparation step.[1-2] In this respect, works performed in our group for fuel cell catalysts will be summarized, they are preparation of by galvanostatic pulse electrodeposition on Nafion-bonded carbon layer [3-5], and effect of carbon supports for catalysts. Improved catalytic performance by choosing appropriate flow channels in the support was also observed.
