        Malassezia sympodialis가 동정된 신생아 Malassezia 농포증 1예

        Neonatal Malassezia pustulosis can be defined as pustules on face and neck, age at onset younger than 1 month, isolation of Malassezia by direct microscopy in pustular material, elimination of other causes of neonatal pustuloses, and response to topical ketoconazole therapy. We report a case of neonatal Malassezia pustulosis in a 20-day-old male. Direct microscopic examination on smears for pustules showed forms of Malassezia yeasts and culture yielded Malassezia sympodialis. The lesions were remarkably improved by topical ketoconazole cream for 14 days. [Kor J Med Mycol 6(4): 229-231] Key Words: Neonatal Malassezia pustulosis

      • 감초추출액이 멜라닌세포의 증식과 멜라닌화에 미치는 영향

        Background: Licorice has been widely used for its anti-inflammatory action. Recently many studies have revealed that licorice has an inhibitory effect on melanin formation by inhibiting tyrosinase, TRP-1(Tyrosinase-related protein 1) and TRP-2(Dopachrome tautomerase) activity. Most studies have reported that licorice extract inhibits melanogenesis in malignant melanoma cell but there are few reports of the effect of licorice on proliferation and melanization in normal human melanocytes. Furthermore, anti-melanogenetic effect of licorice is different according to the cultivated places. Objective: This study was done to investigate the effects of crude extract(CLE) and oil-soluble extract(OLE) of licorice which is cultivated in China and circulates in Korea on the proliferation and melanization of normal cultured human melanocytes. Methods: CLE(100, 500, 1000㎍/mL) and OLE(1, 10, 100㎍/mL) were added into culture medium respectively. Three days after adding CLE, melanocytes were counted by using hemocytometer, and melanocyte proliferation was measured by MTT and SRB assay. And melanin contents were examined by using spectrophotometer. Melanocytes were counted and melanin contents were measured 2 days and 7 days after adding OLE. Results: In CLE-added groups, the number of melanocytes was significantly lower in experimental groups (500㎍/mL and 1000 ㎍/mL) than in control group(p<0.05). On MTT assay, the proliferation of melanocytes was inhibited significantly in experimental groups(500㎍/mL and 1000㎍/mL) (p<0.05). On SRB assay, the proliferation of melanocytes was inhibited significantly in all experimental groups(p<0.05). This anti-proliferative effect was supposed to be related with the cytotoxic effect of CLE. Melanin contents increased significantly in CLE-added experimental group(500㎍/mL and 1000㎍/mL) than control group(p<0.05). It is considered that the increased melanin contents were caused by strong anti-proliferative effect. Two days after adding OLE, the number of melanocytes had no change in 1㎍/mL-added group, which decreased significantly in 10㎍/mL-added group, All cells were detached from the bottom of culture plate in 100 ㎍/ml-added group. The number had no change in 1㎍/mL-added group, while all cells were detached in 10㎍/mL and 100㎍/mL-added groups 7 days after adding OLE. This means that the concentrations of 10㎍/mL and 100㎍/mL of OLE have cytotoxic effect on melanocyte. Melanin contents decreased significantly on the 2nd day(p<0.05), and decreased slightly on the 7th day in 1㎍/mL OLE-added group. Conclusion: Oil-soluble extract of licorice which is cultivated in China and circulates in Korea has anti-melanogenic effect. Therefore, this material can be used to treat hyperpigmented disorders including melasma.

        피부로 전이된 방광암 1예

        Metastasis to the skin from the urinary bladder carcinoma are very rare and their incidence is 1 % to 2% of the cutaneous metastasis. A 78-year-old man developed cutaneous metastasis 2 years later after the diagnosis of carcinoma of the bladder had been made. He visited our department with skin colored, soft, tender, subcutaneous nodule on the lower abdomen which had been detected 5 days ago. Histologic examination of the skin lesion showed atypical cells having hyperchromatic and pleomorphic nuclei. Herein we report a case of cutaneous metastasis from carcinoma of the urinary bladder in a 78-year-old man. (Korean J Dermatol 2003:41(1) : 133∼134)

        Clear cell syringoma has histologic characteristics of clear cells and is known to coexist with diabetes mellitus. Two patients visited our clinic due to asymptomatic skin colored papules around the eyelids. A 68-year-old woman had an increasing number of skin lesions which had initially developed 40 years previously. A 71-year-old woman had a skin lesion which had been found 1 year previously. On past medical history, they were diagnosed as having contracted diabetes mellitus 20 and 30 years previously, respectively.

      • Nicotine이 배양된 정상 인체 멜라닌세포의 멜라닌화에 미치는 영향

        Nicotine, the active ingredient of tobacco, causes increased platelet aggregation, endothelial cell damage and alterations in prostacyclin and thromboxane metabolism. Skin changes with nicotine include brown pigmentary staining of hands and nails, leg ulcers, cyanosis, decreased skin temperature and other signs of arterial insufficiency. However, the effect of nicotine on melanization is not well established. The purpose of this study is to evaluate the effect of nicotine on melanization in cultured normal human melanocytes. In order to evaluate non-cytotoxic dose of nicotine, we exposed various concentrations (0.01, 0.1, 1, 10, 100, and 1000 μg/ml) of nicotine to melanocytes and examined cell cytotoxicity using MTT assay. There was no cytotoxicity below 0.1 μg/ml of nicotine. After exposure to 0.1μg/ml nicotine on cultured normal human melanocytes, we investigated the effects of nicotine on the expression of tyrosinase (TYR), tyrosinase-related protein 1 (TYRP1), DOPAchrome tautomerase (DCT) at the protein levels in human skin melanocytes. There was no significant difference in the expression of TYR, TYRP1, and DCT protein after treatment with nicotine compared to the untreated control. These results suggest that short-term nicotine exposure had no direct melanogenic effect, in vitro.

        Effect of PUVA on Nerve Growth Factor Expression in Cultured Keratinocytes

        Nerve growth factor (NGF) is an important autocrine growth factor and also a survival factor for keratinocytes. NGF may act in the hyperproliferative condition, psoriasis. Clinically, the combination of psoralen and UVA (PUVA) has been used in the treatment of a wide variety of cutaneous disorders, such as psoriasis and vitiligo. However, the precise therapeutic mechanism of PUVA on the dermatologic diseases remains unclear. The purpose of this study was to examine whether the expression of NGF in cultured keratinocytes is influenced by PUVA. Thus, normal human keratinocytes were isolated from neonatal foreskin, and the third to fifth-passaged cells were used in this study. The cells were exposed to various doses of UVA (30, 60, 120 $mJ/cm^2)$ after adding 8-methoxypsoralen (8-MOP) to examine the expression of NGF mRNA. The RNA and protein of the cells were extracted at various time points (1, 8, 24 hours) after UVA irradiation to examine the expression of NGF mRNA and production of NGF protein. In keratinocytes, there were no differences in the expression of NGF mRNA between the different doses of UVA irradiation, however, the expression of NGF mRNA in UVA and PUVA groups tended to increase as the time increased. The expression of NGF mRNA was the highest in PUVA group, followed by UVA group and the lowest in 8-MOP group. The expressions of NGF protein at 1 and 8 hours after UVA irradiation were lower in the PUVA group than in the other groups. This study showed that the expression level of NGF protein in keratinocytes was relatively lower in the PUVA groups than in the other groups, suggesting that the therapeutic mechanism of PUVA in psoriasis is related to the decrease of NGF protein.

        Periumbilical Perforating Pseudoxanthoma Elasticum

        Periumbilical perforating pseudoxanthoma elasticum (PPPXE) is a localized acquired disorder found most frequently in obese, multiparous, middle-aged women. It is characterized clinically by yellowish, lax, well-circumscribed, reticulated or cobblestoned patches or plaques in the periumbilical region. Multiparity, obesity, massive ascites, and abdominal surgery are thought to the initiating factors. There is controversy about the etiology of PPPXE. Some authors have classified it as a separate entity from hereditary pseudoxanthoma elasticum (PXE), while others speculate that this condition merely represents a variable expression of PXE in which systemic associations are likely. We report a case of periumbilical perforating pseudoaranthoma elasticum associated with a clinical PXE lesion on the anterior neck.
