        An Investigation of fan type anchorages applied to end of CFRP strips

        CFRP strips are widely used nowadays for repair/strengthening or capacity increase purposes. Sharp bending at the ends of the CFRP strips is frequently encountered at these applications. In this study, Reinforced Concrete (RC) beam specimens that were produced with 10 MPa compression strength concrete were strengthened by using bonded CFRP strips with end anchorages to tension region. The parameters that were investigated in this study are the width of the strip, the number of applied fan anchorages and whether additional layer of CFRP patch is used or not at the strip ends. Specimens were strengthened with 100 mm wide CFRP strips with one or two anchorages at the ends. In addition CFRP patch with two and three anchorages at the ends were tested for investigating the effect of the patches. Specimens that were strengthened with three anchorages at the ends with patches were repeated with 60 and 80 mm wide CFRP strips. The most successful result was obtained from the specimen that was strengthened with 80 mm wide CFRP strips with 3 end anchorages and patches among the others at the experimental program. The numbers of anchorages that were applied to ends of CFRP strips were more effective than the width of the CFRP strips onto strength and stiffness of the specimens. Due to limited space at the ends of the strips at most three anchorages could be applied.

      • Early Detection and Gemcitabine/Cisplatin Combination Positively Effect Survival in Sarcomatoid Carcinoma of the Urinary Bladder

        Background and Objectives: This study aimed to present the clinicopathological characteristics and treatment of patients with bladder carcinoma with sarcomatoid differentiation at our institution. Methods: Between 1995-2009, 950 patients were followed-up for bladder carcinoma. Among them, 14 patients with sarcomatoid carcinoma were retrospectively reviewed, and their clinical, pathological features and treatment were recorded. Results: Median age of the patients was 65 years (range: 41-86 years), 12 (86%) being male and 2 (14%) female. All the patients presented with hematuria and 11 (88%) had a history of smoking. The tumor growth pattern was solid in 10 patients, papillary in 2, and mixed in 2. In all, 5 of the patients had urothelial carcinoma with sarcomatoid differentiation and 9 were diagnosed with sarcomatoid carcinoma. Five patients underwent radical cystectomy with ileal conduit surgery, 2 patients refused cystectomy, and 8 patients underwent re-TUR. Following diagnosis, 12 of the patients died in mean 10.7 months (range: 1-48 months). Conclusion: Urothelial carcinomas with sarcomatoid features are aggressive and are usually at advanced stage at the time of diagnosis. The outcomes of multimodal treatment are not satisfactory. Significant findings of the present study are that early diagnosis positively affect survival and that gemcitabine and cisplatin in combination can positively affect survival.

        The effect of glazing and aging on the surface properties of CAD/CAM resin blocks

        PURPOSE. To investigate the effect of accelerated aging on surface properties of glazed CAD/CAM resin blocks using a 2D surface profilometer and a 3D non-contact optical profilometer. MATERIALS AND METHODS. Three types of CAD/CAM resin restorative materials, LAVA Ultimate (3M ESPE, St Paul, MN, USA), VITA Enamic (Vita Zahnfabrik H. Rauter, Bad $S\ddot{a}ckingen$, Germany), and Cerasmart (GC Corparation, Tokyo, Japan) were used for this study. CAD/CAM blocks were cut in 3-mm thickness slabs and divided into three groups; Group 1: control group (specimens polished with 600 grit SCI paper); Group 2: specimens sandblasted, silanized, and glazed with Optiglaze Color (GC); Group 3: glazed specimens subjected to 5000 thermocycles (n=15). The surface roughness ($R_a$ and $R_z$) was evaluated using a profilometer and a 3D scanning instrument. Data were analyzed using two-way ANOVA and Tukey's post- hoc test (P<.05). RESULTS. LAVA, VITA, and Cerasmart exhibited statistically similar $R_a$ and $R_z$ values for each group (P>.05). For VITA and Cerasmart, the specimens in Group 1 exhibited significantly higher $R_a$ values than Group 2 (P<.05). Group 1 ($0.502R_a$), Group 2 ($0.384R_a$), and Group 3 ($0.431R_a$) exhibited statistically similar $R_a$ values for LAVA (P=.062). After 5000 thermocycles, surface roughness values did not change significantly for glazed LAVA, VITA, and Cerasmart (P>.05). CONCLUSION. Glaze material Optiglaze Color makes CAD/CAM resin surfaces smooth and glazed CAD/CAM surfaces seem resistant to deterioration under 5000 thermocycles.

        Evaluation of Bone Density Measurement in Type 2 Diabetic Postmenopausal Women with Hypertension and Hyperlipidemia

        Objectives: The aim of the study was to compare bone mineral density (BMD) in healthy postmenopausal women to BMD in type 2 diabetic hypertensive postmenopausal women with hyperlipidemia. Methods: Fifty type 2 diabetic and hypertensive postmenopausal women with hyperlipidemia and 51 age and body mass index (BMI) matched healthy postmenopausal women were included. Lumbar spine and femoral neck BMD were noted in both groups. BMD was measured using dual energy X-ray absorptiometry (DXA). Serum alkaline phosphatase (ALP), calcium and phosphorous were also measured. Pearson correlation coefficients were used to establish the relationship between various clinical characteristics. Results: There were no significant differences between two groups in respect to lumbar and vertebral BMD values, age, BMI, gravidity, parity. Serum cholesterol and fasting glucose levels were significantly different between each groups (P = 0.0001, P = 0.002). Conclusion: We found that, accompanying chronic diseases such as diabetes, hypertension and hyperlipidemia don’t affect the BMD measurements at postmenopausal period. So these postmenopausal women don’t have excess risk regarding osteoporosis. (J Menopausal Med 2015;21:36-40)

        Introduction and Aim:While ischemia and nefrotoxins account for almost 45 % of all acute renal failure cases, prerenal (caused by congestive cardiac failure, liquid insuffi - ciency, sepsis and medication) azotemia accounts for around 20%. Among nefrotoxins, nonstereoid anti-infiammatory drugs (NSAII) may affect kidneys through multiple mechanisms (causing hemodynamic, acute allergic interstitial nephritis, papillary necrosis or glomerular nephritis). Herein, we present a patient who developed analgesic nephropathy associated with total anuria. Case: A patient with a medical history of benign prostatic hyperplasia, coronary artery disease, hypertension and tuberculosis was admitted to our department of emergency with complaints of dyspnea and headache. Laboratory examination revealed the following Results: urea: 138 mg/dL, kreatinin: 5,20 mg/dL, BK: 19300, ALT: 319 U/L AST: 324 U/ L LDH: 1947 U/L. The patient was diagnosed with total anuria. Because the patient had a history of analgesic usage, he was scheduled to undergo hemodialysis 3 times per week with a prediagnosis of analgesic nephropathy. Although initially renal function tests demonstrated a slight deterioration (urea: 177 mg/dL and creatinine: 8 mg/dL) they displayed a gradual decrease following ongoing hemodialysis sessions and consequently returned to normal (urea: 48 mg/dL and creatinine: 1, 25 mg/dL). The patient also received piperacillin/tazobactam 3x 2, 25 gr/day due to a concern for aspiration pneumonia. Diagnostic bronchoscopy results were in compliance with fi brosis at microscopic level. Conclusion/Recommendations: Analgesic nefropathy should be investigated when inquiring for the etiology of acute renal failure. Patients with total anuria should receive comprehensive treatment comprising renal replacement therapy such as hemodialysis. Additionally, interstitial lung disease and overlapping aspiration pneumonia may complicate the condition. Therefore, they also should be considered. Even patients with total anuria may return to normal life following a comprehensive and rigorous approach.
