        Comparison of the Effects of Barefoot, Kinesio Tape, and Dynamic Tape on Static and Dynamic Balance in Subjects With Asymptomatic Flexible

        Background: Flat-footed persons with collapsed medial longitudinal arch lose flexibility after skeletal maturity, resulting in several deformities and soft tissue injuries. Although arch support taping is usually applied in the clinic to support the collapsed arch, research on the use of different types of tape for more efficient arch support in flat-footed persons is lacking. Objects: The purpose of this study was to examine three conditions (barefoot, kinesio tape, and dynamic tape) and compare their effects on static and dynamic balance in persons with asymptomatic flexible flatfoot. Methods: Twenty-two subjects (9 females and 13 males) with asymptomatic flexible flatfoot participated in this study. The subjects performed the Y-balance test to measure the composite reach score. The subjects also performed a 30-second standing test to measure the center of pressure (COP) path length and a walking test to measure anteroposterior and lateral variability using the Zebris FDM system. One-way repeated-measures analysis of variance compared the three conditions applied to the subjects’ feet for each balance variable. Results: The composite reach score significantly increased following the application of dynamic tape compared with barefoot and that of kinesio tape compared with barefoot. There was no significant difference in the COP path length during standing among the three conditions. Anteroposterior and lateral variability during walking significantly with dynamic tape application compared with barefoot. Conclusion: The results of this study suggest that, in persons with asymptomatic flexible flatfoot, application of kinesio tape and dynamic tape may be effective in increasing the composite reach score in Y-balance test, whereas application of dynamic tape may be effective in reducing anteroposterior and lateral variability during walking.

        Effects of Different Knee Flexion Angles According to Three Positions on Abdominal and Pelvic Muscle Activity During Supine Bridging

        This study analyzes how different knee flexion angles affect the abdominal and pelvic muscle activity during supine bridging. Twenty healthy subjects participated in the study. We used surface electromyography (EMG) to measure how three different knee flexion angles (100°, 70°, and 40°) affected the activity of the transverse abdominis/internal oblique (TrA/IO), external oblique (EO), biceps femoris (BF), rectus femoris (RF), and gluteus maximus (GM) muscles on the dominant side during supine bridging. The one-way repeated analysis of variance (ANOVA) was used to determine the statistical significance of TrA/IO, EO, BF, RF and GM muscle activity and the GM/BF activity ratio. For the TrA/IO, EO, BF, and GM muscles, supine bridging with different knee flexion angles resulted in significant differences in abdominal and pelvic muscle activity. For the TrA/IO muscles, the post-hoc test demonstrated that muscle activity significantly increased at 40° compared to 70°; however, there were no significant differences between 100° and 70° or 100° and 40°. For the EO muscle, the post-hoc test demonstrated that muscle activity significantly increased at 40° compared to 100° and 70°; no significant difference was observed between angles 100° and 70°. For the BF muscle, the post-hoc test demonstrated that muscle activity significantly increased according to the knee flexion angle (40°>70°>100°). For the GM muscle, the post-hoc test demonstrated that muscle activity significantly increased according to the knee flexion angle (100°>70°>40°). However, for the RF muscle, there was no significant difference. Additionally, the GM/BF activity ratio significantly increased according to the knee flexion angle (100°>70°>40°). From these results, we can conclude that bridging with a knee flexion of 100° can strengthen the GM muscle, whereas bridging with a knee flexion of 40° is recommended to strengthen the IO, EO, and BF muscles. We can also conclude that knee flexion angles should be modified during supine bridging to increase the muscle activity of different target muscles.

        Biomechanical Effectiveness of the Low-Dye Taping on Peak Plantar Pressure During Treadmill Walking Exercise in Subjects With Flexible Flatfoot

        The purposes of this study were 1) to determine the effects of low-dye taping on peak plantar pressure following treadmill walking exercise, 2) to determine whether the biomechanical effectiveness of low-dye taping in peak plantar pressure was still maintained following removal of the tape during treadmill walking, and 3) to determine the trend towards a medial-to-lateral shift in peak plantar pressure in the midfoot region before and after application of low-dye taping. Twenty subjects with flexible flatfoot were recruited using a navicular drop test. The peak plantar pressure data were recorded during five treadmill walking sessions: (1) un-taped, (2) baseline-taped, (3) after a 10-minute treadmill walking exercise, (4) after a 20-minute treadmill walking exercise, and (5) after removal of the taping. The foot was divided into six parts during the data analysis. One-way repeated measures analysis of variance was performed to investigate peak plantar pressure variations in the six foot parts in the five sessions. This study resulted in significantly increased medial forefoot peak plantar pressure compared to the un-taped condition (p=.017, post 10-minute treadmill walking exercise) and (p=.021, post 20-minute treadmill walking exercise). The peak plantar pressure in the lateral forefoot showed that there was a significant decrease after sessions of baseline-taped (p=.006) and 10-minute of treadmill walking exercise (p=.46) compared to the un-taped condition. The tape removal values were similar to the un-taped values in the five sessions. Thus, the findings of the current study may be helpful when researchers and clinicians estimate single taping effects or consider how frequently taping should be replaced for therapeutic purposes. Further studies are required to investigate the evidence in support of biomechanical effectiveness of low-dye taping in the midfoot region.

        Comparison of Supraspinatus Muscle Architecture During Three Different Shoulder Strengthening Exercises Using Ultrasonography

        Background: Strengthening the supraspinatus is an important aspect of a rehabilitation program for subacromial impingement and tendinopathy. Many authors recommended empty-can (EC), full-can (FC), and prone full-can (PFC) exercises to strengthen the supraspinatus. However, no ultrasonography study has yet investigated supraspinatus muscle architecture (muscle thickness; MT, pennation angle; PA, fiber bundle length; FBL) in relation to supraspinatus strengthening exercises. Objects: The purpose of this study was to compare the architecture (MT, PA, and FBL) of the supraspinatus muscle during three different types of exercises (EC, FC, and PFC) using diagnostic ultrasound. Methods: Participants performed three different exercises: (A) EC; the arm was maintained at 60° abduction with full internal rotation in the sitting position, (B) FC; the arm was maintained at 60° abduction with full external rotation in the sitting position, and (C) PFC; the arm was maintained at 60° abduction with full external rotation in the prone position. Ultrasonography was used to measure the MT, PA and FBL of the supraspinatus. One-way repeated analysis of variance with Bonferroni’s post-hoc test was used to compare between the three exercises and the initial position of each exercise. Results: Compared with each initial position, the FC exercise showed the greatest mean difference in muscle architecture properties and the PFC exercise showed the least mean difference. Conclusion: The findings suggest that the FC exercise position may have an advantage in increasing the amount of contractile tissue or producing muscle power and the PFC exercise position may be useful in a rehabilitation program because it offers the advantage of maintaining the muscle architecture properties

        Selective Muscle Activation With Visual Electromyographic Biofeedback During Scapular Posterior Tilt Exercise in Subjects With Round-Shoulder Posture

        The purpose of this study was to investigate the effects of visual electromyography (EMG) biofeedback on the EMG activity of the lower trapezius (LT), serratus anterior (SA), and upper trapezius (UT) muscles, the LT/UT and SA/UT EMG activity ratios, and the scapular upward rotation angle during scapular posterior tilting exercise (SPTE). Twenty-four subjects with round-shoulder posture participated in this study. The EMG activities of the LT, SA, and UT were collected during SPTE both without and with visual EMG biofeedback. The scapular upward rotation angle was measured at the baseline, after SPTE without visual EMG biofeedback, and after SPTE with visual EMG biofeedback. The LT, SA, and UT EMG activities, and the LT/UT and SA/UT EMG activity ratios were analyzed by paired t-test. The scapular upward rotation angle was statistically analyzed using one-way repeated analysis of variance. If a significant difference was found, a Bonferroni correction was performed (p=.05/3=.017). The EMG activities of LT and SA significantly increased, and the EMG activity of UT significantly decreased during SPTE with visual EMG biofeedback compared to SPTE without visual EMG biofeedback (p<.05). In addition, the LT/UT and SA/UT EMG activity ratios significantly increased during SPTE with visual EMG biofeedback compared to SPTE without visual EMG biofeedback (p<.05). Significant increases were found in the scapular upward rotation angle after SPTE without and with visual EMG biofeedback compared to baseline (p<.017), and no significant differences were observed in the scapular upward rotation angle between SPTE without and with visual EMG biofeedback. In conclusion, SPTE using visual EMG biofeedback may be an effective method for increasing LT and SA activities while reducing UT activity.

        Effects of Different Types of Isometric Hip Contraction on Gluteus Medius and Tensor Fasciae Latae Activity During Squat Exercises

        Hip muscle activation and strengthening exercise programs are often used to prevent and treat various lower extremity injuries. Common exercise programs include squat exercises. The purposes of this study were to investigate gluteus medius (GMED) and tensor fasciae latae (TFL) muscle activity, and to assess the GMED/TFL ratio during squat exercises involving different isometric hip contraction conditions. Different types of isometric hip contraction were standard squat without hip contraction, squats with isometric hip adduction, and squats with isometric hip abduction. Twenty (10 males and 10 females) healthy subjects (23.7±2.8 years old) were recruited. Subjects performed the squat exercises with the back supported by a wall and knees flexed to 60°. Surface electromyography (EMG) was used to measure GMED and TFL activity. One-way repeated analysis of variance was used to compare GMED and TFL muscle activity and the GMED/TFL ratio. GMED and TFL EMG activity was significantly higher during squats with isometric hip adduction and abduction compared with the standard squat without hip contraction (p<.05). Between the isometric hip adduction and abduction contraction conditions, only the TFL EMG activity was significantly higher during squats with isometric hip adduction than isometric hip abduction (p<.05). The GMED/TFL ratio was significantly higher during squats with isometric hip adduction than isometric hip abduction (p<.05). Squats with isometric hip adduction and abduction improved GMED and TFL muscle activity. Furthermore, the GMED/TFL ratio was higher during isometric hip adduction than isometric hip abduction. Our data indicate that squat exercises involving isometric hip adduction enhance GMED muscle activity.

        Foot posture is important in the development of the musculoskeletal structure in the lower limbs because it can change the mechanical alignment. Although foot orthotics are widely used for the correction of malalignments in the lower extremities, the biomechanical effects of wedges have not yet been cleared. The aim of this study was to investigate whether medial wedges affect the electromyographic (EMG) activity of the knee and hip joints in healthy adults that are performing one leg standing. Seventeen healthy volunteers performed the one leg standing under two foot conditions: A level surface, and a 15° medial wedge. The subjects’ EMG data for the gluteus maximus (Gmax), gluteus medius (Gmed) , tensor fasciae latae (TFL), biceps femoris (BF), vastus lateralis (VL), and vastus medialis oblique (VMO) were recorded, along with the surface EMG, and all were analyzed. The EMG activity of the Gmed and TFL had significantly decreased under the medial wedge condition during one leg standing. Further study is needed in order to investigate whether medial wedges influence the EMG activity and kinematic data of the knee and hip joints as well as the ankle joints in adults with flexible flatfoot, while they are performing one leg standing.

        Inter-Rater Reliability of Abdominal Muscles Thickness Using Ultrasonography for Different Probe Locations and Thickness Measurement Techniques

        Ultrasonography (US) is a recent technique that has proven to be useful for assessing muscle thickness and guiding the rehabilitation decision-making of clinicians and researchers. The purpose of this study was to determine the inter-rater reliability of the US measurement of transversus abdominis (TrA), internal oblique (IO), and external oblique (EO) thicknesses for different probe locations and measurement techniques. Twenty healthy volunteers were recruited in this study. Muscle thicknesses of the transversus TrA, IO, and EO were measured three times in the hook-lying position. The three different probe locations were as follows: 1) Probe location 1 (PL1) was below the rib cage in direct vertical alignment with the anterior superior iliac spine (ASIS). 2) Probe location 2 (PL2) was halfway between the ASIS and the ribcage along the mid-axillary line. 3) Probe location 3 (PL3) was halfway between the iliac crest and the inferior angle of the rib cage, with adjustment to ensure the medial edge of the TrA. The two different techniques of thickness measurement from the captured images were as follows: 1) Muscle thickness was measured in the middle of the muscle belly, which was centered within the captured image (technique A; TA). 2) Muscle thickness was measured along a horizontal reference line located 2 ㎝ apart from the medial edge of the TrA in the captured image (technique B; TB). The intraclass correlation coefficient (ICC [3,k]) was used to calculate the inter-rater reliability of the thickness measurement of TrA, IO and EO using the values from beth the first and second examiner. In all three muscles, moderate to excellent reliability was found for all conditions (probe locations and measurement techniques) (ICC=.70~.97). In the PL1-TA condition, inter-rater reliability in the three muscle thicknesses was good to excellent (ICC=.85~.96). The reliability of all measurement conditions was excellent in IO (ICC=.95~.97). Therefore, the findings of this study suggest that TA can be applied to PL1 by clinicians and researchers in order to measure the thickness of abdominal muscles.

        Effects of Tensor Fasciae Latae-Iliotibial Band Self-Stretching on Lumbopelvic Movement Patterns During Active Prone Hip Lateral Rotation in Subjects With Lumbar Extension Rotation Syndrome

        The purpose of this study was to identify the effects of tensor fasciae latae-iliotibial band (TFL-ITB) self-stretching exercise on the lumbopelvic movement patterns during active prone hip lateral rotation (HLR) in subjects with lumbar extension rotation syndrome accompanying TFL-ITB shortness. Eleven subjects (9 male and 2 female) were recruited for the two-week study. A three dimensional ultrasonic motion analysis system was used to measure the lumbopelvic movement patterns. The TFL-ITB length was measured using the modified Ober`s test and was expressed as the hip horizontal adduction angle. The subjects were instructed how to perform TFL-ITB self-stretching exercise program at home. A paired t-test was performed to determine the significant difference in the angle of lumbopelvic rotation, movement onset time of lumbopelvic rotation, TFL-ITB length, and LBP intensity before and after the two-week period of performing the TFL-ITB self-stretching exercise. The results showed that after the intervention, the lumbopelvic rotation angle decreased significantly (p<.05), the movement onset time reduced significantly (p<.05), and LBP intensity decreased slightly but not significantly (p=.07). The hip horizontal adduction angle increased significantly (p<.05) after the intervention. These findings indicate that TFL-ITB stretching exercise increased TFL-ITB length, decreased lumbopelvic rotation angle, and delayed the movement onset time of lumbopelvic rotation after two-weeks. In conclusion, the TFL-ITB self-stretching exercise performed over a period of two weeks may be an effective approach for patients with lumbar extension rotation syndrome accompanying TFL-ITB shortness.

        The Impact of Abdominal Drawing-in Maneuver and Tensor Fasciae Latae-iliotibial Band Self-stretching on Lumbopelvic Kinematics in Individuals With Lumbar Extension Rotation Syndrome

        Background: The abdominal drawing-in maneuver (ADIM), a method of lumbar stabiliza-tion training, is an effective neuromuscular intervention for lumbar instability associated with low back pain (LBP).Objects: The purpose of this study was to compare the effect of a 2-week period of the ADIM and tensor fasciae latae-iliotibial band (TFL-ITB) self-stretching on lumbopelvic rotation angle, lumbopelvic rotation movement onset, TFL-ITB length, and pain intensity during active prone hip lateral rotation.Methods: Twenty-two subjects with lumbar extension rotation syndrome accompanying shortened TFL-ITB (16 males and 6 females) were recruited for this study. The subjects were instructed how to perform ADIM training or ADIM training plus TFL-ITB self-stretching pro-gram at home for a 2-week period. A 3-dimensional ultrasonic motion analysis system was used to measure the lumbopelvic rotation angle and lumbopelvic rotation movement onset. An independent t-test was used to determine between-group differences for each outcome measure (lumbopelvic rotation angle, lumbopelvic rotation movement onset, TFL-ITB length, and pain intensity).Results: The results showed that ADIM training plus TFL-ITB self-stretching decreased the lumbopelvic rotation angle, delayed the lumbopelvic rotation movement onset, and elongated the TFL-ITB significantly more than did ADIM training alone. Pain intensity was lower in the ADIM training plus TFL-ITB self-stretching group than the ADIM training alone group; how-ever, the difference was not significant.Conclusion: ADIM training plus TFL-ITB self-stretching performed for a 2-week period at home may be an effective treatment for modifying lumbopelvic motion and reducing LBP.
