      • P-19 Active tuberculosis in retired dusty workers received health examination for confirming work-related pneumoconiosis

        According to the act on the prevention of pneumoconiosis and protection, etc. of workers suffering from pneumoconiosis, dusty work defines any work requiring handling of earth, stone, rock or minerals which might cause workers engaged in such work suffer from pneumoconiosis, as prescribed by Presidential Decree. Pneumoconiosis is a disease, the main symptom of which is a fibroplastic change occurring in lungs due to inhalation of dust. Active tuberculosis is manifested in one of the most common complications under the act on the prevention of pneumoconiosis and protection, etc. of workers suffering from pneumoconiosis. The present study aimed to assess the prevalence of and risk factors associated with active tuberculosis in 40,623 retired dusty workers received health examination for confirming work-related pneumoconiosis from 1984 to 2015. The mean age of them was 70.6 ± 8.42 years. 1,553 (3.8%) subjects were diagnosed with active tuberculosis. In multivariate analysis, mining workers (odds ratio, 1.518; 95% confidence interval, 1.269-1.816) and cardiopulmonary disability (odds ratio, 2.675; 95% confidence interval, 2.221-3.222) were independently associated with increased risk of development of active tuberculosis in retired dusty workers.

        Aspirin prevents TNF-α-induced endothelial cell dysfunction by regulating the NF-κB-dependent miR-155/eNOS pathway: Role of a miR-155/eNOS axis in preeclampsia

        <P><B>Abstract</B></P> <P>Preeclampsia is an inflammatory disease with endothelial cell dysfunction that occurs via decreased endothelial nitric oxide synthase/nitric oxide (eNOS/NO) activity. Aspirin reduces the incidence of hypertensive pregnancy complications. However, the underlying mechanism has not been clearly explained. Here, we found that tumor necrosis factor (TNF)-α, <I>microRNA (miR)−155</I>, and eNOS levels as well as endothelial redox phenotype were differentially regulated in preeclamptic patients, implying the involvement of TNF-α- and redox signal-mediated miR-155 biogenesis and eNOS downregulation in the pathogenesis of preeclampsia. Aspirin prevented the TNF-α-mediated increase in <I>miR-155</I> biogenesis and decreases in eNOS expression and NO/cGMP production in cultured human umbilical vein endothelial cells (HUVECs). Similar effects of aspirin were also observed in HUVECs treated with H<SUB>2</SUB>O<SUB>2</SUB>. The preventive effects of aspirin was associated with the inhibition of nuclear factor-κB (NF-κB)-dependent <I>MIR155HG</I> (<I>miR-155</I> host gene<B>)</B> expression. Aspirin recovered the TNF-α-mediated decrease in wild-type, but not mutant, eNOS 3′-untranslated region reporter activity, whose effect was blocked by miR-155 mimic. Moreover, aspirin prevented TNF-α-mediated endothelial cell dysfunction associated with impaired vasorelaxation, angiogenesis, and trophoblast invasion, and the preventive effects were blocked by miR-155 mimic or an eNOS inhibitor. Aspirin rescued TNF-α-mediated eNOS downregulation coupled with endothelial dysfunction by inhibiting NF-κB-dependent transcriptional <I>miR-155</I> biogenesis.       • 농촌지역 버스정류장의 디자인 특성 분석에 관한 연구

        오늘날 농촌이 녹색복지 실현을 위한 새로운 장으로서 역할이 증대되면서, 농촌다운 양호한 경관형성을 위한 다양한 시도가 이루어지고 있다. 그러나 농촌경관을 이루는 자연?생태적 요소와 인공적 요소들의 부조화로 농촌경관의 시각적 질은 저하되고 있는 실정이다. 따라서 본 연구는 농촌 경관의 장소성을 부여하고, 농촌 경관이미지를 좌우하는 주요 경관 시설 중 하나인 ‘버스정류장’을 대상으로 버스정류장의 물리적 디자인 특성인 형태, 재료, 색채 특성 분석에 관한 연구를 시행하였다. 분석결과, 각 행정구역별로 버스정류장 디자인특성은 유사한 패턴으로 나타났으며, 농촌만의 지역성이나 풍토성이 결여된 일반 기능위주의 도시환경 이미지를 형성하는 디자인 특성이 나타났다. 즉, 농촌 전 지역에서 농촌만의 지역적 정취와 개성이 결여된 경향이 나타나고 있는 것으로 분석되었다. 따라서 농촌 버스정류장 디자인 계획에 있어, 농촌의 자연적이며 부드러운 이미지와 농촌 각 지역의 정체성을 반영할 수 있는 형태, 재료, 색채 등의 디자인 구성요소에 대한 종합적인 계획이 이루어져야 할 것으로 사료된다. 이처럼 본 연구는 이러한 디자인 현황을 바탕으로 디자인 특성과 문제점을 추출하였고, 농촌경관의 가치를 재인식할 수 있는 계기를 마련하고 농촌지역 버스정류장 디자인 활용에 필요한 기초자료를 제공하는데 의의가 있다. As the role of rural area in becomes greater in green welfare, there are various attempts for landscape management of rural area to be favorable. However disharmony between natural factor and artificial factor deteriorates visual landscape quality of rural area. This study analyzed the design characteristics - such as form, material and color ? of ‘bus stations’s, which assign rural landscape placeness and influence rural landscape image. As a result, There were little differences in the design characteristics of bus stations among different rural areas and the design represented urban environment image instead of regionality or endemicity. In other words, All of rural areas tended to lack regionality and individuality of their own. Therefore consideration about a comprehensive plan including form, material and color in designing bus station in rural area in order to reflect the identity of each rural area. By means of analyzing present condition of design characteristics and problems in rural area, this study is relevant to rediscover the value of rural landscape and providing basic data.

      • 도시가로변 가설시설물의 경관이미지 및 시각적 선호도 분석

        도시학자 제이콥스(Jane Jacob)에 의하면 ‘도시의 거리가 재미있어 보이면, 그 도시도 재미있게 보인다.’라고 하였다. 즉, ‘가로 공간’의 경관이 실질적으로 도시의 이미지 전체를 형성하는 주요인으로 작용하고 있는 것이다. 최근 이러한 ‘가로 공간’은 도시의 경제적 · 기능적 측면이 부각됨에 따라 상업기능이 활성화 되고 있다. 이에 따라 상업시설물인 가설시설물들은 가로 공간 내에 입지가 확산되고, 가로경관 및 도시환경 전체의 이미지에 상당한 영향을 미치고 있다. 따라서 본 연구는 현대 도시의 이미지에 상당한 영향을 미치고 있는 ‘가설시설물’을 대상으로 가설시설물의 주·야간 시각적 및 경관적 특성에 관한 연구를 시행하였다. 즉, 가설시설물의 주·야간 경관의 이미지와 시각적 선호도, 경관이미지 인자와 시각적 선호도와의 관계성을 분석하여 가설시설물로 인해 가로의 경관이 어떻게 인식되는지를 비교·분석하고자 하였다. 이를 통해 그간 부정적으로만 인식되어 온 가설시설물에 대한 가치를 재인식할 수 있는 계기를 마련하고, 조화롭고 특색 있는 가로공간으로 정비하기 위한 방안을 제안하는데 의의가 있다. According to Jane Jacob, urbanologist, if the streets of the city seem to be fun, the city also seems to be fun. That is to say, Landscape is the principal factor which form the image of the entire city. Shedding a new light on economic and functional aspect of the city, the commercial function of the streets is becoming more vitalized. As a result, the number of the street vendors grows more and more, which impacts on landscape and the environment of a city. Therefore this study focused on landscape and visual characteristic of the street vendors, especially comparing day and night. I did comparative analysis on relationship between landscape, landscape variables and visual preference at different time - day and night - in this study. This study will make people reconsider street vendors which were looked at negatively and give us indexes for redesigning street with harmony and character.

      • 농촌 경관향상을 위한 마을회관의 색채이미지 분석에 관한 연구

        최근 ‘농촌’이 새로운 지역개발의 중심지로서 농촌의 역할과 가치가 주목받고 있다. 이에 따라 농촌문화의 흔적을 담은 역사자원이자 자연과 문화의 자원이 되는 농촌경관의 형성과 개발을 위한 다양한 시도가 진행되고 있다. 그러나 아직까지 농촌의 경관은 무분별한 외장색채의 사용 등 자연경관과 인공경관의 색채 부조화현상이 나타나고 있는 실정이다. 따라서 본 연구는 농촌의 다양한 시각적 이미지를 형성시키고, 농촌만의 정체성을 구현하는데 큰 영향을 주는 ‘색채’의 분석을 통해 농촌지역에서 주된 인위적 시각 구성요소인 공공시설 중 마을회관의 외장색채에 대한 색채현황과 이미지에 관한 연구를 시행하였다. 따라서 농촌지역 마을회관의 건축물 입면의 색채의 현황과 특성을 분석하고 I.R.I 색채감성척도를 통해 경관의 이미지를 색채로 도출하여 색채이미지를 분석하였다. 분석결과, 기능적이며 인공적인 색채의 경향이 농촌의 모든 지역에서 동일하게 나타나 일반 기능위주의 도시환경 이미지를 형성하는 것으로 나타났다. 따라서 본 연구는 이를 바탕으로 농촌경관의 가치를 재인식할 수 있는 계기를 마련하여, 자연과 조화를 이루면서 동시에 농촌의 지역적 특성과 정체성이 내재된 특색 있는 농촌경관 색채 환경을 정비하기 위한 방안을 제안하는데 의의가 있다. Recently, the ‘rural areas’ have been engaging public attention for their roles and values as the hub of the new local development. However, when it comes to the landscapes of the rural areas, until now, the exterior colors have been used only thoughtlessly. In the light of that, this study analyzed ‘colors’, and there, the colors have been forming various visual images of the rural areas. Through the analysis, the study looked into the current statuses and images of the exterior colors of community center, these public facilities which are located in the rural areas as the most popular artificial visual component. All the rural areas in South Korea were selected as research subjects, and the study aimed to investigate the current statuses and features of the facades of the community center. In addition, through I.R.I color sensibility scale, the study converted the landscape images into the colors, and analyzed the images of the target areas. The findings of this study are considered academically significant not only for the study has provided any relevant fields with a chance to have new understanding on the rural landscapes but also for it presents ideas about how to repair the color environments of the rural landscapes.

        산재요양기관 종사자에서 잠복결핵감염 유병율 및 위험요인

        Objectives: The major objective of this study was to investigate the prevalence of and risk factors for latent tuberculosis infection (LTBI) among employees at a workers' compensation hospital. Methods: Among the 394 employees at Incheon Hospital, 362 were enrolled in the study. An interferon-gamma release assay(IGRA) for diagnosis of LTBI was performed using QuantiFERON$^{(R)}$ TB Gold In-Tube(QFT-IT). Risk factors for LTBI were analyzed using logistic regression analysis. Results: The overall prevalence of LTBI was 32.0%(116/362). The non-medical departments have a significantly high prevalence compared to medical departments(39.7% vs 23.2%). In multivariate logistic regression analysis, experience working in the pneumoconiosis hospital(OR, 3.6; 95% CI, 1.3-10.3) was associated with development of LTBI. Conclusions: Korean guidelines for the management of tuberculosis recommend annual regular health examinations for TB and LTBI for health care workers(HCWs). Considering the high prevalence of and risk factors for LTBI among non-HCWs, it suggests a need for annual regular health examinations for TB and LTBI for all employees at workers' compensation hospitals, including pneumoconiosis hospitals.

      • Protease and antiprotease imbalance in retired coal miners with airflow limitation

        Occupational exposure to inorganic dusts such as coal and crystalline silica has been identified as a chronic obstructive pulmonary disease (COPD) risk factor. This risk factor causes abnormal lung inflammation and protease-antiprotease imbalance. Abnormal inflammatory response of the lung induces parenchymal tissue destruction and leads to progressive airflow limitation that is characteristics of COPD. The aim of this study was to determine the relationship of proteases such as neutrophil elastase (NE) and matrix metalloproteinase (MMP)-9 and antiproteases such as alpha-1 antitrypsin (AAT) and tissue inhibitors of metalloproteinase (TIMP)-1 with lung function. The study population contained 223 male retired coal miners. We performed lung function test and analyzed MMP-9, AAT, TIMP-1 in serum and NE in plasma by enzyme-linked immonosorbent assay (ELISA). NE, AAT, and The levels of TIMP-1 in workers, who had %FEV<sub>1</sub> < 80%, were higher than those of workers who had %FEV<sub>1</sub> ≥ 80%. The levels of AAT and TIMP-1 in workers with airflow limitation were higher than those of workers with normal. %FEV<sub>1</sub> showed significant negative correlation with AAT, TIMP-1, and NE. %FEV<sub>1</sub>/FVC showed significant negative correlation with NE. From the results of stepwise multiple regression analysis about %FEV<sub>1</sub> and %FEV<sub>1</sub>/FVC, significant independents were NE and AAT in %FEV<sub>1</sub>, and NE in %FEV<sub>1</sub>/FVC. There were significant relationships between airflow limitation and protease and antiprotease concentration. The levels of proteases and antiproteases, however, may be affected by the biological and inflammatory responses. It is necessary to evaluate specimens more reflected the effects of proteases and antiproteases imbalance in the lung such as lung tissue and bronchoalveolar lavage fluid.

        Analysis of Protease and Antiprotease Concentrations in Retired Workers Exposed to Inorganic Dusts

We performed lung function test, including percent of forced expiratory volume in one second (%FEV₁) predicted and %FEV₁/forced vital capacity (FVC). We analyzed serum MMP-9, AAT, TIMP-1 and plasma NE concentrations by sandwich enzyme immunoassay. NE, AAT, and TIMP-1 concentrations in workers, who had %FEV₁<80% predicted, were higher than those of workers who had %FEV₁ ≥80% (P<0.05). Both AAT and TIMP-1 concentrations in workers with airflow limitation were higher than those of workers with normal airflow (P<0.05). %FEV₁ predicted showed significant negative correlation with AAT (r=-0.255, P<0.01), TIMP-1 (r=-0.232, P<0.01), and NE (r=-0.196, P<0.01). %FEV₁/FVC predicted showed significant negative correlation with NE (r=-0.172, P<0.05). From the results of stepwise multiple regression analysis about %FEV₁ and %FEV₁/FVC, significant independents were NE (r=-0.135, P=0.001) and AAT (r=-0.100, P=0.013) in %FEV₁, and NE (r=-0.160, P=0.014) in %FEV₁/FVC. In the present study, there were significant correlations between airflow limitation and protease concentration and between airflow limitation and antiprotease concentration. Serum protease and antiprotease concentrations, however, may be affected by the biological and inflammatory responses. It is necessary to evaluate specimens more reflected the effects of proteases and antiproteases in the lung such as lung tissue, bronchoalveolar lavage fluid, and exhaled breath condensate (EBC).
