        To satisfy the increasing needs for blood transfusion it is necessary not only to find new blood donors but also to repeat the collection of blood from the same donors. It was recognized that too frequent blood donations might become detrimental to the health of the donor, above all in female donors in foreign countries where the blood program relies on voluntary donation. Unfortunately, the sources of much blood depends solely on professional donors except in a few instances in Korea. Thus, anemia of professional donors after repeated blood donation must be controlled. For this purpose, we studied hemoglobin levels and serum iron levels of professional donors, coming to Korean Red Cross Blood Service and Seoul Medical College Hospital. The data obtained in this study were as follows: (1) 6. 0% of male professional donors and 17.98% of female professional donors were disqualified owing to a low hemoglobin levels. On the other heand, only 0.8% of male voluntary donors and 10.6% of female voluntary donors were rejected. (2) 7.3% of male donors and 33.3% of female donors were rejected their donations because of low hemoglobin levels on their 5-6th visits. (3) The serum iron levels appeared to be decreased after repeated blood donations. Specially, serum iron levels of female donors decreased to 73. 6 mcg. /d. after 2-4 blood donations yearly. So, it might be recommended to limit the frequency of blood donations from female.

        재료내 다중결함에 의한 SH형 초음파 산란장의 수치해석

        비파괴검사중의 하나인 초음파 탐상검사는 교량, 발전설비 및 석유화학플랜트 등의 각종 다양한 구조물들의 안전성 확보를 위한 내부결함 및 손상평가를 위하여 일반적으로 폭넓게 사용되고 있다. 초음파를 이용한 비파괴 평가 기술은 각종 구조물에 존재하는 내부결함에 의한 산란신호를 통해 건전성을 평가하는 기법이므로 결함의 신뢰성 높은 정량적 평가를 위해서는 결함으로부터의 초음파 산란신호특성에 대한 기본적 이해가 필수적이며 따라서 이를 위한 모델링 수치해석 연구가 요구된다. 본 연구에서는 동탄성 경계요소법을 이용하여 무한체내에 존재하는 다중기공 결함에 의한 초음파 수평횡파의 근거리 산란특성에 대하여 결함의 형상과 결함사이의 상호작용을 고려하여 해석하였다. 본 연구에서 얻어진 결과들은 결함검출 민감도의 개선 및 역변환 해석에 의한 정량적 비파괴 평가에 큰 도움을 줄 수 있다. In order to assure the reliability and integrity of structures such as bridges, power and petrochemical plants, nondestructive evaluation techniques are recently playing more important roles. Among the various kinds of nondestructive evaluation techniques, ultrasonic technique is one of the most widely used methods for nondestructive inspection of internal defects in structures. For the reliable quantitative evaluation of internal defects from the experimental ultrasonic signals, a numerical analysis of ultrasonic scattering field due to a defect distribution is absolutely required. In this paper, the SH-wave scattering by multi-cavity defects using elastodynamic boundary element method is studied. The effects of shape of defects on transmitted and reflected fields are considered. The interaction of multi-cavity defects in SH-wave scattering is also investigated. Numerical calculation by the boundary element method has been carried out to predict near field solution of scattered fields of ultrasonic SH-wave. The presented results would be useful to improve the sensitivity of flaw defection for inverse analysis and pursue quantitative nondestructive evaluation for inverse problem.

        선천성 수정 대혈관 전위증 - 부검1 예 -

        The corrected transposition of the great arteries is a rare congenital heart anomaly consisting of the discordant connections of both the atria to ventricles and the ventricles to the great arteries. But its circulation is physiologically corrected and therefore it is compatible with life unless there are associated lesions. Our case was associated with VSD, COA, and PDA. He was slightly cyanotic and irritable. Despite of oxygen supply, his symptoms were not improved and he eventually died in 52 hours after delivery. Medical record revealed severe respiratory and metabolic acidosis.

        상악 치아군의 저항중심의 위치에 관한 3차원 유한요소 해석

        최근 골내 고정 형태의 temporary anchorage device (TAD)를 많이 이용하게 되면서 다양한 위치로부터 그리고 강한 교정력을 이용할 수 있게 되었다. 이에 따라 치아군의 이동양상을 예측하고 치료계획을 세우기 위하여 다양한 치아군의 저항중심의 위치에 대한 이해가 필요하게 되었다. 본 연구에서는 3차원 유한요소해석을 이용하여 상악 4전치, 6전치 그리고 상악 전치열에서 3차원적 저항중심의 위치를 조사하고자 하였다. 이를 위하여 상악 전치열 14개 치아와 치근막 및 치조골의 3차원 유한요소모델을 제작하였고, 각 치아군별로 치관부를 협측, 설측 호선, 설측 splint wire로 고정하여 개별 치아이동을 최소화하고 적용된 힘이 치아에 고루 분산되도록 하였다. 상악 중절치 절단연의 중점에서 연장된 와이어 빔에 수직, 수평으로 100 g 또는 200 g의 힘을 가하여 치아의 변위를 해석하고, 각 치아군에 속한 치아들이 최대한 평행이동 되는 힘의 적용부위를 저항중심으로 정의하였다. 연구결과 상악 4전치군의 저항중심은 상악 중절치 절단연으로부터 치근방향 13.5 mm, 후방 12.0 mm, 상악 6전치군은 상악 중절치 절단연으로부터 치근방향 13.5 mm, 후방 14.0 mm에 위치하였으며 상악 전치열군의 저항중심은 상악 중절치 절단연으로부터 치근방향 11.0 mm, 후방 26.5 mm에 위치하였다. 본 유한요소 실험모델을 이용하여 얻은 결과는 교정치료의 효율성을 높일 수 있으리라 생각된다. Objective: The aim of this study was to investigate the 3-dimensional position of the center of resistance of the 4 maxillary anterior teeth, 6 maxillary anterior teeth, and the full maxillary dentition using 3-dimensional finite element analysis. Methods: Finite element models included the whole upper dentition, periodontal ligament, and alveolar bone. The crowns of the teeth in each group were fixed with buccal and lingual arch wires and lingual splint wires to minimize individual tooth movement and to evenly disperse the forces to the teeth. A force of 100 g or 200 g was applied to the wire beam extended from the incisal edge of the upper central incisor, and displacement of teeth was evaluated. The center of resistance was defined as the point where the applied force induced parallel movement. Results: The results of study showed that the center of resistance of the 4 maxillary anterior teeth group, the 6 maxillary anterior teeth group, and the full maxillary dentition group were at 13.5 mm apical and 12.0 mm posterior, 13.5 mm apical and 14.0 mm posterior, and 11.0 mm apical and 26.5 mm posterior to the incisal edge of the upper central incisor, respectively. Conclusions: It is thought that the results from this finite element models will improve the efficiency of orthodontic treatment.

        Cantrell"s pentalogy may be defined as a failure of fusion of the midline from the sternum to the umbilicus. Thus, this malady consists of multiple anomalies of the sternum, heart, pericardium, diaphragm and umbilicus or anterior abdominal wall. According to the degrees of each anomaly, various operations can be planned as a one-stage operation or as a multi-stage operation and then palliative or corrective operations. The authors experienced a case of Cantrell"s pentalogy that consisted of a bifid sternum, ventricular septal defect, atrial septal defect, ventricular diverticulum, dextrocardia, pericardial defect, anterior diaphragmatic defect and diastasis recti; all of these problems were corrected by a one-stage operation.

      • RESEARCH PAPER : Docetaxel-Loaded Thermosensitive and Bioadhesive Nanomicelles as a Rectal Drug Delivery System for Enhanced Chemotherapeutic Effect

        PURPOSE: To investigate the potential of thermosensitive and biadhesive nanomicelles in improving the bioavailability of docetaxel (DCT) and its chemotherapeutic effect. METHOD: DCT-loaded nanomicelles were prepared by emulsufication and characterized in terms of physico-chemical and visco-elastic parameters. The optimzed formulation was evaluated for in vivo localization, pharmacokinetic and anti-tumor efficacy. RESULTS: The hydrodynamic size of DCT-loaded nanomicelles was approximately 13 nm and thenanomicelles exhibited a sufficient gelation strength (9250 mPa·s) and bioadhesive force (2100 dyn/cm²) to be retained in the upper part of rectum. We observed a high rectal bioavailability of 29% DCT compared to that following oral administration in rats, as it successfully evaded the multidrug efflux transporters and hepatic first-pass metabolism. Plasma concentration around ∼50 ng/mL was maintained throughout the study period (12 h) while Taxotere® attained subtherapeutic range within 4 h of drug administration. Results also revealed that the rectally administered DCT-loaded nanomicellesexhibited a significant anti-tumor effect (200 mm³) with a reduced toxicity profile when compared to orally administered DCT (950 mm³). Furthermore, histological study showed that the rectal mucosa was completely intact with no signs of irritation upon treatment with DCT-loaded nanomicelles. CONCLUSIONS: Taken together, our novel thermosensitive and biadhesive nanomicellesdemonstrated the ability to improve the bioavailability and chemotherapeutic potential of DCT in vivo. To the best of our knowledge, this is the first report describing the rectal delivery of DCT-loaded nanomicelles.

        Case Reports : Endobronchial Metastasis of Epithelioid Sarcoma

        Epithelioid sarcomas are rare soft tissue sarcomas with a high tumor grade and high local recurrence and metastasis rates. Although the lung is the most common site of metastasis, endobronchial metastasis hasn`t been reported yet. We now report a case of epithelioid sarcoma with endobronchial metastasis. A 28-year-old man had recurrent pneumothorax and underwent wedge resection. He presented at our hospital with hemoptysis, dyspnea, and chest pain. Chest computed tomography revealed left pneumothorax, multiple lung nodules and endobronchial lesions at the right lower basal lobe. Bronchoscopy showed a hemorrhagic mass obstructing the bronchus of the right lower basal lobe. Magnetic resonance imaging revealed multiple nodular lesions in the left thigh muscles. The bronchoscopic biopsy of the endobronchial lesion and the muscle biopsy of the thigh showed the same feature epithelioid sarcoma. This is the first case report of an epithelioid sarcoma with endobronchial metastasis that was diagnosed by bronchoscopic biopsy.
