        Transforming growth factor-beta and liver injury in an arginine vasopressin-induced pregnant rat model

        Objective: Approximately 30% of preeclamptic pregnancies exhibit abnormal liver function tests. We assessed liver injury-associated enzyme levels and circulating transforming growth factor beta (TGF-β) levels in an arginine vasopressin (AVP)-induced pregnant Sprague-Dawley rat model.Methods: Pregnant and non-pregnant Sprague-Dawley rats (n=24) received AVP (150 ng/hr) subcutaneously via mini-osmotic pumps for 18 days. Blood pressure was measured, urine samples were collected, and all animals were euthanized via isoflurane. Blood was collected to measure circulating levels of TGF-β1-3 isomers and liver injury enzymes in pregnant AVP (PAVP), pregnant saline (PS), non-pregnant AVP (NAVP), and non-pregnant saline (NS) rats. Results: The PAVP group showed significantly higher systolic and diastolic blood pressure than both saline-treated groups. The weight per pup was significantly lower in the AVP-treated group than in the saline group (p<0.05). Circulating TGF-β1-3 isomer levels were significantly higher in the PAVP rats than in the NS rats. However, similar TGF-1 and TGF-3 levels were noted in the PS and PAVP rats, while TGF-2 levels were significantly higher in the PAVP rats. Circulating liver-type arginase-1 and 5'-nucleotidase levels were higher in the PAVP rats than in the saline group. Conclusion: This is the first study to demonstrate higher levels of TGF-β2, arginase, and 5'-nucleotidase activity in PAVP than in PS rats. AVP may cause vasoconstriction and increase peripheral resistance and blood pressure, thereby elevating TGF-β and inducing the preeclampsia-associated inflammatory response. Future studies should explore the mechanisms through which AVP dysregulates liver injury enzymes and TGF-β in pregnant rats.

        Surgery for symptomatic hepatic hemangioma: Resection vs. enucleation, an experience over two decades

        Backgrounds/Aims: Hemangiomas are the most common benign liver lesions; however, they are usually asymptomatic and seldom require surgery. Enucleation and resection are the most commonly performed surgical procedures for symptomatic lesions. This study aims to compare the outcomes of these two surgical techniques. Methods: A retrospective analysis of symptomatic hepatic hemangiomas (HH) operated upon between 2000 and 2021. Patients were categorized into the enucleation and resection groups. Demographic profile, intraoperative bleeding, and morbidity (Clavien-Dindo Grade) were compared. Independent t-test and chi-square tests were used for continuous and categorical variables respectively. p-value of < 0.05 was considered significant. Results: Sixteen symptomatic HH patients aged 30 to 66 years underwent surgery (enucleation = 8, resection = 8) and majority were females (n = 10 [62.5%]). Fifteen patients presented with abdominal pain, and one patient had an interval increase in the size of the lesion from 9 to 12 cm. The size of hemangiomas varied from 6 to 23 cm. The median blood loss (enucleation: 350 vs. resection: 600 mL), operative time (enucleation: 5.8 vs. resection: 7.5 hours), and postoperative hospital stay (enucleation: 6.5 vs. resection: 11 days) were greater in the resection group (statistically insignificant). In the resection group, morbidity was significantly higher (62.6% vs. 12.5%, p = 0.05), including one mortality. All patients remained asymptomatic during the follow-up. Conclusions: Enucleation was simpler with less morbidity as compared to resection in our series. However, considering the small number of patients, further studies are needed with comparable groups to confirm the superiority of enucleation over resection.

        An Evidence for Host Translation Inhibitory Factor Encoded in a Polydnavirus, Cotesia glomerata Bracovirus, Genome and Its Expression in Parasitized Cabbage White Butterfly, Pieris rapae

        Polydnavirus is a DNA virus symbiotic to some endoparasitic wasps and plays a critical role in accomplishing successful parasitic life cycle of host wasps. Host translation inhibitory factor (HTIF) has been found in some polydnaviral genomes and performs parasitic functions leading to host immunosuppression and redirecting host nutrient usage to wasp development. The cabbage white butterfly, Pieris rapae, parasitized by a gregarious endoparasitoid, Cotesia glomerata, undergoes several physiological alterations including immune malfunctioning and failure of pupal metamorphosis. C. glomerata possesses its own symbiotic polydnavirus, C. glomerata bracovirus (CgBV). Its genome consisted of at least 12 segments in unequal amounts. Parasitized P. rapae hemolymph contained HTIF-like protein, which was determined through an immunoblotting assay using HTIF antibody of C. plutellae bracovirus (CpBV). RT-PCR using HTIF primers of CpBV produced an HTIF-like gene in P. rapae larvae parasitized by C. glomerata. Also, this HTIF-like gene was encoded in CgBV genome and its partial sequence of CgBV showed highly homology (98.5%) to amino acid sequence of an HTIF of CpBV, called CpBV15α. These results suggest that a common HTIF- like moiety may be shared among Cotesia-associated bracovirus.

        Interrogation of earthworm (Clitellata: Haplotaxida) taxonomy and the DNA sequence database

        The DNA sequence database assists us with great potential in species identification because it is practicaland affordable to perform. More often, the identification using molecular data helps to identifymorphologically variable individuals of same species, including juvenile specimens. In case of earthworms,the classical standards of taxonomy are mainly based on genital structures, while the collectionof the sexually agile worm is difficult. Hence, typical barcode marker cytochrome c oxidase subunit 1 wasused to explore species richness of the study area. From taxonomic relationship, the valid identificationof voucher specimens was taken into consideration for characterization. The species richness of earthwormsp. viz., Metaphire houlleti, Perionyx sp., Drawida minuta, Eudichogaster prashadi, Lennogasterpusillus, Octochaetona beatrix, and Octochaetona prashadi were recorded and analyzed. Less than 1%divergence within species was found, whereas divergence between species was greater than 15% in allcases. The phylogenetic tree(s) was constructed using the best-fitted substitution model of evolution andspecies delimitation by Automatic Barcode Gap Discovery, operational taxonomic units, and networkanalysis. The prospective and limitations of molecular taxonomy and the vital role of publicly availablenucleotide sequence databases were reviewed to inventor earthworm diversity. The study shows theuniversality of single locus persists utopian and the use of other typical loci may strengthen earthwormdiversity assessment.

        Parasitism by Cotesia glomerata Induces Immunosuppression of Pieris rapae: Effects of Ovarian Protein and Polydnavirus

        A gregarious endoparasitoid wasp, Cotesia glomerata, parasitizes the cabbage butterfly, Pieris rapae. During wandering larval stage for pupal metamorphosis, the parasitoid larvae egress from the parasitized host to form cocoons thus eventually leading to death of the host. This study focused on the effect of C. glomerata parasitization on cellular immune response of P. rapae. For this purpose, an ideal anticoagulant buffer was formulated to procure the hemocytes in native form with morphological, behavioral, and functional characteristics. The hemocytes selectively encapsulated only DEAE beads under in vitro conditions and a quantitative study revealed about 70% of the beads being encapsulated. On the other hand, calyx fluid from C. glomerata injected to P. rapae markedly inhibited the spreading ability of the hemocytes in a dose-dependent manner and also attenuated the in vitro encapsulation response of the hemocytes against the cationic bead. The calyx fluid contained polydnavirus as well as ovarian proteins. The isolated polydnavirus genome consisted of variously sized-segments with their unequal amounts. The P. rapae injected with the calyx fluid expressed several polydnaviral genes within 2 h. These results suggest that the immunosuppression of the parasitized P. rapae may be induced by the polydnaviral gene products as well as ovarian proteins.

        Metachronous Cancer of Breast and Adenocarcinoma of Cervix: A Rare Case Report

        The occurrence of a second primary cancer in a cancer survivor is well documented. It may be synchronous or metachronous. Incidence of metachronous cancer involving cervix is 0.82% to 1.33%. One such metachronous cancer is that of breast and cervix. We present a case of a woman who received tamoxifen for invasive ductal cancer of breast following a modified radical mastectomy and subsequently developed adenocarcinoma of cervix after six month of tamoxifen therapy. The role of tamoxifen in pathogenesis of cervical cancer and that of human papillomavirus infection in pathogenesis of both cancer of cervix and breast cancer has been well recognized. In our patient, the adenocarcinoma of cervix (rare occurrence) which is likely due to six month of tamoxifen therapy is a perplexing question. Women diagnosed and treated for breast cancer need to be followed up for development of other metachronous gynecological cancers. (J Menopausal Med 2017;23:131-134)

        Primary multifocal cystic signet ring neuroendocrine tumor of liver: a case report

        Primary signet ring neuroendocrine tumors of the liver are extremely rare tumors. Morphologically, they mimic signet ring cell adenocarcinomas; however, the absence of mucin by special stains and the expression of neuroendocrine markers help to diagnose these tumors. We herein report a case of a 47-year-old female who presented with multiple solid and cystic lesions in both liver lobes, which were initially suggested to be biliary cystadenocarcinoma on imaging. Liver biopsy of the lesion revealed the presence of a signet ring neoplasm with diffuse expression of synaptophysin and pan-cytokeratin. The case was subsequently diagnosed as a primary hepatic signet ring neuroendocrine tumor. The patient was offered 3 cycles of chemotherapy and is well preserved after 14 months of diagnosis. Although this is an extremely rare entity, its possibility should be considered in the differential diagnosis of neoplasms characterized by signet ring cell morphology.         Case Report : Spontaneous Perforation of Pyometra

        Pyometra is collection of purulent material which occurs when there is interference with its normal drainage. It is an uncommon condition with incidence of 0.1 to 0.5% of all gynecological patients. Spontaneous rupture of uterus is an extremely rare complication of pyometra. A 65-year-old lady presented with pain abdomen and purulent vaginal discharge. Preoperative diagnosis of pyometra was made by magnetic resonance imaging (MRI). Laparotomy followed by peritoneal lavage and repair of perforation was performed. Although spontaneously perforated pyometra is rare, the condition must be borne in mind with regard to elderly women with acute abdominal pain. Preoperative diagnosis of perforated pyometra is absolutely essential. Computed tomography (CT) and MRI are diagnostic tools. In selected cases conservative approach at surgery can be opted.
