N/A Objectives: Pneumocystis carinii pneumonia(PCP) is one of the most common life-threatening opportunistic infections in patients with acquired immunodeficiency syndrome(AIDS). This study reports the clinical characteristics of PCP in the patients with AIDS and prognostic factors for short-term survival of them. Methods: We investigated 43 patients of AIDS to evaluate the frequency of PCP in patients in AIDS by retrospective analysis, and classified the 17 patients with PCP into survivors and non-survivors to compare epidemiology, clinical characteristics and laboratory findings. We also analyzed whether the these findings influenced the short-term survival in patients with PCP that was combined with AIDS. Results: In this retrospective study of 43 patients of AIDS, the frequency of PCP in AIDS patients was relatively high as 17 patients(39%), of whom eight patients(47%) died of PCP. The epidemidologic findings such as age, route of human immunodeficiency virus(HIV) infection and co-existing disease were not significantly different between survivors and non-survivors. Coughing was the most common symptom and bilateral infiltrates of lung was the most common form in the chest X-ray examination. But these clinical features were similar in the both groups. Total lymphocyte count, CD4 cell count, serum albumin level and arterial oxygen tension were decreased and serum LDH was increased in patients with PCP that was the first episode in patients with AIDS. Lymphocyte and CD4 cell count were significantly lower in the non-survivor group (p=.002 and p=.03, respectively), Survivors had higher serum albumin level and arterial oxygen tension than non-survivors (p=.02 and p=.04, respectively). And non-survivors were found to have higher serum LDH level than survivors (p=.02). Conclusion: Lymphocyte and CD4 cell counts, serum albumin and LDH, and arterial oxygen tension may be considered as the prognostic factors for short-term survival of patients with PCP that is combined with AIDS.

Purpose: The prospective study was conducted to assess the efficacy and toxicities of the salvage chemotherapy of capecitabine and cisplatin for the patients with recurrent SCCHN (squamous cell carcinoma of head and neck), who could not be treated anymore by surgery or radiation. Subject and Methods: The patients with recurrent SCCHN were eligible if they had treated with previous chemotherapy and radiotherapy. The treatment consisted of cisplatin (75 mg/m² as a 2 hours infusion on day 1) and capecitabine (1000 mg/m² orally twice daily, on day 2-15), repeated every 21 days. Results: Twenty-four patients received total 79 cycles (mean 4 cycles). Overall response rate was 42% with 1 CR (4%) and 9 PR (38%).5 patients were SD (21%) and 9 patients presented PD (38%). The most common grade 3/4 adverse events were neutropenia and anemia, which documented in 5 and 4 patients, respectively. Fatigue was the most frequent non-hematologic side effect that was noted in 11 patients. Treatment-related mortality was absent. Conclusion: Capecitabine plus cisplatin is an effective salvage regimen with an acceptable toxicity profile for the patient with recurrent SCCHN.

Objective: This study was performed to assess the efficacy and safety profiles of the combination treatment with S-1 and Cisplatin in patients with locally advanced squamous cell carcinoma of the head and neck (SCCHN). Design: Eligibility criteria consisted of histologically confirmed SCCHN, stage III or IV with no evidence of distant metastasis, evaluable lesions, adequate organ function, age of 20-80 years, and a performance status of 2 or less. Cisplatin was infused over 1 hour on day 1 (75 mg/m²) and S-1 was administered orally for 14 consecutive days (days 2-15). The dosages of S-1 were assigned according to the patients' body surface area (BSA): 50 mg twice a day (B5A<1.5m²),or 60 mg twice a day (B5A>1.5m²). Each course was repeated every 3 weeks. After 2 courses, tumor response was evaluated by CT scan and laryngoscopy. If the patients achieved a response (complete response: CR, or partial response: PR), they received one more course of chemotherapy before undergoing radical treatment such as radiotherapy or operation. Results: All 30 patients were assessable for toxicity, and 29 patients for response. Theoverall response was 89.7 % (CR:9, PR:17). 2 year estimated overall survival rate was 79.2 %. Adverse reactions occurred 128 times during 81 courses in the 30 cases. The most common grade 3/4 adverse event was neutropenia, which occurred in 8 patients. Non-hematological grade 3 or 4 toxicity included nausea and vomiting in 4 patients, stomatitis in 2 patients, and diarrhea in 1 patient. Conclusion: S-1 plus cisplatin combination chemotherapy is effective against locally advanced SCCHN with mild toxicity.
Analyzing the Factors Associated With Nocturia in Older People in the United States

Background: The risk factors of nocturia in older adults remain unclear. We aimed to in-vestigate factors associated with nocturia using the National Health and Nutrition Exami-nation Survey (NHANES) data. Methods: Among 40,790 participants, 4,698 participants aged ≥65 years were included from the NHANES dataset between 2005 and 2012. A multivariate logistic regression analysis was performed to determine the odds ratio (OR) for nocturia. A subgroup analysis was conducted based on sex and underlying diseases. Results: In the multivariate logistic regression model, obesity (OR, 1.46; 95% confidence interval [CI], 1.28–1.68), hypertension (OR, 1.28; 95% CI, 1.07–1.52), and diabetes melli-tus (DM) (OR, 1.27; 95% CI, 1.11–1.45) were significantly associated with nocturia. These factors were associated with nocturia regardless of sex. In a subgroup of participants with hypertension, obesity (OR, 1.44; 95% CI, 1.25–1.67) and DM (OR, 1.26; 95% CI, 1.09–1.45) were associated with nocturia. In the additional analysis on patients with DM, noc-turia was associated with obesity (OR, 1.33; 95% CI, 1.06–1.67) and duration of DM (OR, 1.02; 95% CI, 1.01–1.03). Conclusion: This study demonstrated that hypertension, DM, and obesity were significantly associated with the prevalence of nocturia in older adult patients regardless of sex. In particular, obesity was associated with nocturia in every sub-group analysis.

Most Technological laws of Korea are almost identical to those of Japan. Electricity was not introduced to Korea until lighting ceremony in Kyong-Bok palace observed in 1887. Since Korea was annexed to Japan in 1910, Korea have applied Japanese law and regulations made by Chosun government-general. And public works were under Japan control. Korea accepted adopted the power plants of America, codes of NESC, ASME / ANSI, MEMA while Japan accepted or adopted only IEC. Our generation distribution system takes American style while our consumer side takes Japanese style. As global trade system was transferred GATT into WTO, the technological standards, certification, construction and testing are in harmony internationally. The WTO / TBT agreement came in effect after 12th, April, 1979. The sections in the agreement require the members to exchange their own standards and codes for the harmonized ones by degree. The Korean distribution system has a strong resemblance to American system so that the internal engineers are confused in application of the relevant standards. IEC60364 and NEC are technologically similar but practically unconvertible because both have their different originality, that is to say Europe and USA. This paper deals with the fundamental elements of electrical safety system on review of IEC 60364 and NEC. This paper considers how engineers should apply IEC60364 and NFPA 70(NEC) based on all-the-way review of them.

A leaching kinetics was conducted for the purpose of recovery of praseodymium in sulfuric acid (H2SO4) from REE slag concentrated by the smelting reduction process in an arc furnace as a reactant. The concentration of H2SO4 was fixed at an excess ratio under the condition of slurry density of 1.500 g slag/L, 0.3 mol H2SO4, and the effect of temperatures was investigated under the condition of 30 to 80 oC. As a result, praseodymium oxide (Pr6O11) existing in the slag was completely converted into praseodymium sulfate (Pr2(SO4)3o8H2O) after the leaching of 5 h. On the basis of the shrinking core model with a shape of sphere, the first leaching reaction was determined by chemical reaction mechanism. Generally, the solubility of pure REEs decreases with the increase of leaching temperatures in sulfuric acid, but REE slag was oppositely increased with increasing temperatures. It occurs because the ash layer included in the slag is affected as a resistance against the leaching. By using the Arrhenius expression, the apparent activation energy of the first chemical reaction was determined to be 9.195 kJmol-1. In the second stage, the leaching rate is determined by the ash layer diffusion mechanism. The apparent activation energy of the second ash layer diffusion was determined to be 19.106 kJmol-1. These relative low activation energy values were obtained by the existence of unreacted ash layer in the REE slag.
