골절 치유에 대한 Vitamin D와 Calcium의 최신 지견

골다공증성 골절, 부전 골절, 노인 골절로 알려진 취약 골절은 노령화의 가장 큰 문제의 하나로 정형외과의사에게 흔히 접하게 된다. Calcium과 vitamin D는 뼈 건강을 유지하기 위해 필수적이다. 따라서 calcium과 vitamin D의 결핍은 골다공증 발생의 주요 위험 요소이다. 가골의 무기화 시에는 충분한 양의 calcium이 필요하기 때문에 골다공증 환자에서 자주 관찰되는 골 감소는 calcium과 vitamin D 결핍에 기인할 수 있다. 따라서 calcium과 vitamin D 보충제는 골다공증 환자의 취약한 골절 치유를 치료하기 위해 상당히 중요할 수 있겠다. 골절 치유와 외상 후 골의 turnover의 긍정적인 효과의 몇 가지 임상 증거가 있지만 더 많은 연구가 수행되고 있다. calcium과 vitamin D는 골 조송증 환자, calcium 및 vitamin D 농도의 부족, 2차 골다공증 골절 예방을 위한 일차 치료제이며 골 흡수 억제제나 골 형성 촉진제와 같은 다른 1차 골다공성 약물과 함께 보조제로 사용될 수 있다. 골절 치유에서 calcium과 vitamin D의 역할을 여기에서 소개하고자 한다. Fragile fractures, also known as osteoporosis fractures, insufficiency fractures, and senile fractures are a significant problem encountered by orthopedic surgeons. Calcium and vitamin D are essential for maintaining bone health and deficiencies in calcium and vitamin D are major risk factors for the development of osteoporosis. Sufficient amounts of calcium are also required for fracture-callus mineralization. Hence, compromised bone repair that is frequently observed in osteoporotic patients might be attributed to calcium and vitamin D deficiencies. Consequently, calcium and vitamin D supplementation represents a potential strategy for treating compromised fracture healing in osteoporotic patients. There is some clinical evidence of the positive effect of supplementation in fracture healing and posttraumatic bone turnover, but research in this area is ongoing. Calcium and vitamin D should be the primary treatment of choice in osteopenic patients with an insufficiency of calcium and vitamin D and for the prevention of secondary osteoporotic fractures. Calcium and vitamin D can also be used as addition to other primary osteoporotic medications such as antiresorptive or bone-forming agents. The role of calcium and vitamin D in fracture healing and the therapeutic potential of calcium and vitamin D supplementation is summarized in this context.
비대면 패션·뷰티 공연의 특성요인이 고객만족에 미치는 영향 -혁신저항의 조절효과를 중심으로-

본 연구는 2020년 비대면으로 서울에서 성공적으로 진행된 비대면 패션·뷰티 공연 '아시아모델페스티벌'의 특성 요인(현존감, 공감성, 반응성, 상호작용)이 고객만족에 미치는 영향과 특성요인과 고객만족의 관계에서 혁신저항의 조절효과를 실증적으로 검증하는데 목적을 두고 연구하였다. 연구대상은 비대면 '아시아모델페스티벌' 관람객을 대상으로 총 223부의 설문지를 회수하여 SPSS 21.0 통계 프로그램을 사용하여 실증분석 하였다. 분석 결과 특성요인이 고객만족에 긍정적인 영향을 미치는 것으로 나타났고, 혁신저항은 특성요인의 하위요인별 상호작용, 공감성과 고객만족 간의 관계를 조절하는 것으로 나타났다. 본 연구는 코로나로 인해 더욱 확대되고 있는 비대면 관람객의 고객만족에 영향을 미치는 특성요인을 파악하고 혁신저항의 조절효과를 연구함으로 해서, 향후 비대면 관람객이 어떤 것을 요구하고, 어떤 서비스를 제공받기 원하는지를 알 수 있는 근거를 마련하여 실무적인 측면의 시사점을 제시하고 관련 연구를 위한 기초자료를 제공하였다. 하지만, 향후에는 혁신저항의 하위 개념을 세분화하고 이들 각 하위변수들을 살필 필요가 있고, 비대면 공연과 고객만족의 관계를 조절하는 새로운 변인에 대한 연구도 진행될 필요가 있다고 판단된다. 그러할 때에 새로운 비대면 환경의 수용에 관여하는 다양한 변인들의 상호작용적 역할에 대한 이해가 높아질 것으로 기대한다. The purpose of this study aims to empirically verify about the impact of the customer satisfaction on the characteristics factors(presence, empathy, responsiveness, and interaction) on the customer satisfaction of 'Asia Model Festival', which was heldis an as untacted fashion and beauty performance that was successfully conducted in Seoul duringinyear 2020. and tThe regulatory effect of innovation resistance in the relationship of the characteristic factors and the customer satisfaction was also reviewed. The analysis showed that the characteristic factors have a positive effect on customer satisfaction, and the innovation resistance is found to control the relationship between interaction, empathy, and the customer satisfaction of the characteristic factors by the subfactor. By identifying the characteristic factors that affect the customer satisfaction of the un-tacted(Zzero Ccontact) visitors, which are expanding due to COVID-19, and studying the effect of controlling the innovation resistance, this study provides a basis for knowing what un-tacted(Zzero Ccontact) visitors need and what services they want to receive, and it presents practical implications and provides basic data for relevant research.
Failure of a Rotation Control Gamma 3 Lag Screw Used to Treat a Trochanteric Fracture

Gamma 3 rotation control lag screws (U-blade) are particularly useful when treating rotational and unstable fractures of the proximal femur. A 93-year-old woman who underwent closed reduction of a trochanteric fracture and internal fixation with a Gamma 3 nail rotation control lag screw. The patient presented with metal failure and U-blade bending following a fall occurring 4 weeks after surgery. Here, we present a case report summarizing removal of the failed lag screw.

Purpose: This study was performed to assess the radiologic and clinical results of U-blade Gamma3 nail use for the treatment of trochanteric fractures. Materials and Methods: Between September 2015 and May 2018, all patients aged 65 years and older who underwent surgery with U-blade Gamma3 nails were analyzed. A total of 129 patients were selected based on having at least six months of follow-up. Image evaluations included bone quality (T-score), fracture classification on plain radiograph (AO/OTA), computed tomography configuration, union period, position of lag screw, anatomical reduction, tip apex distance (TAD), sliding extent of lag screw, change of neck shaft angle, and complications leading to reoperations were analyzed. Functional outcome were assessed using the Koval grade (ambulatory ability) at the final follow-up. Results: The mean time to union was 19.7 (range, 6-36) weeks. The screw position was centric (93 cases; 72.1%) and anatomical reduction was achieved in 74 cases (57.4%). The mean TAD was 20.3 (range, 12.3-38.1) mm. The mean sliding length of the lag screws was 3.8 (range, 0.1-12.6) mm. The mean change of neck shaft angle was 3.4˚ (range, 0-12.8˚ ). Reoperations were required in two cases (1.6%) due to the cutting out of the lag screw (n=1) and metal failure with U-blade bending (n=1). Finally, Koval grades for 49.8% of patients reached preoperative status. Conclusion: Overall, use of the U-blade Gamma3 nail led to favorable clinical results, suggesting that this system may be a good option for the treatment of trochanteric fractures.
A review of robotic‑assisted total hip arthroplasty

Total hip arthroplasty (THA) is a successful surgical method for hip replacement but still poses challenges and risks. RoboticassistedTHA (rTHA) using new generation robotic systems has emerged to improve surgical precision and outcomes. Thepurpose of this paper is to review the literature on rTHA, with a focus on its advantages, such as individualized preoperativeplanning, intraoperative assistance, and improved accuracy in implantation, especially in complex cases. Additionally, itaims to explore the disadvantages associated with the use of rTHA, including high costs, the learning curve, and prolongedoperation time compared to manual THA (mTHA), which are critical drawbacks that require careful consideration andefforts for minimization. Some financial analyses suggest that rTHA may offer cost-effectiveness and reduced postoperativecosts compared to mTHA. While technological advancements are expected to reduce technical complications, there are stilldebates surrounding long-term outcomes. Practical limitations, such as limited availability and accessibility, also warrantattention. Although the development of rTHA shows promise, it is still in its early stages, necessitating critical evaluationand further research to ensure optimal patient benefits.
