        Psychosocial Determinants of Knee Osteoarthritis Progression: Results from the Promoting Independence in Our Seniors with Arthritis Study

        Background: Knee osteoarthritis (OA) is a common cause of physical disability among older adults. While established risk factors for knee OA include age and increased body weight, few studies have examined psychosocial risk factors or progression of knee OA. Methods: The Promoting Independence in our Seniors with Arthritis study recruited participants aged 65 years and over from orthopedic outpatients and community engagement events. Participants were invited to annual visits during which knee OA symptoms were assessed with the Knee Injury and Osteoarthritis Outcome Score (KOOS), social network using the 6-item Lubben Social Network Scale and anxiety and depression using the Hospital Anxiety and Depression scale. Knee OA worsening was defined by a 5% reduction in mean KOOS scores at the last visit compared to the first visit. Results: Data were available from 148 participants, mean age 66.2±6.5 years and 74.1% female, of whom 28 (18.9%) experienced OA worsening over a median follow-up period of 29 months. Univariate analyses revealed that age, sex, height, grip strength, and social network were associated with OA worsening. Social network remained statistically significantly associated with OA worsening after adjustment for age and sex difference (odds ratio=0.924; 95% confidence interval, 0.857–0.997). The relationship between social network and OA worsening were attenuated by both depression and handgrip strength at baseline. Conclusion: Psychological status and muscle strength may be modifiable risk factors for social network which may in turn prevent knee OA worsening and should be targeted in future intervention studies.

        Outcomes of salvage liver transplant for recurrent hepatocellular carcinoma

        Backgrounds/Aims: Salvage liver transplantation (SLT) is a therapeutic strategy for recurrent hepatocellular carcinoma (HCC). However, it remains controversial with compromised survival outcomes and increased perioperative morbidity compared to primary liver transplant (PLT). In the present work, we describe our institution’s experience on SLT by comparing outcomes of SLT to PLT for HCCs. Methods: Retrospective analysis was conducted for 49 transplant patients from 2006-2017. A comparative analysis was carried out between 14 SLT patients and 35 PLT patients. Results: SLT patients demonstrated significantly shorter time to recurrence than PLT patients (median=5.5 versus 23 months, p<0.001) with a trend towards increased perioperative major morbidity (42.9% versus 37%, p=0.711), inferior 5-year overall survival (61% versus 75%, p=0.345) and inferior 5-year recurrence-free survival (57% versus 72%, p=0.263). However, overall survival from the point of primary resection over a 10-year period showed no statistical difference between the 2 groups (SLT=60% versus PLT=61%, p=0.685). Conclusions: SLT is a viable treatment strategy for HCCs. However, it exhibited poorer short-term perioperative and oncologic outcomes than PLT. SLT requires better patient selection with liver donor grafts for optimization of resource allocation in this era of organ shortage. Considering the worldwide shortages in liver grafts, it is hypothesized that optimization of a salvage transplant strategy may improve resource allocation and reap optimal patient outcomes.

        Free vibration analysis of continuous bridge under the vehicles

        Free vibration analysis for continuous bridge under any number of vehicles is conducted in this paper. Calculation strategy for natural frequency and mode shape is proposed based on Euler-Bernoulli beam theory and numerical assembly method. Firstly, a half-car planar model is adopted; equations of motion and displacement functions for bridge and vehicle are established, respectively. Secondly, the undermined coefficient matrices for wheels, vehicles, intermediate support, left-end support and right-end support are derived. Then, the numerical assembly technique for conventional finite element method is adopted to construct the overall matrix of coefficients for whole system. Finally, natural frequencies and corresponding mode shapes are determined based on iterative method and overall matrix solution. Numerical simulation is presented to verify the effectiveness of the proposed method. The results reveal that the solutions of present method are exact ones. Natural frequencies and associate modal shapes of continuous bridge under different conditions of vehicles are investigated. The influences of vehicle parameters on natural frequencies are also demonstrated.

        New Analysis ofWang Yangming’s Four Dicta

        “四句敎”是王陽明最晩期的思想。後世學者的硏究多是把注意力放在對“四句敎”本體或工夫的把握, 對“四句敎”本身“徹上徹下”的特性硏究尙有不足, 本文擬在此有所發明。王陽明認爲“四句敎”是“徹上徹下”, 可惜的是, 王陽明早逝, 沒有時機對“四句敎”如何“徹上徹下”進行更深入地闡述;意外的是, 王陽明去世後, 錢德洪、王畿二人對“四句敎”均有不同程度的修改, 錢德洪?住的是徹下一節;王畿?住的是徹上一節。這些修改在一定程度上雖模糊了王陽明“四句敎”的本義, 但爲後人理解提供了最相近的注?。 The four dicta (siju jiao 四句敎) whichWang Yangming formulated in his late period of life represent the central tenets of his whole thoughts. The four dicta were mostly analyzed from the perspective of ontology or focusing on the practice of ontology; however, these approaches do not clearly illuminate Wang Yangming’s thoughts. Wang considers the four dicta as the truth which “encompasses the below and the above.” However, Qian Dehong, one of Wang’s students, only focuses on the below: the importance of practical action. In contrast, Wang Ji, another student of Wang, only emphasizes the above: the importance of recognizing the ontology. Thus, neither Qian Dehong nor Wang Ji fully clarifies Wang Yangming’s thoughts. However, as the students who followed Wang for the longest time, their understandings of the four dicta are more profound than others, and thus we can understand Wang Yangming’s thoughts most clearly through an analysis of their explanations.         Theoretical research on the identification method of bridge dynamic parameters using free decay response

        Input excitation and output response of structure are needed in conventional modal analysis methods. However, input excitation is often difficult to be obtained in the dynamic load test of bridge structures. Therefore, what attracts engineers' attention is how to get dynamic parameters from the output response. In this paper, a structural experimental modal analysis method is introduced, which can be used to conveniently obtain dynamic parameters of the structure from the free decay response. With known damping coefficients, this analysis method can be used to identify the natural frequencies and the mode shapes of MDOF structures. Based on the modal analysis theory, the mathematical relationship of damping ratio and frequency is obtained. By using this mathematical relationship to improve the previous method, an improved experimental modal analysis method is proposed in this paper. This improved method can overcome the deficiencies of the previous method, which can not identify damping ratios and requires damping coefficients in advance. Additionally, this improved method can also identify the natural frequencies, mode shapes and damping ratios of the bridge only from the free decay response, and ensure the stability of identification process by using modern mathematical means. Finally, the feasibility and effectiveness of this method are demonstrated by a numerical example of a simply supported reinforced concrete beam.

        A New Approach to Chen Men’s Ridicule of Kongzi

        在“子路宿于石門”篇中, “晨門”對孔子周流列國之擧的態度究竟如何? 對此問題, 先賢巨匠的釋讀觀點旣精彩紛呈又存有爭論。在分析《論語》原典和諸家注釋的基礎上, 本文認爲“晨門”之“知其不可而爲之”一語應是在譏諷孔子;但是“晨門”的譏諷却恰好反證了孔子高尙的品格和對人類社會發展的眞切關懷;同時, “知其不可而爲之”從另外一種側面揭示了儒家的精神骨幹。 The “Xian wen” 憲問chapter in the Lunyu 論語contains Chen Men’s 晨門remark about Kongzi 孔子(551-479 BCE). Kongzi was well known for his travel to feudal states to preach his political ideals. Of this effort, Chen Men remarked that Kongi was “doing things though he know they are unworkable” (zhi qi bu ke er wei zhi 知其不可而爲之). Chen Men’s remark has typically been understood as a ridicule of Kongzi. While there have been various debates on the nature of the Chen Men’s criticism of Kongzi’s behavior and diverse interpretations are possible it seems clear that Chen Men’s ridicule of Kongzi attests to Kongzi’s lofty moral integrity and idealistic devotion to improving human society. The effort and devotion exemplified by Kongzi are usually understood as the essence of Confucian morale and value. The ridicule implied by "knowing it is impossible" can be said to represent the evaluation of Confucius and all Confucian scholars made by the people at that time. Such comments made by them can be regarded as purely a criticism of Confucius, who did not know much about the lives of ordinary people in the real political world at that time, and only pursued ideals that departed from reality. Confucius' perseverance was not accepted by those in power at the time. It is not an appropriate dimension to interpret Confucianism from the perspective of popularization, idealization, and deification. [Article in Chinese]

        Free Vibration of the Cracked Non-uniform Beam with Cross Section Varying as Polynomial Functions

        This paper presents an approach for free vibration analysis of the cracked non-uniform beam with general boundary conditions, whose mass per unit length and bending moment of inertia varying as polynomial functions. Firstly, the general expression of mode shape function of the non-uniform beam with four undetermined coefficients is solved by a generalized power series method based on the Euler-Bernoulli beam theory. Then, the transfer matrix method is combined into the mode shape function of the non-uniform beam to improve the computational efficiency owe to the fewer segments divided by the cracks and joints at the different forms of variable cross-section. The massless rotational springs are adopted to simulate the cracks to derive the transfer relationship of the undetermined coefficients in the same segment. The four undetermined coefficients matrix is obtained by using equilibrium and continuity conditions between two adjacent segments, and then the characteristic equation of the entire cracked beam is formed. Finally, the correctness and reliability of the proposed method in this paper is verified by the methods presented by other published papers and the Finite Element Method (FEM). In addition, the crack parameters on the vibratory characteristics are investigated through the numerical examples.

      • DNA Repair Gene Polymorphisms Do Not Predict Response to Radiotherapy-Based Multimodality Treatment of Patients with Rectal Cancer: a Meta-analysis

        Background: A number of association studies have been carried out to investigate the relationship between genetic polymorphisms in DNA repair genes and response to radiotherapy-based multimodality treatment of patients with rectal cancer. However, their conclusions were inconsistent. The objective of the present study was to assess the role of DNA repair gene genetic polymorphisms in predicting genetic biomarkers of the response in rectal cancer patients treated with neoadjuvant chemoradiation. Materials and Methods: Studies were retrieved by searching the PubMed database, Cochrane Library, Embase, and ISI Web of Knowledge. We conducted a meta-analysis to evaluate the association between genetic polymorphisms and the response in rectal cancer treated with neoadjuvant chemoradiation by checking odds ratios (ORs) and 95% confidence intervals (CIs). Results: Data were extracted from 5 clinical studies for this meta-analysis. The results showed that XRCC1 RS25487, XRCC1 RS179978, XRCC3 RS861539, ERCC1 RS11615 and ERCC2 RS13181 were not associated with the response in the radiotherapy-based multimodality treatment of patients with rectal cancer (p>0.05). Conclusions: This study shows that DNA repair gene common genetic polymorphisms are not significantly correlated with the radiotherapy-based multimodality treatment in rectal cancer patients.
