        비기능성 부신피질암은 발생 빈도가 극히 드문 악성 종양이며, 내분비학적 임상증상이 없어 주로 복부 종물에 의해 우연히 발견된다. 그래서 대부분 종양의 크기가 거대하고 전이로 진행된 경우가 흔하며, 절제술만으로는 치유 불가능한 경우가 많아 예후가 불량한다. 최근 저자들은 약 10년간 간헐적인 좌측 측복통과 흉부 압박감으로 내원하여, 이학적 검사 및 방사선학적 검사상 복부 종물이 확인되고 생화학적 검사에서 비기능성으로 판명된 52세 남자 환자에서 근치적 신절제술을 시행하여 병리조직검사상 비기능성 부신피질암으로 진단되었기에 문헌 고찰과 함께 보고하는 바이다. Nonfunctioning adrenocortical carcinoma is extremely rare, which is diagnosed most often in older man because of mass effect. Most tumors are discovered too late for curative resection, and advanced tumors are associated with a poor prognosis. Herein we report on a 52-year-old man with nonfunctioning adrenocortical carcinoma, treated by radical nephrectomy.

        논문(論文) : 모재(慕齋) 김안국(金安國)의 시문학 연구(詩文學 硏究) -그의 충정시(忠情詩)를 중심으로-

        慕齋 金安國은 성종 9년에서 중종 38년(1478∼1543)까지 살았던 조선 중기의 학자요 문인이다. 이 시기는 정치적으로 사림파와 훈구파의 갈등으로 인해 기묘사화까지 발생했던 매우 혼란한 시기였다. 또한 문학적으로는 사림파 문학의 맹아기인 15세기 후반과 사림파의 성향이 본격적으로 자리잡게 된 16세기 중기 사이에 자리한 과도기적인 시기였다. 지금까지 진행된 모재 김안국에 대한 연구를 살펴보면 그의 방대한 시편에 비하여서 많은 연구가 이루어지지 못하였는데 이것은 사림파라는 집단적 성격 속에 함께 묶여 김안국 개인에 대한 관심은 상대적으로 적었고, 또한 기묘사화와 관련된 정치적인 상황, 성리학자로서의 학자적 태도 등이 부각되어 정작 그의 문학 세계는 많은 관심을 받지 못한 것이라고 여겨진다. 이에 모재의 시세계 속에서 드러나는 그의 관료로서의 의식과 삶에 시선을 고정하여 고찰하였다. 온건적인 개혁 성향으로 인해 관료로서의 忠情을 제대로 인정받지 못하고 세간에서 煩碎하다는 평을 받기도 한 모재 김안국의 시세계속에서 드러나는 忠情의 다양한 면모를 세밀하게 살펴보았다. 그의 충정은 모재의 삶과 정신세계에 있어서 자신을 지탱해 주는 중심축이었으며 詩作活動에 있어서의 중요한 모티브였기에 ``忠``으로 일관한 그의 시세계를 살펴보는 것은 매우 중요한 연구과제라 할 수 있겠다. 1. 出 · 處間의 葛藤, 2. 官僚的 自省意識에서는 모재의 忠情이 뿌리하고 있는 의식세계를 살펴보았고, 3. 戀君之情의 表出, 4. 愛民意識의 發現에서는 모재의 충정이 外的으로 드러나 보여지는 구체적인 면모를 추출해 보았다. 이러한 연구결과는 그동안 조명 받지 못했었던 모재의 관료로서의 진면목을 밝히기 위해서 필요한 작업이며, 아울러 모재의 문학세계에 대한 적극적인 관심을 갖게 되는 계기를 만들기 위한 것이다. Kim Ahn-Kook(Mo-Jae) is a scholar and literary man, who lived from the 9th year of Sungjong`s reign to the 38th year of Joongjong`s reign (1478 ~ 1543) in the middle age of Chosun. The period was chaotic due to conflicts between two political lines, Sa-rim Pa and Hun-gu Pa, culminating with Gi-Myo-Sa-Hwa, a political warfare in the year of Gi-Myo. In terms of literary history, the period was a transitional stage from the newly-emerged Sa-rim Pa in the late 15th century to the flourishing age of Sa-Rim Pa in the mid 16th century. Study on Mo-Jae has not been large enough to address the enormous work of his poems. It is because Mo-Jae has been viewed mainly as one of Sa-Rim Pa group, rather than as an individual, and also because regarding him his political engagements and scholarly achievements as a Neoconfucianist have mainly interested researchers. That is to say, his literary works have been relatively neglected compared to his political stance and academic contributions. This study explored his life and ideas as a scholar-official in his poems, along with consideration to his literary views. Being a moderate reformist his loyal dedication stance was not fully appreciated, and his pursuance of bureaucratic career earned negative evaluation to his literary works for nor being pure from worldly desires and for being miscellaneous. Thus, this study took in-depth look at a variety of aspects of his loyal dedication. Since his loyal dedication was the very basis of his life and was an important motif in his verse making, this study`s approach to his poetic world with a focus on his loyal dedication can have significant meaning. Therefore, this study examined his inner world where his loyal dedication was rooted by looking at 1. conflicts from leading an official life and 2. expressions of his conscience as an officer, and then addressed specific cases of his loyal dedication by looking at 3. expressions of his loyalty to the king and 4. expressions of his love for people. This study was a necessary job to portray Mo-Jae`s true face as an officer and literary man, which had been relatively neglected, and to promote interest in Mo-Jae`s literary world.

        Validation of an automated adenoma detection rate calculating system for quality improvement of colonoscopy

        Purpose: This study aimed to validate an automated calculating system developed for determining the adenoma detection rate (ADR). Methods: To calculate the automated ADR, the data linking processes were as follows: (1) matching the selected colonoscopy results with the pathological results, (2) matching the polyp number from colonoscopy with that from pathology and confirming the histopathological results of each colonic polyp, and (3) confirming the histopathological results, especially the adenoma status of each colonic polyp. To verify the accuracy of the automated ADR calculating system, we manually calculated the ADR for 3 months through medical record review. Accuracy was calculated by measuring the error rate for each value. The cause of error was analyzed by additional order and chart review. Results: After excluding 318 cases, 2,543 patients (1,351 men and 1,192 women; median age, 57.9 years) who underwent colonoscopy were included in this study. When the automated calculating system was used, polyps were found in 1,336 cases (52.6%) and adenomas were found in 1,003 cases (39.4%). When the manual calculating system was used, polyps were found in 1,327 cases (52.2%) and adenomas were found in 1,003 cases (39.4%). The accuracies of the polyp detection rate and ADR according to the automated calculating system were 99.3% and 100%, respectively. Conclusion: We developed a system to automatically calculate the ADR by extracting hospital electronic medical record results and verified that it provided satisfactory results. It may help to improve colonoscopy quality.

        Antenatal Corticosteroids and Clinical Outcomes of Preterm Singleton Neonates with Intrauterine Growth Restriction

        Purpose: We assessed the influence of antenatal corticosteroid (ACS) on the inhospital outcomes of intrauterine growth restriction (IUGR) infants. Methods: A retrospective study was conducted with singletons born at 23+0 to 33+6 weeks of gestation at Seoul National University Hospital from 2007 to 2014. We compared clinical outcomes between infants who received ACS 2 to 7 days before birth (complete ACS), at <2 or >7 days (incomplete ACS), and those who did not receive ACS in IUGR and AGA infants. Multivariate logistic regression using Firth’s penalized likelihood was performed. Results: 304 neonates with 91 IUGR neonates were eligible. Among AGA neonates, mortality (adjusted odds ratio [aOR], 0.13; 95% confidence interval [CI], 0.02 to 0.78), hypotension within 7 postnatal days (aOR, 0.20; 95% CI, 0.06 to 0.64), and severe bronchopulmonary dysplasia (BPD) or death (aOR, 0.24; 95% CI, 0.07 to 0.77) were lower in complete ACS group after adjusting for pregnancy induced hypertension and uncontrolled preterm labor. Mortality (aOR, 0.18; 95% CI, 0.04 to 0.78), hypotension (aOR, 0.26; 95% CI, 0.09 to 0.70), and severe BPD or death (aOR, 0.33; 95% CI, 0.12 to 0.92) were also lower in the incomplete ACS group. Among IUGR infants, after adjusting for birth weight and 5-minute Apgar score, inhaled nitric oxide use within 14 postnatal days was lower in both complete ACS (aOR, 0.07; 95% CI, 0.01 to 0.67) and incomplete ACS (aOR, 0.04; 95% CI, 0.01 to 0.37) groups. Conclusion: ACS was not effective in reducing morbidities in IUGR preterm infants.

        <B>ABSTRACT</B><P>The performance of the SD Bioline rapid antigen test kit for influenza virus detection was evaluated with 295 respiratory specimens during the influenza season. The overall sensitivity and specificity of the SD Bioline test were 61.9% and 96.8% for the influenza A virus antigen and 54.5% and 100% for the influenza B virus antigen, respectively. The results were consistent with peak influenza activities.</P>

        일부재료 ( Oatrim ) 대체가 육류제품의 품질에 미치는 영향

        Meat provides high quality proteins, lipids, minerals and vitamins. The meat protein is especially high in essential amino acids that are crucial for human health, growth & development and for the formation of enzymes, hormones and antibodies. Relatively cheap and nutritionally sound vegetable proteins that are similar to animal proteins are being developed to replace the animal proteins in texture, nutrition and food characteristics. In this study a nutritionally sound meat lipid replacing food Oatrim that has been produced by converting oat starch into maltodextrin by α-amylase, have been partially substituted for beef and general component analysis, texture measurement and sensory tests have been conducted. The results are 1. Water content of the non-treated (0% treated) was 67.1% and the treated (10% treated) was 77%. The treated showed better water holding capacity. 2. Protein content of the non-treated was 21.2g/100g; the 4% treated, 18.4g/100g; the 6% treated, 18.2g/100g; the 8% treated, 17.2g/100g; and the 10% treated, 16.0g/100g. The protein content tended to decrease as the replacing ratio increased but between 4%, 6%, and 8% the difference was not significant. 3. Amino acid analysis results showed that glutamic acid content was the highest in Oatrim and as its amino acid make up is exellent, it is valuable as a fine low fat protein food. 4. Sensory tests show that the increased Oatrim content increased the appearance quality but food characteristics were high only in the 4% and 6% treated groups, indicating that the replacement ratio should not exceed 10%. 5. Texture measurement analysis results show that the higher the replacement content, lower the springness, cohesiveness, hardness, chewiness and gumminess, resulting in relatively soft overall texture. However, in order to better the food chracteristics, more studies must be continuously done, and so by being able to increase vegetable substitution over meat, it may be able to contribute to the prevention of adult disease.

        Perilipin 1 (Plin1) deficiency promotes inflammatory responses in lean adipose tissue through lipid dysregulation

        <P>Lipid droplets are specialized cellular organelles that contain neutral lipid metabolites and play dynamic roles in energy homeostasis. Perilipin 1 (Plin1), one of the major lipid droplet-binding proteins, is highly expressed in adipocytes. In mice, Plin1 deficiency impairs peripheral insulin sensitivity, accompanied with reduced fat mass. However, the mechanisms underlying insulin resistance in lean Plin1 knockout (Plin1(-/-)) mice are largely unknown. The current study demonstrates that Plin1 deficiency promotes inflammatory responses and lipolysis in adipose tissue, resulting in insulin resistance. M1-type adipose tissue macrophages (ATMs) were higher in Plin1(-/-) than in Plin1(+/+) mice on normal chow diet. Moreover, using lipidomics analysis, we discovered that Plin1(-/-) adipocytes promoted secretion of pro-inflammatory lipid metabolites such as prostaglandins, which potentiated monocyte migration. In lean Plin1(-/-) mice, insulin resistance was relieved by macrophage depletion with clodronate, implying that elevated pro-inflammatory ATMs might be attributable for insulin resistance under Plin1 deficiency. Together, these data suggest that Plin1 is required to restrain fat loss and pro-inflammatory responses in adipose tissue by reducing futile lipolysis to maintain metabolic homeostasis.</P>

        Aortic Valve Replacement in the Era of Transcatheter Aortic Valve Implantation: Current Status in Korea

        Background: In the era of transcatheter aortic valve implantation, this study was conducted to investigate the current trend of aortic valve procedures in Korea and to evaluate the early and mid-term outcomes of isolated surgical aortic valve replacement (SAVR) using bioprosthetic valves in contemporary Korea. Methods: Contemporary outcomes of isolated bioprosthetic SAVR in Korea were analyzed using the datasets on a multicenter basis. Patients who underwent isolated SAVR using bioprostheses from June 2015 to May 2019 were included, and those with concomitant cardiac procedures, SAVR with mechanical valve, or SAVR for infective endocarditis were excluded. A total of 456 patients from 4 large-volume centers were enrolled in this study. Median follow-up duration was 43.4 months. Early postoperative outcomes, mid-term clinical outcomes, and echocardiographic outcomes were evaluated. Results: Mean age of the patients was 73.1 ± 7.3 years, and EuroSCORE II was 2.23 ± 2.09. The cardiopulmonary bypass time and aortic cross-clamp times were median 106 and 76 minutes, respectively. SAVR was performed with full median sternotomy (81.8%), right thoracotomy (14.7%), or partial sternotomy (3.5%). Operative mortality was 1.8%. The incidences of stroke and permanent pacemaker implantation were 1.1% and 1.1%, respectively. Paravalvular regurgitation ≥ mild was detected in 2.6% of the patients. Cumulative incidence of all-cause mortality at 5 years was 13.0%. Cumulative incidences of cardiovascular mortality and bioprosthetic valve dysfunction at 5 years were 7.6% and 6.8%, respectively. Conclusion: The most recent data for isolated SAVR using bioprostheses in Korea resulted in excellent early and mid-term outcomes in a multicenter study.
