        Genetic effect of <i>CCR3</i> and <i>IL5RA</i> gene polymorphisms on eosinophilia in asthmatic patients

        <P><B>Background</B></P><P>Eosinophilic infiltration and peripheral blood eosinophilia in asthma require the cooperation of eosinophil-specific cytokines and chemokines and their receptors.</P><P><B>Objective</B></P><P>We investigated the association of polymorphisms in <I>CCR3</I> and <I>IL5RA</I> with asthma susceptibility or peripheral blood eosinophilia and the effects of the polymorphisms on receptor expression.</P><P><B>Methods</B></P><P>Polymorphisms in <I>CCR3</I> and <I>IL5RA</I> were identified and genotyped in 576 asthmatic patients and 180 healthy control subjects. CCR3 and IL-5 receptor α (IL-5Rα) protein expression on eosinophils was measured by means of flow cytometry.</P><P><B>Results</B></P><P>Although polymorphisms in <I>CCR3</I> were not associated with asthma susceptibility, the <I>CCR3</I> haplotype <I>ht2</I> showed a negative gene dose effect on the eosinophil count (<I>P</I> = .003–.009). <I>IL5RA c.−5091G>A</I> was weakly associated with eosinophil count. The effects of <I>ht2</I> were greater when paired with <I>IL5RA c.−5091A</I> (<I>P</I> = .001–.002). CCR3 protein expression was higher on eosinophils of asthmatic patients without <I>ht2</I> than in those with <I>ht2</I>. Asthmatic patients with the <I>IL5RA c.−5091A</I> allele showed higher IL-5Rα expression than those who were homozygous for the G allele.</P><P><B>Conclusion</B></P><P>The genetic association between <I>CCR3</I> polymorphisms and the number of circulating eosinophils was revealed as a novel finding. These associations were more pronounced when the <I>CCR3</I> polymorphisms were paired with polymorphisms in <I>IL5RA</I>. The protein expression levels of CCR3 and IL-5Rα on peripheral blood eosinophils are associated with the polymorphisms on their own genes.</P><P><B>Clinical implications</B></P><P>The identification of single nucleotide polymorphisms and haplotypes of <I>CCR3</I> and <I>IL5RA</I> might be useful in developing markers for intermediate phenotypes of eosinophil number and in designing strategies to control diseases related to hypereosinophilia.</P>

        This study was carried out for reorientation of Rehabilitation Service Delivery System (RSDS) through reviewing reference and observing field practice. The most important results are as follows: 1. The current rehabilitation facility for handicapped persons can be classified into four categories : (a) those for physically disabled persons, (b) those for mentally disturbed persons, (c) those for visually disturbed persons, and (d) those for acoustic and speech disturbed persons. The nationwide number of rehabilitation facilities is 101 of those, 43.5% is for physically disabled persons and 32.9% is for mentally disturbed persons. 2. A small number of circulating rehabilitation centers for physically disability are being run by comprehensive rehabilitation centers since 1992. 3. The hospitals are self-supporting only 20% of their operational costs, and the rest of the operational costs comes from federal tax(40%) and local tax(40%). 4. The number of handicapped persons in Taegu and Kyungpook province is estimated to be about 50,000, but only 20 percent out of them ws registered. The rehabilitation facilities located in this area receives 69% of their operational costs from central government. This figure is higher than the national average of 64.6%. 5. Although RSDS is different from other medical delivery system, nonetheless, it should be developed as a sub-system of national health care delivery system. Its development should also be in accordance with the need of local community. Therefore we suggest the development policy for the development of RMDS must consider the following points.; First, all rehabilitation-related medical systems should be integrated unde ra single comprehensive system so that handicapped persons may easily obtain better service available. Second, rehabilitation programs based on local community should be rejuvenated. In addition to in-patient service, development of intermediate facilities and programs connecting home and society should be encouraged. Third, rehabilitation medical services should be more specialized. Also, more emphasis should be given to team approach to rehabilitation medical services. Fourth, a comprehensive rehabilitation information system should be developed so that handicapped persons may have easy access to public welfare facilities, and the central government should formulate a comprehensive long-term plan for the rehabilitation system.

        The breadmaking characteristics of composite flour containing 10-50% of buckwheat and wheat, were studied to establish the optimum formula for the development of buckwheat bread. As the proportion of buckwheat flour increased in bread formula, the water absorption and loaf volume decreased, but the loaf weight increased. Buckwheat flour was added to wheat flour at a replacement level of 30% without a large adverse effect on bread quality. As the amount of buckwheat flour increased, crust and crumb color were getting dark. According to the sensory acceptance test, buckwheat bread containing higher bran content were preferred to those with less brain. The sensory quality determined by QDA(Quantitative descriptive analysis) showed as follows: The size of air cells, buckwheat flavor, mustiness, bitter taste, aftertaste, firmness, grittiness, and crumbliness increased, but moistness and springiness decreased in buckwheat breads containing 10-30% replacement levels of buckwheat for wheat flours. The overall quality decreased as the amount of buckwheat flour increased.

        The staling of buckwheat bread from composite flour containing 30% of buckwheat and 70% wheat, with the addition of vital wheat gluten and water-soluble hums, were studied. The hardness of the bucktwheat bread appeared significantly high with increasing storage time, the second day after baking , the addition of vital wheat gluten or/ and gums reduced hardness significantly below that of the buckwheat bread. Among those additives, xanthan gum showed the best effect on delay of bread staling Synergistic effects a when the vital wheat gluten and xanthan gum or guar gum were used together.

        Etiology and Patterns of Maxillofacial Fractures in 518 patients in Korea

        Purpose: Different patterns in the causes of maxillofacial injury are thought to correlate with socioeconomic status and regional environment. This study investigated maxillofacial fractures in order to analyze maxillofacial trauma characteristics and the relationship between the causes and injury patterns in Korea. Material and methods: A total of 518 patients with maxillofacial fractures who were treated at the Seoul National University Boramae Hospital between 1996 and 2004 were retrospectively analyzed. Data were obtained from the patients’medical records and radiographs. The male to female ratio in the patient group was 2.78:1, and the mean age was 32.3 years. Results: Midfacial fractures were the most common location of injury (46.1%). The most common etiologic factor was an activity associated with daily life (42.6%) including falls, stumbling, and collisions. The second most common cause was assault (32.4%), followed by traffic accidents (13.7%). In the case of midfacial fractures and mandibular fractures, assault was the most common etiologic factor, whereas in the case of alveolar bone fractures, activities associated with daily life were the most common cause. With regard to age groups, assault was the most common cause for patients between 10 and 39 years old and an activity associated with daily life was the most common cause in those under 10 years and over 40 years. Conclusions: This study concluded that activities associated with daily life and assault causes a large proportion of Korean maxillofacial injuries and that preventive measures should be implemented in order to minimize these risks.

        <P>The vertebrate neural crest is a multipotent cell population that gives rise to a variety of different cell types. We have discovered that postmigratory cranial neural crest cells (CNCCs) maintain mesenchymal stem cell characteristics and show potential utility for the regeneration of craniofacial structures. We are able to induce the osteogenic differentiation of postmigratory CNCCs, and this differentiation is regulated by bone morphogenetic protein (BMP) and transforming growth factor-beta signaling pathways. After transplantation into a host animal, postmigratory CNCCs form bone matrix. CNCC-formed bones are distinct from bones regenerated by bone marrow mesenchymal stem cells. In addition, CNCCs support tooth germ survival via BMP signaling in our CNCC-tooth germ cotransplantation system. Thus, we conclude that postmigratory CNCCs preserve stem cell features, contribute to craniofacial bone formation, and play a fundamental role in supporting tooth organ development. These findings reveal a novel function for postmigratory CNCCs in organ development, and demonstrate the utility of these CNCCs in regenerating craniofacial structures.</P>

        거대설 치료를 위한 혀 중앙부 절제술 :

        Macroglossia can cause dentomusculoskeletal deformities, instability of orthodontic and orthognathic surgical treatment, and create masticatory, speech and airway management problems. To determine whether a reduction glossectomy is necessary, it will important to identify the signs and symptoms of macroglossia. Development of dentoskeletal changes directly related with tongue size, such as an anterior open bite or a Angle Class III malocclusion tendency, would indicate that reduction glossectomy may be beneficial. For reduction glossectomy, several techniques have been reported. However, in most techniques the tip of tongue is removed. So its excision causes the loss of most mobile and sensitive portion of the tongue, and creates ankylosed, globular tongue. To avoid such problems, central tongue reduction technique have been proposed. This article will introduce central tongue reduction for anterior openbite case associated with macroglossia.

      • 우리나라 의료재활 전문인력 수요 공급 및 추계에 관한 연구

        Korea will soon experience a high demand for medical rehabilitation specialists, if it tries to deliver advanced health welfare service. In order to medical rehabilitation manpower policies, this study attempts to analyse, estimate and plan a long-term supply for physiatrists, physical therapists, and occupational therapists. The study analysed both national and foreign statistical data of manpower supply far medical rehabilitation specialists. Based an the above data, the demand of and supply for each specialists were estimated for lang term up to the year 2030. Based an the comparative analysis results of the future demand and supply, the author intended to develop a new supply plan for the three specialist categories. The major finding of the supply plan are as follows: First, as for the supply plan for physiatrists, the author recommends to adopt the demand estimation 1 as the most suitable. In order to prevent an oversupply of physiatrists, the supply plan 1 is recommended which annual enrollment of specialists will maintain with the quota of fixed number of 63 from the year of 1999. Second, it is estimated that there was already an oversupply of physical therapists in Korea. This oversupply is expected to continue even though there would be an increase in rehabilitation hospital beds, rehabilitation facilities for the elderly, and nursing homes, Thus, it would be desirable to cut down the number of students admitted to physical therapy schools each year. Third, there will be a high demand for occupational therapists in the near future as people become more aware of the usefulness of this therapy. Thus, it is urgent to establish a supply plan to meet the demand. Given the close relationship between physical therapy and occupational therapy, the study recommends that the universities already having the the department of physical therapy open the department of occupational therapy as well.
