        농산물 소독과정에서 브롬화메틸에 노출된 근로자에게 발생한 신경병 2례

        목적 : 농산물 소독과정에서 브롬화메틸에 노출된 근로자에게 발생한 신경병 2례 보고. 방법 : 피로감, 전신무력감, 어지럼증, 보행장애 등의 증상을 주소로 내원한 브롬화메틸을 사용하여 방역작업에 종사하는 건강한 20세와 18세의 근로자를 대상으로 혈액검사, 신경정도 및 근전도검사, 뇌자기공명영상 등의 임상검사와 작업력조사를 시행하였다. 결과 : 1례에서 뇌자기공명영상에서 연수와 소뇌의 양측 측충부의 고신호강도가 관찰되었고 11일 후의 추적조사에서 동일 부위의 고신호강토가 감소된 소견이 관찰되었으며 뇌병증으로 최종 진단되었다. 다른 1례는 신경전도검사에서 다발성 신경병 소견과 뇌유발전위검사에서 우측 하부 뇌간부위 병소가 관찰되었고 뇌병증 및 말초신경 병으로 최종 진단되었다. 결론 : 근로자들의 증상은 소독과정에서 브롬화메틸 노출로 인한 신경 병으로 판단되며 훈증소독과방역작업에 종사하는 근로자들의 실태조사와 안전에 대한 대책이 시급하다고 하겠다. Objectives : To report two cases of neuropathy due to methyl bromide intoxication. Methods : Workers, engaged in the fumigating process, complained fatigue, general weakness, ataxia, and hypersomnia. We evaluated them with blood tests, neurophysiologic studies and MRI and investigated their occupational history. Results : Increased signal intensities were found in the medulla oblongata and paraver mian of cerebellum in MRI and after 11 days, high signal intensities were reduced in the following MRI. In the other case, polyneuropathy and rlght lower brainstem lesion were observed In the NCV and BAEP studies. Conclusions : We confirmed that worker's symptoms were related to methyl bromide exposure in the fumigation. It is necessary that we should evaluate present condition of fumigating process and prepare appropriate methods to protect workers engaged in the fumigation.

        The dynamic mechanical properties of thermotropic liquid crystalline polyester (LCP) blends with polystyrene (PS) and Li sulfonated polystyrene (PSSLi) were investigated. It was found that PS forms an immiscible blend with LCP. However, above the matrix T_(g) of the PS, the addition of the LCP increases the modulus of the blend significantly because the LCP acts as filler below its T_(g). In PSSLi/LCP blend system, it was also found that, in the temperature range between two T_(g)s of the ionomer, the LCP acts as filler and thus increases the modulus of the blend. However, the filler effect of the LCP on the modulus decreases rapidly with increasing temperature and disappears above the T_(g) of the LCP. Finally, it was suggested that the PSSLi may be partially miscible with the LCP via ion-dipole interactions between a Li^(+) cation of the PSSLi and a carbonyl group of the LCP.

        The 3-D shape use to effect of movie, animation, industrial design, medical treatment service, education, engineering etc... But it is not easy to make 3-D shape from the information of 2-D image. There are two methods in restoring 3-D video image through 2-D image; First the method of using a laser; Second, the method of acquiring 3-D image through stereo vision. Instead of doing two methods with many difficulties, I study the method of simple 3-D image in this research paper. We present here a simple and efficient method, called direct calibration, which does not require any equations at all. The direct calibration procedure builds a lookup table(LUT) image and 3-D coordinates by a real 3-D triangulation system. The LUT is built by measuring the image coordinates of a grid of known 3-D points, and recording both image and world coordinates for each point; the depth values of all other visible points are obtained by interpolation.

        Inheritance and concurrency are the primary feature of object oriented languages, and are especially important for code re-use. They provide maximum computational power and modeling power through concurrency of objects. But, concurrent objects and inheritance have conflicting characteristics, thereby simultaneously use of them causes the problem, so called inheritance anomaly, which requires code redefinition of inherited methods to maintain integrity of objects. In this paper, to solve the inheritance anomaly problems we introduce concept of state abstraction, in which internal states of encapsulated objects are made available from a part of object's external interface. And we design the abstract states based inheritance interface mechanisms which methods are inherited efficiently. In our scheme, we can solve the typical inheritance anomaly problems.

        Two-Year Clinical Outcome after Carvedilol-Loaded Stent Implantation in Patients with Coronary Artery Disease

        <P><B>Background/Aims</B></P><P>Carvedilol is an antioxidant that inhibits smooth muscle cell proliferation and migration. The aim of this study was to investigate the beneficial effects of carvedilol-loaded stents on 2-year clinical outcomes after stent implantation in patients with coronary artery disease.</P><P><B>Methods</B></P><P>We performed a prospective trial with male subjects to compare the safety and effects of carvedilol-loaded BiodivYsio® stents implanted into 20 patients with those of bare-metal BiodivYsio® stents implanted into 21 patients for de novo coronary lesions. The primary end point was the degree of neointimal hyperplasia, which was measured by intravascular ultrasound (IVUS) 6 months after the procedure; the secondary end point was major adverse cardiac events (MACE) at 2 years after implantation. All carvedilol and control stents were deployed successfully.</P><P><B>Results</B></P><P>A 2-year follow-up was completed for 19 patients (95%) in the carvedilol stent group and 20 patients (95%) in the control stent group. IVUS showed a trend toward a larger luminal area (6.86 ± 2.59 vs. 5.47 ± 1.52 mm<SUP>2</SUP>, <I>p</I> = 0.267), smaller neointimal area (1.34 ± 0.70 vs. 2.40 ± 1.73 mm<SUP>2</SUP>, <I>p</I> = 0.18), and reduced net decrease in luminal area (-0.78 ± 0.97 vs. -1.89 ± 1.78 mm<SUP>2</SUP>, <I>p</I> = 0.106) in the carvedilol stent group compared with the control stent group, respectively. There were no significant differences in the incidence of MACE (10.5 vs. 30.0%, respectively, <I>p</I> = 0.132) between the groups at 2 years after stent implantation. Stent thrombosis did not occur in either group after 2 years.</P><P><B>Conclusions</B></P><P>The carvedilol-loaded stents tended to inhibit neointimal hyperplasia without the occurrence of cardiac death, myocardial infarction, or stent thrombosis at 2-year follow-up.</P>

        A Multi-Center, Double-Blind Randomized Controlled Phase III Clinical Trial to Evaluate the Antiviral Activity and Safety of DA-2802 (Tenofovir Disoproxil Orotate) and Viread (Tenofovir Disoproxil Fumarate) in Chronic Hepatitis B Patients

        Background: Tenofovir disoproxil fumarate (TDF, Viread®) had been used as a standard treatment option of chronic hepatitis B (CHB). This clinical trial was conducted to evaluate the efficacy and safety of DA-2802 (tenofovir disoproxil orotate) compared to TDF. Methods: The present study was a double blind randomized controlled trial. Patients with CHB were recruited from 25 hospitals in Korea and given DA-2802 at a dose of 319 mg once daily or Viread® at a dose of 300 mg once daily for 48 weeks from March 2017 to January 2019. Change in hepatitis B virus (HBV) DNA level at week 48 after dosing compared to baseline was the primary efficacy endpoint. Secondary efficacy endpoints were proportions of subjects with undetectable HBV DNA, those with normal alanine aminotransferase (ALT) levels, and those with loss of hepatitis B envelop antigen (HBeAg), those with loss of hepatitis B surface antigen (HBsAg). Adverse events (AEs) were also investigated. Results: A total of 122 patients (DA-2802 group: n = 61, Viread® group: n = 61) were used as full analysis set for efficacy analysis. Mean age, proportion of males, laboratory results and virologic characteristics were not different between the two groups. The change in HBV DNA level at week 48 from baseline was −5.13 ± 1.40 in the DA-2802 group and −4.97 ± 1.40 log10 copies/mL in the Viread® group. The analysis of primary endpoint using the nonparametric analysis of covariance showed statistically significant results (P < 0.001), which confirmed non-inferiority of DA-2802 to Viread® by a prespecified noninferiority margin of 1. The proportion of undetectable HBV DNA was 78.7% in the DA-2802 group and 75.4% in the Viread® group (P = 0.698). The proportion of subjects who had normal ALT levels was 75.4% in the DA-2802 group and 73.3% in the Viread® group (P = 0.795). The proportion of those with HBeAg loss was 8.1% in the DA-2802 group and 10.8% in the Viread® group (P = 1.000). No subject showed HBsAg loss. The frequency of AEs during treatment was similar between the two groups. Most AEs were mild to moderate in severity. Conclusion: DA-2802 is considered an effective and safe treatment for patients with CHB.

        Development of Kanjang (Traditional Korean Soy Sauce) Supplemented with Glasswort (Salicornia herbacea L.)

        Five types of meju were prepared from 100% defatted soybean (DFSG0), a mixture of 90% DFS and 10% glasswort (DFSG1), a mixture of 80% DFS and 20% glasswort (DFSG2), a mixture of 70% DFS and 30% glasswort (DFSG3), and a mixture of 60% DFS and 40% glasswort (DFSG4). Five types of kanjang were separately prepared from the 5 types of meju by ripening in brine for 6 months. The contents of certain minerals (Mg, Ca, Fe, Mn, and Zn), organic acids (citric acid, malic acid) and the antioxidative effects in the kanjang were increased in proportion to the glasswort content in the meju. However, the free amino acid contents in the kanjang were reduced in proportion to the glasswort content in the meju. DFSG1- and DFSG2-kanjang did not show distinct differences from DFSG0-kanjang based on aroma, flavor, and taste that were compared simply by panel tests. The bacterial and fungal community in the fermented meju and kanjang was not affected by the addition of glasswort to the meju-making process. Bacteria belonging to the Lactobacillus and Bacillus genera and the Lactobacillus family predominated, and yeasts belonging to the Saccharomyces genus and fungi belonging to the Aspergillus genus predominated in the fermented meju and kanjang. In conclusion, the glasswort was a supplement that nutritionally improved the kanjang (except for free amino acid contents) but didn’t influence the growth of microorganisms that are responsible for the fermentation of meju and kanjang.

        Background: It is not well known about the relation between renal function and plaque changes in patients with angina pectoris and hypertension who uses statins. We assessed the impact of renal dysfunction on changes of plaque characteristics in statin-treated patients with angina pectoris and hypertension using virtual histology-intravascular ultrasound (VH-IVUS). Methods: We assessed plaque changes between patients with CKD [n=81, estimated creatinine clearance (CrCl) <60 mL/min) and those without CKD (n=117) who underwent baseline and follow-up VH-IVUS for non-intervened intermediate coronary artery stenosis. Results: %necrotic core (NC) area at minimum lumen area (MLA) site (22.5±11.7% vs. 19.0±11.1%, p=0.035) and %NC volume (20.3±8.0% vs. 15.8±9.4%, p=0.001) were signi ficantly greater, and thin-cap fibroatheroma was observed more frequently (25.9% vs. 10.3%, p=0.004) in CKD group compared with non-CKD group. Follow-up VH-IVUS was performed in about 9 months after baseline VH-IVUS examinations. At follow-up, plaque progressed in CKD group, in contrast plaque regressed in non-CKD group [plaque plus media (P&M) area at MLA site: +0.41±0.72 mm2 vs. -0.78±0.64 mm2, p<0.001). The CrCl correlated with ΔP&M area at MLA site (r=-0.538, p<0.001) and Δ%NC area at MLA site (r=-0.167, p=0.019). CrCl (OR 0.901, 95% CI 0.867-0.936, p<0.001) and baseline %NC area at MLA site (OR 1.251, 95% CI 1.116-1.403, p<0.001) were the independent predictors of plaque progression at follow-up. Conclusions: In patients with angina pectoris and hypertension who uses statins, renal dysfunction is associated with plaque progression and increase of NC component at follow- up.

        <P>Duck hepatitis virus type 1 (DHV-1) was previously classified as an enterovirus, based primarily on observed morphology and physicochemical properties of the virion. The complete nucleotide sequences of two strains (DRL-62 and R85952) of DHV-1 have been determined. Excluding the poly(A) tail, the genomes are 7691 and 7690 nt, respectively, and contain a single, large open reading frame encoding a polyprotein of 2249 aa. The genome of DHV-1 is organized as are those of members of the family Picornaviridae: 5' untranslated region (UTR)-VP0-VP3-VP1-2A1-2A2-2B-2C-3A-3B-3C-3D-3' UTR. Analysis of the genomic and predicted polyprotein sequences revealed several unusual features, including the absence of a predicted maturation cleavage of VP0, the presence of two unrelated 2A protein motifs and a 3' UTR extended markedly compared with that of any other picornavirus. The 2A1 protein motif is related to the 2A protein type of the genus Aphthovirus and the adjacent 2A2 protein is related to the 2A protein type present in the genus Parechovirus. Phylogenetic analysis using the 3D protein sequence shows that the two DHV-1 strains are related more closely to members of the genus Parechovirus than to other picornaviruses. However, the two DHV-1 strains form a monophyletic group, clearly distinct from members of the genus Parechovirus.</P>
