        Decreased Pattern-Recognition Receptor-Mediated Cytokine mRNA Expression in Obese Children With Otitis Media With Effusion

        <P><B>Objectives</B></P><P>To assess innate and humoral immune responses in middle ear effusion of obese pediatric patients with otitis media with effusion (OME).</P><P><B>Methods</B></P><P>We evaluated 219 children with OME, of whom 21 were obese and 198 were non-obese. We compared the expression in middle ear effusion of mRNAs encoding toll-like receptors (TLR) 2, 4, 5, and 9; nucleotide-binding oligomerization domains (NOD) 1 and 2; retinoic acid-inducible gene (RIG)-I; interleukins (IL)-6, -10, and -12; interferon (IFN)-γ; and tumor necrosis factor (TNF)-α mRNAs. We also compared the expression of immunoglobulins IgG, IgA, and IgM and the bacterial detection rate in the two groups.</P><P><B>Results</B></P><P>TLR2-mediated expression of IL-6 mRNA, TLR4-mediated expression of IL-6 and IL-10 mRNA, TLR5-mediated expression of IL-6, IL-10, and TNF-α mRNA, TLR9-mediated expression of IL-6 mRNA, and NOD2-mediated expression of IL-6, IL-12, and TNF-α mRNA were significantly lower in obese than in non-obese children (<I>P</I><0.05). However, concentrations of IgG, IgA, and IgM in middle ear effusion were lower in obese than in non-obese children, but none of these differences was significant (<I>P</I>>0.05).</P><P><B>Conclusion</B></P><P>Mean body mass index was higher and pattern-recognition receptor-mediated cytokine mRNA expression was lower in obese than in non-obese children with OME.</P>

        방풍의 항염 효과 기전

        목적 : 방풍은 임상적으로 관절염을 포함한 다양한 염증성 질환 치료제로 사용되어 왔다. 본 연구에서는 인간 비만세포를 이용하여 세포 독성에 영향을 주지 않는 농도에서 방풍의 항염 효과 및 그 기전을 검토했다. 방법 : 인간의 HMC-1세포를 IMDM에서 페니실린, 스트렙토마이신, 모노티오글리세린를 첨가하여 배양하고 방풍추출액을 투여하였다. 그 다음 MTT, ELISA, RT-PCR, 세포내 칼슘측정, 핵단백분석을 이용하여 TNF-α, IL-6, IL-8 각각의 형성과 mRNA발현, 세포내 칼슘 수준, NF-κB 발현에 대한 방풍추출액의 반응을 측정하고 통계처리 하였다. 결과 : 방풍은 PMA와 calcium ionophore A23187로 활성화된 비만세포에서 세포내 칼슘 수준과 NF-κB, TNF-α와 IL-6의 발현을 억제 시켰고 RT-PCR을 이용한 mRNA 발현에서 TNF-α와 IL-6의 발현을 억제하였다. 결론 : 방풍은 비만세포내 칼슘 수준 및 NF-κB의 활성을 억제하고 염증성 세포 활성 물질인 TNF-α와 IL-6의 분비도 억제하여 항염 효과를 나타냄을 암시하고 있다.

        Selective addition of CXCR3+CCR4-CD4+ Th1 cells enhances generation of cytotoxic T cells by dendritic cells in vitro

        Increasing importance is being given to the stimulation of Th1 response in cancer immunotherapy because its presence can shift the direction of adaptive immune responses toward protective immunity. Based on chemokine receptor expression, CXCR3+CCR4-CD4+ T cells as Th1-type cells were investigated its capacity in monocyte-derived dendritic cell (DC) maturation and polarization, and induction of antigen specific cytotoxic T lymphocytes (CTL) in vitro. The levels of IL-4, IL-5 and IL-10 were decreased to the basal level compared with high production of IFN-γ, TNF-α, and IL-2 in CXCR3+CCR4-CD4+ T cells stimulated with anti-CD3 and anti-CD28 antibodies. Co-incubation of activated CD4+ or CXCR3+CCR4-CD4+ T cells with DC (CD4+/DC or CXCR3+CD4+/DC, respectively) particularly up-regulated IL-12 and CD80 expression compared with DC matured with TNF-α and LPS (mDC). Although there was no significant difference between the effects of the CXCR3+CCR4-CD4+ and CD4+ T cells on DC phenotype expression, CXCR3+CD4+/DC in CTL culture were able to expand number of CD8+ T cells and increased frequencies of IFN-γ secreting cells and overall cytolytic activity against tumor antigen WT-1. These results demonstrated that the selective addition of CXCR3+ CCR4- CD4+ T cells to CTL cultures could enhance the induction of CTLs by DC in vitro, and implicated on a novel strategy for adoptive T cell therapy.         Interferon-γ Inhibits in vitro Mobilization of Eosinophils by Interleukin-5

        <P><I>Background:</I> Th2 cytokines play pivotal roles in allergic inflammation, including eosinophilia, and their actions are antagonized by Th1 cytokines, conferring them therapeutic potential. <I>Methods:</I> In this study, we examined the ability of a number of cytokines to suppress the activation of eosinophils that function as effector cells for allergic airway diseases. <I>Results:</I> Interleukin (IL)-5, IL-6, and tumor necrosis factor (TNF) induced an eosinophil shape change, whereas interferon (IFN)-γ significantly inhibited the shape change. Other cytokines, including IL-1β, IL-4, IL-10 and IL-13, had little or only slightly enhancing or reducing effects on the shape change. We further analyzed the IFN-γ effect, showing that pretreatment with IFN-γ strongly suppressed IL-5-induced eosinophil shape change, and cycloheximide (CHX) abrogated the suppression by IFN-γ, suggesting that new protein synthesis is required for the inhibitory effect by this cytokine. In agreement with these results, IFN-γ blocked the eosinophil migration and ERK phophorylation induced by IL-5, and the addition of CHX restored eosinophil chemotaxis. <I>Conclusions:</I> Collectively, IFN-γ may attenuate eosinophilic inflammation by directly negating eosinophil mobilization.</P><P>Copyright © 2005 S.       • 제2형 당뇨병 환자에서 체내 총항산화능 측정의 의의

        Background: The diabetic patients are at significantly increased risk of developing vascular disease. It's etiology may involve oxidative damage by free radiacals and protection againse such damage can be offered by antioxidants. We investigated that oxidative stress as assessed by measurement of total antioxidant status may play a role in development of diabetes mellitus. Method: We measured total antioxiant status using merchandised kit, glycated hemeglobin(HbA1c) in 46 type 2 diabetes mellitus patients and 50 healthy matched control subjects. Result: The total antioxidant status(TAS) was 2.10(±0.04) mmol/L in uncontrolled type 2 DM patients, 2.60(±0.03) in controlled type 2 DM patients and 2.70(±0.16) in healthy control subjects. TAS was significantly lower(P<0.05) in uncontrolled type 2 DM patients, but no significant association between in controlled type 2 DM patients and healthy control subjects. The TAS was 2.00(±0.17) mmol/L in complicated type 2 DM patients and 2.10(±0.29) uncomplicated type 2 DM patients. There was no significant associations between complicated type 2 DM patients and uncomplicated type 2 DM patients. Conclusion: Poor glycemic control is associated with reduced TAS in type 2 DM patients. TAS was thought indirect index that predict glycemic control of type 2 DM patients.

        IRAK4 as a Molecular Target in the Amelioration of Innate Immunity–Related Endotoxic Shock and Acute Liver Injury by Chlorogenic Acid

        <P>Mice lacking the IL-1R-associated kinase 4 (IRAK4) are completely resistant to LPS-induced endotoxic disorder or the TLR9 agonist CpG DNA plus D-galactosamine-induced acute liver injury (ALI), whereas wild-type strains succumb. However, translational drugs against sepsis or ALI remain elusive. Lonicerae flos extract is undergoing the clinical trial phase I in LPS-injected healthy human volunteers for sepsis treatment. In the current study, chlorogenic acid (CGA), a major anti-inflammatory constituent of lonicerae flos extract, rescued endotoxic mortality of LPS-intoxicated C57BL/6 mice, as well as ameliorated ALI of LPS/D-galactosamine-challenged C57BL/6 mice. As a mechanism, CGA inhibited various TLR agonist-, IL-1 alpha-, or high-mobility group box-1-stimulated autophosphorylation (activation) of IRAK4 in peritoneal macrophages from C57BL/6 or C3H/HeJ mice via directly affecting the kinase activity of IRAK4, a proximal signal transducer in the MyD88-mediated innate immunity that enhances transcriptional activity of NF-kappa B or AP-1. CGA consequently attenuated protein or mRNA levels of NF-kappa B/AP-1 target genes encoding TNF-alpha, IL-1 alpha, IL-6, and high-mobility group box-1 in vivo under endotoxemia or ALI. Finally, this study suggests IRAK4 as a molecular target of CGA in the treatment of innate immunity-related shock and organ dysfunction following insult of various TLR pathogens from bacteria and viruses.</P>

      • The association of disease activity, pro-inflammatory cytokines, and neurotrophic factors with depression in patients with rheumatoid arthritis

        <P><B>Abstract</B></P> <P>Inflammation and trophic factors (brain-derived neurotrophic factor [BDNF], vascular endothelial growth factor, glial cell line-derived neurotrophic factor, and insulin-like growth factor-1) are associated with depression in the general population. Rheumatoid arthritis (RA) is a chronic representative inflammatory autoimmune disease; however, the association of disease activity, pro-inflammatory cytokines, and neurotrophic factors with depression has not been sufficiently investigated. Therefore, we determined the prevalence of depression and risk factors for depression and deterioration of depressive symptoms in RA patients. In addition, we analyzed the association between disease activity, pro-inflammatory cytokines, trophic factors, and depression in RA (N = 474). Demographic and laboratory data were examined, and routine assessment of patient index data 3 (RAPID 3) and disease activity score 28-joint count C-reactive protein (DAS 28-CRP) was performed to assess disease activity of RA. Depression was measured using the Korean version of the Beck Depression Inventory-second edition (K-BDI II). A K-BDI score ≥18 was considered the cut-off for depression in accordance with a previous validation study. The serum level of pro-inflammatory cytokines and neurotrophic factors was assessed by enzyme-linked immune sorbent assay. The prevalence of depression was 32.4% in patients with RA. The severity of disease activity of RA (RAPID 3 score [OR 2.34; 95% confidence interval, CI 1.22–4.51], DAS 28-CRP [≥3.2] [OR 1.60, 95% CI 1.01–2.53]) and severity of fatigue (OR 1.26 95% CI 1.15–1.38) were associated with depression and deterioration of depressive symptoms in the multivariate analysis. Among the components of RAPID 3 and DAS 28-CRP, patient assessment for global health and abilities for daily performance were more related to depression. The level of pro-inflammatory cytokines (IL-1β, IL-6, TNF-alpha) was not related to depression. The level of BDNF was significantly lower in RA patients with depression and was negatively correlated with K-BDI II score. Depression was related with the level of fatigue, low expression of BDNF, and high RA disease activity, which was associated with impaired ability to perform activities of daily life. Strict control of fatigue and disease activity to improve one’s capacity to perform daily life activities would be important to regulate depression.         동진강 유역내 하천의 특성별 영향평가

        본 연구는 동진강 유역의 수질 보전을 위한 기초자료를 얻기 위하여 동진강 본류와 이에 유입되는 정읍천을 대상으로 2001년 5월부터 11월까지 수질 모니터링을 실시하였다. 동진강 본류의 수질은 BOD기준으로 상수원 Ⅱ~Ⅳ등급(2.84~6.45 ㎎/L)으로 지점별로는 DJ6(정읍천 합류후)이 4.07㎎/L로 높게 나타나 Ⅲ급수의 수질을 유지하였다. COD는 지점별로 11.20~32.96 ㎎/L의 범위로 정읍천 합류후의 농도가 32.96 ㎎/L로 가장 높게 나타나 정읍천을 통한 본류의 오염물질량의 증가를 알 수 있었다. T-N의 경우 4.16~5.84 ㎎/L의 범위로 유역 전반에 걸쳐 오염이 이루어지고 있는 것으로 나타났고, T-P의 경우 BOD, COD와 마찬가지로 정읍천 합류후가 0.19 ㎎/L로 타 지점에 비하여 놀게 조사되었다. 본류의 COD의 경우 장마후 갈수기에 높은 농도를 나타냈고, T-P의 경우는 하천의 유량이 증가하는 홍수기보다 장마전 갈수기의 오염정도가 높고 이후 장마후 갈수기로 갈수록 수질이 악화되는 것으로 조사되었다. 동진강 수질의 오염은 주로 하류로 갈수록 그 정도가 심하게 나타나는데, 이것은 정읍천을 통해 유입되는 도시 생활하수와 인근 공장단지의 산업폐수가 그 주 요인으로 오염물질(BOD, COD T-N 및 T-P)의 농도가 상승하는 주원인이 되고 있다. 따라서, 동진강 수계의 수질관리를 위해 주 오염원이 되고 있는 정읍천의 도시생활하수와 공장단지에서 방류되는 산업폐수의 제어에 대한 대책이 먼저 이루어져야 할 것으로 판단된다. Irrigation water quality along Donjin river watershed was monitored to find a possible pollutants, for maintaining water quality to achieve food safety through water quality preservation of river. As a pollution indicators, such as Biological Oxygen Demand(BOD), Chemical Oxygen Demand(COD), Total Nitrogen(T-N), and Total Phosphate(T-P) in Dongjin river were examined from May to November in 2001. The results were as follows : The BOD level of Dongji n river ranged from 2.84 to 6.45 mg/L, which would be in a Ⅱ~Ⅳ grade of the potable water criteria by Ministry of Environment. Averaged BOD level of downstream DJ6(After Jeongupcheon confluence) was 4.07 mg/L. The average COD level of Dongjin river ranged from 11.20 to 32.96 mg/L. COD level of DJ6 rapidly increased rapidly after the junction of Dongjin river and Jungupcheon because it showed the latter had relatively high pollution level. T-N content were significantly high in all sites of Dongjin river ranged through 4.16 to 5.84 mg/L. T-P examined high concentration than another thing point by 0.19 mg/L after Jeongupcheon confluence as BOD and COD. COD of main stream was expressed high concentration to dry season after rainy season. In case of T-P, pollution degree of dry season before rainy season appeared and examined that quality of water was worsened go by dry season after rainy season. The water quality of Dongjin river was deteriorated with inflow of Jungupcheon polluted by municipal and industrial sites near Jungup city.
